Capsule retention caused by duodenal metastases from primary appendiceal adenocarcinoma

Metastatic small bowel cancers are extremely rare in clinical practice. Primary appendiceal adenocarcinoma with gastrointestinal metastasis is seldom reported in the literature. Here, we describe the case of an 80-year-old male patient with primary appendiceal adenocarcinoma, who presented to China Medical University Hospital with mid-gastrointestinal tract bleeding. Capsule endoscopy revealed stenotic bowel lumen, but the capsule was retained in the distal duodenum. Double-balloon enteroscopy demonstrated erosive and erythematous mucosa in the region of capsule retention. The retained capsule was retrieved successfully by using an electrosurgical snare. Histological examination and immunohistochemical staining of the biopsy specimen from the duodenal lesion strongly supported the diagnosis of metastatic appendiceal adenocarcinoma. 문학텍스트 기반 반응저널쓰기를 통한 초등영어쓰기에 대한 연구

The study is to investigate the effect of using literary texts on students` writing in primary English education. To accomplish the objective, the researchers designed a program in which students wrote double-entry journals as their responses to literary texts on a regular basis. The following research questions were proposed: 1) What effect does the utilization of literary texts have on reaction-centered writing activities? 2) What changes can literary text-based writing activities bring about in learners` and their parents` awareness of English education? Data collected from students` journals, classroom teachers` observation diaries, surveys, and interviews, were encoded and categorized. Data analysis revealed the following results. First, students experienced diverse literary texts, and tried to write their own journals creatively utilizing such texts. Second, students and their parents developed a positive attitude about reading and writing in English through writing-activity utilizing literary texts. Based upon positive results of the study, several suggestions are provided for future studies.

Double primary cancer means that more than 2 cancers occur independently in an individual. There have been many reports on double primary cancer since Billroth reported it for the first time in 1889 and Warren and Gates established it. The incidence of esophageal cancer is low, about 1~2% of all cancer and, 7% of all gastrointestinal cancer, but double cancer with including esophageal cancer is 9.5~27% of all double primary cancer. Double primary cancer of the esophagus and stomach has occasionally been reported. We have experienced three cases of double primary cancer of squamous carcinoma in the esophagus and adenocarcinoma in the stomach. In this study we reviewed the clinical characteristics of the reported cases of double primary esophageal and gastric cancer that have been reported in Korea and these three cases we experienced at our hospital. 두 개 이상의 암이 각각 독립해서 발생하고 서로 종속 관계가 없으면 이를 원발성 중복암이라고 한다. 이는 1879년 Billroth가 처음 보고하고 1932년 Warren과 Gates에 의해 정립된 이래 문헌상 보고되는 중복암의 빈도는 점점 증가하고 있다. 중복암의 대부분은 두경부암이 차지하고 다음이 피부암, 유방암, 전립선암, 폐암의 순서로 나타난다. 식도암은 악성종양의 1~2%, 소화기계 악성종양의 7%로 빈도는 높지 않으나 식도암을 포함한 중복암은 9.5~27%로 다양하게 보고되고 있다. 식도와 위에 발생한 원발성 중복암은 국내 보고가 산발적으로 있었다. 저자들은 식도의 편평상피세포암과 위선암이 병발한 3예를 경험하게 되었고 국내에 발표한 증례를 바탕으로 비교 분석하여 문헌고찰과 함께 보고한다.

Double primary cancer is usually accompanied by gastrointestinal cancer. The incidence of histologically different cancers in the same organ is less than that of double cancers arising from different organs. The cancers accompanied by esophageal squamous cell carcinoma are laryngopharyngeal cancer, gastric adenocarcinoma and primary hepatocarcinoma. Esophageal cancer is supposed to be frequently accompanied by squamous cell carcinoma of the head and neck or the upper respiratory tract because of such common carcinogens as smoking and alcohol ingestion. On the other hand, it is vary rare that a synchronous double primary cancer is diagnosed at the esophagus. We present here a case of double esophageal cancer in a 77 year-old patient who visited our hospital because of progressive dysphagia that she’d experienced for 3 weeks. 중복암 중에서 같은 장기 내에 조직학적으로 서로 다른 두 가지 악성 종양이 발생하는 경우는 서로 다른 장기에서 중복암이 발생하는 경우에 비해 드물다. 다발성 원발성 중복암은 대부분 소화기암과 동반된 경우가 대부분으로 이 중 식도암은 주로 두경부 및 상기도의 편평 상피암과 동반되며, 이는 흡연, 음주와 같은 공통의 발암 인자를 가지기 때문인 것으로 알려져 있다. 식도에서 편평상피세포암이 서로 다른 부위에서 동시에 발생한 예는 드물어 국내에서는 다발성 조기 식도암 1예를 보고한 바 있다. 저자들은 3주 동안 점차 악화되는 연하 장애를 주소로 내원한 77세 환자에서 상부 위장관 내시경을 통해 식도의 동시성 중복암을 진단하여 방사선 치료를 시행한 1예를 경험하여 문헌고찰과 함께 보고한다.

Double primary cancer is defined as the case of primary malignant tumors, which must be arise in different sites and have different histological appearances. In the two patients with their seventh decades. synchronous double cancers on colon and stomach are reported. Two patients had ascending colon cancer, sigmoid colon cancer respectively and early gastric cancer, which were diagnosed by endoscopic and organ resection biopsy. We describe the cases with review of literatures.

식도종양과 흉벽종양의 이중성 원발종양 1례

Double primary cancer is a rare disease in which two cancers occur in an individual independently. As prolonged survival of patients with malignant tumors is expected in the future due to advances in methods of treatment, the chance of double primary cancer will be increased. We experienced one case of double primary cancer which was developed in esophagus and chest wall. A 72 year-old male visited our hospital complaining of epigastric discomfort and right chest wall mass. We studied esophagus, chest wall, and other organs including gastrointestinal tract by various methods to exclude the cancer of other sites and could diagnose squamous cell carcinoma of mid-esophagus and adenocarcinoma of chest wall. The patient underwent esophagogastrostomy following esophagectomy and wide-resection of chest wall tumor. The postoperative course was uneventful.

Double Primary Mucinous Eccrine Adenocarcinomas Each Occurring Separately on the Left Eyelid and Right Eyelid with a Time Lag

A 57-year-old man presented with a tender, solitary, 0.3 cm-sized, subdermal nodule on the left periocular area for several months. He had surgery three times for recurrent primary mucinous eccrine adenocarcinoma on the right periocular area. Skin biopsy showed a tumor separated by strands of fibrous tissue and small compartments of tumor cells were surrounded by pale-staining mucin. The cluster of tumor cells contained atypical epithelial cells. The mucin stained with periodic acid-Schiff (PAS) and alcian blue at pH 2.5. Cytokeratin-7 (CK7) was positive for tumor cells, but cytokeratin-20 (CK20) and S-100 were negative. For evaluation of local metastasis, computed tomography was performed and there was no evidence of regional lymph node metastasis. The lesion was removed by local excision and the patient was under close follow up without any sign of recurrence. To our knowledge, double primary mucinous eccrine adenocarcinomas in the opposite body site has never been reported.

Double Primary Cancer of the Gallbladder and Cystic Duct: A Case Report

Primary carcinoma of the gallbladder is an uncommon and aggressive malignancy. Furthermore, double primary cancer of the gallbladder and cystic duct is extremely rare and few such cases have been reported. So, we report here on the case of double primary cancer of the gallbladder and cystic duct, which was pathologically confirmed after an operation, and we discuss the US, CT and ERCP findings.
