        Association between Symptoms of Irritable Bowel Syndrome and Methane and Hydrogen on Lactulose Breath Test

        Whether hydrogen and methane gas produced during lactulose breath test (LBT) are associated with symptoms of irritable bowel syndrome (IBS) is not determined. We aimed to investigate whether hydrogen and methane on LBT are associated with IBS symptoms. Sixty-eight IBS patients meeting the Rome III criteria for IBS, and 55 healthy controls,underwent LBT. The IBS subjects recorded their customary gastrointestinal symptoms on a questionnaire using visual analogue scales. LBT positivity was defined to be above 20 ppm rise of hydrogen or 10 ppm rise of methane within 90 min. Gas amounts produced during LBT were determined by calculating area under the curve of hydrogen and methane excretion. Symptom severity scores were not different between the LBT (+) IBS and LBT (-)IBS subjects and also between methane producers and non-methane producers. Gas amounts produced during LBT were not associated with IBS symptoms, except a weak correlation between total gas amounts and a few IBS symptoms such as bloating (r = 0.324, P = 0.039), flatulence (r = 0.314, P = 0.046) and abdominal pain (r = 0.364,P = 0.018) only in LBT (+) IBS. In conclusion, hydrogen and methane gas on LBT are not useful for predicting the customary symptoms and subtypes of IBS.

        다양한 소화기계 감염 질환을 동반한 후천성면역결핍증 환자

        The number of AIDS patients is currently increasing in Korea, and multiple opportunistic infections, which are due to their immunocompromised state, have become a significant clinical problem. These opportunistic diseases may vary according to the severity of immunosuppression, the mode of transmission, the risk factors, the living environment and treatment; rectal syphilis, perianal condyloma lata and amoebic liver abscess are known to rarely develop in AIDS patients. We report here on a case of a homosexual man who simultaneously presented with an amebic liver abscess, esophageal candidiasis, chronic hepatitis B, rectal syphilis, perianal condyloma lata and asymptomatic neurosyphilis. He was treated with highly active antiretroviral therapy (HARRT) and the appropriate antiviral and antibiotic agents. (Korean J Gastrointest Endosc 2007;35:342-345) 현재 우리나라에서 후천성면역결핍증(AIDS) 환자는 증가 추세에 있기 때문에 면역결핍 상태에서 발생하는 다양한 기회감염이 임상에서 문제가 되고 있다. 면역결핍의 정도, 전파 경로, 위험 인자, 생활 환경, 치료 여부에 따라 다양하게 발생하는 기회질환 중 직장 매독과 항문주위 편평 콘딜로마 및 아메바성 간농양은 후천성면역결핍증 환자에서 비교적 드물게 발생하는 것으로 알려져 있다. 아메바성 간농양, 식도칸디다증, 만성 B형 간염, 직장매독, 항문주위 편평 콘딜로마, 무증후성 신경매독이 동시에 발생한 후천성면역결핍증 환자에서 고활성 항레트로바이러스 요법과 적절한 항바이러스제 및 항생제로 치료한 사례를 보고한다.

