        Purpose: The purpose of this study was to investigate the relationship between teaching effectiveness and clinical practice satisfaction among nursing students. Method: The subjects of this study were 107 junior and senior nursing students in E university. Data were collected using self-reported questionnaire including general and practicum related characteristics, teaching effectiveness of clinical instructors and clinical practice satisfaction from September 12 to September 21, 2011. Collected data were analyzed by SPSS 19 program using t-test, ANOVA with Scheffe test, and Pearson`s correlation coefficients. Result: The mean score of teaching effectiveness was 3.35(±.51), and mean of clinical practice satisfaction was 3.19±.47. There were significant differences of teaching effectiveness of clinical instrutor by satisfaction of overall clinical practicum(F=8.332, p<.001), satisfaction of practice hour(F=3.230, p=.044), and satisfaction of major(F=9.883, p<.001). There were significant differences of clinical practice satisfaction by grade(t=2.274, p=.025), motive of choosing nursing science as a major(F=3.329, p=.007), satisfaction of overall clinical practicum(F=17.437, p<.001), satisfaction of practice hours(F=9.925, p<.001), and satisfaction of nursing major(F=12.748, p<.001). Relationship between teaching effectiveness of clinical instructor and clinical practice satisfaction showed positive correlation(r=.704, p<.001). Conclusion: In this study, teaching effectiveness of clinical instructor was related with clinical practice satisfaction. Therefore, we should consider improving teaching effectiveness of clinical instructor to improve clinical practice satisfaction.

        Clinical efficacy of photodynamic therapy

        Objective The management of cervical intraepithelial neoplasia (CIN) and early invasive cancer of the uterine cervix is very difficult to approach, especially in case of young woman who wants to preserve her fertility. Conization of the cervix may have various kinds of disadvantage. The objective of this clinical retrospective study is to investigate the therapeutic effects and clinical efficacy of photodynamic therapy (PDT) including combined chemo-photodynamic therapy in patients with pre-malignant CIN and malignant invasive cervical cancer. Methods Total number of PDT trial case was 50 cases and total number of patient was 22 patients who registered to PDT clinic. We used photogem sensitizer and 632 nm diode laser in early two cases. After then we performed PDT using photofrin sensitizer and 630 nm diode laser in other cases. We used flat-cut, microlens, cylindrical diffuser, and interstitial type optic fibers in order to irradiate the lesions. 240 J/cm2 energy was irradiated to the lesions. Results CIN 2 were 4 cases (18.2%) and CIN 3 were 15 (68.2%) and invasive cervical cancer were 3 (13.6%). Complete remission (CR) was found in 20 patients (91%). One case of 19 patients with CIN lesion recurred at 18 months after PDT treatment. CR was found in 18 cases in the patients with CIN lesions (95%). CR was found in 2 cases in the patients with invasive cervical cancer (67%). Conclusion Our data showed that CR rate was fantastic in CIN group (95%). This study suggests that PDT can be recommended as new optimistic management modality on the patients with pre-malignant CIN lesions including carcinoma in situ and relatively early invasive cancer of the uterine cervix. Combined chemo-photodynamic therapy is essential in case of invasive cervical cancer. For the young age group who desperately want to preserve their fertility and have a healthy baby, PDT can be a beacon of hope.

        Cholesterol depletion induces anoikis-like apoptosis via FAK down-regulation and caveolae internalization

        <P>Caveolae (lipid rafts), microdomains of the plasma membrane, are known to contain various signalling molecules and consequently are involved in the regulation of many biological functions. To investigate the role of the caveolae in cell survival and adhesion, we disrupted the caveolae by depletion of cholesterol, a major lipid component of the caveolae, with methyl-β cyclodextrin (MβCD) treatment of A431 cells. We found that cholesterol depletion induced an anoikis-like cell death involving actin reorganization, resulting in a decrease in cell spreading and an increase in cell detachment, which was reversed by cholesterol addition. Disruption of caveolae led to the down-regulation of FAK, Src activation, tyrosine phosphorylation of caveolin-1 and mobilization of caveolae markers, GM1 and caveolin-1, from the cell surface to the cytoplasm, which were also recovered by cholesterol addition. The expression of dominant-active FAK was able to delay caveolae internalization and apoptosis and attenuated Akt inactivation by MβCD, whereas dominant-negative FAK expression resulted in enhanced apoptosis. Moreover, FAK down-regulation by si-RNA resulted in Akt inactivation and thus increased cell death by MβCD treatment. Our results suggest that the cholesterol content and/or surface levels of the caveolae affect the activity of FAK, which in turn regulates caveolae internalization and cell survival.       • Activation of WNT/β‐catenin signaling results in resistance to a dual PI3K/mTOR inhibitor in colorectal cancer cells harboring <i>PIK3CA</i> mutations

