순수어농이란 발성언어에 대한 이해와 반복에 장애를 보이지만 읽기, 쓰기, 비발성언어에 대한 이해는 보존되는 경우를 일컫는다. 저자들은 45세 루푸스 환자에서 루푸스 혈관염으로 인해 일차 청각 피질을 포함한 왼쪽 관자이랑에 뇌경색이 발생하여 초래한 순수어농 1예를 경험하였기에 보고하는 바이다. Pure word deafness is characterized by an impairment of auditory comprehension and repetition of speech while a preservation of reading, writing, spontaneous speech and ability to comprehend non-verbal sounds. It is caused by trauma, infection in the temporal lobe of brain. In most cases, the lesions are bilateral, in the middle third of superior temporal gyri, but in a few cases unilateral lesions of dominant temporal lobe are localized. We experienced a case of 45 year-old right handed wonman with systemic lupus erythemato년 (SLE) who presented pure word deafness after left temporal lobe infarction including primary auditory center. Her comprehension was impaired, while reading and visual comprehension were normal. Audiometric tests and auditory evoked potentials disclosed no hearing defect. Brain MRI showed a left temporal lobe infarction without abnormal vascular finding in MRA. She wasdiagnosed as pure word deafness due to SLE vasculitis and managed with high dose steroid and cyclophosphamide therapies.
저선량 γ선 조사가 고추의 광합성과 광 스트레스 경감에 미치는 효과

고추 종자에 저선량 γ선을 조사시킨 고추 식물체의 생육과 광합성 능 및 광 스트레스 반응에 미치는 영향을 조사하였다. 저선량 4 Gy가 조사된 고추 식물체에서 광합성에 의한 산소발생이 대조구에 비해 1.5배정도 높은 것으로 나타났다. 고추 잎에 900 μmol/㎡/s의 빛의 세기로 광저해를 4시간 유도하였을 때 최대 광합성능 (Pmax)이 대조구의 경우 20%정도 감소되는 반면 4 Gy 조사구는 3% 정도의 감소를 보였다. Fv/Fm는 광저해가 진행됨에 따라 감소되는 경향을 보이며 대조구의 경우 4시간 처리시 fv/Fm 값이 50% 정도 감소되는 것을 볼 수 있었다. 반면 4 Gy조사구는 fv/Fm값이 대략 37%정도 감소되어 대조구에 비해 광 스트레스에 대해 덜 민감한 것으로 나타났다. Fo는 광저해가 진행됨에 따라 거의 변화가 없었으며 대조구나 4 Gy 조사구 사이의 차이도 거의 없는 것으로 나타났다. 광계Ⅱ의 광양자 수율인 Φ_PSⅡ과 광계Ⅱ 반응중심에 의한 여기 포획률을 나타내주는 1/Fo-1/Fm 또한 광저해가 진행됨에 따라 감소되었으며 4시간을 처리했을 경우 각각 대조구는 47%, 4 Gy 조사구는 30%의 감소를 볼 수 있었다. 비광화학적 소멸인 NPQ는 광저해가 진행됨에 따라 감소되는 경향을 보였으나 대조구와 4 Gy 조사구간에 차이는 없는 것으로 나타났다. 이러한 결과를 볼 때 종자의 종피를 투과한 저선량의 γ선이 식물의 광합성을 증대시키고 동시에 광 스트레스에 대한 저해를 감소시키는 것으로 보인다. The effect of low dose Y radiation on photosynthesis and the reduction of photoinhibition in red pepper plant was investigated. The seedling height, leaf width and leaf length of pepper were stimulated in plants grown from seeds irradiated with the tow dose of 4 Gy. The O_2 evolution in the 4 Gy irradiation group was 1.5 times greater than in the control. To investigate the effect of low dose Y radiation on response to high light stress, photoinhibition was induced in leaves of pepper by illumination of high light (900 μ㏖/㎡/s). Pmax was decreased with increasing illumination time by 20% in the control, while hardly decreased in the 4 Gy irradiation group. The photochemical yield of PSII, estimated as Fv/Fm, was decreased with increasing illumination time by 50% after 4 hours while Fo did not change. However, Fv/Fm in the 4 Gy irradiation group was decreased by 37% of inhibition, indicating that the photoinhibition was decreased by the low dose Y radiation. Changes in the effective quantum yield of PSII, Φpsu, and 1/Fo-1/Fm, a measure of the rate constant of excitation trapping by the PSII reaction center, showed similar pattern to Fv/Fm. And NPQ was decreased after photoinhibitry treatment, showing no difference between the control and the 4 Gy irradiation group. These results showed the positive effect of low dose Y radiation on the seedling growth and the reduction of photoinhibition.

