        Blockers of renin-angiotensin system(RAS) including ACE inhibitor or ARB are one of the most frequently prescribed medications for the treatment of hypertension, heart failure and proteinuria. One of the major side effects of these RAS blockers is the deterioration of renal function, mainly due to a reduction of intraglomerular pressure. Therefore, close monitoring of renal function is recommended when RAS blockers are initially prescribed, especially for the patients with impaired renal function. We report a patient who was transferred to our hospital due to the sudden development of oliguria and dyspnea after treatment for hypertension with ACEi and ARB. She was finally diagnosed as RAS blocker-induced acute renal failure with pulmonary edema complicated on congenital solitary kidney. After hemodialysis and conservative treatment, her renal function was recovered with maintenance of normal urine output. Conclusion:This case highlights the necessity of the functional and structural evaluation of kidney to prevent the serious complication such as acute renal failure before the administration of ACEi and/or ARB.

        Variation in clinical usefulness of biomarkers of acute kidney injury in young children undergoing cardiac surgery

        Background: Acute kidney injury (AKI) is one of the most significant postoperative complications of pediatric cardiac surgery. Because serum creatinine has limitations as a diagnostic marker of AKI, new biomarkers including neutrophil gelatinase-associated lipocalin (NGAL), kidney injury molecule-1 (KIM-1), and interleukin-18 (IL-18) are being evaluated to overcome these limitations and detect AKI at an early stage after cardiac surgery. Purpose: This study aimed to investigate the clinical usefulness of these biomarkers in young children. Methods: Thirty patients with congenital heart diseases who underwent cardiac surgery using cardiopulmonary bypass (CPB) were selected, and their urine and blood samples were collected at baseline and 6, 24, and 48 hours after surgery. Serum creatinine and blood urea nitrogen levels as well as NGAL, KIM-1, and IL-18 levels in urine samples were measured, and clinical parameters were evaluated. Results: Of the 30 patients, 12 developed AKI within 48 hours after cardiac surgery. In the AKI group, 8 of 12 (66.6%) met AKI criteria after 24 hours, and urine KIM-1/creatinine (Cr) level (with adjustment of urine creatinine) peaked at 24 hours with significant difference from baseline level. Additionally, urine KIM-1/Cr level in the AKI group was significantly higher than in the non-AKI group at 6 hours. However, urine NGAL/Cr and IL-18/Cr levels showed no specific trend with time for 48 hours after cardiac surgery. Conclusion: It is suggested that urine KIM-1/Cr concentration could be considered a good biomarker for early AKI prediction after open cardiac surgery using CPB in young children with congenital heart diseases.

        Effect of Ar Plasma Treatment for Surface of Insert Metal on Property of TLP Bonding Joint for Power Module

        In this study, we investigated the effect of plasma treatment for the surface of an insert bonding metal on the prop�erties of a transient liquid phase (TLP) bonding joint for a power module. Sn preforms, as the insert bonding metal for the TLP bonding, were used with and without Ar plasma treatment. To investigate the effect of Ar plasma treat�ment, the TLP bonding for two structures of Cu-finished Si chip/Sn preform/organic solderability preservative (OSP)-finished direct bond copper (DBC) substrate (Cu/Sn/Cu), and Ni-finished Si chip/Sn preform/Ni(P)-finished DBC substrate (Ni/Sn/Ni) was performed with 1 MPa at 300 ℃ and the bonding times were 10, 30, and 60 min, respectively. After the TLP bonding, we observed interfacial reactions and formations of intermetallic compounds (IMCs) under various bonding conditions. To evaluate mechanical properties, a shear test was also performed. Compared to the TLP bonding joint that used bare Sn preforms without Ar plasma treatment, growth of IMCs at the bonding joint that used Sn preforms with Ar plasma treatment occurred faster, and the IMCs could be formed through the entire joint despite the bonding time of 10 min. Meanwhile, by increasing the bonding time, Cu6Sn5 and Cu3Sn were formed at the Cu/Sn/Cu TLP bonding joint, whereas Ni3Sn4, Ni-Sn-P, and Ni3P were observed at the Ni/Sn/Ni joint. In the case of the Cu/Sn/Cu joint, we observed that increasing Cu3Sn formation while increasing the bonding time could be beneficial to the shear strength of the joint. Further, shear strengths of the joint were not sig�nificantly changed under the bonding conditions after Ni3Sn4, Ni-Sn-P, and Ni3P IMCs were formed at the entire joint of the Ni/Sn/Ni.