        <P><I>PIK3CA</I> is a frequently mutated gene in cancer, including about ~15 to 20% of colorectal cancers (CRC). <I>PIK3CA</I> mutations lead to activation of the PI3K/AKT/mTOR signaling pathway, which plays pivotal roles in tumorigenesis. Here, we investigated the mechanism of resistance of <I>PIK3CA</I>‐mutant CRC cell lines to gedatolisib, a dual PI3K/mTOR inhibitor. Out of a panel of 29 CRC cell lines, we identified 7 harboring one or more <I>PIK3CA</I> mutations; of these, 5 and 2 were found to be sensitive and resistant to gedatolisib, respectively. Both of the gedatolisib‐resistant cell lines expressed high levels of active glycogen synthase kinase 3‐beta (GSK3β) and harbored the same frameshift mutation (c.465_466insC; H155fs*) in <I>TCF7</I>, which encodes a positive transcriptional regulator of the WNT/β‐catenin signaling pathway. Inhibition of GSK3β activity in gedatolisib‐resistant cells by siRNA‐mediated knockdown or treatment with a GSK3β‐specific inhibitor effectively reduced the activity of molecules downstream of mTOR and also decreased signaling through the WNT/β‐catenin pathway. Notably, GSK3β inhibition rendered the resistant cell lines sensitive to gedatolisib cytotoxicity, both <I>in vitro</I> and in a mouse xenograft model. Taken together, these data demonstrate that aberrant regulation of WNT/β‐catenin signaling and active GSK3β induced by the <I>TCF7</I> frameshift mutation cause resistance to the dual PI3K/mTOR inhibitor gedatolisib. Cotreatment with GSK3β inhibitors may be a strategy to overcome the resistance of <I>PIK3CA</I>‐ and <I>TCF7</I>‐mutant CRC to PI3K/mTOR‐targeted therapies.</P>

        만성 B 형 간염환자에서 알파 인터페론의 용량에 따른 치료 효과

        N/A To assess the effect of different dosages of interferon alpha-2b(Intron A), 43 patients with chronic hepatitis B were assigned randomly to 3 groups: Group A, l5 patients, received Intron A in a dose of 3 million units (MU) thrice weekly for 12 weeks (total dosage: 1.8×108); Group B, 14 patients, received in a dose of 3 MU every day for the first month and thereafter every other day for the following 4 months (total dosage: 2.7×108); Group C, 14 patients, received no specific therapy. They included 34 men and 9 women, between 17 to 53 (mean 35) years of ages, with elevated serum AST and ALT levels; 41 to 879 (mean 289), and 57 to 334 (mean 288)U/L, respectively. Needle biopsies of the liver before therapy revealed chronic active hepatitis in 35 cases and active cirrhosis in 7 cases, The average follow-up period was 12 months. The results were as follows; 1) During the therapy, serum ALT levels returned to normal in 12 (80%) of Group A and in 12 (86%) of Group B, but increased again in 8 out of 12 in Group A, and in 4 out of 12 in Group B during the follow-up period, In Group C, the ALT level returned to normal in 3 out of 15 during the same period. 2) Serum HBeAg became negative in 6 out of group A in 11 out of Group B, but became positive again in 2 out of 6 in Group A and in 3 out of 11 in Group B during the follow-up period; The sero-negative conversion rate of HBeAg at the end of this study was 27 in Group A and 57% in Group B. In Group C, the sero-conversion of HBeAg was found in 2 out of 14 (14%). A statistically significant difference was noted between Group B and Group C (p<0.05). 3) Comparison of responders and non-responders in the treated group showed no differences in age, sex, liver fuction test, and tissue pathology. 4) Although flu-like symptoms (100%), loss of hair l (10%), and leukopenia (40%) were observed in the treated group, no serious side effects necessitating discontinuation of the therapy were observed. In conclusion, this study showed that the effect of interferon on patients with chronic hepatitis B might be different depending on the dosage and duration of therapy and that with increased dosage, the response rate could to be higher. But further studies should be carried out to determine the exact duration and dosage of therapy in these patients.

