Claudin-1 expression decreases with increasing pathologic grade in actinic keratosis and may be used as a marker of high-risk actinic keratosis

Background: Actinic keratosis (AK) is a common sun-induced skin disorder of importance for the progression to invasive squamous cell carcinoma. However, there is still no good way to predict high risk AK. Objectives: To identify markers which reflect the biologic behavior of AK and to understand the pathogenesis of AK Methods: A total of 52 patients with AK and site-matched 17 normal controls were included. We evaluated solar elastosis and immunohistochemical features using the following antibodies: p53, vitamin D receptor (VDR), claudin-1, and Langerin. Results: Solar elastosis increased and Langerhans cell (LC) density decreased with aging in both patients and controls. Solar elastosis and p53 expression were higher and VDR expression was lower in patients than controls; however, they showed no statistical difference in relation to the pathologic grade of AK. Claudin-1 expression gradually decreased from normal control to severe AK and decreased in the areas with epidermal atypia. LC density in severe AK was significantly lower than in normal control and mild AK; no difference in LC density was seen among control, mild AK and moderate AK. Conclusion: Claudin-1 can be a useful marker of pathologic severity of AK. In contrast, p53 increases and VDR decreases in AK not in gradual manner but in the early steps of carcinogenesis. LC density is relatively maintained in AK until it reaches severe dysplasia.

Atypical pigmented basal cell carcinoma mimicking traumatic pigmented tattoo

Basal Cell Carcinoma, the most common type of skin cancer in humans, usually develops in the sun-exposed area of head and neck. A 63-year-old woman presented with 1-year history of a bean-sized, slightly protruded bluish dermal patch in her right para-nasal area, and it gradually increased in size after she perceived its presence. It looked like traumatic pigmented tattoo because of bluish color and flat patchy features. A punch biopsy was performed, and the lesion was diagnosed as a basal cell carcinoma (BCC). Histopathologically, melanophages were seen scattered between typical basaloid BCC tumor nests in the reticular dermis, making the lesion appear similar to a traumatic pigmented tattoo by Tyndall effect. Typically pigmented BCC appears as a hyperpigmented, translucenet papule which may also have eroded borders. The patient only showed bluish colored patch without eroded borders, making it appear clinically similar to a pigmented tattoo. Herein, we report an interesting case of pigmented BCC which mimicked traumatic pigmented tattoo but was confirmed to be a BCC by biopsy.

Objectives : In this study, the authors evaluated the correlation between eurocognitive impairments and serum lipids levels among Korean elderly over the age of 65. Methods : A total of 609 elderly individuals participated in this study. Screening for cognitive impairments were carried out using the Mini-Mental Status Examination-Korean version (MMSE-KC). There were 197 subjects above 1.5SD Of MMSE- KC and they were evaluated using the Korean versions of the Consortium to Establish a Registry for Alzheimer's Disease (AD) (CERAD-KC) and Geriatric Depression Scale (GDS-K). Results : There was a correlation between old age/low levels of education and AD (p<0.05, P<0.01). There was an inverse correlation between the serum level of total cholesterol and the word list recognition test scores. The serum levels of LDL cholesterol were inversely correlated with the scores on the constructional praxis test, word list recall test and word list recognition test. Inverse correlations between the serum level of triglyceride and scores on the word list recall test and word list recognition test were also observed. Conclusion : There were inverse correlations between the serum levels of lipids and language/memory function in subjects with AD.

혈액투석 환자에게서 발생한 재생불량성 빈혈 1예

Aplastic anemia is a disease characterized by pancytopenia in the peripheral blood and hypocellular bone marrow with the fatty infiltration. The causes of the acquired aplastic anemia were usually related to drugs and immune-related diseases. A 34-years old man was admitted due to general weakness. He has been going hemodialysis for 2 years because of end stage renal disease (ESRD) due to chronic glomerulonephritis. At admission, laboratory tests showed hemoglobin 6.2 g/dL, hematocrit 18.0 %, WBC 5,710mm (neutrophil 71.8%, lymphocyte 19.1%, monocyte 5.9%), reticulocyte 1.9%, platelet 93,000/mm, Fe 12.8 ug/dL, TIBC 204 ug/dL, ferritin 941.47 ng /ml, haptoglobin 0.72 g/L, vitamin B12 508.17 pmol/L, folate 24 ng/mL, total protein 6.54 g/dL, albumin 3.76 g/dL, Alk. pohsphatase 79 IU/L, AST 30.0 IU/L, ALT 39.7 IU/L, total bilirubin 0.57 mg/dl, BUN 79.4 mg/dl and creatinine 10.18 mg/dl. Peripheral blood smear showed the normocytic normochromic anemia with anisocytosis. Anemia was diagnosed but it was irresponsive to the recombinant human erythropoietin (EPO) and blood transfusion. In the bone marrow aspiration smear were there the hypoplastic myeloid and erythroid precursors. The bone marrow biopsy section showed the hypoplasia of all components (10-20%) and the fatty infiltration. We have experienced one case of the idiopathic aplastic anemia in a patient going through hemodialysis and we just report it with documentary records. 저자들은 혈액 투석 중인 환자에서 약물, 면역계 이상등의 원인을 발견할 수 없었음에도 재생 불량성 빈혈이 발생한 1예를 경험하여 문헌 고찰과 함께 보고하는 바이다.