        유리 측대퇴 피판을 이용한 압궤수부손상 재건술

        Utilizing the lateral thigh flap as originally described by Se-Min Baek. in 1983, we have treated seven cases of reconstruction involving crushing injuries of the hand. This flap is derived from the third perforator of the deep femoral artery and accompanying wein as per Baek`s method. When a large skin flap was needed such as the case of crushing hand injuries, a lateral thigh flap was more useful than any other flap. From our experiences for reconstruction of crushing hand injuries, the following advantages of this flap are noted: 1. It is possible to obtain a large size of all skin flap. 2. It is relatively thin with a small amount of the subcutaneous fat. 3. The vascular pedicle is long (10㎝ or more), facilitating microvascular anastomosis. 4. The lateral thigh flap can be a sensory flap. 5. It can be accommodated easily to any operating position.

        This study is to provide a evaluation method for the penetration depth of emulsified refined bio diesel(ERBD)applied to a surface of the concrete by using water absorption capability of the concrete. The concrete applied with ERBD was immersed at water for 1 min., 5min., and 10 min. and then was checked the brightness with elapse of time. Test results indicated that there was clear difference between ERBD part and non ERBD part in concrete specimen after measuring the brightness until 120min.

      • Comparisons of obstetrical outcomes among vaginal, intramuscular progesterone treatment and conservative management for the prevention of preterm birth in twin pregnancies with a short cervix

        목적: To determine the differences in pregnancy outcomes among cases treated by vaginal progesterone, intramuscular(IM) progesterone and conservative management in twin pregnancies with a short cervix. 방법: This is a retrospective study of 273 twin pregnancies complicated by a short cervix(<2.5cm) who delivered from 2007 to 2016 in CHA Bundang Medical Center. Women who received cervical cerclage were excluded. Treatment groups included 1) group I; conservative management group without progesterone treatment(n=174), 2) group II; vaginal progesterone suppository group(n=30), 3) group III; IM progesterone injection group(n=69). Primary outcomes were spontaneous birth at <28, 32, 34, or 36 weeks of gestational age(GA). The secondary outcomes included hospitalized for tocolytics or antenatal corticosteroids, small for gestational age and low APGAR score(<7) at 5 min. 결과: Preterm birth before 36 weeks of GA was different among 3 groups(29.3%[51/174] vs.20.0%[6/30] vs.43.5%[30/69], p=0.034). Low APGAR score(<7) at 5min was significantly different among 3 groups(7.8%[27/348] vs.5.0%[3/60] vs.15.2%[21/138], p=0.018). After multivariate analysis, preterm birth(<36 weeks) occurred more frequently in group III than group I(aOR 6.90 95%CI:1.06-45.04, p=0.044). However, there was no significant difference between group II and III. Meanwhile, there were more cases with low APGAR score(<7) at 5 min in group III than group I(aOR 6.58 95%CI:1.43-30.21, p=0.015) and group II(aOR 17.04 95%CI:1.56-185.74, p=0.020) after multivariate analysis. 결론: In twin pregnancies complicated by short cervical length, IM progesterone group showed significantly greater occurrence of preterm birth before 36 weeks of GA in comparison with vaginal progesterone group and conservative management group.

