      • Anti-Inflammatory and Analgesic Effects of Pyeongwisan on LPS-Stimulated Murine Macrophages and Mouse Models of Acetic Acid-Induced Writhing Response and Xylene-Induced Ear Edema

        <P>Pyeongwisan (PW) is an herbal medication used in traditional East Asian medicine to treat anorexia, abdominal distension, borborygmus and diarrhea caused by gastric catarrh, atony and dilatation. However, its effects on inflammation-related diseases are unknown. In this study, we investigated the biological effects of PW on lipopolysaccharide (LPS)-mediated inflammation in macrophages and on local inflammation <I>in vivo</I>. We investigated the biological effects of PW on the production of inflammatory mediators, pro-inflammatory cytokines and related products as well as the activation of nuclear factor kappa B (NF-κB) and mitogen-activated protein kinases (MAPKs) in LPS-stimulated macrophages. Additionally, we evaluated the analgesic effect on the acetic acid-induced writhing response and the inhibitory activity on xylene-induced ear edema in mice. PW showed anti-inflammatory effects by inhibiting the production of nitric oxide (NO), tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) and interleukin-1β (IL-1β). In addition, PW strongly suppressed inducible nitric oxide synthase (iNOS), a NO synthesis enzyme, induced heme oxygenase-1 (HO-1) expression and inhibited NF-κB activation and MAPK phosphorylation. Also, PW suppressed TNF-α, IL-6 and IL-1β cytokine production in LPS-stimulated peritoneal macrophage cells. Furthermore, PW showed an analgesic effect on the writhing response and an inhibitory effect on mice ear edema. We demonstrated the anti-inflammatory effects and inhibitory mechanism in macrophages as well as inhibitory activity of PW <I>in vivo</I> for the first time. Our results suggest the potential value of PW as an inflammatory therapeutic agent developed from a natural substance.</P>

        만성 췌장염의 합병증으로 생긴 가성동맥류는 드문질환으로, 반복적으로 다량의 출혈을 일으킬 수 있으며, 치료를 하지 않을 경우 사망율이 높기 때문에, 조기 진단과 치료가 필수적이다. 저자들은 38세 남자 환자에서 췌장염의 합병증으로 생긴 가성낭종내에서 발생한 가성동맥류 1예를 경험하였기에 보고하는 바이다. 가성 동맥류의 색혈류도플러상 낭종내에 양방향 흐름의 와류를 볼 수 있었다. Because of high morbidity and mortality, early detection and treatment of the pseudoaneurysm is needed. We report a case of pseudoaneurysm within pseudocyst from pancreatitis. Color-flow Doppler sonography shows bidirectional flow and turbulent arterial flow, within anechoic mass.

        사염화탄소 투여 흰쥐에서 간섬유화 진행에 따른 Propranolol 의 약동학적 지표들의 변화

        Background/Aims : This study was designed to determine the effect of hepatic fibrotic severity on pharmacokinetics of propranolol in CC1q - treated rats. Methods - 1 mL/kg of 10% CC14 in olive oil was injected intramuscularly to rats twice weekly for 4, 6, 8 and 10 weeks, respectively (n=6). Control (n=6) was a sham - injected equal dose of olive oil for 10 weeks. After intravenous bolus injection of 2 mg/kg propranolol to rats, the serum propranolol concentrations were analyzed for 4 hours at various time points by a HPLC - fluorimetric system, and pharmacokinetic parameters such as Co, MRT, AUC, Vdss, tl/2(p) and CLp were determined. Then, a small amount of hepatic tissue was obtained and subjected to determination of the hepatic 4 - hydroxyproline content, which confirmed the hepatic fibrotic severity. Results : The serum concentrations of propranolol at 0.5, 1, 2 and 4 hours were significantly increased in CC1q-treated rats (p$lt;0.01). In proportion to the duration of CC4 treatment, Co and AUC were significantly increased, and Vdss and CLp were significantly decreased (p$lt;0.001). But MRT and t1/2(β) were not significantly changed. The hepatic 4 -hydroxyproline content was gradually increased in CC4-treated rats (p$lt;0.001). Conclusion - Gradual changes in pharmacokinetic parameters of propranolol were seen to be dependent on the hepatic fibrotic severity. We suggest that gradual dosage modification, according to their hepatic fibrotic severity, is necessary for many drugs administered to patients with chronic liver disease.(Korean J Hepatol 2001;7:181-188)

