To maximize the potential for high material removal rates in simultaneous processes such as parallel milling, developing strategies for successful chatter suppression/avoidance is an important concern for manufacturers. In this study, the effectiveness of the spindle speed difference method (SDM) for chatter suppression is discussed in a parallel end-milling process where a flexible workpiece is machined by two tools rotating in opposite direction. The process model is developed, considering that the dynamic variation due to the regenerative eff ect occurs on a plane perpendicular to the tool axis direction. Through the process simulations and the experiments, this study provides informative discussion for comprehending the process behavior. Additionally, a real-time active chatter suppression system with adaptive SDM, where the spindle speed difference is sequentially optimized during the process according to the tracked chatter frequency, is developed by integrating a chatter monitoring system based on sensorless cutting force estimation with sliding discrete Fourier transform. The results show that the developed real-time adaptive system of spindle speed suppresses chatter vibrations more effectively than non-adaptive SDM system; hence, the integrated system can contribute self-optimizing machining systems oriented to Industry 4.0.

Background: IgA nephropathy (IgAN) is the most common form of primary glomerulonephritis worldwide. Tonsillectomy plus steroid pulse therapy has been able to induce clinical remission in early-stage IgAN. However, its possible effect on systemic and local cytokines and tubular markers has not been fully investigated. Methods: We obtained serum and urine samples from 38 patients just before renal biopsy and third steroid pulse therapy. Markers of tubular damage such as N-acetyl-β- D-glucosaminidase, and kidney injury molecule-1 and inflammation such as interleukin (IL)-6, monocyte chemotactic protein (MCP)-1, intercellular adhesion molecule (ICAM)-1, and vascular cell adhesion molecule (VCAM)-1 were measured by immunoassay. Results: Before renal biopsy, only urinary inflammatory markers, except MCP-1, were associated with glomerular (proteinuria) and/or tubular damage markers. Proteinuria, hematuria, and estimated glomerular filtration rate dramatically improved after therapy. In addition, levels of serum IL-6 and ICAM-1 and all urinary markers declined significantly; however, serum MCP-1 and VCAM-1 levels did not. None of the urinary markers correlated with the serum inflammatory markers. Conclusion: Tonsillectomy plus steroid pulse therapy for patients with IgAN might be useful for improving not only glomerular damage marker but also tubular damage markers through the improvement of local renal inflammation. Background To evaluate the expected postoperative total leg length change using preoperative radiographs during surgical planning of four different methods of double level osteotomy (DLO). Methods This study included 34 patients (44 knees) who underwent DLO for varus knee osteoarthritis. Surgical planning was performed so that the postoperative weight bearing line ratio was 62.5%. In DLO, lateral closed or medial open wedge distal femoral osteotomy (LCWDFO, MOWDFO) was performed so that the postoperative mechanical lateral distal femoral angle was 85°, and residual deformity was corrected with medial open or lateral closed wedge high tibial osteotomy (MOWHTO, LCWHTO). Pre- and surgical planning X-rays in the one-leg standing position were compared to assess the change in leg length, and the factors affecting it, in the various surgical groups. The proportion of cases in which Δ total leg length was greater than 6 mm (symptomatic change) was investigated. Results The mean postoperative total leg length increased significantly with LCWDFO + MOWHTO, MOWDFO + MOWHTO, and MOWDFO + LCWHTO, while it decreased with LCWDFO + LCWHTO. The proportion of cases with a postoperative total leg length change > 6 mm was 72.7%, 2.3%, 100%, and 6.8% in LCWDFO + MOWHTO, LCWDFO + LCWHTO, MOWDFO + MOWHTO, and MOWDFO + LCWHTO, respectively. In addition, the preoperative hip-knee-ankle angle correlated negatively with the postoperative total leg length change in LCWDFO + MOWHTO, MOWDFO + MOWHTO, and MOWDFO + LCWHTO, but not in LCWDFO + LCWHTO. Conclusions MOWDFO + MOWHTO had the largest postoperative leg length change and MOWDFO + LCWHTO had the smallest. Symptomatic leg length change (> 6 mm) should be considered in MOWDFO + MOWHTO and LCWDFO + MOWHTO.
Recovery of the Formation and Function of Oxidized G-Quadruplexes by a Pyrene-Modified Guanine Tract

