To develop a clotrimazole-loaded solid suppository with poloxamer and propylene glycol, the melting points of various formulations composed of poloxamer 188 (P 188) and propylene glycol were investigated. The dissolution study of clotrimazole delivered by the suppository composed of P 188 and propylene glycol was performed. The mixtures composed of P 188 and propylene glycol were homogeneous. Propylene glycol affected the melting points of poloxamer mixtures. In particular, the mixture [P 188/propylene glycol (70/30%)] with the melting point of about 32°C was a solid form at room temperature and instantly melted at physiological temperature. Furthermore, propylene glycol affected greatly the dissolution rates of clotrimazole from the suppository. Dissolution mechanism analysis showed the dissolution of clotrimazole was proportional to the time. Our results indicated that the solid suppository with P 188 and propylene glycol would be a candidate of rectal dosage form for clotrimazole.

제주도에 분포하는 스코리아콘의 사면발달을 밝히기 위하여 49개 스코리아콘을 대상으로 화산체의 비고, 저경에 대한 비고의 비율($H_{co}$/$W_{co}$), 저경과 화구경의 차에 대한 비고의 비율($H_{co}$/$W_{co-cr}$), 및 사면각등의 지형특성치를 계측하였다. 스코리아콘의 형성시기가 오래될수록 지형특성치는 일정한 비율로 감소하므로 화산체의 형태비교학적 방법에 의해 스코리아콘의 형성시기를 추정할 수 있다. 스코리아콘의 화구는 침식작용으로 메워지며 결국에는 사라지게 되므로 화산체의 형성시기와 가장 높은 상관관계를 보이는 지형특성치는 저경과 화구경의 차에 대한 비고의 비율이나 평균사면각($\theta_{ave}$) 이다. 스코리아콘의 평균사면각은 $\theta$$_{ave}$ : $tan^{-1}$〔2.$H_{co}$/$W_{co-cr}$〕로서 구할 수 있다. 제주도는 플라이스토세에 네 번에 걸친 분화활동기를 통하여 형성되었다. 제4분출기(0.1~0.025Ma)에 형성된 최신의 스코리아콘은 평균사면각의 평균치가 23.6$\pm$1.7$^{\circ}$인데 비하여, 제3분출기(0.3~0.1Ma)와 제2출기(0.6~0.3Ma)의 스코리아콘은 각각 18.9$\pm$$1.7^{\circ}C$및 12.6$\pm$$1.9^{\circ}C$이다.다. Morphometric parameters such as cone height, cone height/width ratio($H_{co}$/$W_{co}$), cone height/difference between cone width and crater width ratio($H_{co}$/$W_{co-cr}$) and slope angle are measured to investigate the rates of slope development of scoria cones in Cheju Island. The parameters systematically decrease with increasing age of scoria cones, suggesting that comparative morphology of scoria cones is a useful age indicator. Cone age is most highly correlated with $H_{co}$/($W_{co-cr}$ or average slope angle because summit craters are infilled and eventually removed by erosional processes. The average slope angle is estimated as $\theta_{ave}$= $tan^{-1}$/〔2.$H_{co}$/$W_{co-cr}$〕. Cheju Island was formed through four eruptive stages during Pleistocene. The youngest cones of stage 4(0.1~0.025Ma) have a mean average slope angle of 23.6$\pm$1.7$^{\circ}$, whereas stage 3(0.3~0.1Ma) and stage 2(0.6~0.3Ma) cones have mean values of 18.9$\pm$$1.7^{\circ}C$and 12.6$\pm$$1.9^{\circ}C$, respectively.
Malakoplakia in a healthy young female patient

Malakoplakia is a rare granulomatous disease that occurs commonly in the urinary tract and secondarily in the gastrointestinal tract. Most reported cases of malakoplakia are associated with immunosuppressive diseases or chronic prolonged illness. Here, we report a rare case of malakoplakia in a young healthy adolescent without any underlying disease. A 19-year-old female was referred to our hospital following the discovery of multiple rectal polyps with sigmoidoscopy. She had no specific past medical history but complained of recurrent abdominal pain and diarrhea for 3 months. A colonoscopy revealed diverse mucosal lesions including plaques, polyps, nodules, and mass-like lesions. Histological examination revealed a sheet of histiocytes with pathognomonic Michaelis-Gutmann bodies. We treated the patient with ciprofloxacin, the cholinergic agonist bethanechol, and a multivitamin for 6 months. A follow-up colonoscopy revealed that her condition was resolved with this course of treatment.

