        난치성 백반증에 대한 흡입물집을 이용한 비배양 표피세포이식술 20례

        Background: As nonsurgical interventions for vitiligo are not always successful, various surgical modalities have been used in patients with refractory vitiligo. Of these, non-cultured epidermal suspension transplantation (NCES) was recently introduced to treat large recipient sites using cells from small donor tissue. Objective: We assessed the effectiveness and safety of NCES as a surgical treatment for patients with refractory vitiligo. Methods: We retrospectively reviewed 20 cases in 17 patients (11 females; median age 25 years) who underwent NCES from July 2015 through March 2018. Suction blisters (20 mm in diameter) were collected from the patient’s inner thigh at a donor-to-recipient area ratio of 1:5. After the addition of 5 mL recombinant trypsin solution to the suction blisters, followed by incubation at 37°C for 60 min, epidermal cells were manually scraped off the blister surface, and epidermal cell suspension was obtained by centrifugation at 1,500 RPM for 5 min. The suspension was applied to the vitiligo regions after epidermal ablation of those regions. Phototherapy resumed 1 month later. Treatment success was defined as ≥75% repigmentation of the surgical site, and all adverse events were noted. Results: Overall, 85.0% of cases (17/20) exhibited treatment success. Adverse events included hyperpigmentation (20%) and surgical site infection (5%), but the treatment was tolerable in all cases. Conclusion: NCES is a reliable surgical option for patients with vitiligo refractory to nonsurgical treatment. Large areas of vitiligo can be treated by NCES, and use of this technique should be encouraged in Korea. (Korean J Dermatol 2018;56(7):426∼432)

        The relationship between aerobic capacity and the recruitment of activated sweat gland density with passive heating

        This study investigated whether the recruitment of activated sweat gland density during passive heating [immersion of the lower body into hot water, 41℃ for 30 min (PH), room temperature 23 ± 0.5℃ with 60 ± 3% relative humidity] is different in endurance-trained (Trainee), compared with that of untrained subjects (Control), as indicated by VO2max. Eight trainees and seven control female subjects (38.31 ± 4.91 ml·kg-1·min-1 and 31.8 ± 0.92 ml·kg-1·min-1, respectively, p < 0.01) were similar on all other physical characteristics. To compare the changes between the trainees and controls, serum cortisol and prolactin (PRL), tympanic temperature (TYMP), local activated sweat gland density (L-ASGD) and the mean whole body sweat loss volume (M-WBSLV) changes were measured after PH. The cortisol was significantly higher (p < 0.05) and the PRL tended to be higher in the Trainee than in the Control. The TYMP increased more in the Trainee than in the Control (p < 0.001). The L-ASGD showed a higher tendency in the Trainee than in the Control, there was a significant difference in the mean activated sweat gland density (p < 0.01) and the L-ASGD was significantly correlated with the VO2max (p < 0.001). The M-WBSLV was significantly higher in the Trainee (p < 0.001) and significantly correlated with the VO2max (p < 0.001). The results suggest that in humans, a high aerobic capacity is associated with a greater recruitment of activated sweat glands, whole body loss of volume and TYMP. Therefore, the decline in the heat loss responses, due to decrease in maximal oxygen consumption, may be masked by repeated endurance exercise training.

