        유육종증에서 단핵구의 교원질에 의한 Procoagulant Activity와 종야괴사인자-α의 표현

        이 연구는 간질성 폐질환에서 폐간질내에 축적된 교원질이 섬유소형성에 중요한 PCA와 육아종 형성에 중요한 TNF-α를 분비시켜서 염증반응을 지속시키는 지를 알아보고자 시행한 실험으로서 정상인과 유육종증환자의 말초혈액 단핵구와 폐포대식세포를 교원질로서 자극하여 PCA와 TNF-α를 측정하였다. PCA는 유육종증 환자의 말초혈액 단핵구와 폐포대식세포는 건강대조군에 비하여 기저치가 유의하게 높았으며(P<0.05), LPS는 건강대조군과 유육종증 환자의 말초혈액 단핵구와 폐대식세포를 자극하여 PCA 생성을 유의하게 증가시켰다. 교원질은 건강대조군의 말초혈액 단핵구와 폐포대식세포를 자극하여 PCA를 증가시켰으나 유육종증 환자에서는 말초혈액 단핵구는 자극할 수 있으나 폐포대식세포의 PCA생성은 자극시키지 못하였다. TNF-α는 PCA와 마찬가지로 건강대조군에 비하여 유육종증 환자의 말초혈액 단핵구, 폐포대식세포 모두에서 기저치가 높았으며(P<0.05), 말초혈액 단핵구는 건강대조군과 유육종증 환자 모두에서 교원질과 LPS에 의해서 TNF-α의 생성이 증가되었으나 유육종증 환자의 폐포대식세포는 교원질에 의해서 TNF-α의 생성이 증가되지 않았다. 이상의 성적으로 보아 유육종증은 전신질환이어서 말초혈액 단핵구도 이미 자극되어 PCA, TNF-a를 자극없이 분비할 수 있으며 교원질이 말초혈액 단핵구를 자극하여 PCA, TNF-α생성을 증가시키는 것으로 보아서 간질성 폐질환의 폐조직내에 침착된 교원질은 단핵구를 활성화시켜서 염증반응을 지속 시킬 수 있을 것으로 사료된다. 폐포대식세포는 폐조직내의 교원질과 같은 세포외기질에 이미 노출되어 더 이상 교원질에 의해서는 자극되지 않는 것으로 생각되며 LPS와 같은 강력한 자극에 의해서는 반응이 현저히 나타나는 것으로 생각된다. 단핵구 자극검사방법으로 PCA와 TNF-a의 반응 양상이 비슷하므로 검사방법이 비교적 용이하고 비용이 저렴한 PCA가 TNF-α assay를 대체시킬 수 있을 것으로 사료된다. Extracellular matrix deposition and granulomatous lesions are the hallmarks of a variety of diffuse interstitial lung diseases that can lead to loss of pulmonary function. Sarcoidosis is a systemic granulomatous disease which involves the lungs, lymph nodes, skin, central nervous system, liver and muskuloskeletal system. Activated macrophages and monocytes express procoagulant activity which is an initiator of extrinsic coagulation cascade for fibrin formation. And activated macrophages produce cytokines which are important for chemotaxis and differentiation of fibroblast to produce collagen and fibronectin, which in turn, stimulate monocyte/macrophage to produce interleukin-1. To understand the role of monocyte/macrophage and collagen in the pathogenesis of pulmonary sarcoidosis, ten sujects with pulmonary sarcoidosis (stage Ⅱ and Ⅲ) were investigated by bronchoalveolar lavge cells and peripheral blood mononuclear cells. Spontaneous release of PCA and TNF-αfrom AMs were significantly higher in patients than healthy individuals (p<0.05). Collagen could stimulate PBMCs from sarcoidosis patients and healthy PBMCs and AMs to produce PCA and TNF-αbut could not stimulate AMs from sarcoidosis patients. This dyscoordinate stimulation of AM/PBMC in sarcodosis seems to be related with activation with pre-exposure to the millieu in the collagen-deposited interstitium of sarcoidosis. This data demonstrated that in stage Ⅱ and Ⅲ sarcoidosis, PBMCs and AMs are actively produce cytokines and PCA, and collagen deposited in the sarcoidosis may play role in perpetuating the inflammatory process.

        전자상거래에서 신뢰의 구축 정도와 방법이 서비스 품질에 미치는 영향

        This research addresses familiarity and degree of using Internet site and trust building to affect service quality in e-commerce. We assume that the familiarity and frequency of using Internet site affect perceived service quality of customer. Also it assumes that customer's trust building intermediates their relationships. Several hypotheses are set to test these assumptions empirically. Consequently, the results show that familiarity of using Internet site affects customer's perceived service quality significantly except for tangible. Also it is found that degree of customer's trust building intermediates the relationship between familiarity of using Internet site and customer's perceived service quality significantly. For gaining good trust of customer in Internet site, we suggest that familiarity should fit into customer's needs.

