        감두탕(甘豆湯)의 농약중독 예방효과에 대한 임상적 연구 (I) - 살충제(유기인제)를 중심으로 -

        Objectives: We have accomplished a clinical research about counteracting the pesticides intoxication using oriental medical method and treatment. And we observed the preventive effect of Gamdu-tang(甘豆湯) to intoxication of Organophosphorus Insecticides. Material and Methods: The subjects were 8 volunteers who were healthy and not taking medications. we observed change of cholinesterase(ChE) activity in serum of them after exposed to pesticides Results and Conclusions : In this study of the preventive effects of Gamdu-tang(甘豆湯) on the intoxication of Organophosphate Insecticides, we evaluated that the decreasing value of ChE activity after spraying insecticides of Gamdu-tang medication group was smaller than non-medication group, but it was less significant(P>0.05 P<0.1). We had a difficult to gather participant in this study, so it's necessary for us to get together more larger group in the next study.

        주사전자현미경(SEM)을 이용한 일회용침 끝의 미세 형태에 대한 관찰(2)

        Objective: There have not been so many study about the quality of acupuncture needle tip. In order to ensure safe acupuncture treatment, the quality of needle tip is essential. Therefore we investigated the current condition of the tip of the acupuncture needles in Korea. Methods: We have selected the needles that made by 8 companies in Korea, and selected 50 pieces from 1000 pieces each company by randomized methods. and observed the tip of each needle using a scanning electron microscope at ${\times}800$ magnification. Results & Discussion: We found that needles had several defects such as scratch marks on the surface, metallic scuff, lumps and irregularities of the needle tips, stubbed or malformed tips, tips of point off-center, peeled off coated tips. There was much difference on the quality of needle among the manufacturer, and some needles seem to need to do- thorough quality control. It is necessary to intensify quality management and a concentrated control on manufacturing process of acupuncture needles.

        침의 한국산업규격(KS) 도입 필요성에 대한 고찰

        Quality assurance with high safety profile is one of the most critical issues to medical device manufacturing. In general, this issue was not paid proper attention with respect to acupuncture needle and its related devices. Acupuncture needles are manufactured through various standards ranging from purely hand-made, partially hand-made and partially machine-made, to fully machine-made mainly in China, Korea, and Japan. There is a large difference in quality between needles from different manufacturers. In order to provide a standard manufacturing guideline, it seems necessary to make a Korean Industrial standard (KS). The authors review this topic from an historical view point, investigate the current situation of the standardization of acupuncture needles in other developed countries, and inspect the general procedure to establish a KS in Korea in order to suggest a KS for acupuncture needles.

        급성기 뇌졸중 환자에 대한 임상적 고찰

        Objective : We studied only patients at the acute stage but existing studies on stroke didn't almost classify the stage. We examined 243 patients admitted to the Woo Suk university oriental medical hospital from January 1998 to December 1999 for 2 years. Methods : We analyzed patients into sex, Sasang constitution, the incidence and lesion according to the stroke types, post and family history, onset time, period to admission and physical treatment from onset, and symptom, blood pressure, cholesterol level and herb medicine at entry. Results : Our study was similar to existing studies in the distribution of sex, age and the lesion of stroke, post and family history, and symptoms at entry. But it differed in the constitution, incidence of cerebral infarction and intracranial hemorrhage, attack time, period to admission and physical treatment from onset day, and the symptoms, blood pressure, cholesterol level and herb medication. In comparison between infarction and hemorrhage patients, there was some difference in the distribution of onset time, but wasn't in the cholesterol level. Conclusions : Our study on the acute stage of stroke was similar to existing studies in the general characteristics. But it showed some differences in the herb medicine at entry. In particular, there were much differences in the blood pressure at entry and the incidence of infarction and hemorrhage. We hoped that stroke patients would be synthetically studied in western and oriental medicine.

