      • Azithromycin으로 치료한 임신 중 쓰쓰가무시병 1예

        We report a case of tsutsugamushi disease in a 26 year-old pregnant woman who was treated with azithromycin. Her gestation period was 27 weeks and she admitted with fever, rash, and eschar on the right shoulder. Currently recommended medications for the treatment of scrub typhus are doxycycline or chloramphenicol. But, these drugs are class D drugs according to the U.S. Food and Drug Administration (FDA) Fetal Risk Summary, so they couldn't be used to treat pregnant women. Recently, a few case reports suggested that azithromycin, a relatively new macrolide antibiotic, was effective and safe for the treatment of scrub typhus in pregnant women. And, there is no evidence that azithromycin causes harmful effects to the developing fetus or to children. On the basis of current in vivo test that confirms the effectiveness of azithromycin, it may be the drug of choice for the treatment of scrub typhus in pregnant women and children.         회전식 nickel-titanium file을 이용한 유치의 근관치료

        유치의 근관치료에서 치수절제술은 실활된 유치를 치열궁내에 유지하기 위해 치관과 근관부분의 괴사된 치수조직을 완전하게 제가하는 것을 말한다. 이 술식은 감염된 치수의 세균을 감소시키는 것을 목적으로 하며, 다양하고 불규칙한 유치 근관의 형태가 주된 장애 요소로 작용한다. 더욱이 비협조적인 어린이의 경우에는 오랜 시간동안 정교한 시술을 하기가 곤란하다. 최근 들어 근관치료시에 회전식 기구를 이용하여 근관형성을 하는 장비들이 많이 개발되었으며 저속전기 모터에 회전식 NiTi file을 이용한 술식이 많이 사용되고 있다. 본 증례에서는 회전식 NiTi file과 전기모터를 이용하여 유구치의 근관치료를 시도하였다. 치수강을 개방하고 치관부 치수를 제거한 후 잠정적 작업장을 측정하였으며 적절한 NiTi file을 선택하여 잠정적 작업장까지 crown-down 방법으로 점차적으로 형성하였다. 수용 file로 근단부 도달 및 성형을 하고 전체적인 마무리를 하여 Vitapex로 충전하여 양호한 결과를 얻었기에 보고하는 바이다. The pulpectomy or root canal treatment on primary teeth is to be done when there is evidence of chronic in flammation or necrosis in the radicular pulp. Due to the tortuous and ribbon-shaped anatomy of the primary teeth, the instrumentation of endodontic hand files and barbed broaches is not an easy procedure. Recently, many automatic Ni-Ti rotary instruments have been developed and has made endodontic treatment easier and faster. This report describes two cases of root canal treatment on primary molars using Ni-Ti rotary files. The cervical constricture was eliminated by the crown-down method, as smaller file proceeded unhindered into the apical third of the canal. In addition, the crown-down technique enhanced the efficacy of the endodontic irrigant. The use of rotary instrumentation for primary teeth seemed to be a more effective way to debride the uneven walls of primary teeth.

      • 국내 우유용기 생산 및 사용실태 조사·분석

        Up to now all carton packs used for milk container in our country have been imported abroad. With rapid increase of interest on environment and waste recycling, Environmental Societies and Civil Consumer's Organization restart the Use of Glass Bottle for Milk Movement. From preliminary survey result that glass bottle had many advantages in most test items, we chose the glass bottle as substitute for carton pack, and proposed some regulations such as usable range and detailed regulation, etc. Because at this time it is impossible to rebuild whole pack manufacturing process to substitute carton pack to glass bottle, it may be more favorable to use glass bottle for milk packing under some restricted condition such as easy recollection and reusable circumstances. In addition some governmental regulations to promote glass bottle milk productions and sale and to return all used glass bottles are absolutely required in group catering system. If above is impossible, it will be next choice to set up regulations of milk packing as well as to use glass bottle at some position, and adopt a consumer deposit system in order to increase glass bottle usage, which will help environmental protection and economy.