        늑막삼출액의 fibronectin 농도와 Adenosine Deaminase활성도의 암과 결핵에서의 감별진단적 의의

        The differential diagnosis of pleural effustioin remains to be solve. The aim of this study was to evaluate the diagnostic values of the fibronectin concentration and andevosine deaminase activity in the differentiation between tuberculous and malignant pleural effusion. The activity of adenosine deaminase and the concentration of fibronectin were investigated in pleural effusions from 10 patients with tuberculous pleurisy and 10 patients with malignant effusion. The mean ADA activity was significantly higher in the tuberculous pleurisy (74.01±24.05 U/L) than in the malignant pleural effusion(15.46±13.08 U/L). Based on the lowest value of ADA activity found in the tuberculous pleurisy (45 U/L), the test had a sensitivity of 1 and a specificity of 1. There was no difference between the fibronectin concentration of pleural fluid in the tuberculous pleurisy (306.62±50.52mg/ml) and in the malignant pleural effusion (250.50±144.28㎍/ml) By discriminant analysis, the ADA activity was good indicator for differentiation of tuberculous from malignant pleural effusion (% of correctly classified = 95%) and the simultaneous determinations of both parameters in not more efficient tn the differential diagnosis than in determination of only ADA activity. The present study showed that determination of ADA in pleural fluid is of great value in the differentiation between tuberculous pleurisy and malignant pleural effusion.

      • L1210 세포증식에 대한 Glycyrrhizin의 억제작용 기전

        The purpose of this research was to investigate the mechanism of inhibitory action of Glycyrrhizin(GZ) on the proliferation of mouse leukemia cell-line, L1210 cells. The cytotoxic activity was tested using a colorimetric tertrazolium assay(MTT assay), the apoptosis was tested using flow cytometry. Nitric oxide(NO) production form mouse peritoneal macrophage was tested using a Griess method and the phagocytosis of human polymorphonuclear cells was tested using a lucigenin chemiluminescence. GZ ingibited the proliferation of L1210, BALB/c 3T3 cells and mouse thymocytes at 50 ug/ml/ GZ did not affect nitric oxide production from mouse peritoneal macrophages in vitro, but ingibited nitric oxide production from lipopolysaccharide and y-interferon treated macrophages. Macrophages of GZ-administered mice accelerated NO production. The proliferation of L1210 cells apoptosis of L1210 cells were induced by co-culture with macrophage of GZ-administered mice. The apoptosis of L1210 cells were induced by co-culture with macrophage of GZ-administered mice. GZ increased the phagocytosis of human polymorphonuclear cells. These results suggest that GZ inhibit the proliferation of L1210 cells not only as a direct cytotoxic agent o tumor cells, but also by the enhancement of NO production and phagocytic activity.

        편평세포폐암에서 p53 돌연변이의 발현과 TNM 병기와의 상관관계

        Primary lung cancer is now recognized as a major cause of cancer death in Korea as well as the whole world. Non-small-cell lung cancers(NSCLC) comprise about 75% of lung cancer and squamous cell carcinoma is the most common type in Korea. Mutations of p53 gene are common in variety of human cancers, including lung cancer. The p53 gene appears to inhibit the proliferation of cells from the G1 to the S phase of cell cycle and is able to suppress the transformations of cells by other oncogenes, to inhibit the growth of malignant cells in vitro and suppress the tumorigenic phenotype of transformed cells. Alteration or inactivation of p53 by mutation, or by its interactions with oncogene products of DNA tumor viruses, can lead to cancer. These mutations seems to be the most common genetic changes in human cancers. p53 gene mutation is known to be a poor prognostic marker in breast cancer and has significant association with lymph node involvement. But in human lung cancer the association of p53 mutations and TNM stages are controversial. Immunohistochemical staining can detect only mutant p53 protein bacause of markedly prolonged half life of mutant p53 protein. We performed the immunohistoschemical staining on 48 surgically resected and 10 bronchoscopically biopsized specimens of primary squamous cell carcinoma of the lung with monoclonal antibody(DO 7) and analyzed the relationship between the expression of p53 protein and clinical TNM stages. p53 protein was detected in 60.3% of total 58 cases. p53 protein was positive in 52.9% of stage I(17cases), 83.3% of stage Ⅱ(6 cases), 72.2% of ⅢA(18 cases), 42.9% of ⅢB(7 cases) and 50% of stage Ⅳ(10 cases) which revealed no significant correlation between p53 protein detection and stages. There was no significant correlation between p53 protien detection and No and N₁-₃as well Mo and M₁. In conclusion, we couldn't find any significant correlation between p53 protein detection and TNM stages which suggests p53 mutation is frequent in squamous cell carcinoma of the lung but play different role in progression of clinical stages.

        The mesenteric infarction, as a abdominal disease which show acute abdominal pain and almost need to emergency operation, has very high mortality. This mesenteric infarction has been reported very rarely. Also specific clinical manifestations are not clear, therefore initial diagnosis can be missed easily so that patients usually miss their opportunities for adequate treatment. Especially CAPD patients have high mortality because of their rareness and having similar symptoms like CAPD peritonitis. The authors report that the mesenteric infarction was miss-diagnosed to the CAF'D peritonitis with unspecific symptoms.

        만성경과의 폐렴을 동반한 기관지결석증 1례

        Broncholithiasis has been recognized in medicall literature for centuries. Because of the relative infrequency, broncholithiasis has often been neglecteed in the differential deagnosis of bronchial obstruction. In this case, we could remove the broncholith obstructing the right middle lobar bronchus with the fiberoptic bronchoscope in a 37-year old man without any complications and the complete resolution of the pneumonia was followed.
