        조기 난소 부전의 임상 양상에 관한 연구

        연구목적: 조기 난소 부전은 40세 이전에 무월경, 고성선 자극 호르몬증이 동반되는 질환이다. 본 연구에서는 조기난소 부전의 임상적인 양상을 고찰하기 위하여 일차성 무월경과 이차성 무월경으로 나누어 임상 양상을 비교해 보고자하였다. 연구재료 및 방법: 8개 대학병원에서 시행한 후향성 다기관 연구로 262명의 조기 난소 부전 환자를 68명의 일차성 무월경과 194명의 이차성 무월경으로 나누어 내분비학적 소견과 혈청 검사 소견, 임신율과 배란 유도제에 따른 임신율, 내분비학적 소견에 따른 임신율, 골밀도를 비교하였다. 결과: Estradiol 2치는 일차성 무월경에서 유의하게 이차성 무월경보다 낮았으며 진단 전 임신율은 이차성 무월경에서 43.3%로 일차성 무월경의 0% 보다 의미 있게 높았다. 치료후의 임신율은 일차성 무월경에서 5.9%, 이차성 무월경에서 1.5%였다. 호르몬 치료후의 임신율은 일차성 무월경에서 8.3%, 이차성 무월경에서 2.8%였으며 임신이 성공한 9 케이스에서 7 케이스는 estrogen-progestin (EP), 1 케이스는 clomiphene citrate (CC), 1 케이스는 EP/human menopausal gonodotropin (EP/hMG)로 치료받았다. 또한 9 케이스의 임신 성공 중 6 케이스에서 난자 공여로 성공하였으며 다른 내분비학적 소견에 따라 임신율의 차이는 없었다. 골밀도는 일차성 무월경에서 현저히 낮게 나타났다. 결론: 조기 난소 부전은 젊은 여성의 신체적, 정신적으로 영향을 미치는 중요한 질환으로 효과적인 치료를 위하여 빠른 진단과 증상을 완화시키기 위한 호르몬 보충 요법과 골다공증과 같은 장기간 영향을 미치는 질환에 대비한 적절한 약물 요법이 필요하며 난자 공여와 같은 생식 보조술도 고려하여야 한다. Objectives: Premature ovarian failure (POF) is a syndrome defined as the cessation of ovarian function before the age of 40 years that is characterized by amenorrhoea associated with elevated gonadotropin levels. The aim of this study was to compare clinical manifestation of primary amenorrhea and secondary amenorrhea group. Methods: This study was designed as a retrospective multicenter study of 262 women with premature ovarian failure. Sixty eight women with primary amenorrhea and 194 women with secondary amenorrhea were evaluated and hormonal level, lipid profile, bone mineral density, and pregnancy rates were compared. Results: The estradiol level was markedly lower in primary amenorrhea than secondary amenorrhea. The pregnancy rate of 43.3% before the diagnosis in secondary amenorrhea was markedly higher than the rate of 0% in primary amenorrhea. The pregnancy rates after treatment was 5.9% in primary amenorrhea, but 1.0% after diagnosis and 2.8% after treatment in secondary amenorrhea. The pregnancy rate after hormonal treatment was 3.7% in total, 8.3% in primary amenorrhea, and 2.8% in secondary amenorrhea. In nine cases of pregnancy, seven cases were after estrogen-progestin (EP), one case was after clomiphene citrate and one case was after EP/human menopausal gonodotropin (hMG). And In nine cases of pregnancy, six cases resulted from oocyte donation. The prevalence of osteopenia/osteoporosis was markedly higher in primary amenorrhea than in secondary amenorrhea. Conclusion: Premature ovarian failure has negative influences on the physical and psychological health of young patients. Effective management should include earlier diagnosis and intensive medical intervention to relieve symptoms of estrogen deficiency and to treat long-term disease such as osteoporosis and in assisted pregnancy by oocyte donation.         Identification of molecular mechanism controlling P34 gene expression in soybean

