Purpose: To report a case of general congenital fibrosis syndrome with A-pattern strabismus, bilateral ptosis and hypotropia. Case summary: A six-year-old girl was presented with severe bilateral ptosis and esodeviation of the left eye since birth. No levator action was presented and her chin was elevated at an angle of 25 degrees. In both eyes, an abduction defect was observed, and no vertical movement was presented. There was a left esotropia of 20 prism diopters (PD) and bilateral hypotropia of 35PD. There was an A-pattern strabismus that presents convergence movements during attempts to look upward, and orthotropia during attempts to look downward. Diagnosed as general congenital fibrosis syndrome with A-pattern esotropia, bilateral ptosis and hypotropia, bilateral inferior rectus muscle recessions were performed. After the operation, chin elevation and hypotropia in both eyes were corrected and exodeviation increased. Thus, orthotropia was observed in the primary position, and an A-pattern strabismus occurred with an exotropia of 15PD on the downgaze. Conclusions: A congenital fibrosis syndrome with an A-pattern strabismus was reported for the first time by the authors. On decisions concerningsurgical amounts of horizontal deviation, exodeviation should be considered to increase after bilateral inferior rectus muscle recessions.

Background and Objectives Several studies have assessed the impact of laryngopharyngealreflux disease (LPRD) on the health-related quality of life (HR-QoL), showing significantimpairment of HR-QoL. This study aims to assess the impact of proton pump inhibitor (PPI)treatment of LPRDs to enhance HR-QoL. Subjects and Method We prospectively collected data from LPRD patients from April2017 to July 2019. Patients who have reflux symptom index (RSI) of ≥13 or reflux findingscore (RFS) of ≥7 were enrolled in this study. We assessed HR-QoL using a questionnairewith EORTC QLQ-H&N35, -C30 on the first visit. Patients were treated with PPI (Ilaprazole20 mg/day) on their visits at 4, 8, and 12 weeks. RSI and RFS were measured at each visit andHR-QoL was reevaluated on the last visit. Results Ninety-five patients completed the 3-months follow-up and were enrolled in thisstudy. Female : male ratio was 71:24 and the mean age was 57.0±11.9 (27-80). The initial RSIand RFS were 16.3±8.8 and 12.6±2.9, respectively, but were changed to 11.1±9.7 and 9.7±2.6(p<0.001 in both) at 12 weeks after the treatment. Global health status/QoL, speech problem,dry mouth, and coughing were significantly improved. Conclusion PPI administration is effective in treating LPRD, where effects begin to appear at4 weeks after treatment. HR-QoL was also improved in patients who have RSI improvement.

양안의 각막 결정 침착에 의해 진단된 다발성 골수종 1예

Purpose: Multiple myeloma is characterized by the neoplastic proliferation of a single clone of plasma cells. Multiple myeloma rarely involves the eyeball or the orbital tissues. We report a case of multiple myeloma that presented with corneal crystalline depositions in a patient complaining of decreased vision and irritation of both eyes without any systemic symptoms. Case summary: A 63-year-old woman complained of decreased vision and irritation of both eyes that had started suddenly 20 days before. Uncorrected visual acuity was 0.2 in the right eye and 0.3 in the left eye. Best-corrected visual acuity (BCVA) was 0.9 in the right eye and 1.0 in the left eye. Slit lamp examination showed gray-white crystalline depositions on the epithelium, stroma and Descemet membrane of the cornea extensively, except for the limbus. There were no specific findings on intraocular pressure measurement and fundus examination. The patient did not complain of any systemic symptoms. Multiple myeloma was diagnosed by blood examination and bone marrow biopsy. Two months later, the BCVA decreased to 0.3 in both eyes and artificial tears were prescribed to relieve irritation. Conclusions: When irritation and decreased vision occur abruptly even without systemic symptoms, there is a possibility of multiple myeloma. Systemic evaluations are needed in these cases. 목적: 다발성 골수종은 형질세포가 단클론성으로 증식하여 전신적인 영향을 미치는 질환으로 드물게 안구 및 안와를 침범한다. 저자들은 전신증상 없이 양안의 시력저하 및 안구자극 증상을 주소로 내원한 환자에서 각막의 결정 침착이 발견되어 진단된 다발성 골수종 1예를 경험하였기에 보고하는 바이다. 증례요약: 63세 여자 환자가 20일 전부터 갑자기 시작된 양안의 시력저하 및 안구자극 증상을 주소로 내원하였다. 나안시력은 우안 0.2, 좌안 0.3이었고 최대교정시력은 우안 0.9, 좌안 1.0이었다. 세극등검사에서 각막윤부를 제외한 전범위의 상피 및 간질, 데스메막에 회색-흰색(gray-white)의 결정 침착이 관찰되었으며, 다른 이상소견은 보이지 않았고 전신증상은 호소하지 않았다. 혈액검사 및 골수 생검검사에서 다발성 골수종이 진단되었으며 내원 2개월 후 양안의 최대교정시력이 우안 0.3, 좌안 0.3으로 저하되었고 안구자극 증상이 심해져 인공누액 처방 후 경과관찰 중이다. 결론: 전신증상이 없더라도 갑자기 발현된 시력저하 및 안구자극 증상과 각막 결정 침착이 있는 경우에는 다발성 골수종의 가능성을 염두에 두고 전신 검사를 시행해야 할 것으로 생각된다.

