The authors have reviewed 19 patients of brachial plexus injury who treated by operative methods at Department of Orthopedic Surgery, Korea University Hospital during the period from January 1989 to February 1994. All of these patients were followed up more than one year and following results were obtained. 1. The whole arm type injury was most common(7 of 19 patient) and supraclavicular lesion(15 of 19 patient) was more dominant than infraclavicular lesion(4 of 19 patients). 2. The neurorrhaphy, nurolysis, nerve grafting, and neurotization were performed for the primary neural surgery and secondary reconstructive procedure consist of musculotendinous transfer and free muscle transfer with neurotization. 3. The followed up period was from one year to four years and six months, average being two years and five months. 4. We have obtained satisfactory results in 12 patients among 19 patients.
골육종의 도약전이 진단에서 골스캔의 보완검사로서의 FDG-PET/CT: 증례 보고

골육종에서 드물게 발생하는 도약전이 병소는 불량한 예후 인자로서 중요한 연관성이 있기 때문에 골육종 환자의 광범위 절제술 시에 절제범위에 포함되어야 한다. 따라서 수술 전 검사에서 골육종의 골수 내 침범에 대한 정밀한 평가가 필수적이며, 골스캔 검사는 도약전이의 발견에 있어서는 높은 위음성률을 보일수 있으며 자기공명영상 검사는 종양이 위치한 골 전체를 촬영하지 않는 경우 임상적인 증거가 없으면 간과하기가 쉽다는 단점이 있다. 이에 저자들은 주 종양 부위만을 포함한 국소 자기공명영상 촬영과 골스캔에서 발견되지 않았으나 FDG-PET/CT에서 도약 전이가 진단된 대퇴 원위부의 골육종 증례에 대해 문헌 고찰과 함께 보고하고자 한다. Skip lesion is not uncommon feature in osteosarcoma and considered to be importantly associated with poor prognosis factor, and thus, should be excised with the main mass. The accurate pre-operative evaluation of the intramedullary extent of osteosarcoma is essential, because it determines the level of bone resection. Among the reliable detection methods, bone scan has a drawback of high rate of false negative results and regional MRI has a difficulty to cover the whole involved lesions without clinical suspicion. The authors report a case of osteosarcoma of the distal femur with a proximal skip lesion that was not detected by either regional MR imaging or by bone scan, but which was visualized by FDG-PET/CT.

During forming of large automotive body panel, die components deflect due to the forming loads transmitted through the contact between various press and die components. Because die deflection results in forming defects and non-uniform material flow, time consuming corrective action to maintain uniform gap between dies, so called die spotting, should be done before the completion of dies. In this study, to predict the die deflection accurately, a structural analysis procedure considering the deformation of press-die assembly and the thickness variation of blank simultaneously is presented. The analysis procedure is applied to the optimization of the position and height of balance block for simplified automotive model die. The optimization results show much improved contact state between the blank and dies.
Flexible drill bit를 이용한 척추경 고정시 안정성에 대한 내시경적 확인

Among the numerous spinal instrumentation devices currently available,transpedicular screw fixa- tion has become one of the most popular methods of providing segmental stability. To avoid damage to surrounding dura, nerve roots, or vascular structures, proper placement of the screw is critical and it is influenced by anatomic consideration, such as pedicle size and location, spinal canal shape,ver- Having made screw holes using flexible drill bit, the authors confirmed that they were safely fixed within the pedicle using intraosseous endoscopy for 124 pedicle screws fixed patients from May. 1991 to July. 1994 at Korea University Anam Hospital. In result, every pedicle screws were located within the pedicle and no complications were observed. Therefore, pedicle screw fixation using flexible drill bit and direct visual confirmation of screw holes with intraosseous endoscopy are an efficient method to prevent the complications in pedicle screw fixation.
