Acute calcific tendinitis of the longus colli muscle, which is also known as retropharyngeal calcific tendinitis, is an under-recongnized cause of acute cervical pain produced by an inflammation of the longus colli muscle. Acute calcific tendinitis of the longus colli muscle is a rare benign condition which is characterized by acute sore throat, neck pain and stiffness, dysphagia, odynophagia and a limited range in the cervical spine. This condition can easily be overlooked or misdiagnosed because of its rather non-specific presentation and rare occurrence. The differential diagnosis includes a retropharyngeal abscess, meningitis, infectious spondylitis and herniated disc in the cervical spine. Knowledge of the clinical and imaging findings can prevent a misdiagnosis and inappropriate procedures. We report a case of acute calcific tendinitis of the longus colli muscle misconceived as acute retropharyngeal abscess with a reviews of the related literatures.

Amyloidosis is a rare but devasting condition caused by deposition of misfolded proteins as aggregates in the extracellular tissues in various tissues mainly derived from the mesoderm. Amyloidosis in the head and neck is a rare and benign condition that usually takes the form of localized amyloidosis. We experienced a case of a 63-years old women with a localized unilateral mass at the left vestibular fold and bilateral mass on both side of lateral nasopharynx, which were surgically removed and were diagnosed as amyloidosis pathologically. Concurrent amyloidosis of the larynx and the nasopharynx is rare and there are only about one case that have been reported in the literature worldwide until today. We report this case with a review of literature.

Many surgical approaches for reduction of zygomatic fractures have been reported, which include Gilles approach, transnasl approach, transoral approach, subcilliary approach, lateral canthal approach, eyebrow approach and transconjunctival approach, etc. The choice of surgical approach and materials for reduction of zygomatic fracture depends on the type of fracture, the surgeon’s experience and preference. In case of zygomatic tripod fracture, reduction was performed frequently by open reduction and internal fixation. However open reduction has significant drawbacks such as facial scar, facial nerve injury and blindness. Compared with open reduction, closed reduction has many advantages, which are short operative time, minimal incision, cost-effectiveness. But paucity of closed reduction was exist such as inaccurate reduction and delayed displacement. To overcome theses faults, the authors performed semi-closed reduction for the tripod fracture under intraoperative assessment using C-arm in two cases.

Minor and major complications can occur during endoscopic sinus surgery. Damage to an anatomic structure ad-jacent to a sinus is always a possibility during ESS. The lamina papyracea is very thin and may be fractured withrelatively minor truma. The presence of orbital fat, as well as the distorted anatomy at the fracture site, may makeendoscopy sinus surgey difficult or impossible because orbital fat is a landmark that surgeons should not crossfor fear of injury to orbital content, such as extraocular muscles, vasculature, and neural structures. If there maybe combined disruption of the medial orbital wall with orbital blowout fracture, orbital complication after ESSmay be very high. We report a case of endoscopic sinus surgery of chronic sinusitis and nasal polyps associatedold medial blowout fracture with a review of literature.

Infestation of tissue by fly larvae is termed myiasis, and it is unusual in humans. Nasal myiasis is common in low socioeconomic status individuals due to poor nasal hygiene. It commonly affects the skin and rarely the nasal and paranasal sinuses. Recently an 82-year-old female was admitted to the emergency department because of discharge of live maggots from the nasal cavity. She had been diagnosed with brain infarction and Alzheimer’s disease several years previous. We successfully removed all the maggots from the patient’s nasal cavity and sinuses via endoscopic surgery under local anesthesia. Subsequently, the patient’s nasal problem resolved completely.

Lymphoma is a neoplastic proliferation of the lymphoreticular system and originate most commonly within a lymph node or lymphoid tissue. But extra-nodal involvement occurs about 25-30% in non-Hodgkin’s lymphoma (NHL) and 1% in Hodgkin’s lymphoma (HL). Cutaneous lymphoma has been reported in 4% of malignant lymphoma in South Korea is presented as primary or secondary lymphoma. Especially, cutaneous lymphoma of the external auditory canal is extremely rare, and has been reported 8 cases in the literature worldwide. We report a rare case of secondary cutaneous diffuse large B cell lymphoma of the external auditory meatus which has not been reported in South Korea.

Fibrous dysplasia is a benign pathological condition of bone in which fibrous tissue gradually expands and replaces normal bone. Histologically, it shows various degrees of osseous metaplasia. Fibrous dysplasia frequently affects the maxilla, frontal bone, and mandible. The sign and symptoms of fibrous dysplasia of head and neck vary and are related to the location and extent of bony abnormalities. Facial asymmetry is the most common sign of fibrous dysplasia, while pain and ocular proptosis are the next most frequent symptoms. Fibrous dysplasia is rare in the nasal cavity, especially involving the turbinate and nasal septum. So we report a case of fibrous dysplasia, which extensively involves the middle turbinate and nasal septum, with a review of literature.

Confirmation bias is a cognitive bias in clinical practice where healthcare professionals tend to give more weight to data that support their initial diagnosis while ignoring evidence that contradicts it. This can lead to diagnostic errors and poor medical decision-making, ultimately resulting in adverse outcomes for patients. One potential complication following endoscopic sinus surgery is postoperative bleeding, which can occur immediately or as delayed bleeding several days after the procedure. While spontaneous epistaxis, or nosebleeds, are a common ENT emergency, posterior epistaxis is particularly challenging to detect and treat, which can result in significant bleeding. This report highlights a case in which posterior epistaxis was initially misdiagnosed as postoperative hemorrhage after endoscopic sinus surgery, leading to a delay in treatment. Such diagnostic errors can have severe consequences for patients, underscoring the importance of being vigilant for confirmation bias and its potential impact on clinical decision- making.
생체 흡수성 고정판을 이용한 안와 외향 골절 정복술의 결과

Background and Objectives The purpose of this study was to share our clinical experience on the use of bioresorbable poly L-/DL-lactide 70/30 implant to repair blow out fracture. Subjects and Method The medical records of 130 patients with orbital fractures that were treated surgically from June 2006 to February 2011 were reviewed retrospectively. Patients who had diplopia or limited extraocular motion, significant Enophthalmos (>2 mm), or a large orbital wall fracture on a computed tomographic scan were enrolled for the study. We investigated diplopia, limited extraoacular motion, enophthalmos before and after surgery. Results The mean postoperative follow-up was 28 months. There was no evidence of sinus infection related to the implant. However, six patients showed dislocation of orbital implant in the nasal cavity. After six postoperative months, diplopia was seen in 1 (2%) patient, and significant enophthalmos was seen 2 (2%) patients. We carried out re-operation on six cases and the main cause was incomplete reduction or over reduction. Conclusion Bioresorbable poly L-/DL-lactide 70/30 implants are safe and reliable for the reduction for blowout fracture. Korean J Otorhinolaryngol-Head Neck Surg 2014;57(11):759-65
