The thickness levels of fluids for patients with dysphagia are established according to the guidelines of the National Dysphagia Diet (NDD) and International Dysphagia Diet Standardization Initiative (IDDSI). The nectar- (level 2), honey- (level 3), and pudding-like (level 4) fluids in NDD are consistent with the mildly (level 2), moderately (level 3), and extremely (level 4) thick fluids in IDDSI, respectively. In this study, NDD levels were compared with IDDSI levels by estimating both the apparent viscosity (ηa,50) and the residual volume (mL) in the IDDSI syringe flow test of thickened drinks prepared with a commercial xanthan gum-based thickener at different concentrations (0.1∼3.1%, w/w). The concentration range of the thickener in thickened drinks at each IDDSI and NDD level increased in the following order: water> orange juice> milk. A small difference was noted in the range of thickener concentration in the same NDD and IDDSI levels for thickened milk when compared with other thickened drinks. These results indicate that the thickener concentration ranges of thickened drinks for the classification of NDD levels differed from those of IDDSI levels, and they appeared to be greatly influenced by the type of drink. These findings could provide useful information for practically indicating the reliable thickness levels by the IDDSI flow test in clinical practice.
CFD 시뮬레이션을 활용한 화학물질 누출사고 분석에 관한 연구

Objectives: Chemical accidents cause extensive human and environmental damage. Therefore, it is important to prepare measures to prevent their recurrence and minimize future damage through accident investigation. To this end, it is necessary to identify the accident occurrence process and analyze the extent of damage. In this study, the development process and damage range of actual chemical leakage accidents were analyzed using CFD. Methods: For application to actual chemical leakage accidents using FLACS codes specialized for chemical dispersion simulation among CFD codes, release rate calculation and 3D geometry were created, and scenarios for simulation were derived. Results: The development process of the accident and the dispersion behavior of materials were analyzed considering the influencing factors at the time of the accident. In addition, to confirm the validity of the results, we compared the results of the actual damage impact investigation and the simulation analysis results. As a result, both showed similar damage impact ranges. Conclusions: The FLACS code allows the detailed analysis of the simulated dispersion process and concentration of substances similar to real ones. Therefore, it is judged that the analysis method using CFD simulation can be usefully applied as a chemical accident investigation technique.
누크관 수종 제거 이후 후유증에 대한 한방치료 1례 : 증례보고

Objective: The purpose of this case study is to report the hydrocele of the canal of Nuck and the effectiveness of Korean medicine in post-surgery improvement of symptoms. Method: A patient who was diagnosed with hydrocele of the Nuck canal and had symptoms after Lichtenstein’s operation was treated using Korean medicine, including acupuncture, herbal medicine, and moxibustion, every day. The evaluation was performed using the Numeral Rating Scale (NRS), the 5-level EQ-5D version (EQ-5D-5L), the EQ visual analogue scale (EQ-VAS), and the World Health Organization Quality of Life-Brief (WHOQOL-BREF). Results: After treatment, changes were observed in NRS (from 8 to 2-3) and EQ-VAS (from 60 to 85), whereas there was no difference in EQ-5D-5L (both 6). There was an overall improvement in the domains of WHOQOL-BREF and subjective symptoms. Conclusion: This study suggests that Korean medicine may improve symptoms after surgery of hydrocele of the canal of Nuck and can be evaluated using an overall QoL questionnaire.
초등학교 전이과정에서 나타나는 아동의 행복감 변화유형: 취학 전 아동 및 부모 특성을 중심으로

