        Clozapine 투여후 발생한 급성 간염

        Clozapine 투여 후에 급성 간염이 발생했던 두 명의 환자에 대한 증례로, 두 명 모두 이전에 약물에 이한 간염이 발생한 과거력이 있었던 환자로서 무증사의 급성 간염이 발생한 후에 보존적 치료 후에 호전 되었다. 간염이 발생한 후에 간효소수치에 대해 정기적으로 검사를 하면서, 사용하고 있던 clozapine의 용량을 낮춘 후 간장제를 투여함으로써 간효소수치는 정상화되었다. Clozapine과 비교하면 clozapine은 간세포의 cytochrome P450에 영향을 미치는 것으로 생각되고 있고 chlorpromazine은 간내 담즙저류에 영향을 미치는 것으로 생각된다. 과거 약물에 의한 간염을 경험한 환자는 clozapine에 의해 유도된 감염에 보다 취약성을 보일 가능성이 있으며, 이런 관점에서 이전의 간 기능 이상을 경험했거나 물질 사용 장애 등의 병발이 있을 경우 정기적인 간기능 평가가 임상적으로 중요할 것으로 판단된다. The authors report two cases of clzapine-induced acute hepatitis. Two patients developed asymptomatic hepatitis and got better with conservative care. We decreased the dosage of clozapine and added hepatic protections, resulting in normalized laboratory findings. The authors also reviewed side effects of clozapine in this report. We reviewed the suggested mechanism of either clozapine or chlorpromazine-induced hepatitis. clozapine influences the liver cell via cytochrome P 450 and chlorpromazine does so via mild cholestasis. There may be a possibility that a patient who has experienced drug-induced hepatitis is vulnerable to clozapine-induced acute hepatitis. In this respect, those who have experienced drug-induced hepatitis must be observed more closely.

      • 흉추의 병적골절을 보인 심한 신성 골이영양증 1례 : 증례 보고

        A 33-year-old woman presented with tingling sensation and weakness in the both hands and feet. She had a diagnosis of secondary hyperparathyroidism caused by renal failure. Her height was shortened due to severe kyphosis. On plain radiograph, there were fractures on both femoral necks and pathologic fracture on thoracic vertebra. An MRI scan of the spine demonstrated a expansile, heterogeneous hypointense lesion of the 2nd thoracic spine, which remained hypointense on T2 images. Following total parathyroidectomy the patient underwent a thoracic decompression and spinal stabilization from the back. Hyperparathyroidism may cause brown tumor and tumor associated pathologic fracture in the spine. Involvement of the spine with neural compression is extremely rare. An early spinal decompression with parathyroidectomy leads to remission of symptoms.

        꽃꽂이시 절화조합이 장미의 절화수명과 미생물 증식에 미치는 영향

        Various combinations of cut rose and other cut flowers were accomplished to provide basic data for vase life extension of cut rose. The vase life of cut rose combined with other cut flowers was as follows ;rose (8 days) > rose + gladiolus (6 days) > rose + calla (4 days) > rose + chrysanthemum (4 days) > rose + carnation (3 days) > rose + gerbera (2 days). The vase solution of cut rose without other cut flowers had the lowest concentration (1.2×105 CFU/mL) of bacterial cells. On the other hand, The vase solution of cut rose with gerbera had the highest concentration (9.3×106 CFU/mL) of bacterial cells. From the above results, it is concluded that vase life of cut rose is extended by reducing the bacterial cells in the vase solution though the proper combination with effective cut flowers.

        전북 익산지역에서의 묘화 이용실태 조사

        This study was surveyed to know utilization of the in graveyard the military and the police(MP), the catholic graveyard(CG) and graveyard of mausoleum(GM). The items of investigation were the ratio, kinds of flower, color of cut flower and form of offering a bunch of flowers. The time of investigation were New Year's day, Chusok(the korean thanksgiving day) and a weekday. On a week day, the ratio of offering a bunch of flowers was 6.3 perrcent in GM. CG and GM were 2 and 6 percent, respectively, On Chusok MP was 9 percent, CG was 25 percent and GM was 55.9 percent. On New Year's Day MP was 17.9 percent, CG was 43.3 percent and GM was 49.8 percent. Except for GM, when we compared the chusok with a week day, they were offered 12-24 times. Of the kinds of cut flower offered a bunch of flowers, chrysanthemum was 60 percent or more without regard to MP, CG and GM. About color of cut flower, white was 39.2 percent or more and yellow was 8.8 percent or more. Most of used colors were white and yellow. And they were 48 percent or more. That of on a week day was low remarkably. The form of offered flower on a graveyard were various: a flower, a basket of flower, a potflower, a flower in a case of glass, and a flower in a vase. Of that, a bunch of flowers were 33 percent or more in GC, MP and CG. KEY WORDS : flower, graveyard, new year's day, chusok, chrysanthemum

        건강가정지원센터 가정생활교육 프로그램 운영 및 성과 : 용산구·숙명여자대학교 시범사업을 중심으로

        The purpose of this study is to propose a model for service programs and management strategies for the division of family education in Healthy Families Center. For the purpose, the researchers find out demands for the family education programs of people in Yong-San Gu, Seoul, exam the effect of each family education program of Yong-San Gu & Sookmyung Women's University Healthy Families Center from June to December, 2004. According to the results, people in Yong-San Gu need economic education service mostly, and they want to study at Saturday afternoon. The programs show positive effect on the improvement of family health.