        2003년 국내 중증급성호흡기증후군 진료 현황 및 문제점 분석

        목적 : 2002년 말 중국에서 SARS가 발생한 이후 국내에서도 2003년 10월까지 총 3명의 추정환자, 17명의 의심환자가 보고되었다. 향후 추가적인 SARS의 유행이 우려되는 상황에서, 그간의 SARS 환자 진료에 있어서의 실질적인 준비사항, 진료 현황 등에 대한 조사를 통해 문제점을 파악하여, 향후 더 나은 대비가 될 수 있도록 개선점을 제시하고자 하였다. 재료 및 방법 : SARS로 의심되는 환자를 진료 경험이 있는 병원의료진을 대상으로 2003년 10월에 설문조사를 실시하였다. 설문에는 SARS 환자 진료 시의 실질적인 조치, 진료 현황, 병실, 응급실 및 외래에서의 격리 시설과 준비사항, 보건당국의 관리와 지원에 관한 사항을 포함하였다. 결과 : 대상이 되는 22개 병원 중 17개(17/22, 77.2%) 병원이 설문에 응하였다. SARS 환자를 위한 격리실은 응급실, 외래, 일반병실 및 중환자실에서 각각 9개(9/17, 52.9%), 5개(5/17, 29.4%), 15개(15/16, 93.7%), 4개(4/16, 25%) 병원에서 음압처리가 되어있지 않은 일인실 혹은 다인실이 사용되었고, 1개(1/16, 6.3%) 병원에서만 일반병실에서 음압격리실이 운영되었다. 입원환자의 진찰 시 개인보호구의 착용은 거의 모든 의료기관에서 이루어졌다. 보건당국에서 SARS지정병원의 시설 등을 사전에 확인한 곳은 1곳(1/12, 8.3%)이였고, 14개 병원(14/15, 93.3%)에서는 보건당국에 의뢰한 검사결과를 통하 받지 못하였다. 결론 : 의료기관에서 SARS 환자용 격리실뿐만 아니라 기존의 격리실 설비 등이 미흡하였으며, 특히 중환자실 및 외래의 준비가 더욱 부족하였다. 보건당국의 의료기관에 대한 종합적인 지원이 부족하였고, 병원과의 원활한 연계가 잘 이루어지지 않았다. SARS 만이 아닌 격리를 필요로 하는 질환의 적절한 진료를 위해 향후 병원 시설의 정비와 정부차원에서의 보다 구체적이고 실질적인 대책마련이 필요하다. Background : There was an worldwide outbreak of the Severe Acute Respiratory Syndrome (SARS) originated from China in late 2002. During that period three cases of suspected SARS and 17 cases of probable SARS were reported in Korea. With the concerns about the reemergence of SARS-coV transmission, it is important to be prepared for any possibility. So, this study is aimed to analysis the past measures in managing SARS and propose the amendatory plans to improve the preparedness. Materials & Methods : Questionnaires were collected among clinicians with any experience in managing the probable or suspected SARS cases in Oct. 2003. 17 out of 22 hospitals responded to the questionnaire. The contents in the questionnaire were practical activities, personal equipments, response plans, isolation facilities in emergency centers, outpatient clinics, general wards and intensive care units, and relationship with the public health department. Results : The dedicated isolation rooms in emergency centers, outpatient clinics, general wards, and intensive care units were prepared in 9 (9/17, 52.9%), 5 (5/17, 29.4%), 15 (15/16, 93.7%), and 4 (4/16, 25.0%) hospitals, respectively. Except for one hospital that newly made negative pressure room for SARS, single or multi-bed rooms without airborne infection control were used in all the other hospitals. The personal precaution principles were kept quite well in general wards. Before the designation of SARS hospital by the public health department prior evalution to see if the hospital was suitable for managing SARS was conducted in only 1 (1/12, 8.3%) hospital. The results of laboratory diagnosis were reported back in 1 (1/15, 6.6%) hospital. Conclusions : The isolation facilities which can control airborne infection were almost deficient not only for SARS but also for other respiratory transmissible diseases. For the infection control of transmissible diseases including SARS, more investment is needed on medical facilities and comprehensive support from the public health department required.

        본 논문에서는 비파괴 검사를 통하여 얻은 세라믹 영상에 퍼지 기법과 ART2 기법을 적용하여 결함을 검출하는 방법을 제안한다. 제안된 방법은 세라믹 소재로 얻어진 영상에서 결함의 구간을 설정하기 위해 퍼지 스트레칭 기법을 적용하여 명암도를 대비시킨다. 명암 대비가 강조된 영상에서 퍼지 이진화 기법을 적용한 후, 상/하 경계선에 가장 많이 분포된 곳을 Max, Min으로 설정하고, Max+20, Min-20을 결함 구간으로 설정한다. 설정한 결함 구간 내의 비파괴 세라믹 영상에서 ART2 알고리즘 기법을 적용하여 세라믹 영상의 결함을 검출한다. 본 논문에서 제안된 방법을 비파괴 세라믹 영상을 대상으로 실험한 결과, 제안된 방법이 기존의 세라믹 결함 검출 방법보다 비파괴 세라믹 영상에서 다양한 형태의 결함이 검출되는 것을 확인하였다.