        저자는 과거 20년에 약 320예의 척추결핵환자를 치험한 바 있으며 이를 통해 얻은 약간의 경험을 여기 논술하였다. 저자는 이병의 완전한 치유를 확보하고 변형 및 신경증상의 유발등 후유증의 발현을 막기 위해 조기진단 및 치료를 강조하면서 아울러 감별진단상의 문제점 저자가 개발한 Costotransversetomy)의 변법의 장단점 요추결핵에 있어서의 이단계수술의 권장 치료실패의 원인 후만증의 문제 척추 각 부위에 있어서의 특이성과 치료상의 고려점등 칙추결핵 치료상의 여러 문제점을 고찰 논술하였다. Author treated about 320 cases of spinal tuberculosis personally in the past 20years (1960-1979) and following is the short summary of the observations made through these experiences. 1.Only direct surgical approach to the spinal lesion establishes definite diagnosis. In differential dagnosis, osteomyelitis of the spine should be considered first. The incidence of the osteomyelitis was about 8% of that of tuberculous spine. 2.Author's modified costo-transversectomy approach offers very useful route for the thoracic and some thoraco-lumber lesion, particularly in children, older age and debiliated adults. It provides enough room for thorough de´bridement and fusion while giving the least stress to the patient because of its extrapleural approach. It offers also good access to spinal cord and also make it possible to combine with the posterior fusion in one setting if desired. 3.In the lumber region, when bilateral psoas abscess is present, bilateral approach in two stages is a thorough way of de´bridement and fusion particularly in adults in order to ensure the complete cure of the lesion. 4.In extensive lesion, anterior curetting with graft supplemented with posterior fusion offers best stability of spine to prevent further progression of the kyphosis. 5.The selection of the sensitive antibiotics and its regular administration is essential and indispensable pre-requisite for the successful outcome of the treatment. 6.Early eradication of the lesion and stabilization preferably through combined anterior and posterior route of the best chance of cure and the least residual deformity and disability.

      • Silicone rubber 멤브레인을 이용한 압저항형 저차압센서의 개발

        A 4-beam piezoresistive differential pressure sensor using a silicone rubber membrane has been fabricated on the selectively diffused (100)-oriented n/n^(+)/n silicon substrates by an unique silicon micromachining technique using porous silicon etching. The width, length, and thickness of the beam is 120 μn, 600 μm, and 6 μm, respectively, and the thickness of the silicone rubber membrane was 36 gm. By the use of four-beam structure, the mechanical strength of the differential pressure sensor can be highly improved due to smaller shear stress. And the lower sensitivity of the sensor can be simply solved by combining two output signals of half-bridge. The effectiveness of the sensor is confirmed through an experiment and FEM simulation in which the differential pressure sensor is characterized.

      • 충청북도 단양군 일대의 하부고생대층 코노돈트화석

        이 논문은 충청북도 단양군 어의곡리 지역에 분포하는 하부 고생대층에서 산출된 코노돈트미화석을 채취하여 분류, 기재하고 분류된 코노돈트화석군을 이용하여 연구 대상지역의 하부고생대층의 지질시대를 규명하고는데 그 목적이 있다. 이 연구를 위해 어의곡리 단면을 선정하여 이 단면에서 총 23개의 표품을 채취한 결과 총27개체의 코노돈트화석이 산출되었고, 이는 총3속 3종으로 분류되었다. 연구대상 지층인 어의곡리 단면에서는 Cordylodus proavus, Cambrooistodus cambricus, Glyptoconus bassleri. Proconodontus sp.가 산출되었다. 이와 같은 코노돈트화석군을 기초로하여 북미, 유럽, 북중국,호주 등의 화석군과 대비해보면 어의곡리 단면의 지질시대는 상부 캄브리아기에서 하부 오오도비스기에 이르는 것으로 밝혀졌다. Samples from the lower Paleozoic Sequence were collected from the one measured section in the Danyang Area, Chungchungbuk-do, Korea. A total of 27 identiable conodonts were recovered from 23 samples They are clssified into 3 form species referable to 3 genera. Conodonts found in this section are Cordylodus proavus, Cambrooistodus cambricus, Glyptoconus bassleri. Proconodontus sp. Conodont fauna from the Aekogri section are correlated with those from the upper Cambrian to early Ordovician conodont fauna of North America, northern Europe, China.

