        중,고등학생의 구강건강신념이 구강건강관리행동에 미치는 영향 : 건강신념모형을 적용하여

        Objectives: The aim of the study is to investigate the health belief model affecting the oral health behavior in middle and high school students. Methods: The subjects were 296 middle and high school students in Seoul, Gyeonggi and Incheon from February 15 to March 21, 2014. The students filled out the self-reported questionnaires after receiving informed consents. The instrument was adopted and revised from those of Kim & Hwang, and Choi & Joo. The questionnaire consisted of 4 questions of general characteristics, 9 questions of oral health status including subjective oral health status, frequency of tooth brushing, duration of tooth brushing, method of tooth brushing, use of oral health devices, dental clinic visit, scaling services, snack intake, and smoking. The oral health belief consisted of 25 questions including susceptibility, seriousness, barriers, benefit, and self-efficacy using Likert 5 scale. The reliability of Cronbach’s alpha in the study was 0.725. Data were analyzed using SPSS ver 18.0 for frequency analysis, t-test, ANOVA, χ²-test, and Pearson``s correlation coefficient, simple regression, and binary logistic regression. Results: Oral health beliefs of middle and high school students affected the oral health behaviors. Susceptibility, barriers and self-efficacy also influenced on the oral health behaviors. In order to provide the best oral health education, susceptibility and self-efficacy are the primary factors to increase motivation because the motivation endows the students with correction of oral health behaviors that improve the knowledge, attitudes, and decrease barriers in oral hygiene. Conclusions: It is important to correct oral health behaviors in the middle and high school students by providing the continuing andsystematic oral health education.

      • 재발성 류마티즘 환자에서 발작의 유발인자

        Background and Objectives : Palindromic rheumatism (PR) is known to be triggered without any obviously inciting events in most patients, although, in a few cases, attacks have been noted to relate to another events such as weather change, childbirth, ingestion of certain foods, or over-exercise. The aim of this study was to characterize the triggering factors on palindromic attacks. Materials and Methods : We evaluated sixty-four patients with PR for the presence of triggering factors and the relationship between attacks and triggering factors by interview of patients. We considered that there was an probable association between episodes and PR, if episodes within 24 hours prior to PR had triggered at least 50% of all attacks and a definite association between episodes and PR, if episodes within 24 hours prior to PR had triggered 100% of all attacks. Results : In 15 patients (35.9%), there was a possible association between episodes of PR and initiating events. In 8 patients (12.5%), there was a definite association between episodes of PR and initiating events. The triggering factors were physical over-activity including exercise (19/64: 29.7%), foods, such as rawfish, crab, and chicken, including alcohol (19/96: 19.8%). However, there were no difference between the groups with and without the triggering factor in sex ratio, duration and onset of PR, the positive rate of rheumatoid factor and involved sites. Conclusion : Our observations showed that PR was triggered by physical over-activity more than in previous reports and hypersensitivity to foods was a causative factor in some patients with PR. However, patients with triggering factors had a similiar clinical profile to patients without triggering factors.

