        한방병원 간호사들의 한방간호 필수용어 활용도와 관련된 특성

        Purpose: This study was done to examine the level of use of essential oriental nursing terms and related characteristics among nurses in oriental medicine (OM) hospitals. Method: A cross-sectional survey design with convenience sampling was employed. Questionnaires were used to collect data from 315 nurses in seven university-affiliated OM hospitals. The questionnaire included 174 oriental nursing terms about actions, tests, drugs, and theories. Multiple regression analysis was performed to determine factors associated with the level of use. Results: The response rate was 98.1% (n=309). Overall mean score for use of essential oriental nursing terms was 2.7(±0.7) on a five-point Likert scale. Significant factors associated with the level of use were workplace and knowledge of oriental nursing theory terms. Nurses who worked in nursing care wards and those who had a higher level of knowledge of oriental nursing theories had a higher level of use. Conclusions: These findings indicate that level of use of essential oriental nursing terms were below average. Educational programs to enhance knowledge of oriental nursing theories need to be implemented for nurses in OM hospitals, considering their workplace, in order to facilitate nursing data sharing and communication among healthcare providers.

        응급실에서의 질 향상 기법 적용

        Background : Although a number of studies are related to QA improvement, there are few studies applied various method of QA tools. This study reviewed the availability of general quality assurance(QA) tools according to ten steps in performing quality improvement activities at emergency room of a tertiary teaching hospital which has more than 1,000 beds. Methods : All patients in emergency room from 15th Oct. 1997 to 5th Sep. 1998 were surveyed. The survey .data based on the patient's records of emergency room were evaluated according to length of stay, and we tried to identify problems with management of emergency room. To solve the problems, our team applied general QA tools(brainstorming, flow chart, nominal group technique, benchmarking, cause-and-effect diagram, run chart, control chart) to quality improvement activities and discussed the availability of the tools. Results : After the activities such as changes of staffing, the establishment of local area network and chest pain clinic, application of triage and so on, the percentage of patient who had stayed more than six hours was reduced from 56.0% to 46.8%. The mean number of patients per day in emergency room was increased from 49 to 62. But the reporting time for laboratory test was not changed after these activities. Conclusion : Each QA tool has unique benefit and limitation, but we can implement and evaluate the quality improvement activities more scientifically and systematically by applying these tools to practice according to QA ten steps.

        환자특성에 따른 의료이용에 대한 환자만족도 비교

        Purpose: The purpose of this study was to identify the relationship between patient characteristics and patient satisfaction. Methods: A cross-sectional questionnaire survey was conducted in an acute care hospital. The subjects were 317 patients discharged from general medical and surgical nursing care units during September, 2005. Patient satisfaction was measured using the short-form satisfaction scale of Hwang and Park(2001). Additional information about patient characteristics, including general demographics and health care utilization variables, was collected from the hospital information systems. Multiple regression analysis was performed to determine patient characteristics influencing patient satisfaction. Results: Patients were satisfied with hospital care with an average of 4.10 on a five-point Likert scale. Patient characteristics explained 13.5% of the variance of patient satisfaction. The significant factors influencing patient satisfaction were patients' age and perceived health status. There was no significant relationship between structural variables and patient satisfaction. Conclusion: This study showed that patients' characteristics were significant factors explaining patient satisfaction. Therefore, these characteristics should be adjusted in reporting patient satisfaction as an indicator for hospital-level or department-level rating.

        중소 병원 간호사들의 업무상 손상경험에 영향을 미치는 요인파악

        Purpose: This study was conducted to examine the factors associated with work related injuries of nurses in small and medium sized hospitals. Method: A cross-sectional survey was conducted with nurses in eight hospitals from October 2007 to January 2008. A questionnaire was designed to collect information on nurses' work related injuries, and individual and job related characteristics. The response rate was 65.1%. Data from 294 nurses were analyzed. Multiple logistic regression analysis was performed to determine factors associated with work related injuries. Result: Of the 294 nurses, 19.1% (n=56) responded as having at least one injury during their job performance. The logistic regression analysis revealed that the significant factors influencing work related injuries were job satisfaction, stress recognition, and hospital's location. Nurses with a higher job satisfaction were less likely to experience work related injuries (OR=0.58). Nurses with a higher stress recognition (OR=2.57) and those working at hospitals in metropolitan cities (OR=3.28) were more likely to experience work related injuries. Conclusions: The result of this study indicated that a substantial proportion of nurses in small and medium sized hospitals had experienced injuries related to nursing job. Interventions to prevent work related injuries among nurses should take into account the job satisfaction, stress recognition, and hospital characteristics.

        Performance of Early Warning Scoring Systems Regarding Adverse Events of Unanticipated Clinical Deterioration in Complementary and Alternative Medicine Hospitals