      • 産地에 의한 해송 子葉數의 差異

        우리나라 海岸線에 따라 分布하는 해송의 産地에 의한 稚苗 特性을 알고자 子葉數를 比較 檢討하였든바 그 結果를 要約하면 다음과 같다. 1. 産地別 해송 子葉數는 울진의 6.95로서 가장 높은 값을 보였으며 가장 적은 값은 남해의 6.60이었다. 2. 産地에 의한 子葉數를 分散分析한 結果 産地間은 高度의 有意性이 認定되고 反覆間에는 有意差가 없었다. 그리고 産地別 平均値 間의 有意差를 檢定하였든바 울진과 사천, 남해 間에서는 有意性이 있었고 다른 地域과는 없었다. 3. 産地에 따라 子葉數와 種子長, 種子幅 間에는 모두 높은 相關을 보였고 울주의 것은 낮은 相關을 보였으며 子葉數의 遺傳力은 比較的 낮게 보였다. 해송의 子葉數는 産地에 따라 有意差를 보이고 있으며 遺傳力이 낮은 結果는 環境에 影響되는 形質로 推定된다. The cotyledon numbers of black pine seedlings were examined in order to find out the provenances character along the southern sea coast of Korea. The results obtained are summarized as follows: (1) The number of cotyledons of black pine were showed to be the highest 6.95 in Wuljin, the lowest were 6.60 in Namhae. (2) Among the provenances, the results of analysis of variance showed to be high significant. There were insignificant among the replications. The difference of cotyledon numbers among the provenances showed to be high significant in Wuljin, Sachun and Namhae, while the other provenances did not showed. (3) The coefficient correlations between number of cotyledons and seed length and number of cotyledons and seed width showed to be high at every provenances, but low only at Wuljin. It was estimated that the cotyledon numbers of black pine among the provenances showed significant difference, and the result of low heritability was the environmental.

        From the study of the definition of and intoxication, detoxication poisonous drug, the symptom and treatment of intoxication, andantidotes with DONG YUI BO GAM etc, it can be concluded as follows. 1. In ancint times, the poisonous drug was a drug which could treat diseases but after that it has been a drug which can injure one's health because it has severe property, poison or side effects 2. Intoxication means that poison goes and works in the body. Detoxication means treatment which clears the symptom of intoxication and removes the toxin in the body or on the body 3. At the intoxication's symptom, vomiting, madness, diarrhea, abdomianl pain, consumption are frequent in order 4. At the intoxication's treatment, vomiting method is used when poison is in the stomach, purgative method is used when poison is the intestine. 5. Mostly Hei-dou is used in drugs and Gan-kao. Qing-yan are used in order. 6. Mostly drugs of reliving fever are used in drugs. 7. Mostly cold drugs are used in drugs and warm, even, hot, cool drugs are used in order and mostly mild drugs are used in drugs and bitter, hot, salty, sore drugs are used in order.

      • 素問ㆍ鍼解에 對한 硏究

        This part is to comment the part related to acupuncture among the contents of Ku Chim Sip Ⅰ Won(九鍼十二原) in Young Chu(靈樞) and Bo Myung Jeon Hyung Lon(寶命全形論) in So Moon(素問). This volume divide in three chapters. Chapter Ⅰ deals with rules of Bosa(補瀉) in acupuncture. Chapter Ⅱ deals with desirable attitude in acupuncture. Chapter Ⅲ deals with Ku Chim(九鍼) that correspond to the universe, four seasons and YinYang. So Chim Hea(小鍼解) in Young Chu(靈樞) also comments Ku Chim Sip Ⅰ Won(九鍼十二原) in the same way that this part does. And yet there are not a few differences from this part in noting. Thus we need the comparative study of between this part and So Chim Hea to understand Ku Chim Sip Ⅰ Won(九鍼十二原). Therefore I made a comparative study of views of many physicians to correctly understand.

        Prevention of NSAID-Associated Gastroduodenal Injury in Healthy Volunteers-A Randomized, Double-Blind, Multicenter Study Comparing DA-9601 with Misoprostol

        In addition to inhibiting cyclooxygenase and prostaglandin, nonsteroidal anti-inflammatory drugs (NSAIDs) may cause gastroduodenal injuries due to reactive oxygen species produced by recruited inflammatory cells. DA-9601 is a novel antioxidant with anti-inflammatory and cyto-protective effects. This study was conducted to compare the efficacy and safety of DA-9601 with misoprostol for preventing NSAID-associated gastroduodenal injury. In this randomized, double-blind, multicenter, noninferiority trial we compared the extents of protection of gastric and duodenal mucosae by endoscopy after 4 weeks of treatment with DA-9601 60 mg or misoprostol 200 μg three times daily, in subjects with normal baseline endoscopic findings who received an NSAID twice daily for 4 weeks. A total of 266 subjects were randomized to treatment. At week 4, the gastric protection rates with DA-9601 and misoprostol were 85.1% and 95.2%, respectively; the difference between the groups was -10.1% (var = 0.001), which was shown to indicate noninferiority of DA-9601 compared to misoprostol. Adverse events were lower in the DA-9601 group, 56.4%(95% CI, 48.0%-64.8%) than in the misoprostol group, 69.2% (95% CI, 61.3%-77.0%)(P = 0.031). DA-9601 is not inferior to misoprostol for preventing NSAID-associated gastroduodenal injury, and superior to it with respect to treatment-related side effects.

