        배곧은근피판을 이용한 유방재건 시 복벽의 주름형성과 합성그물을 이용한 복부성형술의 유용성

        Purpose: The transverse rectus abdominis myocutaneous(TRAM) flap has become a mainstay of breast reconstruction. The chief disadvantage of the TRAM flap is its potential to create a weakness in the abdominal wall. Nowadays true hernia is less frequent, but bulging that appears at the muscle donor site, or at the contralateral side, or at the epigastric area is still remained as a problem. To prevent this complications, we have used synthetic mesh as well as abdominal muscle plication. Now we report the result of our methods.Methods: We started to use synthetic mesh and muscle plication as supplementary reinforcement for entire abdominal wall, after TRAM flap harvesting, in an attempt to stabilize it and achieve a superior aesthetic result since 2002. We observed complications of TRAM flap donor site, and compared our results (from January, 2002 to January, 2006) with other operator's result (before 2001) at the same hospital in aspect of incidence of abdominal complications. Results: 42 consecutive patients have been performed routine reinforcement with the extended mesh technique and muscle plication from January, 2002 to January, 2006. Mean patient follow up was 25.2 months. No hernia or mesh related infection were encountered and only one patient had a mild abdominal bulging. Nevertheless the our good results, there were no significant statistical differences were observed between two groups.Conclusion: We recommend the using of synthetic mesh and muscle plication for donor site reconstruction after TRAM flap breast reconstruction to improve strength as well as aesthetic quality of the abdominal wall.

        광배근근피판을 이용한 유방재건술이 림프부종에 미치는 영향

        Lymphedema is one of the most common complications of mastectomy. It decreases quality of life and causes functional or aesthetical problems in post-mastectomy patients. Axillary lymph needs dissection (ALND), and radiation therapy(RTx) is known as the representative factor of lymphedema. Authors discovered that breast reconstructions using latissimus dorsi(LD) myocutaneous flap decrease the incidence of lymphedema in spite of these risk factors. Therefore we compared the incidence of lymphedema between the patients who underwent breast reconstructoins by LD pedicled flap, and the patients who did not undergo breast reconstructions from January 2002 to December 2004. Lymphedema was diagnosed when difference of arm circumference was over 2cm or limitation of joint movement was greater than 20 degrees. Overall incidence of lymphedema was 14.0%, and it was 18.9% in case of ALND, and 21.1% in case of RTx, respectively. But the incidence of breast-reconstructed patients using LD pedicled flap was 3.3%. This result reveals that LD pedicled flap decreases incidence of lymphedema significantly. In the future, it is recommended to identify the causes of decrease in the incidence of lymphedema in case of breast reconstructed by LD myocutaneous pedicled flap, for example lymphoscintigraphy and so on.

        Effect of Age and Severity of Facial Palsy on Taste Thresholds in Bell’s Palsy Patients

        Background and Objectives: To investigate whether taste thresholds, as determined by electrogustometry (EGM) and chemical taste tests, differ by age and the severity of facial palsy in patients with Bell’s palsy. Subjects and Methods: This study included 29 patients diagnosed with Bell’s palsy between January 2014 and May 2015 in our hospital. Patients were assorted into age groups and by severity of facial palsy, as determined by House- Brackmann Scale, and their taste thresholds were assessed by EGM and chemical taste tests. Results: EGM showed that taste thresholds at four locations on the tongue and one location on the central soft palate, 1 cm from the palatine uvula, were significantly higher in Bell’s palsy patients than in controls (p<0.05). In contrast, chemical taste tests showed no significant differences in taste thresholds between the two groups (p>0.05). The severity of facial palsy did not affect taste thresholds, as determined by both EGM and chemical taste tests (p>0.05). The overall mean electrical taste thresholds on EGM were higher in younger Bell’s palsy patients than in healthy subjects, with the difference at the back-right area of the tongue differing significantly (p<0.05). In older individuals, however, no significant differences in taste thresholds were observed between Bell’s palsy patients and healthy subjects (p>0.05). Conclusions: Electrical taste thresholds were higher in Bell’s palsy patients than in controls. These differences were observed in younger, but not in older, individuals

        Preparation and Characterization of Water-soluble Microcapsule for Sustained Drug Release Using Eudragit RS 100

        In this study, ondansetron was encapsulated using hydroxypropyl methyl cellulose (HPMC) mixed with a water-insoluble polymeric barrier using EuRS RS 100 (EuRS), which was expected to be effective in preventing premature drug release. This microcapsule containing ondansetron was prepared using oil-in-water (O/W) solvent evaporation method and confirmed by Fourier transform infrared spectroscopy (FTIR) and X-ray diffraction (XRD). The morphology and particle size of the microcapsules were observed by scanning electron microscopy (SEM). In addition, the drug release behavior of the microcapsules with different EuRS contents were determined by UV/Vis spectroscopy. From the results, the microcapsules were successfully fabricated in all cases, 28-49 μm in size. The drug release rate was decreased with increasing EuRS ratio, which was likely due to the increasing content of quaternary ammonium groups on EuRS.