        Purpose: This study aims to examine the performance of early warning scoring systems regarding adverse events of unanticipated clinical deterioration in complementary and alternative medicine hospitals. Methods: A medical record review of 500 patients from 5-year patient data in two traditional Korean medicine hospitals was conducted. Unanticipated clinical deterioration events included unexpected in-hospital mortality, cardiac arrest, and unplanned transfers to acute-care conventional medicine hospitals. Scores of the Modified Early Warning Score (MEWS), National Early Warning Score (NEWS), and National Early Warning Score 2 (NEWS2) were calculated. Their performance was evaluated by calculating areas under the receiver-operating characteristic curve for the event occurrence. Multiple logistic regression analyses were performed to determine the factors associated with event occurrence. Results: The incidence of unanticipated clinical deterioration events was 1.1% (225/21101). The area under the curve of MEWS, NEWS, and NEWS2 was .68, .72, and .72 at 24 hours before the events, respectively. NEWS and NEWS2, with almost the same performance, were superior to MEWS (p ¼ .009). After adjusting for other variables, patients at low-medium risk (OR ¼ 3.28; 95% CI ¼ 1.02e10.55) and those at medium and high risk (OR ¼ 25.03; 95% CI ¼ 2.78e225.46) on NEWS2 scores were more likely to experience unanticipated clinical deterioration than those at low risk. Other factors associated with the event occurrence included frailty risk scores, clinical worry scores, primary medical diagnosis, prescribed medicine administration, acupuncture treatment, and clinical department. Conclusions: The three early warning scores demonstrated moderate-to-fair performance for clinical deterioration events. NEWS2 can be used for early identification of patients at high risk of deterioration in complementary and alternative medicine hospitals. Additionally, patient, care, and system factors need to be considered to improve patient safety.

        Patient Participation in Patient Safety and Its Relationships with Nurses' Patient-Centered Care Competency, Teamwork, and Safety Climate

        Purpose: This study's aim was to examine degrees of patient participation in patient safety activities in hospitals and to investigate their relationships with nurses' patient-centered care competency (PCC), teamwork, and safety climate. Methods: A cross-sectional study designwas used. Data were collected with 479 nurses from two general hospitals in Seoul, Korea, using a questionnaire designed to collect data on patient participation in patient safety activities, PCC, teamwork perceptions, and safety climate. The response rate was 74.1% (N = 355). Data were analyzed using descriptive statistics and multiple logistic regression analysis. Results: The mean score for patient participation was 2.76 ± 0.46 of 4.0. The mean scores for PCC, teamwork, and safety climate were 3.61 ± 0.46, 3.64 ± 0.41, and 3.35 ± 0.57 of 5.0, respectively. Nurses who experienced high patient participation in patient safety activities ( 3.0) had higher scores for PCC, teamwork, and safety climate. Multiple logistic regression analysis revealed that PCC (OR = 2.31, 95% CI = 1.14e4.70) and safety climate (OR = 2.51, 95% CI = 1.09e5.78) scores were the significant factors associated with patient participation. Conclusion: The degree of patient participation in patient safety activities was not high. Nurses' PCC, teamwork, and safety climate were positively related with patient participation. In particular, the findings indicate that enhancing nurses' competency for patient-centered care and creating a strong safety climate are important to promote patient participation for safer health care.

        결과지표로서의 환자 만족도에 관한 연구

        The Purpose of this study was to identify factors influencing patient satisfaction and to evaluate the utility of patient satisfaction as an outcome indicator. The study was conducted by mailed questionnaire. The subjects were 900 patients discharged from adult nursing units in a tertiary teaching hospital. On the discharge date, questionnaires were distributed by two trained research assistants The questionnaire developed by the researchers was based on Larson(1996)'s study, and consisted of 71 items with the following components: overall satisfaction domain-specific satisfaction(administration process. hospital facility and environment, nurses, and doctors). patients' loyalty(intention to use the health care service of the hospital in the future). recommendation to others. health benefits, and demographic characteristics. Each item was rated using a five point Likert scale ranging from' 1=strongly disagree' to 5= strongly agree'. The response rate was 43%(387/900). The satisfaction level with the health care service was generally high. Perceived health status was the only significant factor influencing satis- faction level. Satisfaction with doctors contri-buted the most to explaining overall satisfaction. Overall satisfaction was significantly correlated with patient loyalty, recommendation, to other and perceived health benefit. It was found that the score of satisfaction was positively correlated with the score of loyalty, recommendation, and health benefit. Therefore. patient satisfaction seems to be a good outcome indicator

        조지 허버트의 『성전』에 나타난 ‘부활’

        기독교에서 부활은 인간을 죽음에서 생명으로 옮기는 중요한 사건이다. 인간은 자신의 죄 값을 치룬 그리스도와 함께 부활함으로 새로운 피조물이 되는 갱생의 은혜와 기쁨을 누리며 영원한 천국에 입성한다. 부활은 인간의 이성적 힘이 아닌 신의 은총에 의지할 때 찾아오는 생기이다. 죄와 절망으로 말라버린 심장이 봄처럼 푸르게 소생하는 것이고 그리스도의 날개에 접붙여 영적 죽음에서 구원의 빛으로 갱생의 천상으로 비상하는 것이다. 부활은 기독교 역사에서 중요한 부분으로서 부활이 없는 기독교는 상상할 수 없다. 본 연구는 그동안 부활에 대해 일반적이고 단편적으로 다룬 기존의 연구에서 벗어나 더욱 세부적으로 접근함으로써 기독교에서 부활이 갖는 의의를 더욱 깊이 있게 조명하고자 한다. Resurrection of Christianity is an important event that moves humans from death to life. Humans enter the eternal kingdom of heaven, enjoying the grace and joy of rebirth to become a new creation by resurrecting with Christ who paid for their sins. Resurrection is the life that comes when we depend on God’s grace, not human rational power. The heart that has dried up due to sin and despair is revived as spring as it is, and it is grafted onto Christ’s wings to rise from spiritual death to the heaven of rebirth with the light of salvation. This study examines the resurrection expressed in George Herbert’s temple. This paper argues that the resurrection is the important part of Christian history, and Christianity without the resurrection is unimaginable.