        GT rotary file을 이용한 만곡 근관형성시 작업 효율 및 file 변형 발생에 관한 연구

        Root canal preparation process is of utmost importance in successful treatment of root canal. Also, one of the most important purpose of the root canal preparation is to enlarge the root canal three dimensionally without changing the curvature of the root canal. However, as the curvature of the root canal increases, there are many difficulties involved in formation of optimum root canal. Therefore, in order to solve the above mentioned problems, new developments in methods of root canal preparation and equipments for such purposes were made. Recently, vigorous studies about newly introduced engine-driven nitckel-ti-tanium rotary file are conducted. As shown in research results to dates, it is well established that the use of nickel-titanium file is better suited for curved root canal than stainless steel file in maintaining the curvature of root canal and reducing the deformation of root canal. However, it is also acknowledged that there are a few discrepancies in research results according to protocol, due to failure to remove variables in experiments. In addition, although it is recommended by the manufacturer that the GT rotary file should maintain a low rotational speed of 150∼350rpm and 'light pressure' as light as not to break the lead of a pencil, academic studies about the vertical force which is not yet standardized are not sufficiently explored. Therefore, this research devised and utilized a special research equipment to standardize the appropriate range of vertical force for GT rotary file through experiments by breaking of the lead of a pencil as expressed by the manufacturer and to accurately measure factors involved through repeating and recreating the environment of root canal preparation. Forming nine experimental groups by varying the vertical forces (150g, 220g, 300g) and rpm (150rpm, 250rpm, 350rpm), the effects of changing vertical forces and rpm on working efficiency were measured in terms of time expended in root canal preparation by crown-down method using a transparent resin block with 35 degree curvature and GT rotary file (z-test). The following research using this special research equipment that involved nine experimental groups and varying the vertical force for root canal preparation from 300g which is within the normal vertical force range to 700g and 1000g which fall outside the normal rpm range. The results were as follows : 1.Analysis of the experiment results revealed that the tome spent in root canal preparation decreased as the vertical forces and rpm increased (p<0.05). Also, the effects of rpm were greater than those of the vertical forces within the normal vertical force range (β-weight test) . 2.Observation of the deformation of GT rotary file revealed that deformation increases in a direct correlation with the vertical force increase and in a reverse correlation with the rpm decrease, In the case of the vertical forces close to the normal range, the probability of GT rotary file deformation were quite different depending on the rpm changes. In the case of◎eater tier◎cal forces, the occurrences of deformation of the file were more frequent regardless of the rpm changes. 3.Deformation and breakage of file were also commonly observed in the expended time measurement experiments and GT rotary file deformation experiments in which low speed rpm (150rpm) was used and at the curved portion of the resin block.         한국인 배자 및 태아에서 유치 발생의 조직학적 변화

        Tooth development is usually described in four stages such as bud stage, cap stage, bell stage and crown stage. Exact time of appearance of tooth primordia is different among reports, and up to now there is no timetable regarding initial tooth development. To understand the congenital malformations and other disorders of the orofacial region, there is need to establish a standard timetable of early tooth development. Till now, studies on the tooth development were mainly on later fetuses, and only few reports on early stage. Also, there were no reports on the time when bud stage turns to cap stage, and cap stage to bell stage. In this study, external morphology of face and the early development of the tooth, and transition of bud stage to cap stage, cap stage to bell stage were studied using 27 staged human embryos and 9 serially sectioned human fetuses. the results are as follows : 1. Mandibular region was formed by union of both mandibular arch at stage 15, and maxillary region by union of maxillary arch, medial nasal prominence, and intermaxillary segment at stage 19. 2. Ectodermal thickening which represents the primordia of tooth appeared in mandibular region at stage 13, and maxillary region at stage 15. 3. Bud stage began from mandibular primary central incisor at stage 17, and maxillary primary central incisor at stage 18. And the sequence of appearance was in the mandibular primary lateral incisor at stage 19, maxillary primary lateral incisor at stage 20, mandibular primary canine at stage 22, maxillary primary canine and primary first molar at stage 23, madibular primary first molar and maxillary primary second molar at 9th week, and mandibular primary second molar at 10th week of development. 4. cap stage began from the primary anterior teeth at 9th week, and primary second molar still had the characteristics of cap stage at 12th week of development. 5. Transition to bell stage started from the primary anterior teeth at 12th week, and primary second molar started at 16th week of development. 6. Trnasition to crown stage started from primary anterior teeth at 16th week, and primary second molar at 26th week of development.

