        카라멜화 반응 생성물의 갈색도와 항산화 효과와의 관계

        The study was carried out to compare the relation between the color intensity and antioxidant activity of caramelization products using xylose(XY), glucose(GL), sucrose(SU), glucose+citric acid(GLCA), glucose+sodium citrate(GLSC), heated at 80, 120 and 140℃ for 24hrs, respectively. The color intensity(absorbance at 490㎚) of the browning mixtures increased as the browning temperature and time increased. But the degrees of color intensity of SU and GLCA changed very little. The hydrogen donating ability(HDA) of browning reaction products was generally enhanced as the browning temperature and time increased. When browning mixtures were heated at 80℃, the HDA of GLGC was the highest, but the HDA of GLSC was the highest when heated at 120 and 140℃. The antioxidant activities for the corn oil substrate containing the anhydrous ethanol extracts from the browning mixtures was inferior to that of SU, but was superior to that of GLCA. The relations among the color intensity, the antioxidant activity, and the hydrogen donating ability(HDA, reducing power) of the browning reaction mixtures were as follows: As the color intensity increased, the antioxidant activity decreased. The correlation coefficient of the color intensity and the antioxidant activity by regression equation was -0.73∼-0.82. As the reducing power increased, the antioxidant activity decreased. The correlation coefficient between the reducing power and the antioxidant activity by regression equation was -0.98∼-0.99. Therefore, the antioxidant activity of browning reaction mixtures seemed not correlated with the color intensity and the reducing power.

        The following, Chung, Am, Chun, Seo, Rok, Shin, Oh, Cho, Po, listed in [Che Min Yo Sul] are as follows; 1. Chung means polutry and meat like pork and beef cooked with Ja. Am is flavoring meat in a seasoned broth, which is seasoned with Yeom Shi, green onion and pepper. Chun is stir-fryed fish, beef or pork in a small amount of broth and Apchunbup, a kind of Chun, is stir-fryed with Yeom Shi Juice. 2. Cheo, Rok stands for boiled meat pickled in vinegar. 3. There are twenty kinds of Chuk, which is broiled polutry, fish, shellfish and meat like pork and beef together with vegetables and spices. 4. Shin, Oh, Po, are a type of meat preservation method by using Nurook, salt, alchol or broiling it in Bong Chuk way and then preserving it.

      • 두경부 암환자의 항암화학요법에 따른 삶의 질 변화

        The purpose of this study was to assess the changes in quality of life wish elapse of chemotherapy in head and neck cancer patients. This study was a longitudinal descriptive design. The subjects were 9 patients who had stage III and stage IV head and neck cancer and received neoadjuvant cisplatin and 5-fluorouracil chemotherapy. Linear Analogue Self Assessment Scale (LASA) of the QOL which it includes eleven items(seven physical items appetite, nausea/vomiting, physical well-being, vigor activity, energy, sleepiness, fatigue and four mental items anger, anxiety, depression & vital power) were used to assess the quality of life eleven times(once in the prechemotherapy period and from the 1st day to 10th day postchemotherapy). The means of scores at each time were analyzed by repealed measures of ANOVA and Bonferroni multiple comparison method. The results are as follows : 1. The degree of appetites decreased significantly on the period between third and sixth day compared with that of appetites on the prechemotherapy day (p<0.001). 2. The degree of physical well-being decreased significantly on the period between first and fifth day compared with that of physical well-being on the prechemotherapy day (p<0.001). 3. The degree of nausea/vomiting decreased significantly on the period between first and fourth day compared with that of appetites on the prechemotherapy day (p<0.001). 4. The degree of anger decreased significantly on the second, fourth, eighth and ninth day after chemotherapy compared with that of anger on the prechemotherapy day(p<0.001). 5. The degree of fatigue decreased significantly on the fourth day compared with that of fatigue on the prechemotherapy day(p<0.001). 6. The increment of nausea/vomiting and poor appetite started on the 2.89th day and 3.67th day after chemotherapy, respectively. 7. The increment of sleepiness started on the 2.56th day after chemotherapy. 8. The increment of depression and anxiety started on the 2.33th day and 3.33th day after chemotherapy, respectively. 9. The lowest degrees of nausea/vomiting and poor appetite were 37.00±17.9 % and 28.64±14.94 % after chemotherapy compared with those of nausea/vomiting and poor appetite on the prechemotherapy day, respectively. 10. The lowest degrees of anger and vigor activity were 48.12±28.3 % and 39.83±24.9 % after chemotherapy compared with those of anger and vigor activity on the prechemotherapy day, respectively. 11. The recovery rates of eight items of quality of life (appetite, vigor activity, energy, fatigue, anger, anxiety, depression & vital power) were shown to reach a peak on the tenth day after chemotherapy and be reached to almost 84 % compared with those of quality of life on the prechemotherapy day. 12. The recovery rates of physical well-being and sleepiness were shown to reach to 67.75 % and 95.68 % on the 10th day after chemotherapy compared with those of physical well-belong and sleepiness on the prechemotherapy day. 13. The recovery rates of seven physical items were faster than those of four mental items on the 10th day after chemotherapy. In conclusion, it is suggested that nursing care strategies to improve the quality of life in patients with chemotherapy would be established both physically and mentally, and be extended at least ten days when patients stayed their home after chemotherapy.

