        병원 유형 및 지역에 따른 임상영양서비스에 대한 간호사의 인식

        The purpose of this study was to evaluate nurses` perception of clinical nutrition services. A cross-sectional survey design was performed. The research was accomplished by using questionnaires developed for this study and administered from September 12 to December 31, 2013 to 343 nurses at 43 tertiary hospitals and 20general hospitals. The percentage of nurses who recognized clinical nutrition certificate as issued from nation was 27.8%. The mean scores of perceived usefulness on clinical nutrition services was 4.23/5.00, whereas that of perceived implementation was 3.76/5.00. The mean scores of necessity of disease-specialized clinical dietitian at capital hospitals were significantly higher for obesity (P<0.01), cancer (P<0.05), and infant & childhood disease (P<0.01) than at local hospitals. The rates of nurses` experience in group education on cancerat capital hospitals (21.7%) was significantly higher than that at local hospitals (10.3%) (P<0.05). The means cores of perceived importance of clinical nutrition services were 4.46/5.00 for ‘group nutrition education’, 4.46/5.00 for ‘individual consultation’, and 4.40/5.00 for ‘nutrition management for enteral nutrition (EN) patients’. The most common reason why clinical nutrition services are important was ‘improving mal nutritional status’. To activate clinical nutrition services especially at local hospitals, clinical dietitians should give systematic assistance to patients and also institutional supports are needed.

        For developing plant anti-inflammatory agents, extracts from Lonicera japonica were obtained and evaluated for their anti-inflammatory and analgesic activities using acute/chronic inflammatory models and writhing tests. It was found that the extracts with a modified extraction method showed higher anti-inflammatory and analgesic activities than those of the extracts based on the ancient literatures.

        입원환자의 영양불량위험 검색도구의 개발 및 평가

        이 연구는 다양한 병명을 가진 입원환자의 영양불량위험의 진단을 간단하고 신속하게 실시할 수 있고 재원기간 내의 임상결과를 유의하게 예측하며 NRS 2002와 비교하여 평가하였을 때 일정 수준 이상의 타당도를 가지는 영양불량위험 검색도구를 (Nutritional Risk Screening Tool, NRST) 개발하고자 하였다. NRST의 개발 및 평가는 서울 소재 일개 종합병원에 1년여의 기간 동안 입원한 성인 환자를 대상으로 수집된 자료를 이용하여 NRST 구성요소의 선정, NRST scoring scheme의 탐색, NRST 평가의 기준점 설정, NRST의 임상결과 예측력 재확인 등의 총 4단계의 과정을 통하여 실시되었다. 아래에 본 연구의 결과 및 결론을 요약하였다. 1) 재원일수, 사망여부, 합병증 유무, 질병예후를 각각 종속변수로 하는 stepwise multiple regression을 시행한 결과 나이와 혈청 내 albumin, TLC, Hct 농도가 NRST의 구성요소로 선정되었다. 2) 선정된 각 NRST 구성요소의 수준에 따른 NRS 2002의 OR에 기준하여 NRST의 scoring scheme을 아래와 같이 설정하였다. NRST = Albumin × 1 + Age × 2.5 + Hct × 1.5 + TLC × 2* *Coding: Albumin < 3.5:1, ? 3.5:0. Age > 65:1, ? 65:0. Hct < 37:1, ? 37:0. TLC < 1800:1, ? 1800:0. 3) NRST의 점수변화에 따른 NRS 2000에 대한 민감도와 특이도를 바탕으로 NRST에 의한 영양불량 위험의 진단 기준점수를 3.5 이상으로 정하였다. 4) NRST 결과에 따른 재원일수, 합병증여부, 질병 예후, 사망여부 등의 차이를 검토한 결과 개발된 NRST가 만족할 만한 수준의 임상결과 예측력을 가지는 것으로 나타났다. 5) NRST는 다양한 진단명을 가진 입원환자의 영양불량위험 여부를 향후의 임상결과와 관련하여 신속하게 진단하는데 유용하리라 사료된다. Malnutrition of hospitalized patients can adversely affect clinical outcomes and cost. Several nutritional screening tools have been developed to identify patients with malnutrition risk. However, many of those possess practical pitfalls of requiring much time and labor to administer and may not be highly applicable to a Korean population. This study sought to develop and evaluate a Nutrition Risk Screening Tool (NRST) which is simple and quick to administer and widely applicable to Korean hospitalized patients with various diseases. The study was also designed to generate a screening tool predictable of various clinical outcomes and to validate it against the Nutritional Risk Screening 2002 (NRS 2002). Electronic medical records of 424 patients hospitalized at a general hospital in Seoul during a 14-month period were abstracted for anthropometric, medical, biochemical, and clinical outcome variables. The study employed a 4-step process consisting of selecting NRST components, searching a scoring scheme, validating against a reference tool, and confirming clinical outcome predictability. NRST components were selected by stepwise multiple regression analysis of each clinical outcome (i.e., hospitalization period, complication, disease progress, and death) on several readily available patient characteristics. Age and serum levels of albumin, hematocrit (Hct), and total lymphocyte count (TLC) remained in the last model for any of 4 dependent variables were decided as NRST components. Odds ratios of malnutrition risk based on NRS 2002 according to levels of the selected components were utilized to frame a scoring scheme of NRST. A NRST score higher than 3.5 was set as a cut-off score for malnutrition risk based on sensitivity and specificity levels against NRS 2002. Lastly differences in clinical outcomes by patients’ NRST results were examined. The results showed that the NRST can significantly predict the in-hospital clinical outcomes. It is concluded that the NRST can be useful to simply and quickly screen patients at high-nutritional risk in relation to prospective clinical outcomes.