        He-Ne 레이저 혈관내 조사(ILIB)가 고지혈증에 미치는 영향에 대한 임상보고

        Purpose : Intravascular Laser Irradiation on Blood(ILlB) therapy has been usedto various circulatory diseases recently. In this study, we sought to find out the effects of ILIB on hyperlipidemia . Material and Methods : In this study of 163 patients with hyperlipidemia , we observed change of total cholesterol and triglyceride in serum after ILIB therapy. Results and Conclusion : In patients with hyperlipidemia , total cholesterol mean values decreased from $240.04{\pm}56.23$mg/dL to $214.90{\pm}45.84$mg/dL and triglyceride mean values decreased from $304.98{\pm}134.37$mg/dL to $244.05{\pm}120.46$mg/dL, after ILIB thetapy. We evaluated that ILIB therapy reduced total cholesterol and triglyceride (p<0.001) in patients with hyperlipidemia and IUB therapy is more effective to patients with hyperlipidemia than normal(p<0.001).

        Objectives : The purpose is to discuss the clinical applications of Orungsan(Goreisan: 五苓散) as an alternative management for increased intracranial pressure in the field of neurosurgery in Japan. Methods and Results : Attention has focused on Kampo medicine(traditional Japanese medicine) for some cerebral disease including chronic subdural hematoma(CSDH) and cerebral infarction in Japan. Orungsan and one of its classes, Sirungtang(Saireto: 柴苓湯) are well known their effects on brain edema. After some studies of Orungsan has the anti-edemic effects by the inhibition of aquaporin, this herbal medicine has been used widely in the neurosurgery field in Japan. It is high time to think about where we are and we go ahead for the progress and the integration in medicine. We have reviewed the studies using Orungsan or Sirungtang, that was reported at the 20th annual meeting of 'the Japan society for Kampo medicine and neurological surgery' was held on November 5, 2011 in Tokyo. Fifteen studies related with Orungsan or Sirungtang were reported among all 32 studies at the meeting. Orungsan in ten, and Sirungtang in five among 14 studies contained specific clinical case. In the aspects of disease, thirteen papers were related with SDH, including CSDH(11), SSDH(1), aneurism clipping for SDH prevention(1), and one was acute cerebral infarction and one was multiple metastatic brain tumor. In the report style, case control study was 7(mostly retrospective), and the case report was 8. Conclusions : Orungsan may be plausible to be an alternative method to reduce brain edema after SDH and other brain injury in the field of neurosurgery.

        오령산 단일 투여를 통한 일차성 고혈압 치험례

        Objectives : To report a case of the treatment of primary hypertension by Oryung-san (五?散: Wulingsan in Chinese, Goreisan in Japanese) monotherapy. Methods : The blood pressure (BP) fluctuation was checked of a woman who had sudden BP elevation without a history of hypertension treatment. There were no specific history of disease and results for the laboratory examination and image diagnosis including MRI. The woman was diagnosed with primary hypertension and she was given supple of Oryung-san extract (Hanpoong Pharm Co.) 3 g, three times a day for three months. The BP has been checked with digital sphygmomanometer (HEM-7111, Omron Japan) in brachial artery at home. Results and Conclusions : The patient had had stable BP since three weeks after Oryung-san treatment was initiated. There were no subjective symptoms, then Oryung-san medication also had quit after three months treatments. This shows some possibility to control hypertension using Oryung-san, which resembles thiazide that acts on the distal convoluted tubule and inhibit sodium-chloride reabsorption. For further evaluation of the effectiveness, well-designed randomized controlled trials should be undertaken.

        구법(灸法)과 intense pulsed light(IPL)의 특성 비교 및 광선구(光線灸)의 한방임상(韓方臨床) 활용(活用)

        Objectives : The purpose of this study is to compare the principles between moxibustion and the 'Intense pulsed light (IPL)' and to investigate the clinical application of the photo-moxa for the Korean medicine fields. Methods : The characteristics of moxibustion, IPL, parabolic reflector moxa and the dermatology surgical use of moxibustion were reviewed in the literature. Results : IPL is one type of light treatments that is employed by radiating the short-pulse wave, which is transformed from the light of high intensity. There has been used parabolic reflector in Asia for more than 2,000 years, and this hand-held device used for moxibustion in traditional medicine. Moxibustion is one of the treatment tools in Korean medicine using the heat energy. The fluence or energy density of IPL in clinical circumstances is similar to parabolic reflector moxa of moxibustion. IPL and parabolic reflector moxa shared same treatment principle employing the heat energy. Therefore IPL could apply for many indications in Korean medicine as a moxibustion tool and acupuncture. Conclusions: IPL may be plausible to be a fascinate method in phototherapy of Korean medicine.