        방문간호서비스 질 평가도구 개발

        본 연구는 2008년 7월 시행을 앞두고 있는 노인장기요양보험제도의 재가급여 중 방문간호의 지속적인 질 관리와 향상을 위하여 실시된 방문간호서비스 질 평가 도구 개발 연구이다. 본 연구의 진행은 노인간호 전문가 7인으로 이루어진 자문위원회 회의를 통하여 이루어졌다. 국내ㆍ외 노인 간호 관련 질 평가지표에 대한 문헌고찰과 자문회의 검토를 통하여 예비 질 평가 도구를 개발하였으며, 예비 질 평가도구에 대하여 50인의 전문가 집단을 구성하여 1차 내용 타당도 검사를 실시하였다. 예비 질 평가도구의 1차 내용타당도 검사결과를 수정ㆍ보완한 후 2차 내용타당도 검사를 실시하였으며, 개발된 방문간호서비스 질 평가도구를 이용하여 훈련된 평가자 4인이 전국 14개 방문간호기관을 직접 방문하여 시범평가를 실시하였다. 시범평가 실시 후 실제 평가도구 적용 시 평가기준을 분명하게 할 필요가 있다고 평가자들이 지적한 문항들에 대하여 자문회의를 통하여 질 평가 도구를 수정ㆍ보완하여 방문간호서비스 질 평가도구를 완성하였다. 최종적으로 개발된 질 평가도구는 구조, 과정, 결과로 구성되어 있으며, 총 20개 영역, 59개 항목, 88개 문항으로 이루어진 4점 척도의 도구이다. 20개 영역은 인사, 업무 공간, 물품확보 및 관리, 조직, 재정, 경영방안, 의사소통 체계, 보고체계, 정보관리, 비용청구, 자원연계, 효과, 방문간호 책임자 역량, 방문간호사 교육, 서비스 계획, 서비스 수행, 서비스 평가, 감염관리, 환경 및 안전대책, 자체평가로 구성되어 있다. 본 연구결과로 개발된 방문간호서비스 질 평가도구를 활용하여 향후 시범사업이 종료되고 본 사업이 진행될 때 체계적인 방문간호 질 관리 체계를 구축해 나갈 필요가 있다. The purpose of this study was designed to develop Home Care Quality Indicators(HCQIs) to evaluate and improve quality of 'Elderly Care Program' that has been conducted in 14 institutions in the 2nd demonstration project. For this study, the preliminary HCQIs were developed through literature reviews and analysis of existing quality indicators. An expert development panel reviewed established and new quality indicators twice for applicability to home care services for long-term care. The final HCQIs were confirmed and applied in fourteen home care service agencies to test feasibility. In results, the preliminary HCQIs had 19 domains, 68 categories and 99 indicators. After two content validity testing, the final HCQIs consisted of 20 domains, 59 categories and 88 indicators. They were applied in fourteen home care service agencies and it was revealed that all the indicators were applicable. The quality indicator set provides a comprehensive home care quality framework for long-term care future and will allow for comparative research. Provision of these evidence-based measures could improve patient quality of life and longevity.

