        Acinar cell carcinoma is a rare tumor that represents 1~2% of al1 pancreatic cancers. Clinical and radiologic findings are inconclusive in this disease Acinar cell carcinoma is characterized by rapid progression and early metastasis, which lead to its poor prognosis. A 41-year-o1d man was admitted to our hospital for abdominal pain. Abdominal computed tomography (CT) and positron emission tomography-computed tomography (PET-CT) showed a splenic mass, which was being invaded by a pancreatic tail mass and which had increased ^(18)F-fluorodeoxyglucose (FDG) uptake Primary radical distal pancreatectomy and splenectomy were performed. Pathologic findings revealed an acinar cell carcinoma of the Pancreas The patient underwent a total gastrectomy three months later because of gastric recurrence Four months later, multiple hepatic metastases were discovered, and the patient underwent a left hepatectomy During treatment with capecitabine, there was no evidence of tumor progression for 14 months. We report a case of metastatic pancreatic acinar cell carcinoma, which did not progress for an extended period while the patient was being treated with capecitabme.

        Primary retroperitoneal mucinous cystadenocarcinoma is a very rare malignancy, and little is known concerning its Pathogenesis, optimal treatment, and prognosis. A 29-year-o1d pregnant woman (21 weeks) Presented with abdominal discomfort CA 19-9, CA 125, and CEA were normal Abdominal CT scanning revealed a 19x15x13 cm retropentoneal tumor Exploratory laparotomy and tumor excision were performed. Mucinous retropentoneal implants were removed as completely as possible Histologically, the tumor showed focal areas of capsular invasion, but free resection margins The uterus and both ovaries were normal in appearance No adjuvant therapy was pursued. Six months later, Peritoneal and bilateral ovarian metastases were discovered. Hence, we report the details of this case of primary retroperitoneal mucinous Cystadeno-carcinoma and present a review of the literature.

      • 도시고형폐기물의 성분분석에 관한 연구

        도시지역에서 배출되는 일반폐기물에 대한 성분분석결과 밀도는 250∼300㎏/㎥, 수분함량은 중량비로 평균 51.83%, 가연성분 42.09%, 회분 6.08%로 각각 측정되었다. 한편, 저위발열량은 주거지역이 700∼1,700㎉/㎏, 상업지역이 2,000㎉/㎏이상으로 측정되어 상업지역이 주거 지역보다 발열량이 높은 폐기물이 배출됨을 알 수 있었는데 이는 수분함량이 높은 주개류의 배출이 상업지역이 주거지역보다 적으며 소비자 포장지로서 종이, 비닐, 플라스틱류 등의 사용이 많은데 기인하는 것으로 생각되어 진다. 한편, 분리수거, 유가품에 대한 자원 재활용, 연탄재 배출의 감소추세 등의 효과로 폐기물 중 불연성 함량은 줄어들고 반면 종이, 비닐, 플라스틱류 등의 가연성 폐기물의 함량은 크게 늘어 전체적으로 폐기물의 질이 향상되고 있음을 알 수 있다. The analysis results of components of municipal solid wastes are as follows : ① Density ranged from 250㎏/㎥ to 300㎏/㎥. ② Water content, combustible content and ash content measured 51.83%, 42.09%, 6.08% respectively. ③ Low calorific value was ranged from 700 to 1,700㎉/㎏ in residential area and measured 2,000㎉/㎏ in commercial one. Therefore, it was known that calorific value of wastes in residential area is smaller than that in commercial area. Also, the quality of wastes was improved by increase of combustible content which results from vitality of valuable recycling, reduction trend of briquette ash generation rate and increase of paper, plastic consumption as wrapping materials.

        Hypoparathyroidism is usually the result of an inadvertent surgical removal of all the parathyroid glands, In some instances, not all the tissues are removed, but the remainder undergoes vascular supply compromise secondary to the fibrotic changes in the neck after surgery, Previously, the surgery for hyperthyroidism was the most frequent cause of acquired hypoparathyroidism. Idiopathic hypoparathyroidism is a relatively rare disease that is characterized by hypocalcemia and hyperphosphatemia due to a parathyroid hormone deficiency of an unknown cause, It usually develops at a young age, and shows various clinical symptoms and signs accompanied with hypocalcemia. In addition, it is rarely associated with polyglandular autoimmune syndrome during the follow-up. Hypocalcemia and idiopathic hypoparathyroidism associated with labor and lactation are rarely reported condition previously. We here describe a case of a woman in whom the symptomatic severe hypocalcemia appeared after her delivery. We reviewed all the previously reported cases and suggest a possible physiological explanation for the association between pregnancy, lactation, and the appearance of symptomatic hypocalcemia.

