BACKGROUND: Agricultural water is known to be one of the major routes in bacterial contamination of fresh vegetable. However, there is a lack of fundamental data on the microbial safety of agricultural water in Korea. METHODS AND RESULTS: We investigated the density of indicator bacteria in the surface water samples from 31 sites collected in April, July, and October 2018, while the groundwater samples were collected from 20 sites within Jeollabuk-do in April and July 2018. In surface water, the mean density of coliform, fecal coliform, and Escherichia coli was 2.7±0.55, 1.9±0.71, and 1.4±0.58 log CFU/100 mL, respectively, showing the highest bacterial density in July. For groundwater, the mean density of coliform, fecal coliform, and E. coli was 1.9±0.58, 1.4±0.37, and 1.0±0.33 log CFU/ 100mL, respectively, showing no significant difference between sampling time. The survival of E. coli O157:H7 were prolonged in water with higher organic matter contents such as total nitrogen (TN), and nitrate-nitrogen (NO3-N). The reduction rates of E. coli O157:H7 in the water showed greater in order of 25, 35, 5, and 15℃. CONCLUSION: These results can be utilized as fundamental data for prediction the microbiological contamination of agricultural water and the development of microbial prevention technology.

Gastrointestinal metastasis of primary lung carcinoma is very rare, although this is found in about 4.7∼14% of cases at autopsy. An 81-year-old man was admitted with masses in the lung, adrenal gland, bladder and colon on his CT scans. We suspected metastases of an unknown origin and we carried out EGD and colonoscopy to differentiate the primary origin of the metastases. The examination revealed a submucosal tumor-like mass with central erosion on the gastric antrum and colonic splenic flexure, and polypoid lesions with a central fissure on the second portion of the duodenum. All the endoscopic forcep biopsies showed poorly differentiated adenocarcinoma without evidence of foci of the preinvasive surface glandular lesions. Immunochemical analysis of the tumor cells showed positivity for thyroid transcription factor-1 and cytokeratin 7, and negativity for cytokeratin 20 and caudal-related homeobox 2. Therefore, we diagnosed this case as multiple gastrointestinal metastases of primary lung cancer. This is first case of gastric, duodenal and colonic metastases from adenocarcinoma of the lung in the medical literature. 원발성 폐암의 위장관 전이 빈도는 부검을 통한 연구에서 4.7∼14%이지만 실제 임상에서의 보고는 드물다. 81세 남자가 전산화 단층 촬영에서 폐, 부신, 방광, 대장에 종괴가 발견되어 내원하였다. 저자들은 원인 불명암을 의심하고 원발암의 감별을 위해 위 및 대장 내시경을 시행하였고 위와 대장에서 중심에 미란을 동반하는 점막하 종양 형태의 병변과 십이지장에서 중심에 균열을 동반한 용종 형태의 병변을 발견하였다. 조직 검사에서 표면 샘의 침범이 없는 저분화 선암종이었다. 면역 화학 검사에서 cytokeratin 7, thyroid transcription factor-1에 양성, cytokeratin 20, caudal-related homeobox 2에 음성을 보여 원발성 폐선암의 다발성 위장관 전이로 진단하였다. 저자들은 위장, 소장, 대장에 동시에 원격성 전이를 일으킨 폐선암을 경험하여 보고한다.
Stress Cardiomyopathy Associated with Diffuse Alveolar Hemorrhage after Colonoscopy

Stress cardiomyopathy (SCM) is usually precipitated by a physiologically or psychologically stressful event. Although it occurs only rarely, hypoxia- and hypercapnia-induced sympathetic activation may also cause SCM. We present the case of a 37-year-old woman affected with SCM after a routine colonoscopy. During the procedure, she aspirated residual polyethylene glycol from her stomach. Hypotension, resting dyspnea, and hemoptysis were subsequently observed. Laboratory findings revealed elevated cardiac enzymes, and a transthoracic echocardiogram revealed left ventricular (LV) global hypokinesia. She was ultimately diagnosed with diffuse alveolar hemorrhage-associated SCM. After successful treatment with a ventilator and corticosteroids, her LV systolic function and dimensions normalized and she was discharged without complications.