        벤조디아제핀 수용체 영상용 양전자 방출 핵종 표지 플루마제닐 유도체 [F-18](3-(2-Fluoro)flumazenil의 합성과 생체 내 분포

        Purpose: Radiotracers that bind to the central benzodiazepine receptor are useful for the investigation of various neurological and psychiatric diseases. [C-11]Flumazenil, a benzodiazepine antagonist, is the most widely used radioligand for central benzodiazepine receptor imaging by PET. We synthesized 3-(2-[F- 18]fluoro)flumazenil, a new fluorine-18 (t1/2=110 min) labeled analogue of benzodiazepine receptor imaging agent, and evaluated in vivo for biodistribution in mice. Materials and Methods: Flumazenil (Ro 15-1788) was synthesized by a modification of the reported method. Precursor of 3-(2-[F-18]fluoro)flumazenil, the tosylated flumazenil derivative was prepared by the tosylation of the ethyl ester by ditosylethane. [F-18] labeling of tosyl substitued flumazenil precursor was performed by adding F-18 ion at 85℃ in the hot cell for 20 min. The reaction mixture was trapped by C18 cartridge, washed with 10% ethanol, and eluted by 40% ethanol. Bidistribution in mice was determined after intravenous injection Results: The total chemical yield of tosylated flumazenil derivative was ∼40%. The efficiency of labeling 3-(2-[F-18]fluoro)flumazenil was 66% with a total synthesis time of 50 min. Brain uptakes of 3-(2-[F-18]fluoro)flumazenil at 10, 30, 60 min after injection, were 2.5±0.37, 2.2±0.26, 2.1±0.11 and blood activities were 3.7±0.43, 3.3±0.07, 3.3±0.09%ID/g, respectively. Conclusion: We synthesized a tosylated flumazenil derivative which was successfully labeled with no-carrier-added F-18 by nucleophilic substitution.

      • 덕동댐 유역의 유출해석에 관한 연구

        Four different methods. Clark area routing method. SCS TR-55, Nakayasu synthetic hint hydrograph, and kajiyama formula were used to calculate the desi후 flood for Deok Dong reservoir watershed located in Kyung Ju. The results obtained using Clark area routing method were determined as the design flood for the watershed. The recurrence interval of 500 years was selected for the watershed. The design runoff hydrographs were routed through reservoir using the Puls method. The actual storm events which were relatively large in their magnitudes were also analyzed. The outflow calculated in this study showed considerable differences from results available in the literatures. The several errors that can not be ignored found in the literatures were indicated and explained to provide more accurate results for the watershed. Some basic informations useful for the prevention of disaster due to heavy storms were proposed.

        Half-loc has been investigated for an alternative material of natural stone consists of filter layer of rubble mound breakwater. In this study, the load on the Half-loc combination gave been set for measuring the load. The data show 1) 40ton Tetrapod of 2-layer placement gives load of about 40ton on a Half-loc 2) the normal load of a slope have bigger than load on plane flat calculated by arithmetic average method.

      • 방파제의 신뢰성 설계에 의한 중량식의 비교

        Hanzawa et a1 (1996)calculated damages of armor blocks using Monte-Carlo simulation for horizontally composite breakwaters. In their calculation, one dimensional wave breaking model by Goda(1975) was used. But in the real sea area, incident waves have variability of wave directions including directional spreading parameter, refraction and deviation of mean wave directions. In this study, reliability based design was executed using three dimensional random wave breaking model by Kweon(1998) instead of one dimensional wave breaking model by Goda(1975). As a result, considering variability of wave directions, thedamages of armor blocks are calculated to be smaller than those of Hanzawa et a1 (1996). The damages of armor blocks calculated using van der Meer(1988b) stability formula for Tetrapods also show similar results. Reliability based design for breakwater armor blocks has been executed on the assumption of unidirectional waves. But, if variability of wave directions is considered, the economic design can be expected.

        The Stamey bladder neck suspension have been performed for female stress incontinence in Soonchunhyang University Hospital from January 1990 to May 1994. This procedure was introduced by Stamey, varying degrees of the success have been reported ranging from 61% to 91% in contemporary series. Someone reported the high recurrence rate with long-term follow-up. Among the 36 patients who received Stamey procedure, 28 patients(78%) receive anterior colporrhaphy for concomitant cystocele simultaneously. The mean follow-up period was 41.4±8.6months, the patient's data were gathered by chart review and the subjective success rate was obtained by questionnaire survey. Urinary retention was the major complication in 8(22.2%),de novo urgency occurred in 4(11%), protracted suprapubic pain occurred in 7(19%), The 20(56%) of the 36 patients were complete dry and 10(28%) were improved. The changes of success rate related to the time of follow-up were; 94.4% immediate after surgery, 88.9% at 6 month, 86.1% at 12 month, 83.3% at 1-5 year. The majority of the failure occurred in the early postoperative period. The 27(90%) of the 30 patients success group received anterior colporrhaphy for concomitant cystocele simultaneously during the Stamey operation. Our success rate was slightly higher than contemporary series, it was suggested that supporting the midurethral segment by anterior colporrhaphy with bladder neck suspension might be the contributing factor for increasing the success rate, but more follow up is warranted.