Comparison Architecture for Large Number of Genomic Sequences

Generally, a suffix tree is an efficient data structure since it reveals the detailed internal structures of given sequences within linear time. However, it is difficult to implement a suffix tree for a large number of sequences because of memory size constraints. Therefore, in order to compare multimega base genomic sequence sets using suffix trees, there is a need to re-construct the suffix tree algorithms. We introduce a new method for constructing a suffix tree on secondary storage of a large number of sequences. Our algorithm divides three files, in a designated sequence, into parts, storing references to the locations of edges in hash tables. To execute experiments, we used 1,300,000 sequences around 300Mbyte in EST to generate a suffix tree on disk.

서픽스 트리는 주어진 모든 문자열의 모든 서픽스를 트리 형태로 나타내는 자료구조로서 선형시간에 구성할 수 있으며 문자열에 대한 많은 문제를 효율적으로 해결할 수 있다. 하지만 이런 효용성에도 불구하고 서픽스 트리로 구성한 문자열을 삽입/삭제하는 경우 트리를 구성하는데 상당히 많은 시간이 소비된다. 본 논문은 이러한 문제를 해결하기 위한 서픽스 트리 재구성 알고리즘을 제안한다. 제안하는 알고리즘은 부 문자열을 삽입하는 경우와 삭제하는 경우로 나눈 다음, 발생할 수 있는 모든 경우의 수를 감안해서 설계했다. 알고리즘의 성능을 평가하기 위해서 기존의 Ukkonen 알고리즘과 비교실험 해 본 결과 서픽스 트리 재구성 시 30% 이상 시간이 절약됨을 알 수 있었다. This paper introduces a new algorithms for reconstructing the suffix tree of character string, when a substring id deleted from the string or a string is inserted into the string as a substring. The algorithem has two main functions, delete-structure and insert-structure. The main objective of this algorithm is to save the time for constructing the suffix tree of an edited string, when the suffix tree of the original string is available. We tested the performance of this algorithm with some DNA sequences. This test shows that delete-reconstructing can save time when the length of the subsequence deleted is less than 30% of the original sequence, and the insert-reconstructing takes less time with regard to the length of inserted sequence.

수정체이탈을 동반한 마르팡증후군에서 앞유리체경계면을 보존하며 실시한 공막봉합 인공수정체삽입술

Purpose: To evaluate the safety and efficacy of transscleral intraocular lens (IOL) fixation while preserving the anterior vitreous face in treating ectopia lentis of Marfan syndrome. Methods: This study included six patients (12 eyes) who had undergone surgical intervention for ectopia lentis with or without lenticular opacity. We compared the best-corrected visual acuity (BCVA) before and after the surgery and evaluated perioperative complications. Results: The mean age at the time of surgery was 18.2±10.7 years. The mean follow-up period was 11.2±7.1 months. Mean BCVA scores changed from 0.96±0.37 (LogMar Value) to 0.14±0.17 (LogMar Value). All patients showed more than two lines of improvement in visual acuity. In two eyes, pupillary capture was found. Medically controllable intraocular pressure elevation was found in three eyes. Conclusions: This study suggests that transscleral IOL fixation with a preserved anterior vitreous face can be a safe and effective technique in treating the ectopia lentis of Marfan syndrome. 목적: 수정체이탈을 동반한 마르팡증후군 환자에서 앞유리체경계면을 보존하며 시행한 공막봉합 인공수정체삽입술의 결과를 알아보 고자 하였다. 대상과 방법: 2002년 2월부터 2007년 2월까지 본원 안과에서 수정체이탈로 관류/흡인술 후 앞유리체경계면을 보존하며 공막봉합 인공 수정체삽입술을 시행 받은 마르팡증후군 환자 6명, 12안을 대상으로 후향적 연구를 시행하였다. 결과: 수술 당시 평균 연령은 18.2세였고 수술 후 평균 11.2개월간 경과관찰하였다. 술 전 최대교정시력 0.96±0.37 (LogMar Value)에서 술 후 최대교정시력 0.14±0.17 (LogMar Value)로 12안 모두에서 2줄 이상의 최대교정시력의 향상을 보였고, 2안에서 인공수정체 동공 물림과 3안에서는 안압 상승을 보였지만 안약으로 조절 가능하였다. 결론: 수정체이탈을 동반한 마르팡증후군 환자에서 앞유리체경계면을 보존하며 실시한 공막봉합 인공수정체삽입술은 안전하며 효율 적인 수술방법으로 생각된다.

만성 육아종성 편도염의 가능한 원인으로서의 톡소플라즈마증: 증례보고

Toxoplasmosis is a parasitic disease, both congenital and acquired, caused by an obligate intracellular parasite known as Toxoplasma gondii. Most immunocompetent individuals who are infected do not show any signs or symptoms of infection while in immunocompromised patients, severe complications that mimic various diseases may follow. Several cases of cervical lymphadenitis associated with toxoplasmosis have been reported, however, toxoplasma manifestations of faucial tonsils have not been dealt with in depth. Herein, we report a case of chronic granulomatous tonsillitis with possible toxoplasmosis origin, in a healthy individual presenting with recurrent mild fever. The etiology, epidemiology, clinicopathological features are also reviewed.

Effective Biological Sequence Alignment Method using Divide Approach

This paper presents a new sequence alignment method using the divide approach, which solves the problem by decomposing sequence alignment into several sub-alignments with respect to exact matching subsequences. Exact matching subsequences in the proposed method are bounded on the generalized suffix tree of two sequences, such as protein domain length more than 7 and less than 7. Experiment results show that protein sequence pairs chosen in PFAM database can be aligned using this method. In addition, this method reduces the time about 15% and space of the conventional dynamic programming approach. And the sequences were classified with 94% of accuracy