투수계수는 지반의 안정성 평가에 중요한 인자로 사용되며, 다양한 실험방법을 통해 유추되고 있다. 본 연구는 기존 방법 외에 물리탐사 기법 중에 하나인 탄성파 속도로 투수계수를 산정할 수 있는 방법을 제시하고자 한다. 탄성파 속도를 이용한 투수계수는 기존에 활용되고 있는 Hazen과 Kozeny-Carman 방정식을 이용하였으며, 각각의 수식에 대입되는 입력변수는 문헌값을 선택하였다. 기존방법과 탄성파 속도를 함께 측정 후 결과값을 비교하기 위하여 탄성파 센서가 장착된 새로운 압밀셀(oedometer)을 활용하였다. 새로운 압밀셀은 탄성파 센서가 추가로 장착되어 기존 압밀셀보다 직경은 확장되었지만 시료 직경 대 높이 비율을 2.5로 고정하여 ASTM에서 추천하고 있는 비율을 만족시켰다. 시료는 원형성이 높은 글라스비즈를 선행적으로 선택하였으며, 측정 데이터의 신뢰성을 확보한 후 주문진사와 점토도 함께 적용하였다. 압밀실험 결과 하중이 증가함에 따라 시료의 침하량은 감소하였으며 탄성파 속도는 증가하는 것으로 나타났다. 탄성파 속도는 일반적으로 입자의 접촉면에 의존하므로 재하 응력이 증가할수록 접촉면적이 커져 그 값이 크게 나타난 것으로 판단된다. 압밀실험에 의해 도출된 투수계수와 탄성파 속도를 도출된 투수계수 값을 비교하였으며, 그 결과 거의 유사한 값을 보이는 것으로 나타났다. 따라서 본 연구는 물리탐사기법을 활용하여 지반의 설계상수를 도출한 것에 의의가 크며, 추후 다양한 조건을 변화하여 현장의 적용성을 극대화 시키고자 한다.
염증성 장질환 환자에서 Azathioprine 사용방법에 따른 골수억제 부작용의 빈도

Background/Aims: The most important adverse effect of azathioprine (AZA) is bone marrow toxicity (BMT). Many physicians have preferred a gradual dose increment (GDI) policy for the prevention of BMT. The aim of this study was to evaluate the efficacy of GDI for the prevention of AZA-induced BMT in inflammatory bowel disease (IBD) patients. Methods: The medical records of IBD patients who received AZA in 6 university hospitals were reviewed. The patients were divided into two groups: the GDI group (initial dose <1.5 mg/kg, gradually increased to a therapeutic dose) and the non-GDI group (initial therapeutic dose ≥2 mg/kg). Results: A total of 308 patients were enrolled (male to female ratio, 1:2.3; mean age, 34.91±14.19 years; ulcerative colitis, 43.5%; Crohn`s disease, 55.2%; and intermediate colitis, 1.3%). The overall incidence of BMT was 16.2% (50/308). BMT developed most frequently between fourth to eighth week (26%, 13/50). The rate of BMT of the non-GDI group was significantly higher than that of the GDI group (27.5%, 11/40 vs. 14.6%, 39/268, P=0.038). A multivariate analysis showed that the only factor related to BMT was a non-GDI policy (P=0.036; odds ratio, 2.41; 95% confidence interval, 1.06-5.49). Conclusions: A GDI policy could be useful for reducing AZA-induced BMT in Korean IBD patients. (Intest Res 2012;10:244-250)

Yersinia enterocolitica infection in adults in Korea is reported rarely. Therefore, the possibility of Yersinia infection as a cause of abdominal pain may be overlooked. Because its clinical features are similar to those of acute appendicitis or other diseases that require operations, Yersinia enterocolitis should be diagnosed before surgery. We recently experienced a case of Y. enterocolitica enterocolitis accompanied by postoperative colonic adhesion. A 39-year-old female patient with a 5-year history of asymptomatic uterine myoma underwent transabdominal hysterectomy due to abdominal pain. However, the pain worsened. On CT, diffuse swelling of the right colon and adhesion of the sigmoid colon with luminal narrowing were found. Colonoscopy showed multiple aphthous ulcers with mucosal hyperemia from the cecum to the sigmoid colon. Multiplex PCR with stool specimens, a hemagglutination test for Y. enterocolitica O:3, and tissue culture were positive for Y. enterocolitica. After fluids and antibiotic therapy, her symptoms were relieved. (Korean J Med 2012;83:757-763)
