        Identification of a sensitive urinary biomarker, selenium-binding protein 1, for early detection of acute kidney injury

        <P>Acute kidney injury (AKI) is associated with increased mortality rate in patients but clinically available biomarkers for disease detection are currently not available. Recently, a new biomarker, selenium-binding protein 1 (SBP1), was identified for detection of nephrotoxicity using proteomic analysis. The aim of this study was to assess the sensitivity of urinary SBP1 levels as an early detection of AKI using animal models such as cisplatin or ischemia/reperfusion (I/R). Sprague-Dawley rats were injected with cisplatin (6 mg/kg, once i.p.) and sacrificed at 1, 3, or 5 days after treatment. Ischemia was achieved by bilaterally occluding both kidneys with a microvascular clamp for 45 min and verified visually by a change in tissue color. After post-reperfusion, urine samples were collected at 9, 24, and 48 hr intervals. Urinary excretion of protein-based biomarkers was measured by Western blot analysis. In cisplatin-treated rats, mild histopathologic alterations were noted at day 1 which became severe at day 3. Blood urea nitrogen (BUN) and serum creatinine (SCr) levels were significantly increased at day 3. Levels of urinary excretion of SBP1, neutrophil gelatinase-associated lipocalin (NGAL), and a tissue inhibitor of metalloproteinase-1 (TIMP-1) were markedly elevated at day 3 and 5 following drug treatment. In the vehicle-treated I/R group, serum levels of BUN and SCr and AST activity were significantly increased compared to sham. Urinary excretion of SBP1 and NGAL rose markedly following I/R. The urinary levels of SBP1, NGAL, TIMP-1, and KIM-1 proteins excreted by AKI patients and normal subjects were compared. Among these proteins, a marked rise in SBP1 was observed in urine of patients with AKI compared to normal subjects. Based upon receiver-operator curves (ROC), SBP1 displayed a higher area under the curve (AUC) scores than levels of SCr, BUN, total protein, and glucose. In particular, SBP1 protein was readily detected in small amounts of urine without purification. Data thus indicate that urinary excretion of SBP1 may be useful as a reliable biomarker for early diagnosis of AKI in patients.</P>

        The incidence of double primary cancer is about 10% in all kinds of cancers. Double primary cancer is usually accompanied by gastrointestinal cancer. Esophageal cancer is supposed to be frequently accompanied by squamous cell carcinoma of the head and neck or the upper respiratory tract, because of such common carcinogens as smoking and alcohol ingestion. Esophageal cancer and hypopharyngeal cancer are usually diagnosed by esophagogastric endoscopy with biopsy. We performed endoscopic ultrasound-guided Trucut biopsy(EUS-TCB) to diagnose a mediastinal mass in a 49 year-old patient, who visited our hospital because of voice change. During procedure, we accidently found enlarged celiac lymph node, and obtained tissue from the mediastinal lymph node and the celiac lymph node. We confirmed double primary cancer of squamous cell carcinoma in the esophagus and the hypopharynx via esophagogastric endoscopy.

        A control scheme to select the appropriate layer between macro- and microcell hierarchical structure is considered. With the assumption that the each mobile speed is known to the system. the algorithm determines the optimal velocity threshold by which the cell section performed. The optimal criterion is to minimize the Grade of Service(GOS), a service criterion in cellular telephone systems. Mobile station with faster speed relative to the determined optimal speed threshold are assigned to a macrocell, while slower moving terminals are assigned to the smaller cells, i,e.. microcells. For the evaluation purpose for the proposed scheme, two different evaluation perspectives are compared and the algorithm proves to work

      • 도로주변 낙하분진의 침착속도와 수계환경에 미치는 영향

        The air pollutants were transported over the long distant by the wind and were deposited to the ground or water surface by the gravitational sedimentation, precipitaion or Brown movement and so on. In this case, the deposited particles might give a bad effect to the water environment, such as eutrophication, damage of aquatic organism. This study was carried out to compare the deposition flux to the 17th national road and Il-San lake, Kyunggi, and to investigate the principal organic components. The deposition flux of 17th national road was 228.2㎎/㎡ day from Feb. to June., and that of Il-San lake was 276.4㎎/㎡ day from Feb., to June. Each components exist in the ratio of COD : T-N : T-P = 100 : 10.8 : 0.9, and the ratio of COD to the VSS is 1.2. The ratio of VSS to TSS was 0.12, the ratio of NH_3-N to T-N was 0.48 and the ratio of PO_4-P to T-P was 0.25. The average deposition velocity was 4.2㎝/s in 17th national road and 4.9 ㎝/s in the Il-San lake. The minimum velocity site was forest at the rate of 1.0㎝/s.

