        Elderly kidney transplant recipients have favorable outcomes but increased infection-related mortality

        Background: The number of elderly patients with end-stage kidney disease has been increasing, but the outcomes of kidney transplants (KT) remain poorly understood in elderly patients. Therefore, we evaluated the clinical outcomes of elderly KT recipients and analyzed the impact of elderly donors. Methods: This retrospective cohort study included patients who underwent KT between 2000 and 2019. KT recipients were divided into four groups according to a combination of recipient and donor age (≥60 or <60 years); elderly recipients: old-to-old (n = 46) and young-to-old (n = 83); young recipients: old-to-young (n = 98) and young-to-young (n = 796). We compared the risks of mortality, graft failure, and acute rejection between groups using Cox regression analysis. Results: The incidence of delayed graft function, graft failure, and acute rejection was not different among groups. Annual mean tacrolimus trough level was not lower in elderly recipients than young recipients during 10-year follow-up. Mortality was significantly higher in elderly recipients (p = 0.001), particularly infection-related mortality (p < 0.001). In multivariable Cox regression analysis, old-toold and young-to-old groups had increased risk of mortality (adjusted hazard ratio [aHR], 2.89; 95% confidence interval [CI], 1.14– 7.32; p = 0.03; aHR, 3.06; 95% CI, 1.51–6.20; p = 0.002). However, graft failure and acute rejection risks were not increased in elderly recipients. Conclusion: In elderly recipients, graft survival and acute rejection-free survival were not inferior to those of young recipients. However, mortality, especially risk of infection-related death, was increased in elderly recipients. Thus, low immunosuppression intensity might help decrease mortality in elderly recipients.

        The purpose of this study was to compare the characteristics, change in physique and physical function of college students majoring physical education and Taekwondo (454 males and 64 females of age groups ranging from 18 to 26). The results are as following: 1. The male group of 23yrs old in physical education showed highest values in 7 items(Weight, Chest Girth, Fore Arm Girth, Thigh Girth, Calf Girth, Scapular Skinfold Thickness and Suprailiac Skinfold Thickness). 2. In the Physical function, the male group of 18yrs old showed highest values in Power, Flexibility, and then 21yrs old at Back Strength, Vital Capacity, and 23yrs old at Grip Strength, and 25yrs old at Blood Pressure, respectively. 3. In the physique, the male group of 22yrs old showed highest values in Weight, Chest Girth, Thigh Girth, Calf Girth, and then 24yrs old Upper Arm Girth, Fore Arm Girth, and 19yrs old at Height, Upper Limb Length, respectively. 4. In Physical Function, the 22yrs old showed highest values in Back Strength, Vital Capacity, and then 23yrs old at Side Step, Trunk Extension, but 18yrs old showed lower Values in Grip Strength, Side Step, Vital Capacity and Blood Pressure. 5. The female group of 22yrs old showed highest values in 8 items(Height, Sitting Height, Waist Girth, Upper Limb Length, Lower Limb Length, Skinfold Thickness), and then 18yrs old at 5 items (Weight, Upper Arm Girth, Fore Arm Girth, Thigh Girth, Calf Girth), respectively. 6. In the physical function, the 22yrs old showed highest Values in Grip strength, Back Strength, Power, 5 Minute Run, and 18yrs old at flexibility, Harvard Step Test, respectively. 7. In the change of the physique for two years(1988-1989) in the male group, majoring Physical education the 22yrs old showed increase in 4 items, and the 20yrs old in 3 items, but 18yrs old showed decrease in 3 items. 8. In the change of the physical function for two years in the male group, the 20yrs old showed increase in 4 items, and the 21, 25, 26yrs old in 3 items, but the 19yrs old showed decrease in 6 items, and 18, 22 yrs old in 5 items, respectively. 9. The change of the physique for two years in all female group, of 18, 19, 20, 21yrs old showed increase in 11 items except Skinfold Thickness. 10. In the change of physical function for two years in female group, the 18yrs old showed increase in 3 items, and then 20yrs old in 5 items, but 19, 22yrs old decreased 12 items, respectively. 11. The highest significant difference in physique between male and female was found in weight(mean Value 17.5%), and the lowest difference was found Thigh Girth, Calf Girth(1,3%), Upper Limb Length, Lower Limb Length(2.5%). 12. The Highest difference in physical function was Strength (mean Value 40%), Vital Capacity (33%), Power(22%), but Lower Trunk Flexion, Foot Balance(3.4%). 13. In the difference of Physique and physical function between physical education major and taekwondo major, the physical education showed higher values in skinfold Thickines (mean Value 16.5%), Weight, Waist Girth(4.7%) and Strength(33%), Endurance(5.6%), but subjects majoring Taekwondo showed higher values in Flexibility and Foot Balance. 14. In all male groups, Waist Girth, Scapular Skinfold Thickness, Back Strength, Sargent Jump, Trunk Extension and Diastolic Blood Pressure were showed a significantly related at 0.001 level.

