        두가지 혈류 유발방법에 의한 새로운 심폐소생술(이중 혈류 유발 심폐소생술)장치의 개발

        Background: There have been many efforts to augment blood flow during cardiopulmonary resuscitation. These efforts have focused on maximizing the effect of cardiac pump or thoracic pump alone. However, considering that the heart is the biggest blood reservoir and increase of intrathoracic pressure can generate blood flow, simultaneous exploitation of both mechanisms may have synergistic effect. We hypothesized that simultaneous chest constriction in addition to sternal compression by standard CPR may have additive hemodynamic effects by preventing deformation of the chest and increase of intrathoracic pressure. Methods and results: we built a new mechanical device to perform compression and thoracic constriction simultaneously. The device consists of two main elements. Piston in the center is to depress the sternum. Strap is to constrict the thorax circumferentially. Strap is attached to both sides of the piston. When the piston is pushed down, it depresses the sternum and pulls on the thoracic strap. To determine strap width to produce optimal hemodynamic effect, we measured hemodynamic parameters with variable widths of strap in two dogs after induction of ventricular fibrillation. Result of the experiment showed that 10cm wide strap was determined to be most effective. We also determined optimal depth of compression to produce maximal hemodynamic effect with animal experiments using two dogs. Animal experiments showed that the highest aortic pressure could be generated when the stemum was depressed to 5 cm. Cardiopulmonary resusciation using a new device could generate higher systolic aortic pressure, coronary perfusion pressure and end-tidal carbon dioxide tension in comparison with standard cardiopulmonary resuscitation in a pilot animal study using two dogs. Conclusion: New cardiopulmonary resuscitation method using a mechanical device designed by us could perform sternal compression and simultaneous thoracic constriction, and generate better hemodynamic effects than standard cardiopulmonary resuscitation in pilot animal experiments.

        Differentiation of Desmoplastic Spitz Nevus from Similar Conditions

        The diagnosis of classic Spitz nevus with characteristic histopathologic findings is often straightforward, but unusual variants can cause diagnostic difficulties. Desmoplastic Spitz nevus (DSN) is of particular importance, as its differential diagnosis from other diseases, including desmoplastic malignant melanoma (DMM), is essential but often difficult<sup>1</sup>. A 38-year-old Caucasian woman presented with a 6-mm brownish papule of unknown onset on the dorsum of her left hand (Fig. 1A). She did not report any change in the papule size, trauma history, or related symptoms, but she wanted to have the lesion removed. Punch excision of the specimen revealed proliferation of individual spindle and epithelioid cells with scanty pigmentation within dense colla-genous dermal stroma (Fig. 1B, C). The specimen was focal positive for S-100 and HMB-45, positive for Melan-A, and 1% positive for Ki-67 (Fig. 2A∼C). The lesion did not recur after punch excision at the 1-year follow-up. Since its first report in 1975, there have been only a few case series of DSN owing to the rarity of this disease and its under-recognition, except for intermittent case reports<sup>1-3</sup>. DSN usually presents as a small red-brown papule on the trunk and extremities. It can occur at any age, but is mostly observed in young adults, with a slight female predominance. The distinctive histopathologic features of DSN―an intradermal growth pattern of large spindle or epithelioid nevus cells embedded in a fibrotic stroma, sparse melanin pigment, no junctional activity, no Kamino bodies, no prominent nest formation―can aid its differentiation from clinical simulators. Additionally, immunohistochemistry is essential for a differential diagnosis. DSN tests positive for S-100, Melan-A, and HMB-45, whereas dermatofibroma is negative for all three<sup>3</sup>. Hypomelanotic blue nevus shows uniform positivity for HMB-45, whereas DSN shows differential expression in most spindled cells3. The distinction between DSN and DMM is the most important. DMM is more common in elderly patients and tends to occur on sun-damaged head and neck areas. It also shows cellular atypia, strong mitotic activity and Ki-67 expression, less frequent S-100 and Melan-A positivity, and almost exclusive negativity for HMB-45<sup>2,3</sup>. Some researchers regarded DSN as an end stage of Spitz nevus that had lost continuity with the epidermis and undergone fibrosis. Paniago-Pereira et al.<sup>2</sup> also reported that DSNs occurred in patients older than 30 years. These findings suggest that desmoplasia might be an aging process of Spitz nevus. However, Barr et al.1 found no significant difference in patient age, disease duration, or trauma history between patients with DSN and common variants of Spitz nevus, and suggested that desmoplasia may be a tumor-induced reactive stromal induction rather than a regressive phenomenon. The pathogenesis of desmoplasia has not yet been clearly elucidated. Moreover, it is controversial whether DSN should be regarded as a variant of Spitz nevus or whether it belongs to a spectrum of desmoplastic nevus as a distinctive entity<sup>3,4</sup>. Some researchers5 suggested strict diagnostic criteria for de-smoplastic nevus, including greater cellularity in the super-ficial portion, and a mixture of melanocytic nevus cells, ovoid and dendritic melanocytes, and spitzoid melanocytes. Further, they mentioned that lesions in which one particular type of melanocyte predominates over others are more likely to represent DSN. Dermoscopic findings can also aid the distinction, because DSN shows dotted vessels and reticular depigmentation whereas desmoplastic nevus demonstrates a delicate pigment network over a pinkish background<sup>4</sup>. Although it is regrettable that we did not acquire dermoscopic image to support the diagnosis, our case overall seems more com-patible with DSN. However, the probability of a morphologic spectrum that embraces DSN and desmoplastic nevus cannot be excluded, and requires further studies. Here, we report an unusual desmoplastic variant of Spitz nevus with a literature review, and propose keynotes for differential diagnosis from its simulators, especially DMM.