      • 경영에서 grab start와 track start의 운동학적 분석

        본 연구는 남녀 수영 선수 6명씩을 대상으로 grab start와 track start를 실시하여 두 집단간과 두 출발법간의 동작 특성을 운동학적으로 분석한 후 변인들을 정량화 하여 선수들에게 출발법에 대한 심층적인 이해를 제공하고 효과적인 훈련에 기여하도록 하는 것이 목적이며 실험으로 카메라 4대를 이용하였으며 분석을 위해 권(1993)이 개발한 Kwon3D 동작 분석 프로그램으로 3차원 영상 분석을 실시하였다. 결과의 통계 처리 자료로는 SPSS 프로그램을 사용하였으며 출발에 관련된 운동학적 변인을 수평방향과 수직방향에서의 인체 중심점 위치와 속도의 변화를 측정하였다. 위치 변화에서는 출발대를 떠나는 지점에서 남자 선수들은 수직 지향을 여자 선수들은 수평 지향을 함을 알 수 있었고 속도 변화에서 출발대를 떠나는 순간 track start가 속도값이 크기는 하나 두 출발법 사이에 유의한 차이는 나타나지 않았다. The research has the aim to analyze and find out kinematic features of grab start and track start between male and femail. With having selected 6 swimmers form each for this experiment four video-cameras(60 frames/sec) and control-box(length-2m, width-1m, hight-1m) to get real space coordinates were used. The DLT method was employed to obtain the 3-dimensional coordinates of the 25 body points for digitizing with Kwon3D Motion Analysis Package developed by Kwon, Young-hoo (1993). SPSS was used for statistic process. Kinematic variable were measured in start: The center of mass of body in horizontal and vertical, variation of velocity, entry distance. The following are the results. 1. In change of the body-center, the starting methods of man and women are same in horizontal directions, but in vertical directions the amount of women´s body changement is decreasing ever, while the amount of man´s body changement is increasing continuously with his foot off the start line. The change of the position and velocity in vertical directions is changing with the same tendency in case of starting ways of both man and women, but in vertical directions man´s amount of changement has steep increasing curve, and women´s one has decreasing curve. 2. Horizontal ANOVA analysis of body-center shows that starting method represent not such difference at taking-off moment but significant difference at initial moving point(F=237.016, p<.01). Vertical analysis shows significant difference in gender and starting method(for each F=31.132, p<.01, f=22.591, p<.01)

        Wear and fracture mode of ceramic tool for hardened Std11 steel was investigated by face milling in this study. The cutting force and acoustic emission(AE) signal were utilized to detect the wear and fracture of ceramic tool. The following conclusions were obtained : (1) The wear and fracture modes of ceramic tool are characterized by the three types : ① wear which has normal wear and notch wear, ② wear caused by scooping on the rake face, ③ large fracture caused by thermal crack in the rake face. (2) The wear behaviour of ceramic tool can be detected by the increase of mean cutting force and the variation of the AE RMS voltage. (3) The catastrophic fracture of ceramic tool can be detected by the cutting force(Fz-component). (4) As the hardness of work material increased, acoustic emission RMS valve and mean cutting force(Fz) increased linearly, but the tool life decrease.

      • 휴대용 근적외분광분석기를 이용한 유탕면의 수분 함량 측정 : Infrared(NIR) System

        This experiment was carried out to determine non-destructively water content in a intact fried noodle in oil by portable near-infrared system. Water content was ranged from 0 to 14.3w/w% and the near infrared (NIR) measurements were made on 70 intact samples over the spectral range 1100-1750nm. We found the variation of absorbance band due to OH vibration of water in the noodles depending on the concentration change around 1450nm in the second derivatives spectra. Partial least squares(PLS) regression models were set up using a calibration set(42 E.A.) and a validation set(28 E.A.) between the NIR response and value from loss on dry. Various pretreatments of the spectra were examined: the smallest stnadard error of prediction(0.35%) was obtained using autoscaling spectra over 1100-1700nm. 10 factors were required for the PLS model to give a good relationship between the spectral information and the noodle water content. In order to validate the developed calibration model, routine analyses were performed using other intact noodles. Water contents from the NIR calibration model were compared with the values from loss on dry and the results showed good correlation with those of loss on dry. This study showed that the rapid and non-destructive determination of water content in a intact fried noodle in oil was successfully performed by portable NIR system.