        P34 has long been known as one of major allergenic proteins in soybean. Recently, two low-P34 soybean accessions, PI603570A and PI567476, were identified. In order to understand the molecular mechanism of a significant reduction of P34 protein level in PI567476, we performed comparative analysis of the P34 cDNAs and genomic sequences from low-P34 and normal soybean accessions. While no sequence variation was detected in P34 coding regions of the soybean accessions tested, the P34 gene from PI567476 contains 'ATGT' 4-bp insertion in front of the start codon. We also found the length polymorphism of 'TA' repeats in the P34 promoter regions among accessions. Northern and western blot analysis revealed that, even though mRNA levels of P34 are similar in low-P34 and normal soybean accessions, lower accumulation of P34 protein is only detected in the low-P34 accession, suggesting that the genetic polymorphisms in P34 promoters have a more significant effect on translation efficiency than transcription of the P34 gene. Transient expression analysis showed that the P34 promoter of PI567476 has significantly reduced activity compared to that of normal accession, cv. Clark. In addition, the chimeric promoter generated by introducing the 4-bp insertion in front of the start codon of the P34 gene from normal Clark accession showed markedly reduced activity. The results suggest that lower accumulation of P34 protein in low-P34 accession is largely due to the reduced translation efficiency caused by the 4-bp insertion in the P34 promoter, thus providing direct evidence for the molecular mechanism to explain the regulation of P34 gene expression in various soybean accessions.

        만성 이황화탄소중독의 신증상과 신조직검사 소견에 대한 연구

        Carbon disulfide(CS₂), a volatile gas used during the processing of viscose rayon synthesis, is known to be a neurotoxin and to cause atherogenesis in the vascular system. This atherogenic effect is thought to be responsible for the renal impairment caused by chronic CS₂ intoxication. The author hereby reports the renal symptoms and kidney biopsy findings of 21 cases of chronic CS₂ intoxication. 1) A total of 21 patients, 16 males and 5 females, with a history of working at a rayon factory were examined. The mean age was 44.4±5.2 years old, and the duration of exposure was 13.4±4.0 years. 2) Of the total 21 cases, four patients were hypertensive and another four had proteinuria. Two of those patients also had azotemia. 3) In kidney biopsy, light microscopy revealed diffuse or nodular glomerular sclerosis in eight cases. Immunoflurorescence microscopy revealed nonspecific focal deposits of C3 and IgM in four cases. Electron microscopy revealed that the basement membranes were significantly thicker with a mean of 656.9±244.7㎚ than the normal thickness(320±10㎚). 4) The above results suggest that kidney biopsy is helpful in the early diagnosis of chronic CS₂ intoxication and that the histologic findings are similar to those of diabetes mellitus.

        급성 심근경색증 환자에서 조기에 적절한 재관류 요법을 시행받지못한 요인

        N/A Objectives: Early application of reperfusion therapy such as thrombolytic agent or direct PTCA is utmost important to amximize the efficacy of reperfusion therapy in patients with aute myocardial infarction. However, early adequate reperfusion thearpy was given only in 15% to 36% pf patients with acute myocardial infarction and substantial patients were not benefited from reperfusion therapy, Therefore, we performed these study to evaluate the reasons for which patients with acute myocardial infarction cannot receive an adequate reperfusion therapy. Methods: We analyzed the initial electrocardiogram and various time delays from chest pain onset time, first hospital arrival time, transfer time, ER arrival time, and door to reperfusion time in 138 patients with acute myocardial infarction from Jan. 1991 to Oct. 1993 admitted to Wonju Christian Hospital. Patients were grouped as reperfusion group(n=55) who had reperfusion therapy such as thrombolytic trial or direct PTCA and conservative grou who had not received reperfusion therapy(n= 83). Results: 1) Eighty-three cases(60.1%) did not received an adequate reperfusion therapy. 2) Hospital arrival time of patients was 237±162min and 786±615min in reperfusion and conservative group(p<0.05) respectively. Patient time delay was 103±98min and 439±511min, first hospital time delay, 93±78min and 333±482min, and transfer time, 81±59min and 105±64min in reperfusion and conservative group respectively(p<0.05). Only 4.8% of patients was transferred from first hospital after reperfusion therapy. 3) Patient time delay was the most common reason of not receiving reperfusion therapy, and time delay and lack of adequate reperfusion therapy at the first hospital inspite of early arrival were the second place. Other reasons were contraindication to lytic therapy(4 cases), definite Q wave MI(11 cases), inconclusive EKG(4 cases), and delay in therapy(4 cases). Conclusion: The most common reason of not receiving reperfusion therapy was patient delay in seeking medical care(45.8%) and time delay at first hospital in 22 cases(26.5%). To maximize the effectiveness of reperfusion therapy, it is important to shorten hospital arrival time delay and widespread use of reperfusion therapy at first hospital is recommeded.