        함초(Saliconia herbacea L.)로부터 베타인 정량

        해안가 및 폐염전 등에 서식하며 각종 미네랄, 아미노산 및 유용 생리활성 물질을 함유하고 있는 함초로부터 혈중 동맥경화 및 심장 질환 예방에 유효한 효능을 보일 것으로 예상되는 성분인 betaine을 HPLC를 이용하여 분석하였다. 함초는 양쪽성 물질로 같이 존재하는 다른 이온성 물질들에 의해 방해를 받으며 UV 흡수가 매우 낮아 일반적인 정량방법으로는 분석이 매우 까다롭다. 따라서 본 연구에서는 이러한 문제를 해소하기 위하여 2단계 이온교환수지 컬럼을 이용하여 방해물질을 제거한 후 betaine을 4-bromophenacyl bromide (PBPB)로 유도체화시켜 UV-labelling 한 후 분석하였다. 이온교환수지를 통과한 함초추출물의 회수율은 83.6%이며 함초추출액 mL당 4.85㎎의 betaine이 함유되어 있는 것으로 나타났다. The betaine content of Saliconia herbacea L. was determined by reverse-phase high performance chromatog-raphy on a C_(18) column. A 50% methanol extract was passed through a anion exchanger Ambelite IRA 400 (quaternary ammonium type, OH-) column and a strong cation exchanger Ambelite IR 120 (sulfonic acid type, H^+) column to remove amino acids, zwitter ions which are interfere with betaine analysis. The betaine extract was derivatized with 18-crown-6-ether and 4-bromophenacyl bromide (PBPB) for UV-labelling. Betaine in Saliconia herbacea L. was analysed on a mobile phase contained 13 mM sodium heptane sulfonic acid and 5 mM Na₂SO₄ in deionized water by isocratic elution for 30 min. The recovery ratio of betaine from Saliconia herbacea L. extract was 83.6%. The mean betaine value for Saliconia herbacea L. determined by the described method is 4.85 mg/mL with a standard deviation of 0.127.

        Finding key vulnerable areas by a climate change vulnerability assessment

        <P>Extreme climate events such as typhoons, heat waves, and floods have increased in frequency with climate change. Many municipalities within the Republic of Korea (ROK) have experienced damage from these events, necessitating countermeasures. Vulnerability assessment has been suggested in the implementation of a national plan for reducing damage resulting from climate change. Thus, in this study, we assess the vulnerability of the ROK and identify key vulnerable municipalities in support of the national adaptation plan. We create a framework for assessing the vulnerability of all 232 municipalities of the ROK with respect to 32 items in 7 fields. The framework regards decision makers' comprehension and availability of data as important factors. We assess the vulnerability index of each municipality by using variables of climate exposure, sensitivity, and adaptation capacity. The weights of variables are determined by the Delphi method. We used the representative concentration pathways 8.5 climate scenario to reflect future climate exposure for the vulnerability assessment. From the analysis, vulnerability maps are prepared for the 32 items of 7 fields, and key vulnerable municipalities are identified by aggregating the maps. The distribution of vulnerable municipalities changes with the future climate conditions. These maps provide a scientific and objective basis for the ROK government to establish adaptation plans and allocate resources. The ROK government can utilize the results to identify the characteristics of highly vulnerable areas, and municipalities can use the results as a basis for requesting support from the national government.</P>

      • Rat의 복강내에 투여한 5-fluorouracil-polyglycolic acid 제형이 장기에 미치는 영향에 관한 연구

        For more effective intraperitoneal chemotherapy, the authers made 5-fluorouracil-polyglycolic acid composite disks(FU-PGA disks) with 5-fluorouracil and polyglycolic acid, a biodegradable polymer. We inserted the FU-PGA disk into the peritoneal cavites of Sprague-Dawley male rats, the control and three experimental groups; one FU-PGA disk insert group(300mg/kg), two FU-PGA disk insert group(600mg/kg), and three FU-PGA disk insert group(900mg/kg). The control group received a similar number of PGA disk inserts. A pharmacokinetic study was performed to measure the 5-fluorouracil concentration in the peritoneal fluid, blood, and tissues(liver, kidney and heart) at 24 hours, 72hours and 168 hours after insertion of the FU-PGA disk. Light and electron microscopic studies were done. The results were as follows: 1) The number of white blood cells and platelets decreased after FU-PGA insertion. The degree was proportional to the duration and amounts inserted. The change in the number of red blood cells varied slightly. 2) Light microscopically, slight changes were noted at 168 hours in the 3 disk insert group (900mg/kg). Mild fatty change and hepatocyte degeneration around the central veins of the liver were noted, with vacuolar degeneration in the proximal convoluted tubules of the kidney. 3) Electron microscopically,the liver showed focal increases of lipid droplets in the hepatocytes, and irregular nuclear membrane with focal nucleolar segregation of the fibrillar and granular elements. Also the double membranous structure of the hepatocytes and Kupffer cells was widened with bleb formation. The kidney showed wide separation of the nuclear double membrane in the proximal convoluted tubular cells and mesangial cells, with vacuolation and myelin figure formation in the mitochondria of the proximal convoluted tubular cells, at 72 hours and 168 hours in the 2 to 3 FU-PGA insert groups. The heart showed focal loss of cristae,vacuolation and myelin figure formation in the mitochondria of the 2 to 3 FU-PGA insert groups. In conclusion, despite the large amont of FU-PGA inserted, the histopathological changes in the liver, kidney and heart were slight, and consonant with the very low amount of 5-fluorouracil concentrations detected in the liver, kidney and heart. The above results suggest that the FU-PGA composite can serve as a new device for releasing drugs in a controlled manner and easily targeted to intraperitoneal organs. This device can improve the efficacy of intraperitoneal chemotherapy for gastric cancer.

