        A 33-year-old female was admitted to our hospital due to removal of lung mass which indicentally detected Chest radiography. Chest computerized tomography revealed lung parenchymal cystic lesion. Open thoracotomy was done for diagnosis and therapy. Cystic lesion was communicated to bronchus and wedge resection including cystic lesion was done. Bronchogenic cyst is congenital disease and mainly locate in mediastinum. It is rare in lung parenchyme. Herein we report a case of intrapulmonary bronchogenic cyst.

      • 아미노산 투여가 운동중 싸이클선수의 젖산 및 심박수에 미치는 영향

        The purpose of the study was to determine whether a 15.6mg.Kg ??' does of glutamate given one hour prior to incremental exercise test would reduce the lactate formation. Five well trained cyclists served as subjects in this experiment which included a placebo and glutamate trial. The subjects initially work on treadmill 200m/min for 3 minutes and here increased the workload by 20m/min every 3minutes until exhaustion. Blood lactate were collected from finger-tip. Subjects in the glutamate trial showed significantly lower lactate formation during exercise then they did in the placebo trial. In both groups no significant difference was found on heart rate. During the recovery lactate and heart, rate were unaltered in either the glutamate or placebo trials. In summary, glutamate ingestion may reduce lactate formation resultly improvement of performance.

      • 대장균 sfs 유전자의 전사조절

        sfs(sugar fermentation stimulation) 유전자는 crp*¹이 도입된 MK2001 균주에서 mal 유전자의 발현을 촉진시키는 특성으로서 클로닝 되어졌다. sfs4 유전자의 발현은 두 개의 promoter에 의하여 조절되어진다. 본 연구는 이들 두 promoter (P₁과 P₂)의 cAMP-CRP에 의한 발현조절을 보고자 조절영역과 CRP 결합영역에 변이가 도입된 DNA가 in vitro transcription 방법에 의해 확인되어졌다. 두 개의 promoter가 공존시에는 cAMP에 의한 P₂의 전사는 촉진되어졌으며, P₁의 전사는 억제되어졌다. P₂만이 존재할 때에도 전사는 cAMP에 의하여 촉진되어졌다. CRP 결합부위에서의 변이는 cAMP에 의한 전사촉진이 크게 감소하였다. sfs7 (nlp) 유전자의 전사산물을 확인하였으며, 이는 cAMP에 의한 발현조절을 받지 않았다. sfs(sugar fermentation stimulation) genes are involved in regulation of mal gene expression with the altered crp*¹ gene. Expression of the sfs4 gene appeared to be regulated by a two promoters. In this research, the cAMP-CRP dependent regulation of P₁ and P₂ promoter of sfs4 was examined by in vitro transcription using the regulatory region and mutated DNA of the CRP binding region. The transcription of sfs4 from P₂ is stimulated, but P₁ was totally repressed by in vitro transcription. In mutant in the CRP binding region, the effect of activation by cAMP was severely reduced. The transcript of sfs7(nlp) was identified and regulation by cAMP was not observed at the mRNA level.

        Rupture of the main bronchus followed by blunt chest trauma is relatively rare but, with increasing traffic accidents, the incidence has been increasing recently. Early diagnosis and management for the traumatic bronchial rupture is essential to reduce morbidity, mortality and late complication but often, it is comcealed in cases of blunt chest trauma and particularly it is associated with multiple other organ injuires. Especially, bronchial pseudoaneurysm which maintains airway patency after complete transection of the bronchus by blunt chest trauma is very unusual case. We report a case of a 15 year old male who had complete bronchial transection with bronchial pseudoaneurysm.The bronchial rupture was comfirmed by spiral chest CT and 3D reconstruction. In the operation, right main bronchus was completely transected and separated but the airway was patent to the right lung by pseudoaneurysm of the bronchus. Resection of the pseudoaneurysm and end to end anastomosis of the bronchus was performed successfully.

