        冬壽의 出自로 본 高句麗의 낙랑군지배

        Main character of the Anak Tomb No.3, Dong-su is from unknown family in China, but he was charged with a top position where only elite worked as in Murong(慕容) regime. That was unusual, so this researcher called his background in question. As the result of analyzing various family names-related data, Dong-su was not from China but from Nakranggun. At that time, Chinese immigrants and original settlers who are Goguryeo’s descendants lived in Nakranggun, and Dong-su was also one of original settlers. Meanwhile, the history book reveals Dong-su’s birthplace as Yodong. When King Micheon of Goguryeo ruled Nakranggun(樂浪郡) in 313, among residents of Nakranggun, there were immigrants who moved to Yoseo in China and they settled themselves in Nakranggun that was newly established near to Geuksung(棘城). Besides, they were immgrated in Changryeogun(昌黎郡) and Yodonggun(遼東郡). As the result, Nakrang Han family received a new domicile of Changryeo Han family, and Nakrang Dong family was changed to Yodong Dong family, which was the reason that Dong-su was expressed as Yodong. For Goguryeo that subjected Nakranggun, controlling powerful families(豪族) who were main power of Nakranggun was the main point. Goguryeo carried through the Nakranggun governance by using the graded powerful family Dong(冬) to restrain Nakranggun large family Wang(王). They received administrative rights from the Goguryeo regime, but military rights were totally under Goguryeo. Han(韓) and Wang(王) families that were ruled like this became the sacrificial lamb of the reinforcement policy of kingship and received a blow after King Jangsu(長壽王) moved the capital from Kuknaesung to Pyeongyang. The fact was turned out in inscription remaining in China through the analysis of the study. As the result, they left Nakrang for China. After that, these two families weakened as much as their trace was not found in the history of the late Goguryeo. However, some people from Nakranggun who just followed Goguryeo’s rule played a important role after the late Goguryeo.

        삼차신경통과 반측안면경련에서 CISS 영상의 진단적 유용성

        Objectives : Trigeminal neuralgia and hemifacial spasm are caused by vascular compression of the REZ(root entry or exit zone) of the 5th and the 7th cranial nerve. Preoperative detection of neurovascular compression is essential for accurate diagnosis, appropriate treatment, and the good operative results. Three dimensional Fourier Transformation-Constructive Interference in Steady State(3DFT-CISS) images are known to give good contrast between CSF, nerve, and vessels. We applied a 3DFT-CISS imaging technique for the preoperative evaluation of patients with these diseases and estimated the diagnostic accuracy and usefulness of this study. Methods : A series of 71 patients with trigeminal neuralgia and hemifacial spasm were treated by microvascular decompression. Among them 34 patients with trigeminal neuralgia and 24 patients with hemifacial spasm had preoperative CISS images. We compared the radiologic finding with the operative finding, and analysed the diagnostic usefulness of 3DFT-CISS imaging. Results : The sensitivity of CISS images of detecting the neurovascular compression was 90.3% in trigeminal neuralgia and 100% in hemifacial spasm. There were one false-positive and three false-negative cases in trigeminal neuralgia, and one false-positive case in hemifacial spasm. The accuracy in diagnosing the causative vessel was 73.5% in trigeminal neuralgia and 83.3% in hemifacial spasm. Conclusion : CISS image is very useful diagnostic tool for preoperative evaluation of neurovascular compression in patients with trigeminal neuralgia and hemifacial spasm. No additional neuroradiologic examination other than CISS image and MRA is needed for preoperative evaluation of patients with trigeminal neuralgia and hemifacial spasm.

