표피성장인자의 창상치유에 대한 효과

Backgound : Epidermal growth factor(EGF), a potent stimuant of epithelialization, has been noted to increase the thickness of the epidermis, increase the epithelial cell proliferartion and keratinization, and accelerate wound contraction. Objective : Our purpose was to evaluate the efficacy of topic 1 application of recombinant human epidermal growth factor(rhEGF) in the healing of full-theckness excision and burn wound. Methods : Full-thickness excision and burn wound were male in the back of the male Wistar rat. Recombinant human epidermal growth factor was apolied twice a day and the size of the wound was measured with planimetry every other day for 21 days. The keratinocytes of the circumcised foreskir were cultured in different concentrations of recombinant human epidermal growth factor and proliferation of the keratinocytes was evaluated. Results : 1. Regardless of wound types or base types, wound heal in the experimental groups (rhEGF 10, 30, 50 g/g) was generally better than that in the control or vehicle group. 2. The duration of wound healing was decreased as followa in order, in full-thickness excision wound, rhEGF 50 g/g, group, rhEGF 30 g/g group, rhEGF 10 g/g group, vehicle group, and control group and in full-thickness burn wound, rhEGF 30 g/g group, rhEGF 50g/g group, rhEGF 10g/g group, vehicb group, and control group. 3. In the biopsy specimen taken from the wound at 9th and 13th day, neodermis, neovascularization, the thicknesa and maturation of the collagen bundes and reepithelialization were more increased in the experimental groups than in the control or vehicle group. 4. In vitro culture of epidermal cells showed similar proheration in the concentration of rhEGF higher than 10 ng/rnl. Conclusion . These findings suggest that topical application of ecombinant humanepidermal growth factor is effective in the healing of full-thickness exoision and burn wound.(Kor J Dermatol 1995; 33(1): 76-84)

Melamine - formaldehyde resin에 의한 직업성 알레르기성 접촉피부염

Melamine-formaldehyde resin is widely used in manufactur ing plastics, tableware, t,extile and paper finishing, laminates, electrical insulations, and adhesives throughout the world, Allergic contact dermatitis to this resin has rarely been reported worldwide and has not been reported in Korean literature. We report a case of occupational allergic contact dermatitis to melamine-formaldehyde resin in a 38-year-old man. He had been working for a month in a textile factory when pruritic multiple erythematous patches and placjues developed on the face, both forearms and dorsa of the hands. In the factory melamine-forrnldehyde resin was used for crease resistant finishing. A Patch test revealed a positive reaction to rnelamine-formaldehyde resin. (Kor J Dermatol 1997;35(2): 369-373)

Herpes-associated erythema multiforme(HAEM) is a common subtype of erythema multiforme (EM) which usually develops 1-3 weeks after the recurrent herpetic episodes. The role of HSV in the pathogenesis of EM has not been fully defined. Recent studies, however, suggest that the host immune response to HSV, especially cell-mediated immune mechanism, is critical to the development of HAEM and, based on these findings, a variety of immunomodulating agents have been tried for the treatment of recurrent EM. Thymopentin, an immunomodulator that influences the maturation of thymocyte and mature T cell function, has been used as a therapeutic agent for various autoimmune diseases and recurrent HSV infection. A 37-year-old man with HAEM, who had failed to respond to intermittent acyclovir therapy for about 5 years, was treated with a subcutaneous injection of thymopentin 50mg three times a week for 6 consecutive weeks. The EM lesions were completely cleared 5 weeks after starting the treatment, without any sigrificant side effects. We herein, report a case of HAEM who has been treated with a 6-week schedule of thymopentin therapy and has showed no recurrence for the 6 months of follow-up period. We report a case of multiple Bowen's disease associated with leprosy. A 68-year-old man was seen because of slightly pruritic multiple erythematous scaly patches and plaques on the trunk and the both extremities for 5 years. He has been treated for leprosy with many kinds of drugs consisting of the herb medications for over 40 years. He also showed decreased cell-mediated immunity in skin tests. It is considered that depressed cellular immunity may be an important predisposing factor in the development of mutiple Bowen's disease in this case, .although arsenics can not be completetly excluded. Male breast cancer is a rare disease, representing approximately 1% of breast cancer and less than 1.5% of malignant tumors occurring in men. We report a case of cuaneous metastasis of male breast cances. a 64-year-old male who presented with multiple nodules and erythematous plaque on the right anterior chest and enlargement of the right axillary lymplinodes. The histopathologic findings of the nodule showed atypical tumor cells with a somew eat gland-like pattern. The cells showed potive reactions in PAS, DPAS and alcian blue stains, and also positive for estrogen and roiesterone receptors. Biopsy specimens taken from the plaque showed cellular arrangement in show line, referred to as Indian filing, between thiclenecin collagen bundles. He was treated not, tamoxifen, 5-fluorouracil, adriamycin, cyclophosphamide and megestrol acetate. 5 months after nitiation of the treatment, the skin lesions were improved vvith a decreased number and size or dules and decreased intensity of erythema. 심상성 루프스에 동반된 스포로트리쿰증

