      • Do we have to test hemoglobin level routinely after total laparoscopic hysterectomy?

        목적: Routine checking of hemoglobin (Hb) level after total laparoscopic hysterectomy (TLH) has been questioned by researchers. Postoperative Hb might help to find out patients who need intervention to correct postoperative anemia. The aim of this study is to investigate perioperative factors affecting to postoperative intervention and cut-off values of the factors for predicting the patients' probability of receiving postoperative intervention. 방법: Medical records of patients who underwent elective TLH in our institution from January 2012 to December 2014 were reviewed. Patients who had intraoperative complications including vaginal laceration, bladder rupture or received transfusion during the operations were excluded. Women who received transfusion or took iron agent before the operations were also excluded. Patients with or without postoperative intervention were classified. Postoperative intervention included intravenous or oral iron agent, transfusion, abdomino-pelvic computer tomography scan, and reoperation. 결과: A total of 966 women underwent TLH during study period. 265 patients (27.4%) were received postoperative intervention. Patients with postoperative intervention had lower preoperative and postoperative Hb level, longer operation time, heavy uterine weight and blood loss during operation compared with patients without postoperative intervention. (p<0.001) Analysis by subgroup divided by preoperative Hb level revealed that lower preoperative Hb level was related with smaller decrease of Hb level after operation and greater opportunity to receive postoperative intervention. (p<0.001) Cutoff value of preoperative Hb level for predicting the patients' probability of receiving postoperative intervention was 10.1 g/dL with an area under curve (AUC) of 0.837. (sensitivity: 0.343, specificity: 0.967) 결론: Possibility of having postoperative intervention after TLH is affected by not decreasing amount of Hb levels but absolute value of Hb level. Patients, whose preoperative Hb level was below 10.1g/dL, had strong possibility to take postoperative intervention to correct postoperative anemia.

        타임으로부터의 플라보노이드 화합물의 추출 조건 최적화

        타임, 바질, 로즈마리 등 20종의 허브 식물에 대한 80% 에탄올 추출물의 항산화 활성을 분석하였다. 탐색 대상 허브 식물 중 타임추출물의 총 폴리페놀 함량은 6.90±0.02mg CAE/100 g로 높은 값을 보였으며, 총 플라보노이드 함량은 1.71±0.19mg NE/100 g으로가장 높았다. 또한 DPPH radical 소거능은 93.14±0.23%로 매우 높은 수준의 항산화 활성을 나타냈다. 타임으로부터 플라보 노이드를 추출하기 위하여 용매로서 물, 에탄올, 메탄올, 헥산 중 추출수율이 높은 헥산을 추출용매로 선정하였다. 반응표면분석법을 이용하여 타임 추출물의 최적화 추출조건으로 추출온도는 35.9 ℃, 농도는 1.63%, 시간은 192분이었다. 예측된 조건으로 제조한 타임 n-헥산 추출물의 총 플라보노이드 함량은 예측 값의 96.3%로 반응표면분석법에 의한 추출조건 최적화의 신뢰성이 검증되었다. 이상의 최적화된 추출 조건은 플라보노이드의 생산을 위한 타임의 용매추출 조건으로서 활용할 수 있을 것으로 기대된다. Total polyphenol contents, flavonoid contents, 1,1- diphenyl-2-picrylhydrazyl (DPPH) free radical scavenging activities of ethanol extracts of 20 herbal plants (sage, turmeric, mace, bayleaf, fenugreek, oregano, blackpepper, whitepepper, clove, marjoram, cinnamon, coriander, basil, dillseed, mustard, cadamon, thyme, celery, rosemary, cumin) were analyzed for the screening of high flavonoid-containing plant resource. Thyme extract, showing the highest flavonoid content and a high degree of antioxidant activity, was selected as a bioactive cosmetic material. The total polyphenols and flavonoids contents of thyme extracts were measured as 6.90 mg chlorogenic acid equivalent (CAE)/ 100 g and 1.71 mg naringin equivalent (NE)/100 g, respectively, and DPPH radical scavenging activities was 90%. Among the tested organic solvents, hexane gave the highest extraction yield. Thus hexane was selected as the most suitable solvent for the extraction of thyme. Response surface method was used to obtain optimized extraction conditions for thyme: reaction temperature of 35.9 ℃, raw material to hexane ratio of 1.63:25 (w/v), and reaction time of 192 min. These predicted extraction conditions was validated by a total flavonoid extraction experiment showing a value equivalent to 96.3% of the predicted total flavonoid content. It is expected that the optimized solvent extraction conditions could be used for the production of flavonoid using thyme.