        Identification of a sensitive urinary biomarker, selenium-binding protein 1, for early detection of acute kidney injury

        <P>Acute kidney injury (AKI) is associated with increased mortality rate in patients but clinically available biomarkers for disease detection are currently not available. Recently, a new biomarker, selenium-binding protein 1 (SBP1), was identified for detection of nephrotoxicity using proteomic analysis. The aim of this study was to assess the sensitivity of urinary SBP1 levels as an early detection of AKI using animal models such as cisplatin or ischemia/reperfusion (I/R). Sprague-Dawley rats were injected with cisplatin (6 mg/kg, once i.p.) and sacrificed at 1, 3, or 5 days after treatment. Ischemia was achieved by bilaterally occluding both kidneys with a microvascular clamp for 45 min and verified visually by a change in tissue color. After post-reperfusion, urine samples were collected at 9, 24, and 48 hr intervals. Urinary excretion of protein-based biomarkers was measured by Western blot analysis. In cisplatin-treated rats, mild histopathologic alterations were noted at day 1 which became severe at day 3. Blood urea nitrogen (BUN) and serum creatinine (SCr) levels were significantly increased at day 3. Levels of urinary excretion of SBP1, neutrophil gelatinase-associated lipocalin (NGAL), and a tissue inhibitor of metalloproteinase-1 (TIMP-1) were markedly elevated at day 3 and 5 following drug treatment. In the vehicle-treated I/R group, serum levels of BUN and SCr and AST activity were significantly increased compared to sham. Urinary excretion of SBP1 and NGAL rose markedly following I/R. The urinary levels of SBP1, NGAL, TIMP-1, and KIM-1 proteins excreted by AKI patients and normal subjects were compared. Among these proteins, a marked rise in SBP1 was observed in urine of patients with AKI compared to normal subjects. Based upon receiver-operator curves (ROC), SBP1 displayed a higher area under the curve (AUC) scores than levels of SCr, BUN, total protein, and glucose. In particular, SBP1 protein was readily detected in small amounts of urine without purification. Data thus indicate that urinary excretion of SBP1 may be useful as a reliable biomarker for early diagnosis of AKI in patients.</P>

      • TiAIN을 코팅한 WC공구의 절삭성능에 관한 연구

        Thin film of TiAlN was deposited onto ISO P20 tungsten carbide cutting insert tip substrate specimen using the FVAS (filtered vacuum arc system) at the substrate temperature of 80℃. Cutting and wear tests have been performed to the TiAlN coated and uncoated WC specimen, respectively under the same cutting conditions for the comparison. Cutting forces and tool wear of coated and uncoated carbide cutting tools were investigated by means of cutting length and cutting forces. Also tool wear and tool life were analyzed using the 3ch tool dynamometer amp lifier and oscilloscope. In this study cutting tests for the cutting performance of TiAlN coated WC-Co P20 to the steel alloy of SNCN420 have been curried out. Through the comparisons of cutting forces during the cutting tests and tool wear, we got the conclusions as follows.; ① In usual cutting conditions, main cutting force component showed higher value than the thrust force normal to the cutting direction especially cutting forces in the of early stage on cutting tests. But, after loosing the protective hard thin film on substrate or insert tip, the thrust force normal to the cutting direction showed much higher than main cutting force component. ② Within the range of 0.3㎜ build up edge in P20 insert tip, TiAlN coated tip was evaluated more stable in flank wear and less in crater wear than the uncoated specimen. ③ Through the wear growth curve of the specimen, tool life differences of uncoated and TiAlN coated tip at high speed (V=250[m/min]) was higher than the low speed(V=200[m/min]) case, compared to the average but the tool wear was almost same in both specimen for the case of normal wear occurred.