        LC-MS Determination and Bioavailability Study of Imidapril Hydrochloride after the Oral Administration of Imidapril Tablets in Human Volunteers

        The purpose of the present study was to develop a standard protocol for imidapril hydrochloride bioequivalence testing. For this reason, a specific LC-MS method was developed and validated for the determination of imidapril in human plasma. A solid-phase extraction cartridge, $Sep-pak^{R}$ C18, was used to extract imidapril and ramipril (an internal standard) from deproteinized plasma. The compounds were separated using a XTerra $MS^{R}$?C18 column ($3.5 {\mu}m, 2.1\times150 mm$) and $acetonitrile-0.1\%$ formic acid (67:33, v/v) adjusted to pH 2.4 by 2 mmol/L ammonium formic acid, as mobile phase at 0.3 mL/min. Imidapril was detected as m/z 406 at a retention time of ca. 2.3 min, and ramipril as m/z 417 at ca. 3.6 min. The described method showed acceptable specificity, linearity from 0.5 to 100 ng/mL, precision (expressed as a relative standard deviation of less than $15\%$), accuracy, and stability. The plasma concentration-versus-time curves of eight healthy male volunteers administered a single dose of imidapril (10 mg), gave an $AUC_{12hr}$ of imidapril of $121.48\pm35.81 ng mL^{-1} h$, and $C_{max} and T_{max}$ values of $32.59\pm9.76 ng/mL and 1.75\pm0.27 h$. The developed method should be useful for the determination of imidapril in plasma with sufficient sensitivity and specificity in bioequivalence study.

        The Prevalence and Distribution of the P and G Genotypes of a Group A Rotavirus Detected in Acute Gastroenteritis Patients from Incheon

        인천지역 병·의원에 내원한 급성위장염환자의 대변 검체 총 11,607건을 효소 면역법(ELISA)을 이용하여 group A rotavirus의 계절별 발생 양상을 조사한 결과 2005년부터 2010년간 인천지역에서는 1~2월이 가장 많이 발생하였으나, 기간별로 차이가 존재했다. 또, group A rotavirus 양성 검체 160건에 유전자 분석 결과 VP4는 P8형, VP7는 G1형, G와 P 조합형으로는 G1P8이 가장 많았다. 그러나 이전의 연구와 비교했을 때 연구 지역과 기간에 따라 혈청형과 유전형의 변화가 빈번하였으므로 효율적인 방역을 위해 지속적인 모니터링이 필요하다고 사려된다. 또 이번 연구는 현재 유통 중인 rotavirus 백신의 효율적인 사용의 기초 자료가 될 것이다. Rotavirus is the main cause of severe diarrhea in infants and young children of the world. However, the frequency of genetic alterations makes it hard to control the prophylaxis. Therefore, continuous monitoring of the rotavirus’s genetic change is inevitable to prevent disease prevalence and is useful in inventing an efficient vaccine. From January 2005 to December 2010, we investigated 11,607 stool samples of acute gastroenteritis patients in the Incheon metropolitan area. About 13.18% (1,530 stool samples) of all samples had a positive reaction against rotavirus using an antigen capture enzyme- linked immunosorbent assay (ELISA). Then, the 160 stool samples were searched for subtypes of group A rotavirus by using a reverse transcription polymerase chain reaction (RT-PCR) and a nested multiplex RCR. In P sub-typing, P8 (56.3%) was an extremely prevalent genotype, followed by P6 (21.3%), and P1A (10.0%). G1 (39.4%) was most widespread in the G subtype, followed by G4 (25.0%) and G3 (18.8%). G1P8 (35.5%) was the most common G and P subtype combination, followed by G4P6 (19.3%) and G3P8 (13.1%). These results might be useful data for understanding the epidemiological status of group A-rotavirus dispersion in the Incheon metropolitan area.

      • A Structural Feature of Sodium Laurate/n-decane/1-pentanol/XN NaCl Microemulsion System from NMR Investigation

        In this study, phase behavior was observed as a function of salinity and aqueous NaCl fraction of sodium laurate/n-decane/1-pentanol/XN NaCl microemulsion system, and NMR spectra have been obtained to investigate the microstructural characteristics of those single phase and middle phase microemulsions. The time for phase equilibrium varied depending on two factors, the volume ratio of oil-to-brine and the salinity of the system. Based on the integration ratio of NMR peaks of (methyl + methylene) /hydroxyl groups, it may be concluded that, the upper portion of the single phase microemulsion contains more of the oil component compared to the lower portion of it; the same phenomenon of decreased oil-distribution at the lower portion of the middle phase microemulsion has also been confirmed. These results lead to a question whether it is proper to define a single phase microemulsion to be either o/w or w/o type, considering the fact that a low density of oil component has a significant influence on the local composition of microemulsion system. The single phase microemulsions formed with equal fraction of oil and aqueous components show the largest variation in the peak ratio of (methyl + inethy1ene)/hydroxyl groups as a function of position of sampling, indicating the presence of a very labile bicontinuous structure despite the homogeneous appearance of a uniform composition.