        동맥혈 가스 검사 방법에 있어 헤파린 수동 코팅 방법과 헤파린 코팅 제품 사용 방법에 따른 전해질 및 혈색소 측정의 정확성 비교

        Purpose: The results of arterial blood gas analysis using conventional liquid sodium heparin syringes are inaccurate due to the dilution effect, chelation of heparin and the electrolyte, and interference of the heparin electrolyte measurement. This study compared the accuracy of using heparin with a liquid sodium heparin syringe (LHs) and balanced lithium/zinc heparin syringe (BHs). Methods: This study evaluated 6,778 cases who underwent an arterial blood gas test, serum electrolytes test, and complete blood count test among patients aged 18 years or older who visited the emergency room from November 1, 2016 to March 3, 2017. Finally, there were 2,383 cases using LHs and 2,584 cases using BHs. The results were compared between the groups using the LHs and BHs for sodium, potassium, and hemoglobin, and the agreement was compared using the Bland-Altman plot. Results: Sodium difference value was the 5.714±5.696 mmol/L in the LHs group, -1.549±3.339 mmol/L in the BHs group. The potassium difference value was -0.650±0.494 mmol/L (LHs group) and -0.257±0.367 mmol/L (BHs group). The hemoglobin difference values were -0.556±1.116 g/dL (LHs group) and -0.170±1.062 g/dL (BHs group). The results showed that the BHs group was improved compared to the LHs group (p<0.001). Conclusion: The results of arterial blood gas analysis of sodium, potassium, and hemoglobin were more accurate for the BHs group than the LHs group based on the serum electrolytes and complete blood counts.

        Penetration of the Descending Colon by a Migrating Intrauterine Contraceptive Device

        Foreign bodies in the gastrointestinal tract often cause serious complications, such as perforation, obstruction, abscess formation, or hemorrhage. This is a case in which a patient visited our hospital and complained of a vague lower abdominal pain that had been present for three months. She had an intrauterine device (IUD) inserted five years earlier. The abdominal X-ray, computed tomography and colonoscopy revealed that the IUD had penetrated into the descending colon. We tried to remove the IUD by colonoscopy but failed due to pain, so we removed the IUD surgically. Thus, we report a case in which a previously inserted IUD had penetrated into the descending colon and was surgically removed. We also present a brief review of the literature.

        비스포스포네이트 장기 복용 후에 생긴 재발성 하악 골수염에서 보이는 하버시안계 결핍 골형성

        A case of chronic osteomyelitis caused by prolonged intake of bisphosphonate showed multiple recurrences involving extensive area of mandibular body. After saucerization the removed bony fragments were decalcified, microsected in 4 ㎛ thickness, and stained with hematoxylin and eosin, Masson trichrome, von Gieson, and periodic acid Schiff reaction. The inflammatory lesion contained fragile osteophytes easily propagated into sequestra. Histologically, this osteomyelitis was relatively less suppurative but almost granulomatous, highly infiltrated with small round cells and macrophages. The osteophytes were frequently deposited on the old lamellate bone, but their ossification was extremely immature and frequently filled with sclerosed collagen bundles positive for von Gieson stain. In the polarizing microscope observation under Masson trichrome stain the newly deposited osteophytes were lack of birefringence image of Haversian system contrast to the old bone nearby. Therefore, we presume that the prolonged intake of bisphosphonate may induce the immature osteophytes lack of Harversian system, which are partly filled with sclerotic collagen bundles, and the immature bone is easily undergone extensive degeneration and necrosis, resulted in the inflammatory foci for multiple recurrent osteomyelitis.

        Citation Laws and Quasi-Impact Factor on Innovation Studies in Korea

        Existing bibliometric laws have been established on the basis of well defined science journals with a long history. However, the history of technology innovation research in Korea is young and the scope of the research is diverse compared with other fields. The main purpose of this research can be summarized as follows : Can the traditional bibliometric laws be used to explain the young and diverse data derived from technology innovation studies in Korea. Second, we want to compare the explain ability of the power law, compared with the traditional laws in the field. Third, we propose a quasi index related to the well-known impact factor to measure the contribution of a journal or a group of journals to the development of innovation research in Korea. We confirmed Lotka's and Bradford's laws which are used to measure the productivity of researchers, but we could not support the validity of Price’s Square Root law as Nicholls (1998) could not. On the citations to journals, Garfield’s laws is not observed. However, the power law fits well the citations to author, journal, article, and book. The estimated parameters between 1.6 and 3.5 are similar to the values in the range of 1.5 and 3 in previous studies. Finally the quasi index shows that the influence of international leading journals on innovation research in Korea is weaker than on innovation studies in the world.