        The current study evaluates the capacity of eugenol to regulate immediate allergic reaction by control of histamine release. Administrations of eugenol (1 M/kg. i.p.) at 60 min before and 5.10 min after the compound 48/80 treatment (8 mg/kg.i.p.) were shown the mortality rates as 0.44.4. and 77.8%. respectively. A 60 min before administered group revealed a singificant inhibition of serum his-tamine release compared with those of 5 and 10 min after the compound 48/80 injection. Eugenol (6-48mM) was also showed a dose-dependent activity on the compound 48/80-induced histamine release from the highly purified population of Alcian Blue-positive peritoneal mast cells. These results indicate that in vitro treatment with exogenous eugenol inhibited the active response of mast cell populations and modulated its characteristics.

        The role of FGF-2 in smoke-induced emphysema and the therapeutic potential of recombinant FGF-2 in patients with COPD

        <▼1><P>Although the positive effects of recombinant fibroblast growth factor-2 (rFGF-2) in chronic obstructive pulmonary disease (COPD) have been implicated in previous studies, knowledge of its role in COPD remains limited. The mechanism of FGF2 in a COPD mouse model and the therapeutic potential of rFGF-2 were investigated in COPD. The mechanism and protective effects of rFGF-2 were evaluated in cigarette smoke-exposed or elastase-induced COPD animal models. Inflammation was assessed in alveolar cells and lung tissues from mice. FGF-2 was decreased in the lungs of cigarette smoke-exposed mice. Intranasal use of rFGF-2 significantly reduced macrophage-dominant inflammation and alveolar destruction in the lungs. In the elastase-induced emphysema model, rFGF-2 improved regeneration of the lungs. In humans, plasma FGF-2 was decreased significantly in COPD compared with normal subjects (10 subjects, <I>P</I> <I>=</I> 0.037). The safety and efficacy of inhaled rFGF-2 use was examined in COPD patients, along with changes in respiratory symptoms and pulmonary function. A 2-week treatment with inhaled rFGF-2 in COPD (<I>n</I> = 6) resulted in significantly improved respiratory symptoms compared with baseline levels (<I>P</I> <I><</I> 0.05); however, the results were not significant compared with the placebo. The pulmonary function test results of COPD improved numerically compared with those in the placebo, but the difference was not statistically significant. No serious adverse events occurred during treatment with inhaled rFGF-2. The loss of FGF-2 production is an important mechanism in the development of COPD. Inhaling rFGF-2 may be a new therapeutic option for patients with COPD because rFGF-2 decreases inflammation in lungs exposed to cigarette smoke.</P></▼1><▼2><P><B>Lung disease: Inhaling a protein might help</B></P><P>Studies on the role of the protein ‘fibroblast growth factor-2’ (FGF-2) in chronic obstructive pulmonary disease (COPD) suggest that inhaled FGF-2 could help treat the emphysema linked to smoking. Researchers in South Korea led by Young-Koo Jee at Dankook University, Cheonan, and Yeon-Mok Oh at the University of Ulsan, Seoul, studied the role of the reduced FGF-2 levels found in mice with lung inflammation caused by exposure to cigarette smoke. They also uncovered details of a protective effect of inhaled FGF-2, identifying specific cellular and lung structure changes attributed to the administered FGF-2. Reduced FGF-2 levels were also found in patients with COPD. Initial trials revealed some improvement in patients treated with FGF-2, but not at a statistically significant level. Nevertheless, the authors suggest their results justify further investigation of the protein’s therapeutic potential.</P></▼2>

        The cutting tests of wood patterns are carried out using CNC milling machine. The cutting forces and surface characteristics of machined surface are investigated at various cutting conditions such as spindle speed, feed speed, cutting direction of wood pattern and wood material. In the CNC end-milling, the surface roughness increases as feed speed increases and decreases as spindle speed increases. However, the cutting force and surface roughness are different along the cutting direction and material of wood pattern.