        Neural Networks을 이용한 Reactive Ion Etching 공정의 실시간 오류 검출에 관한 연구

        본 논문은 정수장에서 사용하는 응집제의 종류를 결정하기 위한 시스템 개발에 관한 내용이다. 정수장은 여러 단위 처리장으로 구성되며, 불순물을 제거하기 위하여 혼화지에서 응집제를 주입하여 침전을 시킨다. 현재까지 응집제 결정을 위해 Jar-test를 이용하는데, 이 방법은 사람의 주관적인 판단에 의존하므로 실험 오차가 발생할 수 있다. 특히 정수장의 자동화를 위한 시스템 개발에서 가장 큰 걸림돌로 작용하고 있다. 본 논문은 이러한 문제점을 해결하기 위하여 로드맵에 기초한 데이터마이닝 기법을 이용하여 응집제를 선택할 수 있는 제어기를 개발하였다. 제어 규칙은 클러스터링 기법으로 도출하였는데, 군집의 초기 값과 개수는 통계적 지수 값을 사용하여 결정하였다. In coagulant control of water treatment plants, rule extraction, one of datamining categories, was performed for coagulant control of a water treatment plant. Clustering methods were applied to extract control rules from data. These control rules can be used for fully automation of water treatment plants instead of operator's knowledge for plant control. To perform fuzzy clustering, there are some coefficients to be determined and these kinds of studies have been performed over decades such as clustering indices. In this study, statistical indices were taken to calculate the number of clusters. Simultaneously, seed points were found out based on hierarchical clustering. These statistical approaches give information about features of clusters, so it can reduce computing cost and increase accuracy of clustering. The proposed algorithm can play an important role in datamining and knowledge discovery.

