        몽골 전통의학 방제(方劑)의 현대적 활용 고찰 - 『Маш гайхамшигт хуримын сан』 방제편(方劑篇) 소재 탕약(湯藥)을 중심으로 -

        This study is aimed to determine the modern application and transmission of decoctions written in Persimmon’s Treasury of Wonders - ngo mtshar dga’ ston, Mash Gaihamshigt Hurimin San (hereinafter abbreviated as <MGHS>), one valuable source of traditional Mongolian medicine which was compiled in the early 20th century. The study is focused on three aspects such as ingredients, compounding methods and uses of decoctions in order to determine their application and importance in modern medicine. There are 45 and 70 kinds of decoctions described respectively in the chapter of prescriptions of <MGHS> and the Guidebook of Traditional Medicinal Substances and Prescriptions published from the Traditional Medical Science and Technology Corporation of Mongolia. Overlaps of 11 kinds of decoctions observed between the two books. These include Tsarvan-5 (Царван-5), Lider-7(Лидэр-7), Manu-4 (Ману-4), Lider-5 (Лидэр-5), Jonlon-5 (Жонлон-5), Zandan-3 (Зандан-3), Aglig-4 (Аглиг-4), Indar-4 (Индар-4), Bamsel (Бамсэл), Baragshun-5 (Барагшун-5) and Tsarvan-4 (Царван-4). The following are the results obtained to the three focuses mentioned above. 1. No great methodological differences were observed in compounding of the 11 kinds of decoctions. Besides, no other medicinal forms of these 11 and the remaining 34 and 59 kinds of decoctions described in the two books respectively were found in the Guidebook of Traditional Medicines and Prescriptions. 2. There are great differences in the indications and actions of the 11 kinds of decoctions observed between the two books. This might be related to the scientific research activities on traditional medicines and interpretations of traditional concepts of diseases by modern medical concepts. 3. No differences were observed in the ingredients between the 11 kinds of decoctions written in the two books. This might be related with that the herbs which are grown mostly in Mongolia are included in the decoctions. Mostly the same terms were used for the herbs in the two books. However it is important to standardize the nomenclatures of medicinal row materials in order to avoid possible confusions. As mentioned above Mongolia’s traditional method of compounding decoctions was respected and handed down to modern times and it was revealed that there were no great differences in the traditional and modern applications. However the effort to improve medical credibility was noteworthy through scientific study for the application of traditional compounding method. In this process there were some observations on the cases that the actions of the medicines and the related diseases introduced in <MGHS> are differently used in modern times. It can be said that Mongolian decoctions were transmitted in diverse ways such as respect to the traditional knowledge and scientific reverification process for application to the modern Mongolian medicine. Traditional Mongolian medicine has played an important role in global tradtional medical history as the heritage of rich life culture and unlimited potential for modern applications. In particular, the traditional knowledge related with traditional decoctions and medication can be said to be the important foundation for the development of modern medical knowledge. Therefore we will conduct research step by step, not only the decoctions introduced in <MGHS> but also undiscovered many traditonal medicines introduced in Mongolian medical literatures.