        Efficacy of Intracytoplasmic Sperm Injection for Leukocytospermia

        White blood cells (WBCs) are present in most human ejaculates, but abnormally high concentration of seminal leukocytes map reflect an underlying pathological condition. The World Health Organization (WHO) has defined leukocytospermia as status of more than 10^(6) WBC/mL of semen. The purpose of this study was firstly, to compare the outcomes between conventional IVF and ICSl in leukocytospermia, and secondly, to investigate whether ICSI may be an alternation treatment for patients with leukocytospermia. Total 346 cycles of conventional IVF and ICSI candidates underwent IVF cycles at Eulji Hospital Infertility Clinic. Semen Parameters including concentration, motility, morphology of spermatozoa and concentration of leukocytes were assessed from the raw ejaculates. There was no difference in sperm concentration, motility and morphology. The rates of fertilization and good embryo development from ICSI were significantly higher than those from conventional IVF in leukocytospermia (60.4% & 32.5% respectively for ICSI group and 44.4% & 28.5%, respectively for IVF group, P<0.001). The pregnancy rate after ICSI was also higher than that from conventional IVF (34.0% vs 29.1%. P<0.05). These results indicate that the presence of seminal leukocyes (>1×10^(6) WBC/mL of semen) is adversely related with fertilization, embryo development and pregnancy rate. Therfore the measurement of seminal leukocytes in routine semen analysis appears to be of prognostic value with regard to male fertilizing potential. In conclusion. it is suggested that ICSI is an alternative choice of treatment for patients with leukocytospermia.

      • 임상 검체에서 분리된 vancomycin 내성 장구균에 대한 Quinupristin/dalfopristin(Synercid®)을 포함한 10종 항균제의 시험관내 항균력

        목적 : 본 연구에서는 고려대 병원과 전남대 병원에서 임상 검체로부터 분리된 VRE 16균주를 대상으로 하여 장구균 감염의 치료에 흔히 사용되는 항균제인 ampicillin, gentamicin, vancomycin, teicoplanin과 VRE에 대해서도 비교적 항균력이 있는 것으로 알려진 기존의 chloramphenicol, doxycycline, erythromycin, ciprofloxacin, rifampicin, 그리고 다제 내성 그람 양성균의 치료를 위하여 새로이 개발된 quinupristin/dalfopristin을 이용한 시험관내 항균력을 조사하고자 하였다. 방법 : 총 16균주의 VRE faecium을 대상으로 quinupristin/dalfopristin (Synercid^(R))를 포함한 10종 항균제에 대한 최소 발육 억제 농도(MIC)를 한천희석법을 이용하여 측정하였다. 각각의 항균제에 대한 감수성은 NCCLS에서 제시한 기준에 의거하여 해석하였다. 결과 : 1) 5균주의 vanB VRE에 대한 항균제의 MIC_(90)은 ampicillin, erythromycin, gentamicin, cipro- floxacin에서 각각 64㎍/㎖, 512㎍/㎖, 512㎍/㎖, 4㎍/㎖로서 100%에서 내성을 보였고, doxycycline, chloramphenicol의 경우 4㎍/㎖, 4㎍/㎖로서 100%에서 감수성을 보였다. Teicoplanin의 MIC_(90)은 1㎍/㎖로서 모두 감수성을 보였고 quinupristin/dalfopristin에 대해서도 MIC_(90) 0.25㎍/㎖로서 감수성을 보였다. 2) 11균주의 vanA VRE에 대한 항균제들의 MIC 범위는 vanB VRE보다는 다양하였다. MIC_(90)을 보면 amplicillin, erythromycin의 경우 256㎍/㎖, 512㎍/㎖으로 100%에서 내성을 보였응며, gentamicin은 MIC_(90) ≥500㎍/㎖의 고도내성을 보인 경우가 8 균주로서 73%에서 내성을 보였다. Ciprofloxacin은 vanB VRE에서와 마찬가지로 MIC≥32㎍/㎖의 100%의 내성을 보였다. Chloramphenicol의 경우는 MIC_(90)이 16㎍/㎖로서 vanA VRE faecium에 대해서도 감수성이 있는 약제로 남아 있었다. 반면에 doxycycline은 MIC_(90)이 4㎍/㎖로서 시험관내에서는 100% 감수성을 보였고, quinupristin/dalfopristin은 MIC_(90)이 2㎍/㎖로서 내성을 보인 균주는 한 균주도 없었다. 결론 : 국내 임상분리 VRE 16균주는 기존에 사용되어 오던 chloramphenicol, doxycyline등의 항균제에 대하여 시험관내에서 100% 감수성을 보였으며 quinupristin/dalfopristin과 같은 새로운 항균제에 대해서도 중등도 또는 완전 감수성을 보여 향후 VRE에 의한 감염증 치료에 있어 이들 약제의 병합요법등이 고려될 수 있을 것이라 사료된다. Background : Vancomcyin-resistant enterococci (VRE) have become a major nosocomial pathogen in the USA during 1990s. In Korea, VRE have recently emerged as a important nosocomial pathogen and posed therapeutic difficulty. We evaluated the antimicrobial susceptibility of clinical VRE isolates to the antimicrobial agents that are commonly used to treat enterococcal infections and to some alternative drugs and to a newer antibiotic such as quinupristin/dalfopristin(Synercid®). Methods : We evaluated 16 clinical VR-E. faecium (5 vanB VRE, 11 vanA VRE) strains that were isolated from Korea Universtiy and Chonnam University hospital during the last five years (1994-1998). The isolates were identified to the species level by using API 20system. The in vitro susceptibility testing was performed by the agar dilution methods, following the criteria described by the National Committee for Clinical Laboratory Standards (NCCLS). The VRE isolates were tested against antimicrobial agents commonly used to treat enterococcal infections (vancomycin, teicoplanin, ampicillin, gentamicin), potential alternative drugs (chloramphenicol, doxycycline, ciprofloxacin, erythromycin, rifampicin) and a newer antibiotic (quinupristin/dalfopristin). Results : 100% (5/5) of vanB VR-E. faecium were resistant to ampicillin, gentamicin and ciprofloxacm, but 100% (5/5) were susceptible to chloramphenicol, doxycycime, teicoplanin and quinupristin/dalfopristm. 100% (11/11) of vanA VR-E. faecium were resistant to ampicillin, ciprofloxacm, 73% were resistant to gentamicin, and 100% (11/11) were susceptible to chloramphenicol and doxycycline. Quinupristin/dalfopristm showed intermediate or high level susceptibility against all vanA VRE isolates. Rifampin is less susceptible (MIC: 4-16 ㎍/mL) against both vanA and vanB VR-E. faecium. Conclusion : The treatment options for infections caused by VRE seem to be very narrow since a small percentage of those isolates were susceptible to the other antmicrobial agents commonly used to treat these infections. And only a few of the alternative drugs including quinupristin/dalfopristm tested showed good in vitro activity. This study suggested that chloramphenicol, doxycycline and qumuprisitin/dalfopristin would be a good candidate to treat VRE infection, but further studies are necessary to demonstrate the clinical role of these antimicrobial agents.