      • 경계요소법을 이용한 위상변이 마스크의 단차 효과 분석

        3차원 위상변이 마스크의 단차 효과를 분석하기 위해 투명 경계조건, 주기적인 경계조건, 및 연속조건을 가진 경계요소법을 광 리소그래피 공정 시뮬레이션에 새로이 적용하였으며, 해석적인 해와 참고문헌의 결과와 비교함으로써 구현된 모듈의 정확성을 검증하였다. 또한, 기존의 rigorous coupled wave analysis에 의한 방법에 비해 수렴성과 계산 시간 측면에서 경계요소법을 이용하는 것이 더 효율적임을 확인하였다. 끝으로 비교적 간단한 위상변이 마스크와 다층-위상변이 마스크에 대한 최적 설계 과정을 기술하였다. The boundary element method was newly implemented into an optical lithography simulator so that it could evaluate rigorously the topological effects of 2dimensional phase-shifting masks. Both transparent and periodic boundary conditions were applied for the method, and the continuity conditions were used for treating interface nodes. The accuracy of the module developed for simulating aerial images was verified by comparison with analytic solutions and published results. In addition, it was found that our simulator would be more efficient than the conventional method based on the rigorous coupled wave analysis in views of the convergence and the calculation speed. Finally, the optimal design of two phase-shifting masks was performed.

        염소 가스 중독에서 나트륨 중탄산염의 흡입치료 2례

        A chlorine gas is a common irritant and when exposed, it result in mild occular, oropharyngeal, or respiratory symptoms. In severe case, however, it may result in pulmonary edema, interstitial pneumonia, or respiratory failure. We report the case of 29-year-old and 46-year-old men is accidentally exposed to chlorine gas during cleaning water. The patients complained dyspnea, chest tightness, cough and both eye pain. During hospitalization, they were treated with inhalation of humidified oxygen, beta-adrenergic agonist and $2\%$ sodium bicarbonate. After several days, patients were discharged without respiratory symptoms and complication. In treatment of chlorine gas toxicity the inhalation of sodium bicarbonate is a possible initial therapy can improve respiratory symptoms in spite of lack of evidence.

        본방사선 치료 후에 후두 절제술을 시행할 경우 인두피부누공, 감염, 출혈 등 상처부위 합병증의 가능성이 높다. 저자들은 최근에 항암치료와 방사선 치료 후에 재발한 성문상부암에 대해 후두 전적출술을 시행하였고, 수술 66일째 생명을 위협하는 인두의 동맥 출혈을 경험하였기에 문헌고찰과 함께 보고하는 바이다.

        평활근육종은 주로 사지와 몸통에 주로 발생하며, 악하선의 원발성 평활근육종은 매우 드물다. 악하선의 원발성 평활근육종은 증상과 방사선 검사 모두에서 비특이적이며, 치료는 광범위 절제술이다. 최근 저자들은 악하선에 발생한 원발성 평활근육종을 광범위 적출술을 시행한 후에 술후 방사선 치료를 시행하여 치험하였기에 문헌 고찰과 함께 보고하는 바이다. 악하선의 평활근육종은 드물지만, 방사선 검사에서 경계가 분명하고 조영 증강이 잘되며, 세침흡입검사에서 방추세포가 발견될 경우에 감별 진단으로 고려해야 한다.

        Goltz 증후군 1례

        저자들은 특징적인 진찰 소견과 피부 조직 생검에서 제 I형 교원질 유전자 발현의 감소를 보인 Goltz 증후군 1례를 경험하였기에 보고하는 바이다. Goltz syndrome(focal dermal hypoplasia) is a rare disorder characterized by ectodermal and mesodermal dysplasia described in 1962 by Goltz. In Korea, one case of Goltz syndrome was reported in 1994. The inheritance mode is mostly X-linked dominant. Skin abnormality is the most common manifestation including hypoplasia of the dermis. Skeletal involvement such as syndactyly, polydactyly, scoliosis, kyphosis and spina bifida occulta may be present, also ocular and dental abnormalities are reported. Radiologic findings are the osteopathy and striation of the long bone. We experienced a case of Goltz syndrome in a 9-year old female who was presented with right side hypotrophy, focal dermal hypoplasia, ocular(anidria, microcornea), dental(oligodontia, amelogenesis) and skeletal(syndactyly) abnormalities. Skin biopsy was performed and showed decreased expression of type I collagen gene with Northern blotting.