        현장 및 실내 측정 탄성파 속도에 근거한 암반평가 기준에 대한 고찰

        이 연구에서는 국내 토목현장에서 수행된 하향식 탄성파탐사 및 굴절법 탄성파탐사 자료(177개)와 시추조사 시료(1,035개)에 대해 연암과 경암(보통암 포함)으로 분류한 후, 건설표준품셈과 지반조사표준품셈의 탄성파속도에 의한 암반분류 기준을 비교하였다. 현장에서의 하향식 탄성파탐사 및 굴절법 탄성파탐사에 의한 탄성파속도는 연암의 경우 1,400∼2,900 m/s의 범위로 건설표준품셈 A그룹(1,200∼1,900 m/s)과 지반조사표준품셈(1,200∼2,500 m/s)의 기준보다 빠르게 나타났으며, 보통암과 경암의 경우 2,300∼3,800 m/s의 범위로 기준범위와 유사하게 나타나는 것으로 나타났다. 실내암석시험에서 구해진 연암과 보통암∼경암의 탄성파속도 또한 현장 탐사 결과와 유사한 경향을 보이는 것으로 나타났다. 암반 탄성파 속도와 품셈간의 상이점을 품셈이 절대적으로 옳다는 관점에서 본다면, 현장 탄성파 속도의 경우 하부지반의 영향을 받아 속도가 빨라지는 것과 실내암석시험의 경우에는 연암구간에서의 시료선별 시 무결암의 선별에 의한 것으로 여길 수 있다. 반대로 상이점의 원인을 품셈에 오류가 있는 것으로 본다면, 품셈상의 지층경계가 점이적이지 않은 뚜렷한 경계가 인위적으로 설정된 점, 지질 양상이 다른 외국의 기준을 그대로 차용하여 사용한다는 점, 품셈상 지층의 탄성파 속도에 대한 독립된 검증이 이루어지지 않은 점 등의 문제가 있음을 알 수 있다. 이 연구에서는 현장에서의 향후 이러한 검증 연구를 제안하며, 널리 쓰이는 품셈에 의한 지층분류에는 내포된 문제가 있음에 대한 인식이 중요하다. Seismic velocities measured from in-situ tests (n=177) and through rock core samples (n=1,035) are reviewed in light of construction standards, widely used standards as a first-hand approximation of rock classification solely based on seismic velocities. In-situ down hole tests and refraction survey for soft rocks showed seismic velocities of 1,400∼2,900 m/s which is faster than those specified in construction standards. For moderate∼ hard rocks, in-situ down hole tests and refraction survey showed 2,300∼3,800 m/s which roughly corresponds with the range specified in the construction standards. A similar trend is also observed for seismic velocities measured from rock core samples. The observed differences between construction standards and seismic velocities can be explained in two ways. If construction standards are correct the observed differences may be explained with seismic velocities affected by underlying fast velocities and also possibly with selection of intact cores for velocity measurement. Alternatively, construction standards may have intrinsic problems, namely artificial discrete boundaries between soft rocks and moderate rocks, application of foreign standards without consideration of geologic setting and lack of independent verification steps. Therefore, we suggest a carefully designed verification studies from a test site. We also suggest that care must be exercised when applying construction standards for the interpretation and accessment of rock mass properties.