        우리나라 어린이와 청소년의 구강건강을 위한 공공서비스의 현황과 전망

        소아치과학이 목표로 하는 어린이와 청소년의 구강건강을 달성하기 위하여는 민간 치과의료기관에서 내원 환자를 대상으로 하는 진료만으로는 한계가 있으며 공공서비스(公共service)의 역할이 필요하다. 학교구강보건설이 설치된 초등학교는 전체의 7.2%이었고,초등학교 중에서 불소용액양치사업을 수행한 비율은 57.5%, 급식 후 집단잇솔질을 한 비율은 46.9%이었으며,초등학생 중에서 구강보건교육을 받은 비율은 48.0%, 치아홈메우기를 받은 비율은 12.4%이었다(2006년). 0∼6세 영유아의 약 42%가 지난 1년간 구강검진,불소도포,치면세마,치아홈메우기 중 하나 이상의 예방적 치과진료를 받았으며,받은 장소는 교육시설이 약 18%, 보건소가 약 1%이었다(2005년). 불소농도가 조정된 수돗물을 마시는 인구 비율은 전국 평균이 5.7%이었다(2006년). 학교구강보건사업의 발전을 위해서 행정의 일원화,보건교사에 대한 교육,구강보건교사제도의 도입,순회 학교치과의사와 학교치과병원을 통한 포괄적 진료서비스의 제공 등을 고려할 수 있을 것이다. 또한 아동·청소년 주치의제의 도입으로 민간기관을 통해 공공서비스를 강화하는 방안이 검토되고 있다. To achieve the dental health of children and adolescents which is the objective of the pediatric dentistry, the role of the public service should be increased. The basis of the public service is established by many laws of which the Dental Health Law is most important. The percentages of primary schools that had the school dental health clinic, that implemented the fluoride rinsing program, and that implemented the group toothbrushing after lunch were 7.2%, 57.5%, and 46.9%, respectively, and the percentages of primary school children that received the dental health education and that received the fissure sealing were 48.0% and 12.4%, respective1y(2006). About 42% of infants and preschool children from 0 to 6 years received preventive dental care in the last one year, at the nursery or kindergarten(18%) or at the health center(1%)(2005). The percentage of the health centers that implemented water fluohdation was 11.3%, and the percentage of the population who drank the fluoridated water was 5.7 %(2006). It was suggested that the school dental health administration should be unified, that dental health teachers should be employed, and that the comprehensive dental health care should be supplied to all the children and adolescents through the circuit school dentists and the school dental hospitals in the long term. Also, the dentist in charge system for the children and adolescent was suggested.