        2003년 국내 중증급성호흡기증후군 진료 현황 및 문제점 분석

        목적 : 2002년 말 중국에서 SARS가 발생한 이후 국내에서도 2003년 10월까지 총 3명의 추정환자, 17명의 의심환자가 보고되었다. 향후 추가적인 SARS의 유행이 우려되는 상황에서, 그간의 SARS 환자 진료에 있어서의 실질적인 준비사항, 진료 현황 등에 대한 조사를 통해 문제점을 파악하여, 향후 더 나은 대비가 될 수 있도록 개선점을 제시하고자 하였다. 재료 및 방법 : SARS로 의심되는 환자를 진료 경험이 있는 병원의료진을 대상으로 2003년 10월에 설문조사를 실시하였다. 설문에는 SARS 환자 진료 시의 실질적인 조치, 진료 현황, 병실, 응급실 및 외래에서의 격리 시설과 준비사항, 보건당국의 관리와 지원에 관한 사항을 포함하였다. 결과 : 대상이 되는 22개 병원 중 17개(17/22, 77.2%) 병원이 설문에 응하였다. SARS 환자를 위한 격리실은 응급실, 외래, 일반병실 및 중환자실에서 각각 9개(9/17, 52.9%), 5개(5/17, 29.4%), 15개(15/16, 93.7%), 4개(4/16, 25%) 병원에서 음압처리가 되어있지 않은 일인실 혹은 다인실이 사용되었고, 1개(1/16, 6.3%) 병원에서만 일반병실에서 음압격리실이 운영되었다. 입원환자의 진찰 시 개인보호구의 착용은 거의 모든 의료기관에서 이루어졌다. 보건당국에서 SARS지정병원의 시설 등을 사전에 확인한 곳은 1곳(1/12, 8.3%)이였고, 14개 병원(14/15, 93.3%)에서는 보건당국에 의뢰한 검사결과를 통하 받지 못하였다. 결론 : 의료기관에서 SARS 환자용 격리실뿐만 아니라 기존의 격리실 설비 등이 미흡하였으며, 특히 중환자실 및 외래의 준비가 더욱 부족하였다. 보건당국의 의료기관에 대한 종합적인 지원이 부족하였고, 병원과의 원활한 연계가 잘 이루어지지 않았다. SARS 만이 아닌 격리를 필요로 하는 질환의 적절한 진료를 위해 향후 병원 시설의 정비와 정부차원에서의 보다 구체적이고 실질적인 대책마련이 필요하다. Background : There was an worldwide outbreak of the Severe Acute Respiratory Syndrome (SARS) originated from China in late 2002. During that period three cases of suspected SARS and 17 cases of probable SARS were reported in Korea. With the concerns about the reemergence of SARS-coV transmission, it is important to be prepared for any possibility. So, this study is aimed to analysis the past measures in managing SARS and propose the amendatory plans to improve the preparedness. Materials & Methods : Questionnaires were collected among clinicians with any experience in managing the probable or suspected SARS cases in Oct. 2003. 17 out of 22 hospitals responded to the questionnaire. The contents in the questionnaire were practical activities, personal equipments, response plans, isolation facilities in emergency centers, outpatient clinics, general wards and intensive care units, and relationship with the public health department. Results : The dedicated isolation rooms in emergency centers, outpatient clinics, general wards, and intensive care units were prepared in 9 (9/17, 52.9%), 5 (5/17, 29.4%), 15 (15/16, 93.7%), and 4 (4/16, 25.0%) hospitals, respectively. Except for one hospital that newly made negative pressure room for SARS, single or multi-bed rooms without airborne infection control were used in all the other hospitals. The personal precaution principles were kept quite well in general wards. Before the designation of SARS hospital by the public health department prior evalution to see if the hospital was suitable for managing SARS was conducted in only 1 (1/12, 8.3%) hospital. The results of laboratory diagnosis were reported back in 1 (1/15, 6.6%) hospital. Conclusions : The isolation facilities which can control airborne infection were almost deficient not only for SARS but also for other respiratory transmissible diseases. For the infection control of transmissible diseases including SARS, more investment is needed on medical facilities and comprehensive support from the public health department required.

      • Molecularly designed, dual-doped mesoporous carbon/SWCNT nanoshields for lithium battery electrode materials

        <P>Formidable challenges facing lithium-ion rechargeable batteries, which involve performance degradations and safety failures during charge/discharge cycling, mostly arise from electrode-electrolyte interface instability. Here, as a polymeric ionic liquid (PIL)-mediated interfacial control strategy to address this long-standing issue, we demonstrate a new class of molecularly designed, ion/electron-conductive nanoshields based on single-walled carbon nanotube (SWCNT)-embedded, dual-doped mesoporous carbon (referred to as “SMC”) shells for electrode materials. The SMC shell is formed on cathode materials through solution deposition of the SWCNT/PIL mixture and subsequent carbonization. The PIL (denoted as “PVIm[DS]”) synthesized in this study consists of poly(1-vinyl-3-ethylimidazolium) cations and dodecyl sulfate counter anions, whose molecular structures are rationally designed to achieve the following multiple functions: (i) precursor for the conformal/continuous nanothickness carbon shell, (ii) dual (N and S)-doping source, (iii) porogen for the mesoporous structure, and (iv) SWCNT dispersant. Driven by such chemical/structural uniqueness, the SMC shell prevents direct exposure of cathode materials to bulk liquid electrolytes while facilitating redox reaction kinetics. As a consequence, the SMC-coated cathode materials enable significant improvements in cell performance and also thermal stability. We envision that the SMC shell can be suggested as a new concept of effective and versatile surface modification strategy for next-generation high-performance electrode materials.</P>