        중증급성호흡기증후군(SARS)의 한약 치료에 관련된 WHO 임상논문 보고에 대한 개관

        Objective: In the winter of 2002, severe acute respiratory syndrome(SARS) began to spread throughout the world. More than 5,000 cases were reported in China, including over 1,700 cases in Hong Kong Special Administrative Region(Hong Kong SAR). The total number of cases reported from Canada and Singapore was more than 200. The total number of SARS cases world-wide reached 8,437 with incidences in 29 counties. Mortality from SARS is estimated at $10{\sim}12%$. When the SARS outbreak occurred in China, the State Administration of Traditional Chinese Medicine of China immediately initiated clinical research projects on the use of integrated herbal medicine and Western medicine for treating SARS. and, in Hong Kong SAR, research on the use of herbal medicine for the prevention and treatment of SARS. Reports were released during convalescence. The objective of this study is to overview twelve clinical SARS reports of WHO on the treatment of SARS with herbal medicine and evaluate the efficacy and safety of treatment of SARS with herbal medicine, and further to share experiences and knowledge of the treatment of SARS. Methods: Twelve clinical reports about SARS from the WHO were selected, overviewed and evaluated for efficacy and safety of treatments of SARS. Results and Conclusion: Twelve clinical reports about SARS showed that the integrated treatment may have advantages, and the advantages are reflected in the following findings: Firstly, herbal medicine is not targeted only at a specific etiology or a certain pathological link, but also at the pathological status of the patients at that particular time. Therefore, comprehensive readjustment was made through various angles, targets and channels to restore the balance of the body. Secondly, there are advantages in the differentiation of the disease and the treatment. Based on the various symptoms, herbal medicine enables the physician to adopt the most suitable principle, provide individual treatment, and to administer medicine in accordance with the actual process and nature of the illness. Thirdly, there are advantages in the results of the treatment; herbal medicine can relieve symptoms, promote absorption of lung inflammation, improve the degree of blood oxygen saturation, regulate immunological functions, reduce the required dosage of glucocorticoid and other Western medicines, and reduce case fatality rate, in addition to lowering the cost of treatment.

        중증급성호흡기증후군(SARS) 치료에 사용된 한약 처방에 대한 고찰 - WHO 보고서(2004)에 사용된 처방을 중심으로

        Background : Early in November 2002, severe acute respiratory syndrome (SARS) began to spread throughout thirty-two countries around the world. A total of 5,327 cases were reported in China, including 1,755 cases in Hong Kong and 655 in Taiwan. The total number of cases reported from Canada and Singapore was more than 200. The total number of SARS cases world-wide reached 8,437 with incidences. Mortality from SARS is estimated at around 11%. Under the guidance of the World Health Organization (WHO), clinical studies on the use of integrated herbal medicine and western medicine for treating SARS were carried out in China and Hong Kong. The official report suggested that integrated treatment was more effective than western medicine alone in clinical symptoms, lung inflammation, blood oxygen saturation, immunological functions and others. Objective : To overview the treatment principle and the prescriptions according to the regimens and the stages for SARS of the reports, and further to broaden our knowledge of treatment of acute infectious diseases using natural herbal medicine. Methods : We reviewed nine of WHO SARS reports that comprehensively described the principles and methods of treatment and summarized them into eight treatment methods. We analyzed the herbal formulae on the basis of their treatment principles, evaluated them in accordance with warm disease study, listed frequently used herbs, and assessed patent prescriptions and herbal injections that were mentioned in the reports. Results and Conclusion : The reports divided the course of SARS into 3 to 6 stages such as high fever stage, fastigium stage, and convalescent stage. Frequently used herbs were Radix Scutellariae, Radix Paeoniae, Radix Astragali, Semen Armeniacae, Radix Pseudostellariae, Radix Ophiopogonis, and Fructus Schizandrae. Herbal decoctions were the primary method of treatment, while extracts or injections were secondary. Prescriptions were mainly based on the warm disease study. SARS is a viral disease caused by corona virus, and herbal medicine is proven to be effective against it. We believe that evidences and experiences from SARS cases can be a good reference to further researches on acute infectious diseases.