        Differentiation of intraductal papillary mucinous neoplasms from other pancreatic cystic masses: Comparison of multirow-detector CT and MR imaging using ROC analysis

        <B>Purpose</B><P>To compare the diagnostic performance of multirow-detector computed tomography (MDCT) and magnetic resonance imaging (MRI) in the differentiation of intraductal papillary mucinous neoplasms (IPMNs) from other pancreatic cystic masses.</P><B>Materials and Methods</B><P>A total of 53 patients with pathologically proven pancreatic cystic lesions who had undergone MDCT and MRI were included in this study. Two radiologists analyzed the morphologic features of the lesions and graded the lesion conspicuity on each examination. The readers assigned their confidence level regarding the differentiation of IPMN from other lesions and predicting ductal communication of the lesion. The radiologists' diagnostic confidence was compared using receiver operating characteristic (ROC) analysis.</P><B>Results</B><P>The Az values for each observer for predicting ductal communication of the lesion and differentiating IPMN from other lesions were as follows: For MRI they were respectively 0.949 and 0.995 for reader 1, and 0.916 and 0.932 for reader 2. For MDCT they were respectively 0.790 and 0.875 for reader 1, and 0.774 and 0.850 for reader 2. In addition, for differentiating IPMNs from other lesions, MRI was significantly more accurate than MDCT (P < 0.05) for one observer, but for the other observer there was no significant difference between the two examinations (P = 0.059). For predicting ductal communication of the cystic lesions for both observers, MRI was significantly more accurate than MDCT (P < 0.05). The weighted κ values indicate good agreement (κ = 0.61) between observers for MDCT, and excellent agreement (κ = 0.82) for MRI.</P><B>Conclusion</B><P>Pancreatic MRI shows better diagnostic performance than MDCT for differentiating IPMNs from other cystic lesions of the pancreas.         최근 많은 의미적 정보처리에 관한 연구들은 지식베이스를 기반으로 수행되고 있다. 대표적 지식베이스인 워드넷은 거의 모든 개념들에 대한 정의와 관계를 형성하고 있지만, 실제로 관용어처럼 사용되는 많은 명사구들에 대한 정의가 부족하다. 이러한 명사구들은 대부분 단의어로써 의미적 정보처리에 중요한 단서가 된다. 이에 본 연구실에서는 다양한 문서집합으로부터 관용어처럼 사용되는 명사구들을 추출하고 워드넷에 확장하는 방법을 연구하고 있다. 특히, 본 논문에서는 명사구들을 추출하고, 추출된 명사구들 중에서 중요한 것들만을 선정할 수 있는 방법을 다룬다.

        SVD 및 트리플릿 기반의 디지털 워터마킹 기법

        In this paper, we proposed a robust watermark scheme for image based on SVD(Singular Value Transform) and Triplet. First, the original image is decomposed by using 3-level DWT, and then used the singular values changed for embedding and extracting of the watermark sequence in LL3 band. Since the matrix of singular values is not easily altered with various signal processing noises, the embedded watermark sequence has the ability to withstand various signal processing noise attacks. Nevertheless, this method does not guarantee geometric transformation (such as rotation, cropping, etc.) because the geometric transformation changes the matrix size. In this case, the watermark sequence cannot be extracted. To compensate for the above weaknesses, a method which uses the triplet for embedding a barcode image watermark in the middle of frequency band is proposed. In order to generate the barcode image watermark, the pattern of the watermark sequence embedded in a LL3 band is used. According to this method, the watermark information can be extracted from attacked images.

      • Monoclonal Antibody CFC-6, which Binds to Helix II, Inhibits Erythropoietin-Induced Bioactivity

        It was discovered that monoclonal anti-erythropoietin (EPO) antibody CFC-6 can neutralize EPO-induced cell activation. To know the binding site of CFC-6, recombinant human erythropoietin (rhEPO) was digested with Glu-C, followed by a separation using high performance liquid chromato graphy (HPLC). Each HPLC fraction was blotted on the nitrocellulose membrane and the membrane was treated with anti-EPO antibody CFC-6 and anti-mouse antibody which is modified with peroxidase. Only one spot showed the color and the fraction was sequenced by Edman degradation. The results suggest that CFC-6 recognizes amino acid sequence V63-W-Q-G-L-A-L-L-S-E72 which is a part of helix II of the EPO molecule. Binding of CFC-6 to EPO may inhibit EPO binding to its receptor, which implies that the antibody binding site and the receptor binding site are close or overlapping.
