        백서두개골 결손부에서 BMP전달체의 골재생효과분석

        Bone morphogenetic proteins have been shown to possess significant osteoinSductive potential, but in order to take advantage of this effect for tissue engineering, carrier systems are essential. Successful carrier systems must enable vascular and cellular invasion, allowing BMP to act as a differentiation factor. The carrier should be reproducible, non-immunogenic, moldable, and space-providing, to define the contours of the resulting bone. The purpose of this study was to review available literature, in comparing various carriers of BMP on rat calvarial defect model. The following conclusions were deduced. 1. Bone regeneration of ACS/BMP, ${\beta}-TCP/BMP$, FFSS/BMP, $FFSS/{\beta}-TCP/BMP$, MBCP/BMP group were significantly greater than the control groups. 2. Bone density in the ACS/BMP group was greater than that in ${\beta}-TCP$, FFSS, $FFSS/{\beta}-TCP$ carrier group. 3. Bone regeneration in FFSS/BMP group was less than in ACS/BMP, ${\beta}-TCP/BMP$, MBCP/BMP group. However, New bone area of $FFSS/{\beta}-TCP/BMP$ carrier group were more greater than that of FFSS/BMP group. ACS, ${\beta}-TCP$, FFSS, $FFSS/{\beta}-TCP$, MBCP were used for carrier of BMP. However, an ideal carrier which was reproducible, non-immunogenic, moldable, and space-providing did not exist. Therefore, further investigation are required in developing a new carrier system.

        한국적 특수성을 고려한 지역특성과 개인의 건강

        Recently much attention has been paid to the effect of neighborhood characteristics on the health of individuals, independent of individual demographic and/or socioeconomic characteristics. Although many empirical studies of a kind, mostly based on Western society, have appeared on various international journals, few studies have shown empirical evidence of neighborhood characteristics as an independent and significant risk factor of ill health in Korea. This paper discusses possible reasons that neighborhood seems to be neither significant nor substantial regarding its impact on the health of Koreans. Addressing the uniqueness of Korean society regarding the concept of neighborhood, attributes of neighborhoods, and methodological challenges, authors suggest that more sophisticated conceptual and methodological approach, specific to Korean society, should enable to reveal the effect of neighborhood characteristics on individual health in Korea.

      • 플랜트 하부의 강파일 인접 토양의 부식성 평가

        플랜트 하부 강파일 기초 주변 토양의 부식성을 평가하기 위해서 현장조사, 현장 보링작업을 통한 시료 채취 및 실험실 분석을 거친 결과를 검토하였다. 토양 분석 결과 강관파일이 타설된 토양은 여러 개의 특성이 다른 토양층으로 구성되어 있었으며, 각 지층의 화학적, 물리적 성질이 다르므로 강관파일의 타설깊이에 따른 부식성의 차이가 발생할 것을 예상할 수 있었다. 분석 결과를 토대로 정성적인 부식성 평가, 평균부식속도 및 최대부식속도를 예측한 결과, 일부 지층에서는 일반적으로 강관파일 설계시 적용되는 부식속도인 0.01-0.02mm/y 이상의 평균부식속도를 보이고 있었다 (0.33mm/y). The corrosivity evaluations on soil surrounding the buried steel piles under manufacturing plants. It was proven that the soil structure of this sample area is composed of several soil layers, and that each layer show the different corrosivity. The predicted average corrosion rates of some layers are about 0.33mm/y, which is over than the general corrosion rate, 0.01-0.02mm/y.

        Protein transduction domain을 이용한 recombinant human bone morphogenetic protein-2의 골재생효과

        Bone morphogenetic proteins(BMPs) are regarded as members of the transforming growth $factor-{\beta}$ superfamily with characteristic features in their amino acid sequences. A number of studies have demonstrated the biologic activities of BMPs, which include the induction of cartilage and bone formation. Recently there was a attempt to overcome a limitation of mass production, and economical efficieny of rh-BMPs. The method producing PTD by using bacteria have advantages of acquiry a mass of proteins. Hences, a new treatment which deliver protein employed by protein transduction domain(PTD) has been tried. The purpose of this study was to evaluate the bone regenerative effect of TATBMP-2 and TAT-HA2-BMP-2 employed by PTD from HlV-1 TAT protein for protein translocation in the rat calvarial model. An 8mm calvarial, critical size osteotomy defect was created in each of 32 male Spraque-Dawley rats(weight $250{\sim}300g$). The animals were divided into 4 groups of 32 animals each (4 animals/group/healing interval). The defect was treated with TATBMP-2/ACS(Absorbable collagen sponge) (TATBMP-2 0.1mg/ml), TAT-HA2-BMP-2/ACS(TAT-HA2-BMP-2 0.1mg/ml), ACS alone or left untreated for surgical control(negative control). The rats were sacrificed at 2 or 8 weeks postsurgery, and the results were evaluated histologically. The results were as follows: New bone formation were not significantly greater in the TATBMP-2/ACS group relative to negative, and positive control groups. New bone was evident at the defect sites in TAT-HA2-BMP-2/ACS group relative to negative, positive control and TATBMP-2 groups. There were a little bone regeneration in TATBMP-2 groups. While, enhanced local bone formation were observed in TAT-HA2-BMP-2 group. But, The results was not the same in all rat defects. Therefore, further investigations are required to develop a method. which disperse homogenously, and adhere to target cells.