      • 강원도 삼척시 일대에 분포하는 하부 고생대층의 Conodont

        본 연구의 목적은 강원도 삼척시 서부 지역에 분포하는 하부 고생대층의 코노돈트 연구를 통하여 이들의 지질시대를 규명하고, 색변화지수를 이용하여 이들 지층의 변질온도를 밝히는데 있다. 코노돈트 미화석 연구를 위하여 삼척군 내미노리 지역과 무사리 지역에서 22개의 시료를 채취하였으며, 이 중 10개의 시료에서 총 64개체가 산출되었다. 이들 화석은 13속 16종로 분류하였으며, 각 층에서 산출된 대표적인 종은 다음과 같다. Teridontodus sp. Rossodus manitouensis, Oneotodus sp, Acanthodus sp, Teridontodus nakamurai, Serratognathus bilobatus, Scolopodus kelpi, Rhipidognathus lawuensis, Aurilobodus leptosomatus, Tangshanodus tanshanensis등이다. 산출된 두무골층의 코노돈트와 북미 지역의 코노돈트를 대비한 결과 두무골층의 지질시대는 북미지역의 코노돈트 C대 상부에서 D대 중부까지며, 이는 유럽의 Tremadocian의 상부에 해당한다. 막골층의 코노돈트는 북 중국의 Liangchiashan층과 하부 Machiagou층의 코노돈트 화석군과, 북미지역의 Fauna 1에서 Fauna 3에 각각 대비된다. 이는 Arenigian 하부에서 Llanvirnian 최 하부에 해당한다. 또한 산출된 코노돈트 미화석의 색깔은 암흑색에서 투명백색으로 색변화지수는 5~8에 해당하며, 변질온도는 약 300℃ 이상으로 추정된다. This study is to clarify the geologic time and temperature of the metamorphism of the Lower Paleozoic Sequence using conodont fauna from the Samcheock Area. A total of 64 identifiable conodonts were recovered from 22 samples collected from 2 columnar sections in the Samcheck Area. Kaneweondo. They are classified into 16 species referable to 13 genera. The representative species of these are Teridontodus sp. Rossodus manitouensis, Oneotodus sp, Acanthodus sp, Teridontodus nakamurai, Serratognathus bilobatus, Scolopodus kelpi, Rhipidognathus lawuensis, Aurilobodus leptosomatus, tangshanodus tanshanensis. Conodont fauna of the Dumugol Formation is correlated with those of from the Fauna C to Fauna D of North America. This is from late Tremadocian to early Arenigian in age. Conodont fauna of the Maggol Formation is correlated with those of from the Fauna 1 to Fauna 3 of North America, those of Liangchiashan Formation too lower Machiagou Formation of North China. This is from late Arenigian to Llanvirnian in age. Conodont is dark grey to white in color. 5-6 of CAI value, it indicates over 300 ℃ inmetamorphic temperature.

      • LC/MS/MS 분석법을 이용한 Amlodipine의 약물동태연구

        The aim of this study were to confirm the analysis method and also to estimate the pharmacokinetic parameters of amlodipine in human volunteers. In an open-label single-dose pharmacokinetic study, a group, consisting of 24 healthy volunteers, received single oral dose of 5mg amlodipine. Blood sample were taken for up to 120 hours. The concentration of amlodipine in these body fluids was determinated using a validated high-performance liquid chromatography(HPLC) method with tandem mass spectrometry. Amlodipine and ketoconazole, an internal standard, were extracted from plasma using ethyl acetate in the presence of 0.1M sodium carbonate. After drying the organic layer, the residue was reconstituted in mobile phase(acetonitrile : water = 70 : 30 v/v (0.1% formic acid)) and injected onto a Zorvax C18 column (2.1 × 100 mm, 3.5 ㎛ particles). The isocratic mobile phase was eluted at 0.2ml/min. The ion transitions monitored in multiple reaction-monitoring mode were m/z 410.10 → 294.95 and 532.11 → 81.95, respectively. The coefficient of variance of the assay precision was less than 12%, and the accuracy exceeded 99.1%. The limit of quantification was 0.1 ng/ml. The pharmacokinetic parameters were calculated with non-compartmental(AUC, C_(max), T_(max), CL_(t), V/F) and compartmental(K_(el), K_(a), t_(lag)) pharmacokinetic analysis using WinNonlin program. The estimated means of AUC_(0-120hr), C_(max) and T_(max) were 196.90 ± 5.02 ng·hr/ml, 3.36 ± 0.09 ng/ml and 10.44 ± 0.61 hr, respectively. The means of other pharmacokinetic parameters(V/F, CL_(t), K_(el), K_(a) and t_(lag)) were 1208.06 ± 50.61 L, 25.39 ± 0.65 L/hr, 0.2806 ± 0.0294 hr^(-1), 0.0210 ± 0.0008 hr-1 and 0.4574 ± 0.0635 hours, respectively.