        1차 의료기관에서 코감기에 처방한 향균제

        Background : Upper respiratory infections (URIs) are common benign illnesses that are associated with significant economic burden, adverse effects of medications, and antimicrobial resistance. These effects have been evaluated chiefly at the tertiary health care centers. The purpose of this study was to investigate the prescribing habits, including antibiotic use, for the management of URIs among the primary care physicians. Methods : Between July and August 2006, 23 medical students visited 122 clinics of primary care physicians for the management of simulated common cold. They were instructed to present symptoms that simulated common cold; clear rhinorrhea and nasal stuffiness that lasted for 3 days, and neither was accompanied with fever nor myalgia. Results : The physicians' specialties were as follows: 43 were general internists; 40, otolaryngologists; 23, general practitioners; 15, family practitioners; and 1, pediatrician. The mean number of prescribed oral medications was 4.71 (S.D.±0.951). Glucocorticoids were prescribed in 10 (8.2%) clinics. Antibiotics were prescribed in 50 (41.0%) clinics - cephalosporins in 17, amoxicillin-clavulanate in 16, amoxicillin in 8, fluoroquinolones in 5, macrolides in 3, and trimethoprim-sulfamethoxazole in 1 clinic. There was no difference in the rates of antibiotic prescription among the specialties. Injections were recommended in 32 (26.4%) clinics. With the exception of chest X-ray recommended in 1 clinic, no laboratory or radiologic evaluation was performed. Conclusion : The antibiotic prescription rate observed in this study is lower than those of the previous studies. Since diagnostic tests are seldom performed, diagnostic ambiguity might be a key cause for the overuse of antibiotics in the management of URI. The reasons for the high rates of prescription of parenteral medications need to be evaluated. 목적 : 상기도 감염은 매우 흔한 질환으로 개개인으로 보면 중요하지 않은 병이나 국가 전체로 보면 비용, 부작용, 항균제 내성에 미치는 영향이 더 문제가 되는 병이다. 이런 영향에 대한 연구가 주로 3차 병원에서만 되므로, 3차 병원에 도착하기 전까지 받은 치료 내역이 연구 결과에 큰 영향을 미칠 수 있지만, 국내에서는 의약분업 후로는 의원에서 감기 처방에 대한 조사가 없어 현재 진료 상황을 알 수가 없다. 이에 의약분업 이후, 1차 의료에 종사하는 의원들을 대상으로 코감기에 관련된 진단의 다양성, 주사제의 사용, 항균제 사용에 대해 조사를 하였다. 재료 및 방법 : 2006년 7월과 8월에 걸쳐 서울, 경기, 인천에 개업하고 있는 일반의, 내과의, 이비인후과의, 가정의를 대상으로 122개 의원을 조사 하였다. 23명의 의과대학생들이 코감기 환자로 모의를 하여 개원의들의 진찰을 받은 후, 진단과 처방을 분석하였다. 코감기 증상은, 3일간 지속한 콧물과 코막힘을 주소로 했고, 발열과 오한은 없었으며, 목아픔과 기침은 약간 있는 정도로 하였다. 결과 : 조사한 의원에서 코감기에 사용한 경구 약제 수는 평균 4.71 (SD±0.951), 과별 차이는 없었다(P=0.99 by chi-square test). 부신피질호르몬은 10개(8.2%) 의원에서 처방하였다. 항균제는 50개(41.0%) 의원에서 처방하였으며, 세팔로스포린 17건, Augmentin 16건, amoxicillin (7) 또는 bacampicillin (1) 8건, 퀴놀론 5건, macrolide 3건, trimethoprim-sulfamethoxazole 1건이 었다. 과별로는 내과 34.9%, 이비인후과 52.5%, 가정의학과 33.3%, 일반외과 39.1%, 소아과 0%였고 통계적 의미는 없었다(P=0.41 by chi-square test). 주사를 권한 경우는 32 (26.4%)예였다. X-선 검사를 권유받은 1건 외에는 검사를 권유받지 않았다. 결론 : 이전 보고보다는 감기에서 항균제 사용률은 낮았으며, 진단을 하기 위한 검사가 적어 진단의 불분명을 극복하기 위한 것도 항균제 남용의 한 원인으로 생각된다. 주사제 처방률도 높은 듯하며 이는 빨리 증상이 호전되지 않을 때 병원을 바꾸는 환자들의 행위에 대한 반응일 가능성이 있으며, 항균제 사용 역시 빠른 증상 호전을 위한 것이어서, 환자의 행동 양상에 대한 조사가 필요하다.

        특성화고등학교 광고디자인 교과목의 창의적 교수방안 제시 : 신문광고 디자인 제작과정을 중심으로

        Education on design at professional high schools, in particular, has been widely overlooked for its importance, which can be regarded as the basic stage for design education. This paper intends to identify imagination and potentiality of students by suggesting creative teaching methods through design courses offered at professional high schools and to seek creative teaching methods by utilizing various methods of striking idea from design for newspaper ads at ad design courses. It also tried to suggest more systematic and effective teaching methods for ad design instead of the current lecture-style training focusing on memorization of representation technique. Given those, the conclusions of this paper are as follows First, ideas or thoughts can be developed by means of repetitive training in ad design courses, Transformation of ideas away from day to day thinking rituals requires creative thinking. Second, education must make sure that students learn how to secure basic approaches to rational inference of ideas and unfold them, thus building foundation for original and creative ad design. More people need to participate in the development of basic studies of design for the sake of progress in design education, and better educational programs should be sought to foster materialization of the potential of creative thinking. To this end, governmental support is needed to work out various ways to enhance basic knowledge, problem solving ability, representation skill of ideas, flexible thinking, tolerance, interest, intellectual curiosity and creativity.