        There is a new evolution in technological advancement taking place called the Internet of Things (IoT), The IoT enables physical world objects in our surrounding to be connected to the Internet. ISM (Industrial Scientific Medical) band that is 2.4GHz band authorized free of charge is being widely used for smart devices. Accordingly studies have been continuously conducted on the possibility of coexistence among nodes using ISM band. In particular, the interference of IEEE 802.11b based Wi-Fi devices using overlapping channel during communication among IEEE 802.15.4 based wireless sensor nodes suitable for low-power, low-speed communication using ISM band. Because serious network performance deterioration of wireless sensor networks. In this paper, we will propose an algorithm that identifies the possibility of using more accurate channels by mixing utilization of interference signal and RSSI (Received Signal Strength Indicator) Min/Max/Activity of Interference signal by wireless sensor nodes. In addition, it will verify our algorithm by using OPNET Network verification simulator.

      • 외측광근을 이용한 대전자부 피부결손의 치료

        Purpose: We performed vastus lateralis (VL) muslce or myocutaneous flap for close of the trochanteric skin defect usually happened in paraplegia and report our 6 cases. Materials and Methods: Between March 2004 and August 2005 we performed 4 cases of VL muscule flap with skin graft and 2 cases of VL myocutaneous flap for close of the trochanteric skin defect in 6 paraplegia patients. There were 5 men and 1 woman and mean age was 52.2 years and mean diameters of skin defect was $8.3{\times}8.3\;cm$. The mean follow up period was 18 months. The survival of flap, complications, healing time and patient's satisfaction were evaluated. Results: All flaps were survived except 1 case of margin necrosis. In 2 cases, blood-serous discharges were continued after operation which might be due to dead space and treated with $2{\sim}3$ times debridement and delayed close. Mean time to heal the skin defect was 7.6 weeks. No infection and recurrence in follow up periods. Cosmetic results judged by patients are that 3 cases are good and 3 cases are fair. Conclusion: VL muscle and myocutaneous flap is good treatment method among the numerous methods in treatment-failed cases of trochanteric skin defects of paraplegia patients. This surgical procedure is simple, constant blood supply, good pliability, cosmetic results and also appliable to other skin defect of pelvis girdle like sacrum and ischium.

