This paper reports the establishment of a proficiency testing (PT) scheme for pesticide residue in crops using relevant international standards (KS Q ISO 13528 & ISO/IEC 17043). The proficiency testing materials (PTMs) are made from freeze-dried Welsh onion powder spiked with the pesticide Procymidone. The results of the analysis were reported by 25 participating organizations and evaluated via Horwitz standard deviation and zscore. The histogram and box plot of the Procymidone analyses clearly show that 22 (88%) of the 25 participating organizations can be approved as qualified test institutions for Procymidone residue analysis in crops.

Background: Mucosal-associated invariant T (MAIT) cells contribute to protection against certain microorganism infections. However, little is known about the detailed information of MAIT cells in patients with mycobacterial infection. Methods: Patients with pulmonary tuberculosis (TB; n=35), nontuberculous mycobacterial lung disease (NTM; n=29), and healthy controls (n=75) were enrolled in the study. MAIT cell levels and functions were measured by flow cytometry. Results: Circluating MAIT cell levels were found to be reduced in TB and NTM patients. Serum C-reactive protein was an independent predictor of MAIT cell deficiency. MAIT cells in TB patients failed to produce interferon-γ irrespective of the mode of stimulation, whereas NTM patients displayed a defect in MR1-dependent signaling pathway. TB patients showed the positive correlation between MAIT cell levels and natural killer T (NKT) cell levels. In vitro experiments revealed that MAIT cell dysfunction was related to NKT cell dysfunction. Notably, an elevated expression of programmed death-1 was associated with MAIT cell deficiency in TB. Recently there is an increasing trend of facial fracture, especially complex fractures. These complex facial fractures need implant for optimal treatment. Titanium is the most commonly used material, but there are few reports of complications after 10 years of implantation. The authors report a case of delayed complication of titanium implant in complex orbital wall fracture in a 69-year-old man.

Fibrous dysplasia is a benign pathological condition of bone in which fibrous tissue gradually expands and replaces normal bone. Histologically, it shows various degrees of osseous metaplasia. Fibrous dysplasia frequently affects the maxilla, frontal bone, and mandible. The sign and symptoms of fibrous dysplasia of head and neck vary and are related to the location and extent of bony abnormalities. Facial asymmetry is the most common sign of fibrous dysplasia, while pain and ocular proptosis are the next most frequent symptoms. Fibrous dysplasia is rare in the nasal cavity, especially involving the turbinate and nasal septum. So we report a case of fibrous dysplasia, which extensively involves the middle turbinate and nasal septum, with a review of literature.

Minor and major complications can occur during endoscopic sinus surgery. Damage to an anatomic structure ad-jacent to a sinus is always a possibility during ESS. The lamina papyracea is very thin and may be fractured withrelatively minor truma. The presence of orbital fat, as well as the distorted anatomy at the fracture site, may makeendoscopy sinus surgey difficult or impossible because orbital fat is a landmark that surgeons should not crossfor fear of injury to orbital content, such as extraocular muscles, vasculature, and neural structures. If there maybe combined disruption of the medial orbital wall with orbital blowout fracture, orbital complication after ESSmay be very high. We report a case of endoscopic sinus surgery of chronic sinusitis and nasal polyps associatedold medial blowout fracture with a review of literature.

Mucosal-associated invariant T (MAIT) cells and natural killer T (NKT) cells are known to play crucial roles in a variety of diseases, including autoimmunity, infectious diseases, and cancers. However, little is known about the roles of these invariant T cells in acute cholecystitis. The purposes of this study were to examine the levels of MAIT cells and NKT cells in patients with acute cholecystitis and to investigate potential relationships between clinical parameters and these cell levels. Thirty patients with pathologically proven acute cholecystitis and 47 age- and sex-matched healthy controls were enrolled. Disease grades were classified according to the revised Tokyo guidelines (TG13) for the severity assessment for acute cholecystitis. Levels of MAIT and NKT cells in peripheral blood were measured by flow cytometry. Circulating MAIT and NKT cell numbers were significantly lower in acute cholecystitis patients than in healthy controls, and these deficiencies in MAIT cells and NKT cell numbers were associated with aging in acute cholecystitis patients. Notably, a reduction in NKT cell numbers was found to be associated with severe TG13 grade, death, and high blood urea nitrogen levels. The study shows numerical deficiencies of circulating MAIT and NKT cells and age-related decline of these invariant T cells. In addition, NKT cell deficiency was associated with acute cholecystitis severity and outcome. These findings provide an information regarding the monitoring of these changes in circulating MAIT and NKT cell numbers during the course of acute cholecystitis and predicting prognosis.

First Fatal Oseltamivir-Resistant 2009 Pandemic Influenza A (H1N1) Case in an Adult in Korea

It has been suggested that oseltamivir-resistant influenza viruses harboring the H274/275Y mutation are less virulent than are those that are oseltamivir-sensitive,and fatality attributed to infection with an oseltamivir-resistant virus is very rare. Here we report the first fatal adult case of oseltamivir-resistant 2009 pandemic influenza A (H1N1) in Korea. A 60-year-old Korean male who had hypertension, diabetes mellitus,chronic kidney disease, and dilated cardiomyopathy visited Chonnam National University Hospital because of a 7-day history of chest pain and dyspnea. The patient was at another clinic and had been medicated with oseltamivir (75 mg twice daily) beginning 7 days before admission. Empirical antibiotics were started on the first day of hospitalization. Reverse-transcriptase polymerase chain reaction for 2009 pandemic influenza A (H1N1) was reported to be positive, and a double dose of oseltamivir (150mg twice per day) was started on day four of hospitalization. However, the pneumonia worsened and the patient died, despite 3 days of high-dose antiviral therapy and 6 days of antibacterial therapy. An H275Y mutation was detected in the neuraminidase gene sequence. This case shows that oseltamivir resistance after short-term drug exposure is possible and can be fatal, emphasizing that early use of zanamivir should be considered in suspicious cases.