        정지용 문학에서의 ‘조선’이라는 기표의 의미 - ‘기행 산문’과 ‘고향 시편’을 중심으로

        The aesthetic achievement of Jeong Ji-yong’s later work is that it had independently reconstructed Joseon’s traditions based on the possibility of ‘creating modern literature in Korean.’ This paper will focus on the re-examination of the theme ‘Joseon-esque objects/styles’ which was shown in the travel essays of Jeong Ji-yong. It will also review the specialities of ‘Rhyme, Color, Joy, Sadness, and Happiness’ which is a part of the theme ‘Joseon-esque style’ seen in Jeong Ji-yong’s work. Moreover, the study will take a deeper look at the concept of ‘Spirit of Joseon’ which appears repetitively in Jeong Ji-yong’s work. Especially in his travel essays, Jeong Ji-yong reconstructs various objects such as the Dadohae, Kisaeng, and the women of Pyongyang to expand the nature and landscape of Joseon into a space of ‘Joseon Shim,’ where historic significance can be recalled. In addition, the ‘Joseon Shim’ shown in many psalms –such as 「Nostalgia」 and 「Hometown」- has an impression that it borrows the camera technique of documentary films. Through this, we can discover the beauty of ‘arrangement’ that has not been noticed in other poems of Jeong Ji-yong. Therefore, Jeong’s indulgence in ‘Joseon-esque objects/styles’ cannot be considered as a distinctive feature that is only shown in his late work. Instead, we can understand that it was a solid foundation that can be seen in all of his work. 후기 정지용 문학의 미적 성과는 “조선어로 근대문학을 한다”는 가능성을 토대로 문학에서의 조선적 전통을 자립적으로 재구성해냈다는 것이다. 본고는 기존 연구사에서 정지용의 기행 산문에서 ‘조선적인 것’에 대한 탐구를 재독한다. 정지용 문학에서 나타나는 朝鮮的인 音, 色, 喜, 愛樂의 특수성을 고찰하고, 정지용 문학 전반에 흐르고 있는 ‘조선 혼’의 개념을 고찰한다. 특히 그의 기행 산문에서는 다도해, 진주 기생, 평양 여인 등을 재구성해내면서 조선의 자연과 풍토를 단순한 장소가 아닌 역사적인 의의를 호출할 수 있는 ‘조선 심’의 공간으로 확장한다. 그뿐만 아니라 「鄕愁」, 「故鄕」 등의 시편에서 드러나는 ‘조선 심’은 기록 영화의 카메라 기법을 차용하는 듯한 인상을 보여준다. 이는 그간 정지용 시에서 주목되지 않았던 배치의 미학을 발견해볼 수 저점이다. 정지용이 ‘조선적인 것’에 대해 탐미했던 것은 후기 시의 특수성이라고만 볼 수 없다. ‘조선적인 것’의 탐미는 정지용 시 전반에서 고찰할 수 있는 토대 감각이다.

        A Case of Cushing Syndrome Diagnosed by Recurrent Pathologic Fractures in a Young Woman

        Cushing's syndrome is characterized by central obesity, fatigability, weakness, amenorrhea, hirsutism, edema,hypertension, impaired glucose tolerance, and osteoporosis due to excessive production of steroids. Cushing's syndrome is an important cause of secondary osteoporosis. Patients with Cushing's syndrome have a high incidence of osteoporotic fractures. At least, 30-50% of patients with Cushing's syndrome experience fractures, particularly in the vertebral body. And it is consistent with the 50% prevalence of osteoporosis in patients with Cushing's syndrome. However,reports of multiple pathological fractures in young patients with Cushing's syndrome are rare. Thus, we describe the case of a 26-year-old woman with Cushing's syndrome accompanied with recurrent multiple osteoporotic fractures and being treated by parathyroid hormone. Careful consideration for the possibility of Cushing's syndrome will be necessary in case of young patients with a spontaneous multiple compression fractures in spine.

        Serum Chemerin Levels Are Associated with Abdominal Visceral Fat in Type 2 Diabetes

        Chemerin is a recently identified adipokine suggested to play a role in obesity and its metabolic complications. The relationship between visceral obesity and serum chemerin levels in type 2 diabetes (T2DM) is unknown and may differ from that of subjects without diabetes. Therefore, we evaluated whether serum chemerin was associated with visceral abdominal obesity in patients with T2DM. A total of 218 Korean patients with T2DM were enrolled and metabolic parameters, abdominal visceral and subcutaneous fat areas, and serum chemerin levels were measured. Serum chemerin level showed positive correlation with fasting insulin, HOMA-IR, serum triglyceride, serum creatinine, urine albumin/ creatinine ratio, high-sensitivity C-reactive protein (hsCRP), fibrinogen, abdominal visceral fat area, visceral to subcutaneous fat area ratio, and negatively correlation with high density lipoprotein cholesterol and creatinine clearance (CCr) after adjusting for age, gender and body mass index. Multiple linear stepwise regression analysis showed that abdominal visceral fat area (β = 0.001, P < 0.001), serum triglyceride (β = 0.001, P < 0.001), CCr (β = -0.003, P = 0.001), hsCRP (β = 0.157, P = 0.001), fibrinogen (β = 0.001, P < 0.001), and BMI (β = 0.02, P = 0.008) independently affected log transformed serum chemerin levels. Higher serum chemerin level was associated with higher level of abdominal visceral fat area, serum triglyceride, hsCRP and fibrinogen and lower level of CCr in patients with T2DM. Serum chemerin may be used as a biomarker of visceral adiposity and chemerin may play a role in inflammation, decreased renal function, and increased cardiovascular risk in T2DM.

