        멀티캐스트 통신환경을 이용하는 많은 응용들의 다양한 요구들을 효율적으로 관리하기 위해 제안된 통합 관리모델인 IPME의 요소중 Agent 부분을 설계하고 구현한다. 제안된 IPME 구조를 기본으로, Manager-Agent-멀티캐스트 참여자의 구조에서 핵심적인 부분인 Agent 부분을 설계하고, 구현하여 멀티캐스트 환경에서의 여러 참여 송수신자들의 다양한 QoS요구들을 효율적으로 관리할 수 있게 한다. 마지막으로 구현된 Agent를 실제 멀티캐스트 환경을 이용하는 화상회의 시스템에 적용하여 그 기능과 역할에 대한 타당성을 검증해 본다.

      • 사회적 관계 형성을 지향하는 한국어 교육에 대한 고찰

        This paper aims to explore new direction of Korean languge education in Japan. Firstly, a project on foreign language education in Japan (Meyasu) is reviewed, which takes foreign language education as ‘pedagogy’, not as training. In the project [Meyasu], the goal of foreign language education is not only to acquire the skill of ‘how to communicate’, but also to make/find out ‘what to communicate’ , to build a relationship ‘with the person to communicate with’. As a methodology for this global society oriented Korean language education project, [Meyasu] introduces project based learning (PBL) and backward curriculum design(BCD). Secondly, a case of a PBL class which is designed for sophomore class of Korean languguage department of a university in Japan. There are 6 groups in the class, the goal of the project is making a short video clip which gives a brief instruction about Gaigosai, the camapus festival of the university. There are many positive comments from students. Every group of the project made a team, showed consideration. And the video clips are well finished and many students mentioned the improvement of pronunciation, vocabulary, and expression skills through the project period. There are some work to be done, the problem of how to reconcile the linguistic contents of traditional language learning with the new social networking approach in class. Points are as follows : 1) Listing up the linguistic items linked to the social networking approach in Korean language education. 2) Developing different meaningful and learner oriented activities.

      • 일본의 한국어 교육에 있어서 문화교육의 현황과 전망

        This paper aims to review the state of cultural education in Korean language education of Japan and to explore new direction of cultural education, especially at university level. Firstly, as the feature of cultural education, intercultural perspective, learner centerness and the balance between universality and uniqueness are suggested. Secondly, the state of foreign language education in the schools of Japan is reviewed. Although the policy on foreign language education is English centered and skill oriented, there also are some attempts to promote communicative competence of many other languages in Japan. Thirdly, the Korean culture items in the university textbooks are examined. In most cases, the Korean culture is presented as a subject of knowledge and a little critical perspective on the culture suggested. It is necessary for the teachers to take extra effort in order to get proper outcomes of the features of cultural education in their own classes. Finally, a case of project based learning (PBL) class which is designed for sophomore class of Korean language department of a university in Japan is brought out. There are some works to be done, the problem of how to introduce the attainments of Korean Japanese contrastive pragmatic studies to Korean language education.

      • Do we have to test hemoglobin level routinely after total laparoscopic hysterectomy?

        목적: Routine checking of hemoglobin (Hb) level after total laparoscopic hysterectomy (TLH) has been questioned by researchers. Postoperative Hb might help to find out patients who need intervention to correct postoperative anemia. The aim of this study is to investigate perioperative factors affecting to postoperative intervention and cut-off values of the factors for predicting the patients' probability of receiving postoperative intervention. 방법: Medical records of patients who underwent elective TLH in our institution from January 2012 to December 2014 were reviewed. Patients who had intraoperative complications including vaginal laceration, bladder rupture or received transfusion during the operations were excluded. Women who received transfusion or took iron agent before the operations were also excluded. Patients with or without postoperative intervention were classified. Postoperative intervention included intravenous or oral iron agent, transfusion, abdomino-pelvic computer tomography scan, and reoperation. 결과: A total of 966 women underwent TLH during study period. 265 patients (27.4%) were received postoperative intervention. Patients with postoperative intervention had lower preoperative and postoperative Hb level, longer operation time, heavy uterine weight and blood loss during operation compared with patients without postoperative intervention. (p<0.001) Analysis by subgroup divided by preoperative Hb level revealed that lower preoperative Hb level was related with smaller decrease of Hb level after operation and greater opportunity to receive postoperative intervention. (p<0.001) Cutoff value of preoperative Hb level for predicting the patients' probability of receiving postoperative intervention was 10.1 g/dL with an area under curve (AUC) of 0.837. (sensitivity: 0.343, specificity: 0.967) 결론: Possibility of having postoperative intervention after TLH is affected by not decreasing amount of Hb levels but absolute value of Hb level. Patients, whose preoperative Hb level was below 10.1g/dL, had strong possibility to take postoperative intervention to correct postoperative anemia.