      • Swanson의 주의력결핍 장애척도의 한국판 표준화

        연구목적 : 한국판 Swanson의 주의력 결핍 장애 척도(이하 SNAP)의 신뢰도와 타당도를 검증하고, 이를 국내 실정에서 주의력결핍 과잉행동장애 환자를 선별할 수 있는 평가용 도구로서 적합한지 여부와 연구에 사용할 수 있는지를 알아보고자 하였다. 방 법 : 1998년 7월 1일부터 2001년 9월 30일까지 영남대학교 의료원 신경정신과 외래를 방문한 주의력결핍 과잉행동장애 환자 392명과 정상대조군 432명을 대상으로 SNAP을 시행하였다. 결 과 : 남녀 간의 SNAP 척도 점수비교에서, 환자군에서는 유의한 차이가 없었으나 정상 대조군에서는 남자가 유의하게 높게 나타났다(p<.01 ∼ .001). 검사-재검사 신뢰도 검증에서 신뢰도 계수는 각 하위 척도에서 .85에서 .92로 모두 만족할 만큼 높았다(p<.05). 내적일관성에 의한 신뢰도 검증에서 정상 및 환자군에서 각 하위척도별로 Cronbach α값이 .80(p.05)이상으로 나타났고, 반분신뢰도는 정상 및 환자군에서 각 하위척도별로 0.731에서 .866사이였다(p<.05). 주의결핍장애 평가 척도-가정관(Attention Deficit Disorder Evaluation Scale-Home Version) 및 소아기 집중력 문제 척도(Childhood Attentin Problem Scale)와의 준거 타당도는 ADHD 환자군에서 .342에서 .713사이였고(p<.05), 정상 대조군에서 .551에서 .789였다(p<.05). 요인분석을 해 본 결과 본 척도는 3개의 요인으로 분류되었는데, 부주의(inattention) 요인, 충동성/과잉행동(impulsivity/hyperactivity)요인, 또래 관계(peer interaction)요인으로 구분되었다. 본 척도가 정상인과 환자를 감별할 수 있는 척도인가를 알아보기 위한 감별 타당도 검사를 해본 결과 87.5%를 분류하여 높게 나타났다. 환자와 정상인을 구분하는 점수인 절단점은 표준편차 1.5를 절단점으로 잡을 때 이는 백분율로 93percentile에 해당하는 점수로서 각 소척도 점수에서 남자(0.98∼1.65)와 여자(0.64∼1.38)의 절단점이 다르게 나타났으며 남자가 높게 나타났다. 결 론 : 이상의 결과에서 볼 때 한국판 SNAP 평가 척도의 신뢰도와 타당도가 입증되었고 환자를 판별하는 능력이 우수하여 향후 ADHD 연구에 유용한 도구로 사료된다. Objectives : The purpose of this study was to examine reliability and validity of Swanson's attention deficit disorder sacale(SNAP). Method : SNAP was administered to 432 elementary school students and 392 patients who were diagnosed as attention deficit hyperactivity disorder in Yeungnam university hospital from 1998 July to 2001 September. Results : In the reliability test, the test-retest reliability coefficient was significantly high and that of inattention was 0.85, that of impulsivity was 0.90, that of hyperactivity was 0.90 and that of peer interaction was 0.92(p<.05). In the reliability test by internal consistency, the Cronbach α coefficient was above 0.80 at each subscale in both patients and normal control group(p<.05). The split-half reliability coefficient by first-second half method showed high correlation and those of each scales were from 0.731 to 0.866 in both patients and normal control group(p<.05). The criterion validity of SNAP with Attention Deficit Disorder Evaluation Scale-Home Version(ADDES-HA) and Childhood Attention Problem Scale(CAP) scale was from 0.342 to 0.713 in patients group, from 0.551 to 0.789 in normal control group(p<0.05). The discriminant ability between ADHD and normal subjects by SNAP was 87.5%. Conclusion : In this point of view, SNAP scale showed high reliability and validity in applying to Korean subjects and was proved to be useful screening tool for attention deficit hyperactivity disorder reserch.

      • 광주전남지역 정상 신생아의 코의 형태와 계측치에 대한 연구

        Background and Objectives: Cleft lip and palate are common defects in congenital deformity. They are the most frequent deformity in the face. Nose deformity is most common among various facial deformity at the center of face. So we must operate to correct the deformity at the earliest stage to give a patient and parent spiritual relief from anxiety, and it is the trend of the treatment recently. We must know the facial anthropometry in newborn to try at the early stage, but there is a lack of literature and research on newborns. So we carried out this research to form base data for operations. Materials and Methods : The noses of 90 newborns in Gwangju-Cheonnam area were analyzed quantitatively, based on 10 nasal measuring points and 13 anthropometric values taken directly from the face of newborns. The relationship between the nasal measurements were studied in 11 proportion indices. Results : Following are our mean values in newborn. Width of the nasal root was 13. 9mm. Width of the nose was 22. lrnm, Width of the columella was 4. 5mm. Height of the nose was 22.6mm. Length of the nasal bridge was 20.1mm. Nasal tip protrusion was 8.9mm. Width of the nostril floor was 6.4mm. Thickness of the nasal ala and length of the nasal ala were 4.0mm, 15.4mm. Length of the columella was 3.9mm. The most common nostril shape was Type II (0-45 degrees in inclination of the nostril axis from the horizontal). Nasal index was 97.8. Columella-nasal tip protrusion length index was 43.9. Conclusion : The development of the columella was relatively incomplete in the case of newborn and was the result to appear. The normal indicies could be used clinically in identifying the it's normality as well as morphologic characteristics of the craniofacial structures. They also could be used in differential diagnosis between the morphologically similar congenital craniofacial anomalies. In preoperative design, in setting the criteria of the postoperative results, and assessing the effect of the operation, Finally, they supply the plastic surgeon and the other specialists in the craniofacial surgery with basic normal population data.