        국내 건설산업 특성을 고려한 기업의 벤치마킹 프로세스

        벤치마킹은 기업의 경쟁력 향상 및 경영전략수립을 위한 효과적인 도구이나 대상과 방법이 적절치 못한 경우 그 실효성이 저하될 수 있다. 건설업의 경우 제조업과는 생산과 경영구조 그리고 수익구조 등 여러 가지 측면에서 다르기 때문에 다른 방식의 벤치마킹이 필요하다. 국내 건설업의 특성은 동일 유형의 경쟁기업이 다수 존재하고 법규 및 정책 등 외부환경변화에 영향을 많이 받는 등 다양하다. 따라서 건설기업의 벤치마킹은 특성상 기업군을 대상으로 하는 것이 효율적이며 자료수집 시자기업의 성과인 내부자료와 벤치마킹 대상의 성과인 기업자료 뿐 아니라 외부환경변화자료도 수집되어야 하며 기업 간 성과 비교 시 외부환경변화가 반드시 고려되어야 한다. 건설업 특성상 건설기업은 차입금과 자기자본 비율의 변화가 크기 때문에 최적자본구조에 대한 분석도 필요하다. 본 연구는 이와 같은 건설업 특성과 국내 기업의 상황을 분석하여 기존의 벤치마킹 프로세스와 차별된 건설기업 벤치마킹 프로세스를 제안한다. Benchmarking is an effective tool supporting improvement of corporate competitiveness and development of management strategy, however, its effectiveness may be compromised if its target and approach are not appropriate. Notably, the construction industry of Korea is unique in that it has a number of competitors sharing the same category and is significantly exposed to changes in external environment in terms of legal, regulatory and policy frameworks. Such characteristics make target corporate category rather than single company more suitable as benchmarking target and necessitate analysis of external environment.         해상응급환자를 위한 원격진료 시스템의 설계 및 구현

        Though there were many research and development about telemedicine on land recently, not yet applied to that on the shore. In this paper, when emergency clinic situation were generated in a ship on shore, the telemedicine system was designed and implemented for transfer and clinic service to doctors on the land by measuring basic body signals of patients. Presently, wireless communication and inmarsat telephone are usually used by simple questions and clinic consults to remote doctors when emergency situation were happened in the ship. In this paper, the telemedicine system on the shore were developed for improving this problems by measuring patient's fundamental conditions such as the blood pressure, pulse, the respiratory condition, electrocardiogram, body temperature, patient image and sending these information to remote doctors on land for more accurate prescription. The developed system can supply the high level clinic service to emergency patients on the shore and cope with the emergency situation in ship. 최근 육상에서 원격진료에 대한 많은 연구와 기술개발이 수행되어 왔으나 해상에는 적용되지 않고 있다. 본 논문에서는 해상의 선박에서 긴급한 응급의료 지원상황이 발생했을 때 환자의 생체정보를 측정하여 원격지에 있는 의사에 게 전달하고 진료 받기 위한 원격 진료 시스템을 설계하고 구현하였다. 현재 해상에서 응급상황이 발생 시 주로 무선통신이나 위성 전화를 통한 단순한 문답에 의한 의학 자문에 의존하고 있다. 본 논문에서는 이를 개선하기 위해 전문적으로 진단에 필요한 환자의 기초 신체상태 즉, 혈압, 맥박, 호흡, 심전도, 체온 및 영상 등을 선박 내 현장에서 측정한 후 전문 의료진에 원격 전송함으로써 보다 정확한 처방이 가능하고 보다 높은 의료서비스를 제공할 수 있도록 개발하였다. 개발된 시스템으로 해상상의 응급환자에게 의료 서비스의 질을 높여 응급상황에 적절히 대처할 수 있다고 본다.

        모사 배기관 내 HC-DeNOx 촉매용 인젝터의 분무 거동 특성

        A new method that optimizes a control of hydrocarbon (HC) addition to diesel exhaust gas for HC type DeNOx catalyst system has been developed. These catalysts are called as the HC-DeNOx catalyst in this paper. The system using HC-DeNOx catalyst requires a resonable quantity of hydrocarbons addition in the inlet gas of the catalyst, because the HC concentration in a diesel engine is so low that the HC is not sufficient for NOx conversion. Generally ambient temperature in the exhaust manifold is $250{\sim}350^{\circ}C$, so spray behavior in this case is different from that of any other condions. This research shows spray behavior of injected hydrocarbons in the transparent exhaust manifold.