      • 수온에 따른 Norfloxacin의 잉어(Cyprinus carpio)와 뱀장어(Anguilla japonica)체내에서 약물동태학적 특성

        양식어류의 세균성 질병 치료약제로서 사용량이 증가하고 있는 NFX의 정량분석 방법을 확립하고, 사육수온과 관련된 NFX의 어체내에서 흡수, 분포, 배설 등 NFX의 합목적적 사용 방법 설정에 필요한 약물동태학적 요인을 조사하였다. 수온에 따른 NFX의 어체 내에서의 혈중최대 농도는 뱀장어에서는 유의적인 차이는 없었으나 잉어에서는 혈중최대농도가 23℃에서 유의적으로 높게 (p〈0.05) 나타났을 뿐만 아니라 NFX의 흡수 및 배설속도정수, 최대혈중농도 도달시간 등 어종별, 수온별 약물동태학적 요인에 차이가 있었다. 이러한 결과로 NFX의 효과적인 사용방법과 휴약기간 설정을 위하여 어류의 사육환경과 관련된 임상 약물동력학적 특성을 충분히 고려하여 결정되어야 할 것으로 판단된다. Effects of Temperature on the pharmacokinetics of norfloxacin(NFX) were studied in the cultured carp, Cyprinus caripo, and cel, Angulla japonica, using high performance liquid chromatography(HPLC) originally developed for quinolone determination in livestocks. Pharmacokinetics of NFX was apparently affected by amibent water temperature. In a two-compartment model for carp after oral dosage of 20㎎/㎏, K01 at 13℃ and 23℃ were 2.60 and 5.20/hr, respectively. In carp the K_(w), T_(max) and C_(max) for carp at 13℃ were 13.30/hr, 17.44 ug/ml and 7.00 ug/ml, respectively. The corresponding values at 23℃ were 3.93/hr, 15.40㎍/㎖ and 9.44 ㎍/㎖, respectively. The AUC and T_(1/2) were 355.66 ㎍:hr/㎖ and 12.70 hr at 13℃ and 417.24 ㎍:hr/㎖ and 13.86 hrs at 23℃, respectively, Similar trends were revealed in the NFX pharmacokinetics of cel kept under the two water temperature regimes after oral NFX dosage of 20 mg/kg. These pharmacokinetical results have some implication in the optimal usage of recently introduced antibacterials in farmed fish, which were originally adapted for poultry and mammalian species.