The purpose of this study is to develop an improved summer Gateball wear that will be much more comfortable and convenient than what is currently worn for the game by those between the ages of 60s and 70s. The investigation has been conducted to evaluate current clothing to find out any problems and discomforts related to the current clothing. For accurate data, hands-on investigation and a short interview have been performed. The questionnaires were answered by 230 persons. The result of the questionnaire enables us to design a sample Gateball wear. material, pattern and design have been improved. A sample Gateball wear was evaluated through a wearing test and an appearance test to compare it to current Gateball clothing. The sample gate-ball wear received more positive assessment than the current clothing.

비흑색종 피부암에서 고에너지 X-선 방사선치료

Purpose: To evaluated the efficacy of radiotherapy in the nonmelanoma skin cancer. Materials and Methods: Eleven patients were diagnosed as non-melanoma skin cancer of the head and trunk. There were 9 women and 2 men. The mean age was 70 years with a range of 35 years to 93 years. Seven patients had squamous cell carcinoma and 4 patients had basal cell carcinoma. The mean tumor size was 3.1 cm (range 1 to 8 cm). All patients were treated with 6-megavoltage X-ray radiotherapy. Mean biological effective dose (BED₂) was 50.9 Gy/43 days (range: 37.5 to 59.4 Gy). Median follow-up period was 49 months (range: 18 to 107 months). Results: The local tumor control was 100% and median disease free survival time was 30 months. Local recurrence was noted in 3 patients and distant metastasis in 1 patient. Affecting factor in local recurrence was only radiation dose. By BED₂, median survival time was 30 months for ≥45 Gy and 12 months for <45 Gy (p=0.027). Radiotherapy related chronic complication was observed in 1 patient. Conclusions: High energy X-ray radiotherapy is an effective and safe treatment for nonmelanoma skin cancer, although the number of patients is small.

손마사지가 허혈성 심질환 환자의 불안에 미치는 효과

The purpose of this study was to investigate the effect of a hand massage program on anxiety, vital sign and pain in clients with ischemic heart disease. The design utilized for this stuffy was quail-experimental with a nonequivalent control group non-synchronized design The subjects were fifty-four patients, twenty-eight for the experimental and twenty-six for the control group, who were admitted with ischemic heart diease at a cardiac intensive care unfit in K medical center of K university. This study was carried out from May, 1999 to March, 2000. The level of anxiety and pain measured by Visual Analogue Scale, systolic blood pressure, diastolic blood pressure and pulse rate were measured before and after hand massage, the state of Anxiety was measured by the Spielberger (1970) scale at admission and after hand massage for three days. The collected data were processed by using the SPSS PC program and analyzed using χ2-test and t-test. The result of this study are as follows : 1. The scores of VAS anxiety and State of anxiety of the experimental group were lower than those of the control group. 2. The degree of systolic blond pressure, diastolic blood pressure and pulse rate of the experimental group were lower than that of the control group. 3. The score of VAS pain of the experimental group was lower than that of the control group. The results suggested that hand massage can decrease VAS anxiety, State of anxiety, vital signs and VAS pain of patients who were admitted with ischemic heart disease at cardiac intensive care unit Therefore, It Is proposed that hand massage is an appropriate nursing intervention to relieve anxiety of the patients who were admitted with ischemic heart disease at a cardiac intensive care unit.

정신분열병 환자의 인지기능 상태

목적: 정신분열병 환자의 적응 상태와 증상 형성에 영향을 주는 것으로 생각되는 인지기능 상태를 주의력, 형태 인식, 단어에 대한 구별과 종합 기능을 평가해봄으로써 알아보고자 하였다 방법: 정신분열병 환자 27명과 대조군 27명을 대상으로 하였고, 평가 도구로써 주의력은 K-WAIS의 숫자외우기 문제로, 형태 인식은 K-WAIS의 모양 맞추기 문제와 숨은 그림 찾기로, 단어 기능은 K-WAIS의 공통성 문제와 단어 차이 검사로 각각 이루어졌다. 통계 처리는 SPSS 11.5를 이용하여 Chi-Square test와 Student's t-test로 검증을 하였다. 결과: 정신분열군은 숫자외우기, 숨은 그림 찾기, 모양 맞추기에서는 대조군보다 낮은 점수를 보였다. 단어 차이 검사와 공통성 문제에서는 양군에서 차이를 보이지 않았다. 결론: 본 연구에서 정신분열군의 인지 기능은 정상인 대조군에 비해 주의력과 형태 인식 기능이 유의하게 떨어져 있었으며, 단어들을 구분하고 종합하는 능력은 차이가 없었다. 이런 결과는 임상적으로 정신분열병에서 보이는 망상이 언어적인 측면에서는 어느 정도 전달이 되는 현상과 일치하지만 주의나 형태인지와 같은 기본적인 과정에서 장애가 있다는 추정을 하게 한다. Purpose: This study aimed to investigate the cognitive function in patients with schizophrenia by assessing attention, recognition of a shape, and verbal function. Methods: The subjects were 27 patients with schizophrenia group and 27 control group. Attention was assessed by the digit span task(K-WAIS subtest). The recognition of a shape was assessed by the object assembly task(K-WAIS subtest) and hidden figure task. Verbal function was assessed by Similiarities(K-WAIS subtest) and the different word task. Analysis was performed by the Chi-Square test and Student's t-test via SPSS 11.5. Result: Schizophrenia group had statistically lower scores than control group in digital span, hidden figure and object assembly. There was no difference between two groups in dimensions of difference in words and similarity. Conclusion: This study showed that patients with schizophrenia had impairments in attention and recognition of shape, and no difference between two groups in verbal ability. These explained the partial communicative function of delusion in verbal level. The formation of delusion might be related to the more basic elements such as attention and recognition of shape.
