전형적인 DISIDA scan 소견의 Rotor 증후군 1예

만성의 비용혈성 직접형 고빌리루빈혈증은 Rotor 증후군과 Dubin-Johnson 증후군의 특징적인 소견으로, Rotor 증후군은 드문 양성 유전성 질환이며, Dubin-Johnson 증후군과 감별을 필요로 한다. 본 저자들은 황달을 주소로 내원한 26세 남자 환자에서 고빌리루빈혈증에 대한 검사를 시행하여 담도폐쇄 소견없이 간담도 스캔상 전형적인 Rotor 증후군의 소견 즉, 강한 심장 배후방사능이 지속되면서, 간담도계의 조영은 지연 촬영에서도 관찰되지 않으며 신장으로의 배설이 계속 관찰되는 특징적인 소견과 정상 간조직 검사 결과로 Rotor 증후군으로 진단한 1예를 경험하였기에 문헌 고찰과 함께 보고하는 바이다. Chronic nonhemolytic conjugated hyperbilirubinemia characterizes both Rotor and Dubin-Johnson syndromes. Rotor syndrome is a rare, benign familial disorder and needs to be differentiated from Dubin-Johnson syndrome. Cholescintigraphy is a simple and informative procedure for the diagnosis, which can differentiate Rotor syndrome from Dubin-Johnson syndrome. A 26-year-old man was admitted due to asymptomatic persistent jaundice. Physical examination revealed icteric sclera without hepatosplenomegaly. Laboratory findings showed increased serum bilirubin with direct bilirubinemia. Markedly decreased hepatic uptake and poor visualization of the gallbladder and biliary tract were shown in ^(99m)Tc-DISIDA scan. Histology of the liver showed no diagnostic abnormality without pigmentation. But, we report a case of Rotor syndrome that showed characteristic cholescintigraphic findings with normal urinary total coproporphyrin excretion.

Corrugated steel-plate resist the external force by the combined action of retained soils and structures. Since it is easy to transport and assemblage of in-situ, the duration and cost of construction can be reduced. Corrugated plate structures, therefore, are widely used in culverts, hydro passages and simple bridges. In this study, behavior analysis is performed to corrugated steel-plate in earth with elastic-plastic soil element method and spring soil element method. An object of this study is to decide more suitable method which indicate practical behavior of corrugated steel-plate in earth

일차성 간 결핵은 특이 증상이 없고 드물어, 진단이 늦어지거나 다른 질환으로 오인되기 쉽다. 저자들은 소화불량과 체중감소를 주소로 내원해 시행한 복부 컴퓨터 촬영에서 간 좌엽에 단일공간점유병변으로 담관암을 의심하였으나 복부초음파 유도하 간 종괴 생검에서 만성 육아종성 염증 및 괴사 소견을 보여 간 결핵으로 진단하였다. 치료로 동반된 위 배출구 폐쇄에 의한 증상이 심해 간좌엽 절제술 및 위-공장 문합술을 실시하였고, 이후 6개월간 항결핵제를 복용하였다. 본 증례를 통해 간내에 단일 종괴의 감별진단에 있어 일차성 간결핵을 고려해야 하고 반드시 간조직 생검을 통해 감별진단이 이루어져야 함을 경험하였기에 문헌고찰과 함께 보고하는 바이다. Primary hepatic tuberculoma is a rare malady that is not accompanied by local symptoms, so the diagnosis can frequently be delayed or misconceived as other disease. We report here on an unusual case of primary hepatic tuberculoma that was misconceived as a cholangiocarcinoma on the imaging study. A 54-year-old man presented with dyspepsia and weight loss for 1 month. Abdominal computerized tomography demonstrated a solitary space-occupying lesion on the left lobe of the liver that suggested there was a cholangiocarcinoma accompanied with gastric outlet obstruction. The lesion was diagnosed by the ultrasonographic guided liver biopsy as a chronic granulomatous inflammation with necrosis, and the patient was treated via surgical resection with gastro-jejunostomy followed by antituberculosis chemotherapy. Twelve months later, no evidence of recurrence was noted when examining the patient`s symptoms and the imaging studies.(Korean J Med 73:423-427, 2007)
Comparison in vivo knock-in strategy using dual adeno associated virus

Genetic disorders with loss of function could overcome AAV-mediated gene therapy, but AAV has a limitation of capsid-mediated immunogenicity and the possibility of random integration. In contrast, genome editing offers the potential to get permanent treatment without repeated transduction. However, in vivo knock-in efficiency is extremely low; thus, a new innovative strategy to overcome this is needed. We tried to evaluate the knockin platform against human genetic disorder by inserting the protein-coding gene. In this experiment, hemophilia B was selected as a candidate genetic disorder, and the KI efficiency and therapeutic effect were verified by dual transduction of AAV-packed Campylobacter jejuni Cas9 and donor template. HR strategy did not develop any evidence of in vivo therapeutic KI, and it seemed to be reasoned by the non-dividing characteristics of target hepatocytes. In contrast, NHEJ mediated integration and bidirectional template AAV mediated integration exhibited evidence of in vivo KI and produced therapeutic functional proteins (human F9 blood concentration- NHEJ mediated KI: 50ng/ml, bidirectional AAV: 250 ng/ml). Besides, the AAV mediated integration strategy created a more precise or unbroken KI than NHEJ mediated KI. Although the reason for the incomplete KI of the HITI strategy was unclear, the AAV mediated integration would be better in vivo KI strategy than other strategies. There were also several interesting findings; 1) AAV transduction interference between dual AAVs, 2) no enhancement of KI efficiency by interval AAV transduction. We present bidirectional AAV donor template mediated KI to develop relative high and preservative therapeutic gene KI into target cell under in vivo situation.

As the movement for carbon neutrality spreads around the world, research on hydrogen energy is also being actively conducted. In the hydrogen value chain, liquefaction is a particularly energy-intensive process. Although the operating energy of the hydrogen liquefaction process can vary greatly depending on the season or regional cooling water temperature, previous studies have not taken this into account. In this study, we quantitatively identify the effect of the change in cooling water temperature on exergy destruction, specific energy consumption (SEC), and coefficient of performance (COP) of the liquefaction process. In addition, we propose a design improvement to reduce exergy destruction with the exergy analysis of multi-stream heat exchangers. A new design (auxiliary process) that reduces exergy destruction is proposed by analyzing the device where energy destruction occurs the most. When the cooling water temperature increases from 20 °C to 35 °C, there is a tendency for SEC and exergy destruction to increase and the COP to decrease. The new design with an auxiliary process shows a decrease in SEC and a reduced rate of increase in SEC in response to the increase in cooling water temperature. The base case without the auxiliary cycle shows that the SEC with cooling water of 35 °C is 14.66% greater than that with cooling water of 20 °C, while the proposed process shows the rate of increase of 9.70%. This means that adding the auxiliary cycle can improve the energy efficiency and increase robustness to variations in ambient conditions.

The Diesel Multiple Units (DMU) can be classified into three types by the type of the transmission. One of those is a Diesel Hydraulic Multiple Unit which is operated with a hydraulic transmission and its propulsion system consists of a diesel engine, transmission, cooling unit, fuel system, intake air and exhaust system, etc. In designing and integrating the propulsion system components, the operation and environment conditions should be considered as well as related standards. This paper describes points to be considered on design with related to propulsion system of Diesel Hydraulic Multiple Unit that is now being developed for Turkish Republic State Railway -TCDD.