        Zinc-Bromine 레독스 플로우 배터리를 이용한 독립형 마이크로그리드 ESS DC-DC 컨버터 설계 및 실증

        This paper proposes ESS DC-DC Converter using Redox Flow Battery (RFB) for stand-alone microgrid. Price, safety, expandability and dynamics are crucial in ESS. Reports show that Zinc-bromine (ZnBr) RFB is the best choice in ESS. Simple electrical ZnBr RFB model is obtained from charging test. DC-DC converter Inductor current-DClink Voltage model is proposed for the DC microgrid. For the controller design in z-domain, the K-factor method is by considering nature of the digital controller. The control performance has been verified with simulation and hardware experiments. Lastly 10kW DC microgrid using RFB test result is shown.

        Background: Protein-Energy malnutrition is prevalent in hemodialysis (HD) patients. Although International Dietetics and Nutrition Terminology (IDNT) is effective tool for the nutritional diagnosis, IDNT has been rarely used to HD patients so far in Korea. In present study, we applied IDNT to HD patients and evaluated correlation of the IDNT results to other clinical parameters. Methods: Adult patients on HD for more than 3 months were recruited from single tertiary center. A clinical dietitian carried out individual interview of all patients and made nutritional diagnosis using nutritional diagnostic codes of IDNT. Dietary data were obtained by 24-hour recall method of usual meal of the day before HD. Demographic and clinical data were also analyzed. Results: Total 90 patients were included. 28 patients (31.1%) were diagnosed as normal nutritional status. 5 patients (5.6%) were malnutrition (NI-5.2), 19 patients (21.1%) were inadequate protein-energy (NI-5.3) and 38 patients (42.2%) were inadequate protein (NI-5.7.1). There were signifi cant difference among normal group, inadequate protein-energy group and inadequate protein group in the body mass index (BMI) (23.9±3.0, 19.3±2.4, 22.0±2.7, p=.001), blood urea nitrogen (68.2±16.8, 52.7±14.2, 54.6±14.4, p=.001), ratio of dry-weight to standard weight (1.1±0.1, 0.9±0.1, 1.0±0.1, p<0.001), ratio of actual to recommended protein intake (1.0±0.1, 0.7±0.1, 0.8±0.1, p<0.001), and ratio of actual to recommended energy intake (1.1± 0.1, 0.8±0.1, 1.0±0.1, p<0.001). There was no signifi cant difference in serum albumin, nPCR and interdialytic weight gain. When we classifi ed patients according to BMI or nPCR, high BMI or nPCR group showed higher proportion of normal nutritional status. Conclusions: Nutritional diagnosis by IDNT was signifi cantly correlated with weight maintenance and recommended protein and energy intake. The higher BMI or nPCR were associated normal nutritional status. IDNT is thought to be effective diagnostic tool for HD patients.