        천해환경에서 전구음원을 이용한 지음향인자의 역추정

        천해환경에서 저주파 광대역신호와 수직선배열을 이용하여, 퇴적층의 지음향인자(층두께, 종파속도, 종파감쇠계수, 밀도)를 역추정하였다. 역산방법은 모델 기반의 역산으로 유전알고리즘 (Genetic Algorithm)을 이용한 일관적 광대역 정합장처리(Coherent Broadband Matched Field Processing)기법을 사용하였다. 저주파 광대역음원으로 사용된 상업용 전구의 내폭신 호는 짧은 시간동안 많은 변화를 포함하는 천이신호이기 때문에, 분석시 시간과 주파수에 따른 창함수의 조절이 요구되는데, 주기신호분석에 주로 사용되는 퓨리에 기반의 분석방법은 이러한 점에서 많은 어려움이 있다. 본 논문에서는 해양도파관에서 근거리 음파전달 시 계측된 시계열신호로부터 다중경로성분을 구분하고 추출하기 위하여 시간-주파수영역에서 창함수의 크기조절이 가능한 웨이블릿 변환을 통한 신호 분석을 수행하였고, 분석된 실측음장과 계산된 복제음장의 연속웨이블릿 계수를 상호상관 시킴으로써 비용함수를 정의하였다. 비용함수의 전역최고점을 찾는 최적화 과정을 통하여 각 퇴적층의 지음향인자들을 역추정하였다. 특히 역산인자의 민감도에 따른 퇴적층별, 인자별, 분리연산을 수행함으로써 최적화과정에서 참값으로의 수렴효율을 높였다. 역산의 결과 실험해역 퇴적물 상층부에는 두께 44.43m, 음속 1549 m/s의 모래-실트-점토질(sand-silt-clay)층이 존재하고, 그 하부에는 12.28m 음속 1993 m/s의 거친모래질(Coarse sand)층의 존재를 추정해 내었다. 또한 역산 결과를 시추자료 및 탄성파 자료와 비교함으로써 본 논문에서 제안한 역산 방법의 유효성을 확인하였다. An inversion method is presented for the determination of the compressional wave speed, compressional wave attenuation, thickness of the sediment layer and density as a function of depth for a horizontally stratified ocean bottom. An experiment for estimating those properties was conducted in the shallow water of South Sea in Korea. In the experiment, a light bulb implosion and the propagating sound were measured using a VLA (vertical line array). As a method for estimating the geoacoustic properties, a coherent broadband matched field processing combined with Genetic Algorithm was employed. When a time-dependent signal is very short, the Fourier transform results are not accurate, since the frequency components are not locatable in time and the windowed Fourier transform is limited by the length of the window. However, it is possible to do this using the wavelet transform a transform that yields a time-frequency representation of a signal. In this study, this transform is used to identify and extract the acoustic components from multipath time series. The inversion is formulated as an optimization problem which maximizes the cost function defined as a normalized correlation between the measured and modeled signals in the wavelet transform coefficient vector. The experiments and procedures for deploying the light bulbs and the coherent broadband inversion method are described, and the estimated geoacoustic profile in the vicinity of the VLA site is presented.

        염산테트라싸이클린의 적용시간에 따른 특수 가공된 임프란트 표면 변화

        The present study was performed to evaluate the effect of tetracycline-HCl on the change of implant surface microstructure according to application time. Implant with pure titanium machined surface, HA-coated surface and dual acid etched surface were utilized. Implant surface was rubbed with $50mg/m{\ell}$ tetracycline-HCL solution for ${\frac{1}{2}}$min., 1min., $1{\frac{1}{2}}$min., 2min., and $2{\frac{1}{2}}$min. respectively in the test group. Then, specimens were processed for scanning electron microscopic observation. The results of this study were as follows. 1. Both test and control group showed a few shallow grooves and ridges in pure titanium machined surface implants. There were not significant differences between two groups. 2. In HA-coated surfaces, round particles were deposited irregularly. The roughness of surfaces conditioned with tetracycline-HCL was lessened and the cracks were increased relative to the application time. 3. The etched surfaces showed the honey comb structures. The surface conditioning with tetracycline-HCI didn't influence on its micro-morphology. In conclusion, the detoxification with $50mg/m{\ell}$ tetracycline-HCI must be applied respectively with different time according to various implant surfaces.