      • sfs1 유전자의 cAMP-cAMP receptor protein에 의한 발현 조절

        crp* 유전자가 도입된 대장균 MK2001(crp, cya::km)을 숙주로 사용하여 mal 유전자 발현을 촉진시키는 유전자의 하나인 sfs1(sugar fermentation stimulation)의 구조해석 결과에 의하면, 잠정적인 sfs1의 promoter영역에는 CRP단백질과의 결합영역으로 보이는 염기배열이 존재하였다. 본 실험에서는 sfs1 유전자의 cAMP-CRP에 의한 발현 조절을 확인하고자, lacZ와의 융합 유전자를 작성하였다. 작성된 융합 유전자는 cya결손주인 TP2010에서 cAMP의 첨가에 의해 B-galactosidase 활성이 크게 증가하였으며, Western blotting의 실험에서도 같은 결과를 나타냈다. in vivo에서 발현이 확인된 전사산물은 cAMP에 의해 전사 촉진이 일어났으며, CRP의 결합부위로 예상되는 DNA영역은 cAMP가 존재하면 CRP단백질과 결합하는 특성을 나타내었다. 이상의 결과로 보아, sfs1 유전자의 발현은 cAMP-CRP에 의한 전사촉진 현상을 받는 것으로 나타났다. We have cloned several E. coli sfs genes which stimulate mal gene expression with ??. One the sfs genes(pPVC2) was sequenced and potential CRP binding site is located in the upstream of the putative promoter in the regulatry region. In order to investigate the regulation of the sfs1 gene by the cAMP-CRP complex, we have constructed the sfs-lacZ fusion gene in this research. The overall transcriptional stimulations of sfs1 gene in the presence cAMP were confirmed by B-galactosidase activity and Western blot analysis of sfs1-lacZ fusion gene. Transcriptional regulation by cAMP-CRP was also confirmed by Northern blot analysis. End-labelled DNA of the DNA fragement in sfs1 regulatory region were used for gel retardation assay to examine the CRP-DNA complex in the presence of cAMP. Results here indicate that CRP binding site in the regulatory region of sfs1 gene is positive regulator for the expression of sfs1 gene.

      • Cellulose 생합성 세균의 분리 및 특성

        발효시켜서 만드는 감식초에서 시료를 채취하여 배양하고, pellicle을 형성하는 single colony를 cellulose생합성균으로 분리하였다. 분리된 균주는 형태적 특성, 알콜의 재산화 등의 생리, 생화학적 특성 등에 의하여 Acetobacter속으로 분류되었으며, Acetobacter CBI-2라고 명명하였다. Acetobacter CBI-2는 정지배양 시에 기존의 생합성 균주로 알려진 Acetobacter xylinum과 대등한 cellulose생합성 능력을 나타내었다. Acetobacter CBI-2이 생성한 고분자물질의 분해산물을 TLC법으로 확인한 결과 기존 cellulose의 것과 일치하였다. Acetobacter CBI-2에서 genomic DNA를 분리 정제하여 cel A 영역을 probe로 하여 hybridization시킨 결과, 상동성을 나타내어 분리 균주에는 cellulose 생합성 관련 유전자가 존재함을 나타내었다.(1997년 1월 7일 접수, 1997년 2월 24일 수리). A screening was performed to isolate the cellulose-producing microorganisms from vinegar in Korea. The isolated strain was identified as Acetobacter sp. with respect to physiological and biochemical characteristics and designated as Acetobacter CBI-2. Cellulose production of Acetobacter CBI-2 was equal with the well known cellulose-producing bacteria, A.xylinum. The result of separation on thin layer chromatography (TLC) was consistent with the degradation product of native cellulose. The presence of genes required for the cellulose biosynthesis in Acetobacter CBI-2 was confirmed by Southern hybrization.

        In this paper, we developed a simulation model of a car parts assembly line to improve the system performance such as worker's utilization balancing, productivity. This simulation model has been developed using QUEST, a true 3D discrete event simulation packcage that is designed for modeling and analysis of manufacturing systems. We have suggested the results obtained to improve the system performances of an existing production line.