      • 유착성 관절낭염 치료에 있어서 한방 치료와 양방 치료의 임상적 고찰 : 동서협진 모델 개발을 위한 기초 연구를 중심으로

        Objectives : To observe the effect of acupuncture treatment and western medical treatment on frozen shoulder patients. Methods : 39 voluntary patients were randomly assigned to the Eastern treatment group(E group, n=22) and the Western treatment group(W group, n=17). The E group received acupuncture treatment on LI15, TE14, GB21 and Master Dong's acupuncture points, Shin-gwan and Gyun-joong, twice a week for 4 weeks. The W group received suprascapular nerve block, subacromial injection and trigger point injection, twice a week for 4 weeks. Both groups were instructed to practice self exercise during their daily lives. Evaluations were made before treatment and after treatment using Constant Shoulder Assessment(CSA), Shoulder Pain and Disability Index(SPADI) and the patient's satisfaction concerning the treatment was measured by Visual Analogue Scale(VAS). The obtained data were analyzed and compared. Results : The E group showed significant improvement(p<0.05) according to the CSA and SPADI. The patient's satisfaction scored 5.67 on a scale of 10. The W group showed significant improvement(p<0.05) according to the CSA and SPADI. The patient's satisfaction scored 7.73. But the difference between the two groups were insignificant according to CSA and SPADI. Conclusion : Both acupuncture and nerve block treatment significantly improved frozen shoulder. But the difference of the two treatments was insignificant.

        새로운 물질이 개발되어 신약으로 탄생하기까지 많은 시간과 노력, 예산이 필요하다. 최근, 우리 나라에서도, 신약캐발이 어릴고 힘든 분야이지만 새로운 신약개발을 총해 얻어지는 분가가치가 막대함을 인식, 연구에 박차를 가하 있는 실정이다. 본 연구는, 의약품 개발 선진국인 미국 FDA에서 발행하는 참고자료 중 항균제 임상편가에 대한 가이드라인 및 주요 임상적응증별 항균제 기발(지역사획회득성 폐렴, 원내감염성 계렴, 급성기관지염의 2차세균감염, 만성기지염의 급성세균성 악화, 연쇄구균에 의한 인두영 및 편도염 , 단순성 요로감염증, 복합성 요로감염증 및 신우신영, 급성 세균성 부비동옆, 항 바이러스제 개발시 전임상단계에서 고려사항, 항 바이러스제 허가와 된련 잉상적 고려사항, 카레타 괸련 혈류감염)에 있어서의 파이드라인 등의 자료를 소개함으로져, 향후 제약사의 항균제 신약개발 및 허가 신청된 의악품의 전임상차료 및 잉상시험자료의 검토 평가업무에 참고자료로 활용될 수 있을 것이다. Changing or unclear interpretations of clinical trial data needed to demonstrate the effectiveness and safety of antimicrobial drug products have a times led to confusion and frustration on the part of both applicants and division of these drug products reviewers. The FDA published guidance on desing clinical protocols, implementing, and analyzing data from clinical trials for antimicrobial drug products has been presented and additional companion guidances introduced specific issues to individual infections. This document provides applicants and reviewers with minimal information appropriate for the clinical development of routine antimicrobial drug products and identifies issues common to many antimicrobial drug applications. The agency can use the kind of information to determine whether the antimicrobial drug product under study is safe and effective in the treatment of the specific infection studied.