        류마티스 질환 환자와 비류마티스 환자에 있어 요추부 후측방 유합술의 결과 비교 -류마티스 질환 환자에서의 척추 유합술-

        Objective: To evaluate the clinical and radiological results and to compare the results of posterolateral lumbar fusion in rheumatic and non-rheumatic disease. Methods: A retrospective review of results was carried out in 20 patients who had posterolateral lumbar fusion with rheumatic disease from Jul. 1996 to Aug. 2002. And same cases of non-rheumatic disease were compared. The diagnosis of rheumatic disease was confirmed by the ARA revised criteria. Bony union was evaluated by Lenke`s criteria and the clinical results by Katz`s satisfaction degree. Statistical analysis was performed by paired T-test and ANOVA test. Results: In rheumatic disease group, there were 7 males and 13 females. Mean age was 56.6 (20∼68) years and mean fused segments were 2.7 (1∼7). Mean follow-up period was 41 months (12∼80) after surgery. In non-rheumatic group, there was 6 males and 14 females. Mean age was 57.1 (35∼71) years and mean fusion segments were 2.9 (1∼4), Mean follow-up period was 40.2 (12∼88) months. In age and fusion segments between two group, there was no statistical difference. In rheumatic disease group, the diagnosis were rheumatoid arthritis in 18 patients, ankylosing spondylitis in 1, and systemic lupus erythematosus in 1 patient. The other operations for combined disease were 8 total knee arthroplasty and 3 total hip arthroplasty. There were no statistical differences in operation time (p=0.527), perioperative bleeding (p=0.653) and postoperative (p=0.830) bleeding between two group. In radiological bony union, all patients of two groups showed A and B grade by Lenke`s criteria. Bony union was complete at 5.5 (5∼8), 5.1 (4∼7) months after surgery. There was no significant difference in clinical satisfaction (p=0.756). Conclusion: There were no significant differences in clinical and radiological results between the rheumatic and non-rheumatic patients with disease of the lumbar spine.

Live_Board의 실시간 스트리밍을 활용하여 영상·이미지기반 의사소통을 지원하고, SNS적 요소인 TimeLine, News Feed를 통해 다른 사용자들과 정보 공유를 할 수 있고, Planner를 통해 개인 스케줄 관리를 할 수 있는 플랫폼이다. 1)주

        STAT3 Potentiates SIAH-1 Mediated Proteasomal Degradation of beta-Catenin in Human Embryonic Kidney Cells

        The -catenin functions as an adhesion molecule and a component of the Wnt signaling pathway. In the absence of the Wnt ligand, -catenin is constantly phosphorylated, which designates it for degradation by the APC complex. This process is one of the key regulatory mechanisms of -catenin. The level of -catenin is also controlled by the E3 ubiquitin protein ligase SIAH-1 via a phosphorylation-independent degradation pathway. Similar to -catenin, STAT3 is responsible for various cellular processes, such as survival, proliferation, and differentiation. However, little is known about how these molecules work together to regulate diverse cellular processes. In this study, we investigated the regulatory relationship between STAT3 and -catenin in HEK293T cells. To our knowledge, this is the first study to report that -catenin-TCF-4 transcriptional activity was suppressed by phosphorylated STAT3; furthermore, STAT3 inactivation abolished this effect and elevated activated -catenin levels. STAT3 also showed a strong interaction with SIAH-1, a regulator of active -catenin via degradation, which stabilized SIAH-1 and increased its interaction with -catenin. These results suggest that activated STAT3 regulates active -catenin protein levels via stabilization of SIAH-1 and the subsequent ubiquitin-dependent proteasomal degradation of -catenin in HEK293T cells.