<P>Oxidation is one of the frequent causes of DNA damage, especially to guanine bases. Guanine bases in the G-quadruplex (G4) are sensitive to damage by oxidation, resulting in transformation to 8-oxo-7,8-dihydroguanine (8-oxoG). Because the formation of G4 represses the expression of some cancer-related genes, the presence of 8-oxoG in a G4 sequence might affect G4 formation and induce cancer progression. Thus, oxidized-G4 formation must be controlled using a chemical approach. In the present study, we investigated the effect of introduction of 8-oxoG into a G4 sequence on the formation and function of the G4 structure. The 8-oxoG-containing G4 derived from the promoter region of the human vascular endothelial growth factor (<I>VEGF</I>) gene differed topologically from unoxidized G4. The oxidized <I>VEGF</I> G4 did not act as a replication block and was not stabilized by the G4-binding protein nucleolin. To recover G4 function, we developed an oligonucleotide consisting of a pyrene-modified guanine tract that replaces the oxidized guanine tract and forms stable intermolecular G4s with the other intact guanine tracts. When this oligonucleotide was used, the oxidized G4 stalled replication and was stabilized by nucleolin as with the unmodified G4. This strategy generally enables recovery of the function of any oxidized G4s and therefore has potential for cancer therapy.</P> [FIG OMISSION]</BR>
Development of a New Model Based Air-Fuel Ratio Control System

The second-generation air-fuel ratio control method has been developed to reduce exhaust gas emissions in accordance with the improvements in catalysts. The control system consists of a feedforward control using a fuelbehavior model, a feedback control using an universal exhaust gas oxygen (UEGO) sensor and a feedback controlutilizing the heated exhaust gas oxygen (HEGO) sensor. Experimental results on actual vehicles show the effectivenessof presented method.

From the designs of product and industrial process perspectives, the solubility of a target material is oneof the most valuable qualities. We proposed the ‘solid dispersion melt crystallization (SDMC) concept’ asa original strategy for dispersing a target material in a matrix one, in which the target material is naturallydispersed between the crevices formed by the matrix material precipitatedfirst. Furthermore, the effectsof introducing some operations, such as cooling, agitation and aeration, to the SDMC concept on thedissolution rate were investigated by a binary eutectic system. According to the experimental results, thedissolution rate was enhanced by the operations (cooling, agitation and aeration). This improvement inthe dissolution rate was attributed to the SDMC concept-based enhancement of the nucleation rate. Therefore, it was revealed that an increase in the nucleation rate due to the SDMC concept had effectivelyimproved the dissolution rate.
Risk Factors for Prolonged Hospital Stay after Endoscopy

Background/Aims: The recovery room used after endoscopy has limited capacity, and an efficient flow of the endoscopy unitis desired. We investigated the duration of hospital stay after endoscopy and the risk factors for prolonged hospital stay amongoutpatients. Methods: We retrospectively studied consecutive patients who underwent esophagogastroduodenoscopy or colonoscopy at theToyoshima Endoscopy Clinic. We collected data on age, sex, body weight, midazolam and pethidine dosage, respiratory depressionduring endoscopy, and duration of hospital stay after endoscopy (scope out to check out). Risk factors for prolonged hospital stay (>100minutes) were identified using multiple logistic regression analysis. Results: We enrolled 3,898 patients, including 3,517 (90.2%) patients tested under sedation and 381 (9.8%) patients tested withoutsedation. Overall, 442 (11.3%) patients had prolonged stay (>100 min). The mean time difference between sedation group and nonsedationgroup was 44.2 minutes for esophagogastroduodenoscopy and 39.1 minutes for colonoscopy. Age (odds ratio [OR], 1.025;95% confidence interval [CI], 1.014−1.036), female sex (OR, 1.657; 95% CI, 1.220−2.249), and midazolam dose (OR, 1.019; 95% CI,1.013−1.026) were independently associated with prolonged hospital stay after esophagogastroduodenoscopy, with similar results forcolonoscopy. Conclusions: Old age, female sex, and midazolam dose were independent risk factors for prolonged hospital stay after endoscopy.

Quality control of the crystalline materials obtained from melts with the difficulty of nucleation is challenging. When nucleation is enhanced in such melts, the quality distribution is improved. Herein, the aerationmethod, which enhances nucleation in solution systems, was employed in a melt to investigate theaeration effect on nucleation in a melt and the effect of the crystallization conditions with aeration on theinduction time sind. Experimental results show that aeration enabled nucleation in a melt under supercoolingconditions, where nucleation could not occur without aeration. Furthermore, sind depended onthe operating conditions with aeration (initial supercooling, aeration period, and atmospheric temperature). The crystal size in the obtained solid phase decreased as the atmospheric temperature decreased. These results suggest that the desired nucleation timing and crystal size can be obtained by manipulatingthe crystallizer and aeration conditions. Thus, aeration is promising for improving the quality distributionsin crystalline materials.