Higher education institutions in the 21st century are feeling more and more pressure to show and prove their due accountability toward the society. The fulfillment of their social responsibilities can be accomplished by teaching and research. The quality of teaching in higher education institutions have been approached from various perspectives and tried to be measured by different methods. Among others, students' evaluation and ratings have been viewed as useful tools to evaluate the quality of education and supporting services of the institutions. This study analyzed 5 aspects of the three major student's evaluation and engagement questionnaires: National Survey of Student Engagement, Course Experience Questionnaire, Australian Survey of Student Engagement. In comparative analysis, researchers concluded CEQ has several shortcomings to be an efficient and effective student engagement scale while it still has some merits such as offering a opportunity to every college graduate to evaluate their college teaching experience. NSSE and AUSSE, taking the sample study framework, are reviewed to be easier to be administrated and contain more plentiful information about various facets of college students' experience and its relationship with achievements in college. This findings are expected to offer useful implications to those who develop the Korean scale of student experience and engagement. 본 연구는 세계적으로 사용되고 있는 대학생 학습과정 조사도구들 중 NSSE, CEQ, AUSSE의 개발 배경과 변천 과정, 개발 목적, 주요 내용, 결과의 활용 다섯 가지 측면을 조사한 후 이를 비교․분석함으로써 우리나라 고등교육기관의 대학생 학습과정 조사도구 개발을 위한 정책적 시사점을 제안하는 것을 목적으로 한다. 이들을 비교한 결과 다섯 가지 준거를 통한 비교․분석 결과 대학 강의 경험을 주요 내용으로 하는 전수조사 방식의 CEQ보다는 강의와 강의실 밖의 다양한 학습 기회에 대한 학생 참여도를 기준으로 대학생의 학습과정과 대학 교육의 질을 평가하는 표본조사 방식의 NSSE와 AUSSE가 우리의 고등교육 시스템에 더 적합할 뿐만 아니라 국제적 비교․분석도 가능한 조사도구로 평가되었다.
만성 C형 간염 환자에서 페그인터페론 알파2a와 리바비린 병합 치료중 발생한 벨마비 1예

페그인터페론과 리바비린 병합요법은 만성 C형 간염의 일차 치료법이다. 저자들은 만성 C형 간염 환자에서 페그인터페론 과 리바비린 병합 요법 중에 발생한 벨마비 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. 환자는 5년 전부터 만성 C형 간염을 앓아온 48세 남자이며, PEG-IFN α-2a 135μgm 피하주사 주1회와 하루 1200㎎의 리바비린을 투여하였다. 치료시작 후 9개월째 환자는 오른쪽 안면의 근력약화를 호소하였으며 벨마비로 진단되었다. 페그인터페론과 리바비린 병합요법을 지속하면서 관찰하였다. 환자의 벨마비는 페그인터페론 치료를 중단하지 않았음에도 3개월후 증상이 회복되고 이후 벨마비 재발 없이 현재 경과관찰 중이다. 만성 C형 간염에서 페그인터페론과 리바비린 병합 요법시 벨마비의 발생 가능성을 염두에 두어야 하겠다. A Case of Bell's Palsy Associated with Combination Therapy of Pegylated Interferon Alfa-2a (PEG-IFN) and Ribavirin for Chronic Hepatitis C Virus Infection Pegylated interferon alfa(PEG-IFN α) and ribavirin therapy is the first line treatment for chronic hepatitis C. Mild complications of the therapy are common, but more serious complications are rare. We report here a case of Bell's palsy that occurred in a patient with chronic hepatitis C virus infection during combination therapy of PEG-IFN α-2a and ribavirin. The patient was 49-year-old man with chronic hepatitis C (genotype 1b) for 8 years. He had compensated liver cirrhosis with splenomegaly. Therapy with PEG-IFN α- 2a 135mcg/week and ribavirin 1200mg/day was initiated. After 9 months of the therapy, the patient showed a loss of muscular tone on the right side of his face. A diagnosis of Bell's palsy was made. The Bell's palsy resolved over 3 months despite continuation of the combination therapy.

<P><B>Background</B></P><P>Non-dippers were reported as showing different left atrial function, compared to dippers, but no study to date investigated the changes in the left atrial function according to the diurnal blood pressure pattern, using tissue Doppler and strain imaging.</P><P><B>Methods</B></P><P>Forty never treated hypertensive patients between 30 and 80 years of age were enrolled in this study. Patients were classified as non-dippers when, during night time, they had a blood pressure decrease of less than 10%. Strain of the left atrium was measured during late systole, and peak strain rates of the left atrium were measured during systole, early and late diastolic periods.</P><P><B>Results</B></P><P>The left atrial expansion index, left atrial active emptying volume and left atrial active emptying fraction were all significantly increased in non-dippers. They also had increased values of mean peak left atrial strain (dippers = 21.26 ± 4.23% vs. non-dippers = 24.91 ± 5.20%, <I>p</I> = 0.02), strain rate during reservoir (dippers = 1.29 ± 0.23 s<SUP>-1</SUP> vs. non-dippers =1.52 ± 0.27 s<SUP>-1</SUP>, <I>p</I> = 0.01) and contractile period (dippers = -1.38 ± 0.24 s<SUP>-1</SUP> vs. non-dippers = -1.68 ± 0.32 s<SUP>-1</SUP>, <I>p</I> < 0.01).</P><P><B>Conclusion</B></P><P>Strain and strain rate acquired from color Doppler tissue imaging demonstrate exaggerated reservoir and booster pump function in never-treated, non-dipper hypertensive patients. These methods are simple and sensitive for the early detection of subtle changes in the left atrial function.</P>