        Body temperature is controlled by thermoregulatory center of the hypothalamus. We report a case of 24 years old man with central fever. He was subjected to a partial excision of pituitary tumor compressing optic chiasm four years ago. He has received hormonal therapy for panhypothyroidism after removal of pituitary tumor. And He received gamma knife operation for partially contrast-enhancing masses in suprasellar and both hypothalamic areas that is probably postoperatively remnant or recurrent tumor of pituitary adenoma. One year after gamma knife operation, he presented with a febrile syndrome of unknown origin including rhabdomyolysis. All usual investigations proved negative. We diagnosed him as central fever with rhabdomyolysis. He received medical ICU care with cooling bed, ice pack. And his symptom improved. Postoperative hyperthermia may result following resection of the pituitary tumor. When central fever is suspected taking note of past history, a quick recognition of course of fever can help reduce the using of unnecessary antibiotics and hospital stay.

        한국무속의 '신병'에 대한 진단분류연구

        To study nosologically the Korean shaman's initiation sickness, 'Shin-Byong', the authors investigated 29 shamans with Symptom Check List-90-R(SCL-90-R) and questionnaire without visiting and interviewed 70 shamans using SCL-90-R, questionnaire and The Schedule for Affective Disorder and Schizophrenia-Lifetime(SADS-L). All shamans were located in Seoul. The results were as follows : 1) Among symptom groups of SCL-90-R, 'Shin-Byong' were associated with higher scores in depression, somatization, psychoticism and hostility group in descending order. 2) With DSM-Ⅲ-R classification, 87.2% of 'Shin-Byong' cases were mood disorders, Another 8.6% of cases were somatization and dissociative disorders and two disorders were related with dissociative phenomenon. Considering the diagnosis of manic episodes in this study, it seems to be a part of dissociative phenomenon, 'Shin-Byong' is the syndrome that is mainly composed of depression and disorders, which are related with dissociative phenomenon. 3) Shamans have teleologic thinking that 'Shin-Byong' is the phenomenon to receive spirits and diagnose intuitively. They think the main therapeutic method is recieving spirits, In conclusion, 'Shin-Byong' isn't a real disease entity but it is a shamanistic disease concept transformed during the shamanistic therapeutic process for depression patients with high dissociative tendency and dissociative disorder.

        Livestock manure is an important source of ammonia emissions globally, contributing to poor air quality through the formation of secondary inorganic aerosols. As per capita meat consumption increases, concentrated animal feeding operations (CAFOs) have become increasingly common, leading to large quantities of excreta that need to be treated in dedicated manure processing facilities. However, little is known about the scale of gaseous emissions from these facilities, and the potential impact they have on air quality and the surrounding environment. This paper presents NH<SUB>3</SUB>, SO<SUB>2</SUB>, NO<sub>x</sub> and PM<SUB>2.5</SUB> monitoring data from a modern commercial manure composting facility located in Paju, South Korea. During the five-week monitoring period during June – July 2020, average internal concentrations of NH<SUB>3</SUB>, SO<SUB>2</SUB>, NO, NO<SUB>2</SUB> and PM<SUB>2.5</SUB> were found to be 57.1 ± 1.3 mg m<sup>-3</sup>, 307.2 ± 17.4 μg m<sup>-3</sup>, 33.4 ± 3.2 μg m<sup>-3</sup>, 7.3 ± 0.8 μg m<sup>-3</sup>, and 36.9 ± 2.6 μg m<sup>-3</sup>, respectively. The NH<SUB>3</SUB> emission rate was calculated to be 7.28 ± 0.41 NH<SUB>3</SUB>-N mg min<sup>-1</sup> m<sup>-3</sup>, with scaled emissions rates of 1.85 ± 0.1 NH<SUB>3</SUB>-N g h<sup>-1</sup> m<sup>-3</sup> of manure compost, or 1.52 ± 0.09 NH<SUB>3</SUB>-N g kg<sup>-1</sup> of manure compost over the full 18-day composting period. The evidence suggests that the high-volume ventilation and scrubber system, maintaining a negative pressure environment inside the facility, was highly effective at limiting external emissions, and can be recommended as a suitable technology to reduce pollution from manure composting facilities.