        The purpose of this study was to compare the characterists and change in physique and physical function by college student of physical education major(287males and 40 females of age groups ranging from 18 to 26). The results were as following; 1) The characteristics of physique and physical function by sex and age. ① The male group of 18yrs old showed higher values in Height, Weight, Upper Limb Length, Strength, Flexibility, and Vital Capacity than any other groups, but female group showed lower values in Height, Weight, Chest Girth, Lower Limb Length, Thigh, Girth, Calf Girth and all of the Physical function. ② The male group of 19yrs old showed high values in power and Agility, then female group showed in Strength and Flexibility. ③ The male group of 20yrs old showed the lower values in Weight, Waist Girth, Upper Limb Length, Fore Arm Girth, Thigh Girh.Calf Girth, Vital Capacity, and female group are very lowed in Upper Arm Lenght, Fore Arm Length, Skin Fold Thickness and 5 minute Run. ④ The male group of 23yrs old showed the high values in Upper Arm Girth, Fore Arm Girth and Balance, and then female group higher in Weight, Chest Girth, Waist Girth, Upper Arm Girth, Fore Arm Girth, Thigh Girth, Power, Shuttle Run, Balance and vital capacity than any other age Groups. ⑤ In male group, Chest Girth, and Blood Pressure were significantly related, and Side Step and Trunk Extension were nagatively related to the age at 0.1% level, and then in female group, Sargent Jump, Standing Long Jump and Vital Capacity were showed a significantly related at 0.5% level. 2) The difference of physique and physical function between male and female by age. ① Everage difference in whole physique items except skinfold thickness was 9.8%, and that 21.1% in all of physical function except Trunk Flextion between male and female group subjects. ② About the ages, the 18yrs old male group showed the 22.8% higher values of 26 items, but 22yrs old male group were about 12.2% in 23 items, thus it is very lower than any other groups. 3) The difference of physique and physical function between General group and physical education group in age-matched. ① In the physique male physical education groups showed about 7% higher in Weight, Chest Girth, Upper Limb Length, Upper Arm Girth, Thigh Girth, and female group showed about 7% higher in Weight and Upper Limb Length. ② In the physical function, Trunk Flextion, 5 minute Run, and Blood Pressure(Diastolic) were about 8% high values in whole group, and female group showed about 8% higher in Standing Long Jump, Shuttle Run, and Trunk Extension. ③ In 22yrs old male and female physical education group were very dominated in physique and physical function than the other age group against the general group.

      • Causal Relationships between Firms Performance, Usage Level, and Customer Satisfaction in the Organizational Market of Hospitality

        The objectives of this study were to develop theoretical model that outlined the antecedents of satisfaction in an industrial buying context, to investigate product usage level as an outcome of the two economic factors, and to examine the relationships of product, usage level and service factor with customer satisfaction as the organizational market for hospitality is growing. To figure out the relevant constructs and variables for this study, related literature was reviewed. Then, to satisfy the purposes of the study, structural equation modeling method was utilized with a confirmatory factor analysis. A confirmatory factor analysis examined a four-factor solution developed from the factor analysis. Also, goodness-of-fits for measurement model and structural model were evaluated to test the reliability of both models. The various measures of overall model goodness-of-fit lend acceptable level of support to deeming the results an acceptable representation of the hypothesized constructs. Based on the acceptable level of support for the goodness-of-fit tests, the relationships among the constructs in the model were examined. Finally, two alternative models were proposed and compared to the original model.