        신기능 저하 환자에서 위막성 대장염 발생의 임상적 특징

        목적: Clostridium difficile-associated diarrhea (CDAD)는 항생제 사용 후 장내세균 변화로 인해 C. difficile 독소 A에 의해 발생되는 위막성 대장염에 의한 것으로 알려져 있다. 본 연구는 대장내시경 후 조직검사를 통해 위막성 대장염으로 확진된 CDAD 환자에서 임상적 및 검사실적 소견의 분석을 통해 신기능의 저하가 CDAD 발생에 어떤 영향을 미치는지 알아보고자 하였다. 방법: 1998년 1월부터 2006년 12월까지 원광대학교 의과대학 병원에서 2일 이상 묽은 변과 고열이 발생한 환자에서 CDAD가 의심되어 C. difficile 독소 A를 시행하고 대장내시경을 통해 조직학적으로 CDCA로 확진된 환자 56명을 대상으로 신기능 저하, 당뇨 및 악성종양의 유무를 조사하여 항생제 투여 기간에 따른 CDAD 발생 빈도를 비교하였다. 결과: 연구 기간 동안 CDAD가 의심되어 C. difficile 독소 A를 검사한 환자는 452명이었으며, 이중 양성 환자 416명 (92.03%), 음성 환자 36명 (7.96%)이었다. 452명 중 대장내시경을 시행한 환자는 92명 (20.7%)이었고, 이중 조직검사에서 위막성 대장염으로 진단된 환자는 56명 (60.8%)이었다. 조직 검사로 진단된 위막성 대장염 환자 56명 (남자 26명; 여자 30명)이었고, 이중 신기능 저하 환자 8명 (14.2%), 당뇨환자 10명 (17.8%), 악성종양 환자 13명 (23.2%)이었다. 또한 독소 A가 음성인 환자는 36명 (64.3%)었다. 조직검사로 확진된 신기능 저하 환자 8명 중 5명 (62.5%)은 C. difficile 독소 A 검사에서 음성으로 신기능 저하 유무에 따른 독소 반응의 차이는 없었다 (62.5% vs. 64.6%). 신기능이 저하된 환자에서 당뇨 2명 (25%), 악성종양 환자 1명 (12.5%)이었다. CDAD 발생까지의 항생제 투여 기간은 신기능이 정상인 환자 보다 신기능이 저하된 환자에서 짧았고 (23.44±3.28일 vs. 11.13±2.386일, p=0.034), 당뇨 및 악성 종양 유무에 따른 비교에서 (20.85±2.221일 vs. 25.50±13.07일; 23.30±3.67일 vs. 16.31±2.31일) 차이가 없었다. 결론: 면역력이 저하된 당뇨 및 악성종양 환자에 비해 신기능이 저하된 환자에서 CDAD 발생까지의 항생제 투여 일수가 적어 신기능 저하 상태에서 특히 항생제 투여에 주의가 필요하다. 또한 항생제 투여 후 발생된 설사 환자에서 C. difficile 독소 A가 음성이라 할지라도 환자가 대장내시경을 시행할 수 있는 상태인 경우 반드시 대장내시경과 조직검사를 시행하여 CDAD를 감별해야 할 것으로 생각한다. Purpose: Clostridium difficile-associated diarrhea (CDAD) is a potentially life-threatening illness which has been shown to be more common and more severe in patient with chronic renal failure. The aim of this study was to investigate clinical characteristics of renal insufficiency patients with clostridium difficile-associated pseudomembranous colitis. Methods: We reviewed charts of fifty-six patients with clostridium difficile-associated pseudomembranous colitis, who have clostridial toxin A assay in stool and a diagnosis made on histology of colonic biopsies. Results: There was no difference in age, serum albumin, C-reactive protein (CRP) and negative incidence of clostridial toxin A between patients who had renal insufficiency with serum creatinine more than 1.5 mg/dL and those who did not. But duration of antibiotic use administered prior to development of the clostridium difficile infection was more shorter in patients with impaired renal function than in patients with normal renal function. Conclusion: These data suggest that it may take a short period to development of the clostridium difficile infection in patients with impaired renal function, and histologic evaluation by sigmoidoscopy should be performed to make a diagnosis in CDAD-suggested patients, who have impaired renal function and even negative clostridial toxin A.

        Depression is the second most common chronic disease in primary care. However, depression is commonly misdiagnosed, or though diagnosed properly, it is easy to treat inadequate doses or duration of antidepressants. It is of importance to diagnose and treat properly. Because of a wide range of symptoms and comorbidities, it is difficult to make a diagnosis of depression. Rating scales can be useful for the diagnosis of depression. It is feasible that the clinicians assess the patients with depression appropriately and determine therapeutic strategies by rating scales for depression. We have reviewed the purpose of rating scales for depression, including screening, ratings of symptoms, diagnosis, the progress, and the results of treatment. We also have described the actual condition of rating scales in Korea, which were performed by clinical research center for depression. It is restricted within marrow limits to select certain rating scale for depression and poorly known that some scales have been developed. Therefore, it is important task to announce the rating scales for depression so that the clinicians and researchers can be familiar with these scales. It is also important to develop a diversity of scales so that the clinicians can select appropriate scales.

        베이지안 망 연결 구조에 대한 데이터 군집별 기여도의 정량화 방법에 대한 연구

        본 연구에서는 베이지안 망 구조 학습에서, 학습 데이터의 특정 부분집합이 학습된 망의 각 연결 구조(edge)의 형성에 기여하는 정도를 정량화하는 방법을 제안한다. 생물학 정보의 분석 등에 베이지안 망 학습을 이용하는 경우, 제안된 방법은 망의 각 연결 구조의 형성에 특정 군집 데이터가 기여하는 정도의 정량화가 가능하다. 제안된 방법의 유효성을 보이기 위해, 벤치마크 베이지안 망을 이용하여 제안된 방법이 망 연결 구조에 대한 데이터 군집별 기여도를 효과적으로 정량화할 수 있음을 보인다. We propose a quatitative annotation method for edges in Bayesian networks using given sets of condition-specific data. Bayesian network model has been used widely in various fields to infer probabilistic dependency relationships between entities in target systems. Besides the need for identifying dependency relationships, the annotation of edges in Bayesian networks is required to analyze the meaning of learned Bayesian networks. We assume the training data is composed of several condition-specific data sets. The contribution of each condition-specific data set to each edge in the learned Bayesian network is measured using the ratio of likelihoods between network structures of including and missing the specific edge. The proposed method can be a good approach to make quantitative annotation for learned Bayesian network structures while previous annotation approaches only give qualitative one.