        암 치료 소외계층을 위한 암 의료이용 체계 구축의 모색: 환자 네비게이션(Patient Navigation) 프로그램 중심으로

        전 세계적으로 암 발생률과 사망률, 암관리비용이 증가하고 있다. 우리나라 또한 사회경제적 수준에 따라 암 치명률과 사망률의 격차를 보일 뿐 아니라, 오히려 사회경제 적 취약계층 일수록 암 치료를 위한 의료이용에서 소외되는 건강불평등 양상이 나타나 기 시작했다. 따라서 효과적이고 효율적인 의료재원 활용의 필요성이 제기되고 있다. 외국의 경우 우리보다 앞서 암 치료 소외계층이 의료 서비스 이용 과정에서 겪게 되는 물리적 ? 경제적 장애요소에 적절히 개입하여 의료이용 접근성을 높이기 위한 `환자 네 비게이션 프로그램`이 개발 ? 적용되고 있으며 지속적으로 여러 연구를 통해 프로그램의 효과가 검증되고 있다. 미국 및 캐나다에서 진행된 200개 이상의 환자 네비게이션 프로그램을 통해 사회경제적 취약계층이 암 의료이용 시 겪게 되는 경제적 어려움, 물리 적으로 낮은 이용 접근성, 문화적 차이와 언어적 제약, 분절된 서비스 전달체계, 인종에 따른 차별 등을 줄이고 암 의료이용에의 접근성을 높인 결과 암 생존율이 높아진 것으로 보고되었다. 이러한 검토에 따라 우리나라 암 치료 소외계층의 의료이용 형평성 제 고를 위한 환자 네비게이션 프로그램 적용 가능성 탐색을 위해서는 우선 암 환자 및 가족 등 프로그램 대상자의 요구도 조사가 이뤄져야 하며 운영 주체를 통한 원활한 프로그램의 기틀 마련, 그리고 네비게이터의 발굴이 이뤄져야 한다. 한국형 환자 네비게 이션 프로그램의 성공적인 적용을 위해서는 암 환자 의료이용 실태에 대한 체계적인 조 사와 네비게이터 양성을 위한 교육 프로그램이 과제로 제시되었다. The rate of cancer and the cost of treatment for cancer have increased exponentially worldwide. Given the social costs attributed to the rising rate of cancer-related mortality and death, medical inequality became an issue, and there is a recognized need for effective medical support for cancer patients. In several countries, the Patient Navigation Program, a systematic cancer prevention and intervention program designed to be accessible to Health care disparities in need of cancer treatment, has been reported to be effective. The focus of the study is to investigate the characteristics of Patient Navigation Program and to examine the feasibility of the program in South Korea. The navigation program train paraprofessionals to provide intervention services for cancer patients who experience barriers to medical services. Presently, there are over 200 Navigation Programs in the United States and Canada. As a result, the program has been found to be effective as increasing number of low-income patients have access to medical service which has reportedly decreased the rate of cancer-related mortality. In order to examine whether such program would be as effective in South Korea, our study examines the background and characteristics of Navigation Program use. There is a need for survey for data base for cancer patient service utility and develop navigator training program. The navigation program will decrease the rate of cancer related mortality and serve for medical inequality.

        전북지역 전통 된장의 품질특성 및 항산화 활성

        The purpose of this study was to investigate the quality characteristics and antioxidant activity by collecting four times on six samples of Jeonbuk regional soybean pastes (Doenjang) traditionally prepared for this study. The water content of samples decreased according to progress of the aging period and the water content of the fourth sample collected was 42.40~59.64% (p<0.05). The salinity of the fourth Doenjang samples was 11.80~18.60%. The amino-type nitrogen content was 122.67~540.33 mg% immediately after the preparation of Doenjang in the Jeonbuk region and the content of the fourth collection samples increased from 251.49 to 982.36 mg% (p<0.05). The isoflavone glycosides decreased but daidzein, genistein and glycitein, which are aglycones, increased during aging periods. The total polyphenol content of the fourth collected samples was 11.99~19.27 mg GAE/g (p<0.05). The DPPH radical scavenging activity, ABTS radical scavenging activity and FARP of the fourth Doenjang samples were 51.88~81.21%, 84.14~90.32%, and 1.08~3.11 mg Trolox/g, respectively. As a result of quality analysis on Doenjang, the superiority of traditional Doenjang has been proven by the increase of amino nitrogen content and antioxidant activity according to the aging period. However, factor analysis on quality differences of regional Doenjang should be conducted to ensure standardization and quality improvement.

        검은콩 첨가 비율에 따른 앙금의 품질 특성

        This study investigated the quality characteristics of black soybean sediments to diversify the availability of soybean. The cooking method selected for black soybean sediment preparation was a pressure cooking process without soaking, considering the isoflavone content. The black soybean sediments were prepared by the addition of 0, 10, 30, 50 and 100% (w/w) black soybean. When 0% to 100% black soybean was added to the black soybean sediments, the moisture and crude protein contents increased from 53.17% to 54.41% and from 12.07% to 21.68%, respectively. The total isoflavone content of the black soybean sediments was increased from 2.69 ㎍/g to 696.09 ㎍/g, respectively, by the addition of black soybean. The anthocyanin content of the black soybean sediments ranged from 279.29 ㎍/g to 387.8 ㎍/g by the addition of black soybean. The total polyphenol content and the total flavonoid content of the black soybean sediments range from 1.72 mg/g to 2.00 mg/g and 0.89 mg/g to 0.92 mg/g, respectively, by the addition of black soybean. Given the isoflavones, total polyphenol, and anthocyanin content of the black soybean sediments, it is appropriate that the ratio of added black soybeans is at least 50% after the pressure-cooking process, regardless of soaking.