        Effect of spark plasma sintering conditions on the thermoelectric properties of (Bi<sub>0.25</sub>Sb<sub>0.75</sub>)<sub>2</sub>Te<sub>3</sub> alloys

        <P><B>Abstract</B></P> <P>As a field-assisted technique, spark plasma sintering (SPS) enables densification of specimens in a very short period of time compared to other sintering techniques. For high performance thermoelectric material synthesis, SPS is widely used to fabricate nanograin-structured thermoelectric materials by rapidly densifying the nanopowders suppressing grain growth. However, the microstructural evolution behavior of thermoelectric materials by SPS, another important process during sintering, has been rarely studied. Here, we explore SPS as a tool to control the microstructure by long-time SPS. Using p-type (Bi<SUB>0.25</SUB>Sb<SUB>0.75</SUB>)<SUB>2</SUB>Te<SUB>3</SUB> thermoelectric materials as a model system, we systematically vary SPS temperature and time to understand the correlations between SPS conditions, microstructural evolution, and the thermoelectric properties. Our results show that the relatively low eutectic temperature (∼420 °C) and the existence of volatile tellurium (Te) are critical factors to determine both microstructure and thermoelectric property. In the liquid-phase sintering regime, rapid evaporation of Te leads to a strong dependence of thermoelectric property on SPS time. On the other hand, in the solid-phase sintering regime, there is a weak dependence on SPS time. The optimum thermoelectric figure-of-merit (Z) of 2.93 × 10<SUP>−3</SUP>/K is achieved by SPS at 500 °C for 30 min.         저온 플라즈마 처리한 쌀국수의 품질 특성

        쌀국수의 유통 안전성 확보를 위한 기초자료를 확보하기 위하여 최근 각광받고 있는 저온 플라즈마 처리에 의한 쌀국수의 품질 특성 분석을 하였다. 본 연구에서 이용한 플라즈마는 컨테이너형 유전격벽 플라즈마로 방전 가스는 공기를 활용하여 0, 5, 10 및 20분 처리하였고 미생물 감균효과, 색도, 경도 및 지질산패도 변화를 측정하였다. 쌀국수에 B. cereus 및 E. coli O157:H7을 접종한 후 20분간 플라즈마 처리 시 E. coli O157:H7은 2.75 log CFU/g 감소하였고, B. cereus는 검출되지 않았다. 색도 측정결과 플라즈마 처리에 의해 명도(L<SUP>*</SUP>), 적색도(a<SUP>*</SUP>) 및 황색도(b<SUP>*</SUP>) 모두 증가하였으며, 경도는 처리시간이 증가함에 따라 감소하는 경향을 보였다. 하지만 플라즈마 처리에 의해 지질산화가 일어나는 경향을 보였다. 따라서 공기로 방전된 저온 플라즈마 기술은 쌀국수의 품질안전성을 개선될 것이라고 판단되나 관능적 품질 특성 개선을 위한 적합 플라즈마 모델 선정, 포장방법 개선 등의 후속연구가 필요하다고 판단된다. Cold plasma has been applied to improve quality of food product; however, studies on its effects on microbial and physicochemical qualities of rice noodles are rarely conducted. In this study, changes in the quality characteristics of rice noodles treated by cold plasma were determined. Cold plasma was generated in a square-shaped plastic container (250 W, 15 kHz, ambient air), and dielectric barrier discharge plasma treatments were applied to rice noodle samples for 0, 10, or 20 min. Rice noodles inoculated with either Bacillus cereus or Escherichia coli O157:H7 were subjected to plasma treatment for 20 min, and the approximate bacterial count reduction were 4.10 and 2.75 log CFU/g, respectively. The Hunter color values of the sample were increased after cold plasma treatment. Peroxide values and thiobarbituric acid reactive substance (TBARS) were increased with an increase in cold plasma treatment time. Futhermore, lipid oxidation was enhanced. Although further studies are warranted to evaluate changes in chemical qualities, such as lipid oxidation of rice noodles, induced by cold plasma, the results suggest that cold plasma can improve the microbial and physical qualities of rice noodles.