        Fusion Length Requiring Spinopelvic Fixation in Lumbosacral Fusion with Anterior Column Support at L5–S1: Assessment of Fusion Status Using Computed Tomography

        Background: The lumbosacral (LS) junction has a higher nonunion rate than other lumbar segments, especially in long-level fusion. Nonunion at L5–S1 would result in low back pain, spinal imbalance, and poor surgical outcomes. Although anterior column support at L5–S1 has been recommended to prevent nonunion in long-level LS fusion, fusion length requiring additional spinopelvic fixation (SPF) in LS fusion with anterior column support at L5–S1 has not been evaluated thoroughly. This study aimed to determine the number of fused levels requiring SPF in LS fusion with anterior column support at L5–S1 by assessing the interbody fusion status using computed tomography (CT) depending on the fusion length. Methods: Patients who underwent instrumented LS fusion with L5–S1 interbody fusion without additional augmentation and CT > 1 year postoperatively were included. The fusion rates were assessed based on the number of fused segments. Patients were divided into two groups depending on the L5–S1 interbody fusion status: those with union vs. those with nonunion. Binary logistic regression analyses were performed to identify risk factors for LS junctional nonunion. Results: Fusion rates of L5–S1 interbody fusion were 94.9%, 90.3%, 80.0%, 50.0%, 52.6%, and 43.5% for fusion of 1, 2, 3, 4, 5, and ≥ 6 levels, respectively. The number of spinal levels fused ≥ 4 (p < 0.001), low preoperative bone mineral density (BMD; adjusted odds ratio [aOR], 0.667; p = 0.035), and postoperative pelvic incidence (PI) – lumbar lordosis (LL) mismatch (aOR, 1.034; p = 0.040) were identified as significant risk factors for nonunion of L5–S1 interbody fusion according to the multivariate logistic regression analysis. Conclusions: Exhibiting ≥ 4 fused spinal levels, low preoperative BMD, and large postoperative PI–LL mismatch were identified as independent risk factors for nonunion of anterior column support at L5–S1 in LS fusion without additional fixation. Therefore, SPF should be considered in LS fusion extending to or above L2 to prevent LS junctional nonunion.

        A study on spatial filtering of a speckle image reflected from a scattering medium in a coherent beam combining system

        We conducted a study to explore a real-time monitoring variable of a coherently combined beam returned from a scattering target. The effect of a spatial filtering on the speckle pattern generated from the interference of the scattered beam was examined in a coherent beam combining (CBC) system, including three-channel laser output, the scattering target, and receiver optics in free space. We were able to recognize the arrangement of the original beams from the speckle pattern by applying a proper spatial filter with a size of 2 mm. The power-in-the-bucket value in the monitoring system was also investigated as a function of the spatial filter size. Our research implies that a complex speckle pattern can be analyzed using spatial filtering to derive a single-output real-time variable in the CBC monitoring system.