        Development of SNP Markers to Distinguish Various Watermelon Traits and Validation Using Fluidigm Genotyping Assay

        Watermelon [Citrullus lanatus (Thunb.) Matsum and Nakai] is one of the economically most important fruit crops of the Cucurbitaceae family. Among different watermelon traits, disease resistance and fruit quality are the important traits for growers and consumers. The single nucleotide polymorphism (SNP) markers similar to those traits can potentially and cost-effectively distinguish the genetic variations among these traits. Consequently, we developed 33 SNP makers linked to different watermelon traits associated with fruit quality and disease resistance, and validated in the genetic resources of watermelon and F1 breeding lines using ‘Fluidigm SNP Genotyping’ assay. Most of the SNP markers distinguished the alleles into three different types such as reference allele, alternative allele and heterozygous from watermelon genotypes for various traits. The SNP markers ‘ZymFL-T81P’ (ZYMVresistance), ‘FON1-U161’ and ‘FON1-S075’ (Fusarium wilt-resistance), ‘Pmr21-Cla831’ (PM-resistance), and ‘ClGBS-J168’ and ‘GBS-GC230’ (GSB-resistance) can successfully differentiate resistant (R), susceptible (S) and heterozygous watermelon genotypes. Similarly, the SNP marker associated with sugar content, citrulline content, arginine content, rind hardness, flesh firmness, fruit shape, rind strip pattern of watermelon fruit and seed coat colour can successfully distinguished the watermelon genetic resources and F1 breeding lines as reference allele (A) type, alternative allele (B) type and heterozygous (H). These SNP markers could be utilized for marker assisted selection as well as screening of a large number of watermelon germplasm for fruit quality and disease resistance. However, further validation like artificial inoculation of pathogens for the traits related to disease resistance is required in watermelon crops.

        악성 난소갑상선종 1예

        난소종양의 15~20%는 생식세포에서 기원하며 이중 95%는 낭포성 기형종이다. 기형종중 갑상선 조직이 전부 또는 대부분 차지할 때 난소갑상선종이라 일컫는다. 이 정의에 의하면 낭포성 기형중 2.7%를 차지한다. 악성 난소갑상선종의 발생율은 저자마다 진단기준의 차이로 인하여 구하기 힘들지만 전 난소갑상선종의 5% 미만으로 추정한다. 저자들은 본 병원 산부인과에서 우연히 발견된 난소 종양에 의해 자궁적출술과 왼쪽 부속기 절제술을 시행받은 46세의 환자의 병리조직검사에서 난소갑상선종에서 유두상 악성변화를 일으킨 1예를 경험하였기에 간단한 문헌 고찰과 더불어 보고하는 바이다. About 15 to 20% of ovarian tumors are of germ cell origin, and about 95% of these are cystic teratoma. when the teratoma is composed either entirely or predominantly of thyroid tissue, it called struma ovarii. Struma ovarii represents about 2.7% of cystic teratoma. The incidence of malignant struma ovarii is probably much less than 5% of all struma ovarii. We have observed a case of malignant struma ovarii originated from the left ovary of a 46-year-old woman, and present this case with a brief review of concerned literatures.

        <P><B>Summary</B></P> <P>In the inner ear, endolymph fluid surrounds the organ of Corti, which is important for auditory function; notably, even slight environmental changes mediated by trauma or infection can have significant consequences. However, it is unclear how the immune response is modulated in these tissues. Here, we report the local immune surveillance role of cleaved cochlin LCCL (<I>Limulus</I> factor C, Cochlin, and Lgl1) during <I>Pseudomonas aeruginosa</I> infection in the cochlea. Upon infection, the LCCL domain is cleaved from cochlin and secreted into the perilymph. This cleaved fragment sequesters infiltrating bacteria in the scala tympani and subsequently recruits resident immune cells to eliminate the bacteria. Importantly, hearing loss in a cochlin knockout mouse model is remedied by treatment with a cochlin LCCL peptide.