        The effect of unsaturated fatty acids on photosynthetic electron transport reaction in isolated choroplasts is investigated to clarify the changes of PS Ⅱ and PS Ⅰ electron transport rates, Chl. a fluorescence intensity, and photosynthetic oxygen evolution. In photosystem Ⅰ, an increase in the electron transport rates occurs in the presence of 10 to 80 μM linolenic acid, followed by a decline in rate from 100 to 300 μM linolenic acid. In photosystem Ⅱ, a decrease in the electron transport rates occurs in the presence of 10 to 20 μM linolenic acid. The efficiency of C_18 unsaturated fatty acids/linolenic (C_(18:3)). linoleic(C_(18:2)) oleic(C_(18:1)) on PS Ⅱ and PS Ⅰ electron transport rates is established in order C_(18:3)>C_(18:2)>C_(18:1) At a low concentration(10-80 μM) of linolenic acid a decrease in Chl. a fluorescence yield was due mostly to the inhibition on the donor side of PS Ⅱ. At a high concentration (100-300 μM), an increase in Chl. a fluorescence yield was due additionally to the inhibition on the acceptor side of PS Ⅱ.

        Objective : As the average lifespan of human increases, osteoporosis and osteoporosis-related fractures have become major health care problems. Despite recent advances in medical treatment, few studies have assessed the recognition of osteoporosis in general adults. This study examined the recognition of osteoporosis and analyzed the relating factors. Method : A population-based sample of 1086 adults in Asan-city, Korea was investigated with questionnaires and height, weight, Information regarding the general characteristics(gender, age, economic state), lifestylef exercise, smoking, alcohol),medical history(progestin, GH, steroid, calcitonin, PTH), demographic parameter, obesity, stress, and family history was collected through an interview using a structural questionnaire. The level of obesity was measured by the body mass index(BMI). BMD(Bone mineral density) at the lumbar spine and femoral neck was measured by dual energy X-ray absorptiometry. Osteoporosis was considered to be T-score below -2.5. Results : The prevalence of osteoporosis increased progressively with age, decreased with BMI and predominant in women by the result of x^(2) -test, T-test. Age, weight, BMI were significantly correlated with osteoporosis by the result of Pearson correlation ana1ysis(P<0.05). And age, gender, BMI, family history of osteoporosis were proved to be significant risk factors of osteoporosis by the result of multiple logistic regression(P<0.05). Conclusion : These results suggest that age, gender, BMI might be the most important risk factors of osteoporosis. And the history taking about family history of osteoporosis is helpful for diagnosing osteoporosis.

        점오염원의 대기확산에 관한 민감도 분석과 모델링

        The sensitivity analysis of two short-term models (ISCST3, INPUFF2.5) is performed to improve the model accuracy. It appears that the sensitivities on the changes of wind speed, stack height and stack inner diameter in the near distance from source, stability and mixing height in the remote distance from source, are significant. Also, the gas exit velocity, stack inner diameter, gas temperature and air temperature which affect the plume rise have some effects on the concentration values of each model within the downwind distance where final plume rise is determined. And in modeling for the atmospheric dispersion of point pollutant source INPUFF2.5 can calculate amount, trajectory of puff and concentration versus time at each receptors. So, it is compatible to analyze distribution of point pollutants concentration at modeling area.

      • 결핵균항원에 대한 폐결핵환자의 임파구 증식반응에 관한 연구

        T lymphocytes are thought to play a central role in cell mediated immune response. To study the T lymphocyte proliferative response to purified 30-kDa antigen from Mycobacterium tuberculosis H37Rv, peripheral blood lymphocytes(PBL) isolated from healthy controls and tuberculosis patients were stimulated with the 30-kDa antigen, crude antigen and PHA. Healthy controls and tuberculosis patients were divided into PPD(+), PPD(-) groups and AFB(-), AFB(+) groups, respectively. PBL proliferation were determined by the ^3H-thymidine incorporation assay and MTT colorimetric assay. PBL proliferation with 30-kDa and crude protein antigens measured by both methods were almost identical(r=0.74, p<0.001). The lymphocyte proliferation to 30-kDa antigen and crude antigen were singnificantly increased in PPD(+) healthy controls and tuberculosis patients when compared to those in PPD(-) controls, but response to PHA was no significant difference. Analysis to T cell subsets of proliferated lymphocytes performed by the indirect immunoalkaline phosphatase techniques were no difference between not only healthy controls and patients groups, but also stimulated antigen or PHA, and especially, at all groups, the percentage of T8 cell was higher than that of T4 cell.