Spatholobus suberectus belonging to the family Leguminosae has been used in Oriental medicine for promoting blood circulation, removing blood stasis, tonifying the blood, relaxing tendons, stopping internal bleeding and eliminating dampness. This study investigates whether the water extracts of S. suberectus induce apoptotic cell death in Jurkat T-acute lymphoblastic leukemia (ALL) cells. Cell viability of Jurkat cells was inhibited in a concentration-dependent manner by S. suberectus, as measured by cell morphology. The capability of S. suberectus to induce apoptosis was associated with proteolytic cleavage of specific target protein such as poly (ADP-ribose) polymerase protein suggesting the possible involvement of caspases. The purpose of this study is also to investigate the effect of S. suberectus on cell cycle progression. G1 checkpoint related gene products (cyclin D1, cyclin dependent kinase 4. retinoblastoma, E2F1) were decreased in their protein levels in a dose-dependent manners after treatment of the extract. These results indicate that the increase of apoptotic cell death by S. suberectus may be due to the inhibition of cell cycle progression in wild type p53-lacking Jurkat cells.

Apoptosis is a morphologically and biochemically distinct form of cell death that occurs in many different cell types in a wide variety of organisms.Albizzia julibrissin belonging the family Leguminosae has been used for the treatment of contusion, sore throat, amnesia, and insomnia in oriental traditional medicine. This study investigates whether the water extract of A. julibrissin induce apoptotic cell death in Jurkat T-acute Iymphoblastic leukemia (ALL) cells. Jurkat cells were increased inhibitions of cell viability in a concentration-dependent manner by A. julibrissin. This herbal medicine also caused apoptosis as measured by cell morphology and DNA fragmentation. The capability of A. julibrissin to induce apoptosis was associated with proteolytic cleavage of specific target protein such as poly (ADP-ribose) polymerase (PARP) protein suggesting the possible involvement of caspases. Our result showed that Bcl-2 and Bax protein levels were not changed in all A. julibrissin-treated groups compared to control group. These results suggest that A. julibrissin-mediated apoptosis is independent with Bcl-2 related signaling pathway in this cells.
한방병원 간호사들의 한방간호 필수용어 활용도와 관련된 특성

Purpose: This study was done to examine the level of use of essential oriental nursing terms and related characteristics among nurses in oriental medicine (OM) hospitals. Method: A cross-sectional survey design with convenience sampling was employed. Questionnaires were used to collect data from 315 nurses in seven university-affiliated OM hospitals. The questionnaire included 174 oriental nursing terms about actions, tests, drugs, and theories. Multiple regression analysis was performed to determine factors associated with the level of use. Results: The response rate was 98.1% (n=309). Overall mean score for use of essential oriental nursing terms was 2.7(±0.7) on a five-point Likert scale. Significant factors associated with the level of use were workplace and knowledge of oriental nursing theory terms. Nurses who worked in nursing care wards and those who had a higher level of knowledge of oriental nursing theories had a higher level of use. Conclusions: These findings indicate that level of use of essential oriental nursing terms were below average. Educational programs to enhance knowledge of oriental nursing theories need to be implemented for nurses in OM hospitals, considering their workplace, in order to facilitate nursing data sharing and communication among healthcare providers.
폴리감마글루탐산을 함유하는 생분해성 나노섬유의 제조 및 생의학적 응용

One of the serious complications that have not been solved yet in modern medicine is a postoperative tissue adhesion of internal organs. Tissue adhesions are associated with numerous postoperative complications, including pain, functional obstruction and difficult re-operative surgery. In this study, poly($\gamma$-glutamic acid)($\gamma$-PGA) based nanofiber sheets were prepared by using electrospinning technique to evaluate the ability of the prevention of postoperative tissue adhesion. The anti-adhesion membranes were prepared from $\gamma$-PGA and poly(D,L-lactic-co-glycolic acid)(PLGA) with different compositions by electrospinning. Also non-steroidal anti-inflammatory drug (ibuprofen) was incorporated during fabrication of nanofibers. Various electrospun nanofibers were characterized by SEM, FTIR, water contact angle measurement, biodegradability test, in vitro drug release profile, cell culture test, and in vivo animal study using Sprague Dawley rat model. The average diameter of the nanofibers electrospun from the various biodegradable polymer solutions ranged from 300 nm to 900 nm, approximately. From in vivo animal study, it was observed that ibuprofen-incorporated $\gamma$-PGA nanofiber sheet (crosslinked) was significantly effective in preventing post-surgical tissue adhesion and wound healing, probably due to the appropriate hydrophilicity preventing shrinkage of the sheet. On the other hand, PLGA nanofibrous mat was dramatically contracted in vivo due to its high hydrophobicity and resulted in insufficient coverage of wound.