      • Is it worth expending energy to convert biliverdin into bilirubin?

        <P><B>Abstract</B></P> <P>Bilirubin (BR) is generated by the reduction of biliverdin (BV), a metabolite that results from the catalytic degradation of heme by the isoforms of heme oxygenase (HO). BV is nontoxic and water-soluble but BR is potentially toxic and lipophilic. Therefore, a further metabolic step is required for BR before excretion is possible. The reductive conversion of BV to BR costs energy and is evolutionarily conserved in human physiology. There must be a compelling reason for this apparently nonsensical evolutionary conservation. In addition to the differences between BR and BV—such as water solubility, antioxidant activity, and participation as a receptor ligand—in the present study, we focused on the chemistry of the two metabolites with regard to an electrophilic functional group called a Michael reaction acceptor (MRA). Our data reveal that the BR reacts with thiol compounds forming adducts, whereas no reaction occurs with BV. Furthermore, the binding of biotin-tagged BR to Kelch-like ECH-associated protein 1 (KEAP1)—a biological electrophile sensor—was prevented by pretreatment with BR or a thiol compound, but was not by pretreatment with BV. In cells, BR could bind to KEAP1 to release and activate nuclear factor-erythroid 2 (NF-E2) p45-related factor 2, a cytoprotective transcription factor, leading to the induction of HO-1.         5-Aminosalicylic Acid Azo-Linked to Procainamide Acts as an Anticolitic Mutual Prodrug via Additive Inhibition of Nuclear Factor kappaB

        <P>To improve the anticolitic efficacy of 5-aminosalicylic acid (5-ASA), a colon-specific mutual prodrug of 5-ASA was designed. 5-ASA was coupled to procainamide (PA), a local anesthetic, via an azo bond to prepare 5-(4-{[2-(diethylamino)ethyl]carbamoyl}phenylazo)salicylic acid (5-ASA-azo-PA). 5-ASA-azo-PA was cleaved to 5-ASA and PA up to about 76% at 10 h in the cecal contents while remaining stable in the small intestinal contents. Oral gavage of 5-ASA-azo-PA and sulfasalazine, a colon-specific prodrug currently used in clinic, to rats showed similar efficiency in delivery of 5-ASA to the large intestine, and PA was not detectable in the blood after 5-ASA-azo-PA administration. Oral gavage of 5-ASA-azo-PA alleviated 2,4,6-trinitrobenzenesulfonic acid-induced rat colitis. Moreover, combined intracolonic treatment with 5-ASA and PA elicited an additive ameliorative effect. Furthermore, combined treatment with 5-ASA and PA additively inhibited nuclear factor-kappaB (NF kappa B) activity in human colon carcinoma cells and inflamed colonic tissues. Finally, 5-ASA-azo-PA administered orally was able to reduce inflammatory mediators, NF kappa B target gene products, in the inflamed colon. 5-ASA-azo-PA may be a colon-specific mutual prodrug acting against colitis, and the mutual anticolitic effects occurred at least partly through the cooperative inhibition of NF kappa B activity.</P>

      • Development and Validation of a Semi-Quantitative Food Frequency Questionnaire to Assess Diets of Korean Type 2 Diabetic Patients