        Loss-of-function mutations in the Transcription Factor 7 (T cell factor-1) gene in hepatogastrointestinal cancers

        Inappropriate activation of the Wnt signaling pathway has been repeatedly implicated in the tumorigenesis of colon, liver, and gastric cancers. There is accumulating evidences that transcriptional factor 7 (TCF7; also called T cell factor 1) might also be one of the tumor suppressor genes in the Wnt pathway. We performed PCR-based sequencing analysis of the TCF7 gene in 234 alimentary tract cancers. The TCF7 mutants detected in this study were functionally analyzed after they were generated by a QuickChange site-directed mutagenesis kit. We detected 7 somatic mutations in the TCF7 gene, including 4 missense, 2 frameshift, and one 28-bp deletion. In a yeast two-hybrid assay, most of the mutants showed varying degrees of decreased binding to an amino-terminal enhancer of split (ABS), a truncated form of Groucho-related protein lacking WD40 repeats. To determine whether mutant TCF7 proteins had decreased DNA binding, we performed electrophoretic mobility shift assays, and the 2 frameshift mutants were shown to have no DNA binding activity. Furthermore, luciferase reporter assays revealed that TCF7 mutants in the presence of AES failed in the AES-dependent transcriptional repression of the reporter gene. In addition, human embryonic kidney 293 cells transfected with TCF7 mutants expressed high levels of cyclin D1, up to 6 times more than cells transfected with wild-type TCF7. Therefore, the TCF7 mutations detected in this study seem to be "loss-of-function mutations" caused y loss of TCF7 repressor activity through decreased binding to Groucho-related protein and/or DNA, thereby contributing to neoplastic transformation by causing accumulation of cylin D1.

      • Probucol inhibits LPS-induced microglia activation and ameliorates brain ischemic injury in normal and hyperlipidemic mice

        <P>Aim: Increasing evidence suggests that probucol, a lipid-lowering agent with anti-oxidant activities, may be useful for the treatment of ischemic stroke with hyperlipidemia via reduction in cholesterol and neuroinflammation. In this study we examined whether probucol could protect against brain ischemic injury via anti-neuroinflammatory action in normal and hyperlipidemic mice. Methods: Primary mouse microglia and murine BV2 microglia were exposed to lipopolysaccharide (LPS) for 3 h, and the release NO, PGE2, IL-1 beta and IL-6, as well as the changes in NF-kappa B, MAPK and AP-1 signaling pathways were assessed. ApoE KO mice were fed a high-fat diet containing 0.004%, 0.02%, 0.1% (wt/wt) probucol for 10 weeks, whereas normal C57BL/6J mice received probucol (3, 10, 30 mg.kg(-1).d(-1), po) for 4 d. Then all the mice were subjected to focal cerebral ischemia through middle cerebral artery occlusion (MCAO). The neurological deficits were scored 24 h after the surgery, and then brains were removed for measuring the cerebral infarct size and the production of pro-inflammatory mediators. Results: In LPS-treated BV2 cells and primary microglial cells, pretreatment with probucol (1, 5, 10 mu mol/L) dose-dependently inhibited the release of NO, PGE2, IL-1 beta and IL-6, which occurred at the transcription levels. Furthermore, the inhibitory actions of probucol were associated with the downregulation of the NF-kappa B, MAPK and AP-1 signaling pathways. In the normal mice with MCAO, pre-administration of probucol dose-dependently decreased the infarct volume and improved neurological function. These effects were accompanied by the decreased production of pro-inflammatory mediators (iNOS, COX-2, IL-1, IL-6). In ApoE KO mice fed a high-fat diet, pre-administration of 0.1% probucol significantly reduced the infarct volume, improved the neurological deficits following MCAO, and decreased the total-and LDL-cholesterol levels. Conclusion: Probucol inhibits LPS-induced microglia activation and ameliorates cerebral ischemic injury in normal and hyperlipidemic mice via its anti-neuroinflammatory actions, suggesting that probucol has potential for the treatment of patients with or at risk for ischemic stroke and hyperlipidemia.</P>