      • Melatonin suppresses tumor angiogenesis by inhibiting HIF-1&agr; stabilization under hypoxia

        <P>Abstract: </P><P>Angiogenesis is an important mediator of tumor progression. As tumors expand, diffusion distances from the existing vascular supply increases, resulting in hypoxia in the cancer cells. Sustained expansion of a tumor mass requires new blood vessel formation to provide rapidly proliferating tumor cells with an adequate supply of oxygen and nutrients. The key regulator of hypoxia-induced angiogenesis is the transcription factor known as hypoxia-inducible factor (HIF)-1. HIF-1&agr; is stabilized by hypoxia-induced reactive oxygen species (ROS) and enhances the expression of several types of hypoxic genes, including that of the angiogenic activator known as vascular endothelial cell growth factor (VEGF). In this study, we found that melatonin, a small lipophilic molecule secreted primarily by the pineal gland, destabilizes hypoxia-induced HIF-1&agr; protein levels in the HCT116 human colon cancer cell line. This destabilization of HIF-1&agr; resulted from the antioxidant activity of melatonin against ROS induced by hypoxia. Moreover, under hypoxia, melatonin suppressed HIF-1 transcriptional activity, leading to a decrease in VEGF expression. Melatonin also blocked in vitro tube formation and invasion and migration of human umbilical vein endothelial cells induced by hypoxia-stimulated conditioned media of HCT116 cells. These findings suggest that melatonin could play a pivotal role in tumor suppression via inhibition of HIF-1-mediated angiogenesis.</P>

      • 6가 크롬 제거 특성을 갖는 양이온화 나노셀룰로오스 하이드로겔의 제조

        최근 오염된 수자원의 정화 및 개선을 위해 친환경 천연 고분자 기반의 고성능 수처리 소재에 관한 연구가 활발히 진행되고 있다. 그중에서도 셀룰로오스 나노섬유는 직경이 20-30 nm 이며 종횡비가 긴 섬유상 물질로 구성되어 있으며 기능기의 보유, 우수한 기계적 강도와 생분해성 등과 같은 셀룰로오스의 장점을 지님과 동시에 기존의 셀룰로오스 보다 넓은 표면적을 가지고 있어서 수처리 공정의 시간을 단축시킬 수 있다. 이처럼 나노셀룰로오스는 수처리 소재로서 유리한 특성을 가졌지만 다양한 형태로의 가공성이 떨어져 그 응용이 제한되어왔다. 따라서, 본 연구에서는 나노 몰폴로지를 가지는 재생 셀룰로오스 하이드로겔 (Regenerated cellulose hydrogel, (RC))을 제조하고, PEI 개질을 통해 흡착제 표면을 양이온화하여 중금속 흡착제로서의 가능성을 확인하였다(PEI/RC). FESEM을 통해 RC의 나노 섬유 및 기공 구조를 확인했고, FTIR XPS, Elemental analysis를 통해 PEI의 도입을 확인했다. 그 결과 PEI/RC는 578 mg/g의 우수한 6가 크롬 제거 효율을 가짐을 나타냈으며, 수차례 재사용한 후에도 초기 형태를 유지하였다.

      • Fabrication of a thin-layer solid optical tissue phantom by a spin-coating method: pilot study.

        <P>Solid optical tissue phantoms (OTPs) have been widely used for many purposes. This study introduces a spin-coating method (SCM) to fabricate a thin-layer solid OTP (TSOTP) with epidermal thickness. TSOTPs are fabricated by controlling the spin speed (250 to 2500 rpm), absorber concentration (0.2% to 1.0%), and the number of layers. The results show that the thicknesses of the TSOTPs are homogeneous in the region of interest. The one-layer TSOTP achieves maximum and minimum thicknesses of 650.28 μm (250 rpm) and 5.10.17 μm (2500 rpm), respectively, decreasing exponentially as a function of the spin speed. The thicknesses of the multilayer TSOTPs increases as a function of the number of layers and are correlated strongly with the spin speed (R20.95). The concentration of the OTP mixture does not directly affect the thickness of the TSOTP; however, the absorption coefficients exponentially increase as a function of absorber concentration (R20.98). These results suggest that the SCM can be used to fabricate homogeneous TSOTPs with various thicknesses by controlling the spin speed and number of layers. Finally, a double-layer OTP that combines epidermal TSOTP and dermal OTP is manufactured as a preliminary study to investigate the practical feasibility of TSOTPs.</P>