        <P><B>Background</B></P><P>Our aim was to assess the validity of a semi-quantitative food frequency questionnaire (FFQ) by comparison with the 3-day diet record (DR) in patients with type 2 diabetes.</P><P><B>Methods</B></P><P>Eighty five type 2 diabetic patients (aged 33 to 70 years) from the Korean National Diabetes Program (KNDP) completed 3-day DR and FFQ. The FFQ was designed to reflect the eating pattern of Korean type 2 diabetic patients, and was based on the 2003 Korean National Health and Nutrition Examination Survey. The FFQ consists of 85 food items and 12 food groups. The validity of FFQ was assessed by comparison with the 3-day DR.</P><P><B>Results</B></P><P>The mean age was 49 ± 10 years. Clinical characteristic including body weight, diabetic duration, and HbA1c were not different from the total cohort subjects (<I>n</I> = 1,478). There were no significant differences in the mean intake of protein, fat and calcium estimated by the FFQ and the 3-day DR. Energy and carbohydrate estimated by the FFQ were higher than those estimated by the 3-day DR. The correlation coefficient was highest for energy (<I>r</I> = 0.740; <I>P</I> < 0.00) and lowest for iron (<I>r</I> = 0.269; <I>P</I> < 0.05). The Kappa values for energy, carbohydrate, protein, fat and calcium were 0.54, 0.37, 0.36, 0.46, and 0.19, respectively.</P><P><B>Conclusion</B></P><P>The FFQ is a reasonable instrument for assessing the intake of most macronutrients in Korean type 2 diabetes, although careful consideration is required for the food groups and nutrients for which the FFQ had low validity.</P>

        Delayed hemorrhage effect of local anesthesia with epinephrine in the loop electrosurgical excisional procedure

        Objective The aim of this study was to investigate factors preventing delayed hemorrhage after the loop electrosurgical excisional procedure (LEEP). Methods Medical records of patients who underwent LEEP at one university affiliated hospital from October 2013 to January 2015 were reviewed. Patients with or without delayed hemorrhage were classified. LEEP was performed either in an operating room under general anesthesia or in a procedure room with local anesthesia in the outpatient clinic. Delayed hemorrhage was defined as excisional site bleeding occurring between 1 and 30 days after the LEEP requiring intervention such as electro-cauterization, gauze packing, or application of another hemostatic agent. Results During the study period, 369 patients underwent LEEP. Twenty-three (6.2%) patients with delayed hemorrhage returned to our hospital either to the outpatient clinic or to the emergency unit. A third of the population (103, 27.9%) underwent LEEP in the operating room under general anesthesia without injection of local anesthesia. The remaining patients (266, 72.1%) underwent LEEP with local anesthesia (lidocaine HCl 2% with epinephrine 1:100,000) in the office procedure room. Patients given local anesthesia including epinephrine had significantly lower delayed hemorrhage compared to patients with general anesthesia without injection of local anesthesia (P=0.001). Hemostats, such as fibrin glue or patch, were used for the majority of patients (346, 93.8%) during the procedure. However, using hemostats was not statistically associated with delayed hemorrhage (P=0.163). Conclusion Local anesthesia with the powerful vasoconstrictor epinephrine is effective not only to control perioperative bleeding, but also to prevent delayed hemorrhage after LEEP.

      • Evaluation of glycine-bearing celecoxib derivatives as a colon-specific mutual prodrug acting on nuclear factor-κB, an anti-inflammatory target

        <P>In an inflammatory state where HOCl is generated, glycine readily reacts with HOCl to produce glycine chloramine, an anti-inflammatory oxidant. Colonic delivery of celecoxib elicits anticolitic effects in a trinitrobenzene sulfonic acid-induced rat colitis model. Glycine-bearing celecoxib derivatives were prepared and evaluated as a colon-specific mutual prodrug acting on nuclear factor-κB (NFκB), an anticolitic target. Glycylcelecoxib (GC), <I>N</I>-glycylaspart-1-ylcelecoxib (N-GA1C), and <I>C</I>-glycylaspart-1-ylcelecoxib (C-GA1C) were synthesized and their structures identified using infrared and proton nuclear magnetic resonance spectrometer. The celecoxib derivatives were chemically stable in pH 6.8 and 1.2 buffers. GC and C-GA1C were resistant to degradation in the small intestinal contents, while N-GA1C was substantially cleaved to release celecoxib. In contrast, all the celecoxib derivatives were degraded to liberate celecoxib in the cecal content. These results suggest that GC and C-GA1C could be delivered to and liberate celecoxib and glycine in the large intestine. In human colon carcinoma HCT116 and murine macrophage RAW264.7 cells, combined celecoxib–glycine chloramine treatment additively suppressed the production of proinflammatory NFκB target gene products. Collectively, our data suggest that C-GA1C is a potential colon-specific mutual prodrug acting against NFκB.</P>