Sporotrichosis is a chronic infection that follows accidental implantation of the fungus Sporothrix schenckii into the skin. The infection begins with skin trauma under conditions when conidia may be present. A 48-year-old woman presented with a 5-year history of an 8X 12 cm sized, centrally depressed, erythematous plaque on the left wrist. She had been treated with antifungal agents at an other hospital for about 6 months but there was no improvement. The histopathological findings showed an infiltration of mononuclear cells and tuberculoid structures on the whole dermis and subcutis. According to the histopathological findings and clinical manifestations, she was diagnosed as having lupus vulgaris. There was much improvement of the lesion after antituberculous therapy. However, 2 months later she developed asymptomatic multiple erythematous papules around the lesion of lupus vulgaris and along the lymphatic drainage of the left arm. The histopathological findings of the newly developed lesion showed a granulomatous reaction and an infiltrate of acute and chronic inflammatory cells. Moist and creamy colonies were changed into brown-to-black ones with a leathery appearance with age. This appearance and clinical manifestations confirmed the diagnosis of lymphocutaneous sporotrichosis. This clinical appearance reflects that the sporotrichosis developed on the ulcerative lesion or biopsy site of lupus vulgaris. (Korean J Dermatol 1998;36(1): 181-185)

Background and Purposes: Over the past three decades, numerous studies performed in Korea have reported that job stress is a determinant risk factor for chronic diseases and work disability. Every society has its own culture and occupational climate particular to their organizations, and hence experiences different occupational stress. An occupational stress measurement tool therefore needs to be developed to estimate it objectively. The purpose of this study is to develop and standardize the Korean Occupational Stress Scale (KOSS) which is considered to be unique and specific occupational stressors in Korean employees. Subjects and Methods: Data were obtained from the National Study for Development and Standardization of Occupational Stress (NSDSOS Project: 2002-2004). A total of 12,631 employees from a nationwide sample proportional to the Korean Standard Industrial Classification and the Korean Standard Occupational Classification were administered. The KOSS was developed for 2 years (2002-2004). In the first year, we collected 255 items from the most popular job stress measurement tools such as JCQ, ERI, NIOSH and OSI, and 44 items derived from the a qualitative study (depth interview). Forty-three items of KOSS, in the second year, were retained for use in the final version of the KOSS by using Delphi and factor analysis. Items were scored using conventional 1-2-3-4 Likert scores for the response categories. Results: We developed eight subscales by using factor analysis and validation process: physical environment (3 items), job demand (8 items), insufficient job control (5 items), interpersonal conflict (4 items), job insecurity (6 items), organizational system (7 items), lack of reward (6 items), and occupational climate (4 items). Together they explained 50.0% of total variance. Internal consistency alpha scores were ranged from 0.51 to 0.82. Twenty-four items of the short form of the KOSS (KOSS-SF) were also developed to estimate job stress in the work setting. Because the levels of the subscales of occupational stress were gender dependent, gender-specific standard norms for both the 43-item full version and the 24-item short form using a quartile for the subscales of KOSS were presented. Conclusion: The results of this study suggest that KOSS might be an appropriate measurement scale to estimate occupational stress of Korean employees. Further and more detailed study needs to be conducted to improve the validity of this scale.

Livedo Reticularis Idiopathica Associated with Mononeuropathy Multiplex Syndrome and Bilateral Ulnar - median Nerve Anastomosis

A 31-year-old woman had a livedo reticularis of reticulated, bluish discoloration on both arms and legs for 18 months with a burning pain in the right hand and numbness in both lower legs. The findings of the electromyography were consistent with mononeuropathy multiplex and bilateral ulnar-median nerve anastomosis. A biopsy specimen from the right sural nerve showed perivascular lymphocytic infiltration and occasional myelin digestion chambers, which were consistent with vasculitic neuropathy. We present a patient with livedo reticularis idiopathica associated with mononeuropathy multiplex syndrome who also has bilateral ulnar-median nerve anastomosis.