The purpose of this study is to investigate the change patterns of children’s self-rated happiness during the transition to elementary school and to identify the characteristics of each pattern. To achieve these goals, 7th (7-year-old) and 8th(8-year-old) year data of the Panel Study on Korean Children (PSKC) were used for investigation. Results are as follows. First, the level of children’s overall happiness had significantly been lowered during the transition to elementary school. Second, a cluster analysis revealed four distinct patterns of change in children’s happiness: high-maintaining, high-to-low, low-to-high, and low-maintaining. Third, the aspects of an individual child’s school readiness, parents’ parenting behavior, parents’ subjective happiness, and children’s peer play behavior were significantly different among the subgroups. Specifically, school readiness was significantly higher in high-maintaining than in high-to-low; affective parenting was higher in high-maintaining than in low-maintaining or high-to-low. Children in groups of high-maintaining and low-to-high had higher levels of parental happiness in preschool period, compared to that of high-to-low and low-maintaining. High-maintaining children were higher in positive peer interaction but lower in play cessation. The results provide theoretical rationale of individualized intervention approach that would support children’s happy transition to elementary school. 본 연구는 초등학교 전이를 경험하는 아동의 행복감 변화 유형을 살펴보고, 각 유형 별 취학 전 시기의 아동 및 부모 특성을 알아보는 것을 목적으로 한다. 이를 위해 한국아동패널조사 7차년도(7세)와 8차년도(8세) 자료(N=1,480)를 사용하였다. 본 연구의 결과는 다음과 같다. 첫째, 초등학교 전이기를 거치며 아동의 행복감은 유의하게 낮아졌다. 둘째, 아동의 행복감 변화유형은 높음 유지형, 상승형, 감소형, 낮음 유지형으로 분류되었다. 셋째, 초등학교 전이기 아동의 행복감 변화 유형에 따라 학교준비도, 부모 양육행동 및 행복감, 또래놀이행동이 달랐다. 구체적으로 높음 유지형이 감소형에 비해 학교준비도가 높았으며, 낮음 유지형과 감소형에 비해 부모 양육행동이 온정적이었다. 또한 높음 유지형과 상승형의 부모 행복감이 높았으며, 높음 유지형이 감소형에 비해 긍정적 또래놀이행동이 높았다. 이 연구는 초등학교로의 행복한 전이를 위한 지원 및 개입의 이론적 근거를 제시한 점에서 의의가 있다.
Emodin induces apoptosis of concanavalin A-stimulated murine splenocytes

Emodin, one of the major compounds in the herb Reynoutria elliptica, is known to maintain immunosuppressive, anti-allergic, anti-cancer, and anti-inflammatory effects. In this study, we assessed the possibility of using emodin to induce apoptosis in stimulated immune cells in vitro. After treatment with emodin and concanavalin A (Con A), we observed DNA damage-induced apoptosis in splenocytes. Moreover, treatment with emodin and Con A increased the number of apoptotic splenocytes compared with untreated controls. Emodin also diminished the size of CD45R/B220+ cells, CD19+CD69+ cells, and cDC populations. These results indicate that emodin-induced apoptosis was involved in attenuating the immune activity promoted by DNA damage and in decreasing the number of CD45R/ B220+ B cells and CD19+CD69+ activating B cells. This demonstration of emodin inducing apoptosis of Con A-stimulated immune cells indicates its potential utility as a therapy for diseases caused by abnormally activated immune cells.

In this study, characteristics and immuno-modulatory effects of Weissella cibaria JW15 isolated from Kimchi, traditional Korean fermented food, were examined for investigation of the capacity of potentially probiotic strains. We measured acid, bile, and heat tolerance, adhesive properties to intestinal epithelial cells, and inhibitory activity against pathogens. JW15 could survive at pH 3.0 for 2 hr, but not at pH 2.0. JW15 also showed tolerance to 0.3% oxgall bile salt, and heat tolerance at 70°C and 80°C for 5 min, respectively. Adhesive ability to Caco-2 cells was similar to that of Lactobacillus rhamnosus GG (LGG), a well-known commercial probiotic. JW15 exhibited antimicrobial activities to pathogenic bacteria such as Escherichia coli, Listeria monocytogenes, Staphylococcus aureus, and Salmonella enteritidis. The immuno-modulatory effects of JW15 were compared with those of LGG, a well-known immune enhancer. For analysis, production of nitric oxide (NO), NF-κB (Nuclear factor κB), Interleukin-1β (IL-1β), and tumor necrosis factor-α (TNF-α) was measured. The concentration of NO induced by JW15 was higher than that by LGG at low concentration (1 × 107 cfu/mL). Low and high (5 × 107 CFU/mL) concentration of JW15 induced statistically higher production of NF-κB, IL-1β, and TNF-α than that produced by LGG, respectively. In conclusion, Weissella cibaria JW15 had ability as a probiotic strain, including acid, bile, and heat tolerance, adhesive properties to intestinal epithelial cells, and inhibitory activity against pathogens. In addition, JW15 showed better immuno-modulatory effects than LGG when NO, NF-κB, IL-1β, and TNF-α were measured. According to these results, the characteristics and immunomodulating activity of Weissella cibaria JW15 are suitable for consideration as a potential probiotic.