      • 신장수여자의 수술경과시간에 따른 자기존경감, 희망감, 염려 및 스트레스원의 차이

        This study attempted to examine the differences of self - esteem, hopes, concerns and total stress scores of 110 kidney recipients in before(N=32) and fellowing kidney transplantation [1 to 6 months(N=29), 6 to 12 months(N=23), 12 to 36 months(N=26)] and utilizing severity rating scale of 44 potential stressors on a 5-point scale of 110 kidney recipients in before(N=32) and following kidney transplantation operation [1 to 6 months(N=29), 6 to 12 months(N=23), 12 to 24 months(N=15) and 24 to 36 months(N=11)]. On scores of hopes and total stress, pretransplant group exhibited significantly higher scores than three posttransplant recipient groups. But, it was found that there was no significant differences among four transplant kidney recipient groups on the scores of self - esteem and concerns. In the pretransplant group, craving for foods was identified as the most stressful event, on the contrary, changes in friends was identified as the lowest stressful events. In the 1 to 6 months posttransplant group, side effect of medications was identified as the most stressful event, however, loss of contact with dialysis patients was identified as the lowest stressful event. In the 6 to 12 months posttransplant group, cost of medication, changes in body appearance were identified as the most stressful event, on the other hand, managing of medications at home was identified as the lowest stressful event. In the 12 to 24 months posttransplant group, cost of medication and insurance coverage were identified as the most stressful event, but possibility of dying and competence of nursing staff were identified as the lowest stressful event. In the 24 to 36 months posttransplant group, changes of body appearance was identified as the most stressful event, on the contrary, possibility of infection was identified as the lowest stressful event. Insurance coverage was a continuing concern throughout posttransplant period.

        최근 30년간 스포로트리쿰증 임상 병형의 추세에 대한 관찰

        Background: The clinical types of sporotrichosis are generally classified into four group: lymphocutaneous, fixed cutaneous, and cutaneous disseminated, and hematogenous. Of these, the lymphocutaneous variety is the most frequently encountered pattern, constituting 80% of sporotrichosis patients and the fixed cutaneous sporotrichosis is the next. Potassium iodide (KI) was the most commonly used drug for the treatment of sporotrichosis. Many studies about these clinical findings of sporotrichosis have been reported, but comparative study on the changes in clinical findings has not been reported. Objective: The purpose of this study is to clarify the changes of the clinical findings in sporotrichosis in Korea. Methods: A total of 58 patients with sporotrichosis who had visited the Department of Dermatology, Pusan National University were divided into 2 groups according to the period and then the clinical findings of sporotrichosis were compared. Results: The results of study are summarized as follows: 1. During this period, there were 58 cases of sporotrichosis. In the past period (1967~1979) there were 35 cases of sporotrichosis, and there were 23 cases in the recent period (1980~1997) 2. In the past period, most cases (71.4%) were lymphocutaneous type, however in the recent period, most cases (69.2%) were fixed cutaneous type. 3. The most commonly affected site was right upper extremity in the past period, but face fright upper extremity were the most commonly affected sites in the recent period. 4. In the past period, 2nd and 3rd decades were prevalent age group, but 5th to 7th decades were prevalent age group in the recent period. 5. In the treatment of sporotrichosis, potassium iodide (KI) was the choice of treatment in the past period, however itraconazole is gradually used in the recent period. Conclusion: According to this study, many changes in clinical findings of sporotrichosis were noted, especially in the clinical type. We found that clnical type of sporotrichosis changed from lymphocutaneous type to the fixed cutaneous type with time. And we suggest that further observation would be necessary to define the meaning of these changes.

        The purpose of this study is to investigate the propriety of TMA clathrate as a cold storage medium. Particularly, this is to examine the extent of subcooling improvement when the surfactants are added to the TMA clathrate, because water used cold storage material has low phase change temperature and subcooling. this study has been analysed and compared with TMA 30wt% clathrate how phase change temperature, subcooling and specific heat in the various concentrations are changed. This results view to be improved phase change temperature and subcooling of TMA30wt% as a cold storage medium, when it had some surfactant. Additionally, it is that surfactant must be controlled under available solution limit and study for new additive must be lasted to know its effect.
