        Sinonasal Undifferentiated Carcinoma (SNUC) is a very rare, highly aggressive malignant tumor of the nasal cavity and paranasal sinuses. SNUC tends to present with advanced-stage disease, often with intracranial invasion. It requires an aggressive multimodality therapy that includes surgical resection. A cure rate of less than 20% is generally reported in the literature, with most patients dying within 1 year of onset of the disease. Three patients diagnosed as SNUC were treated at the Yeungnam University Medical Center between the years 2000 and 2003 were analyzed retrospectively. All patients presented with the disease very advanced. The three cases were given chemotherapy or chemotherapy with radiotherapy. Two patients died of the disease, surviving only 6 and 11 months following treatment, respectively. We did a follow-up on just the one remaining case with incomplete controlled disease for 27 months. The overall prognosis of SNUC is very poor. We consider that more intensive multimodality therapies are recommended for all patients with SNUC.

      • Phenyl ester계열 강유전성 액정혼합물의 전기광학과 유전적 성질에 관한 연구

        Phenyl ester계열 강유전성 액정혼합물의 카이랄 스멕틱 C상(SmC* phase), 카이랄 스멕틱 A상(SmA* phase) 그리고 카이랄 네마틱상(N* phase)들에서의 전기광학과 유전적 성질을 연구하였다. 강유전성 액정혼합물(KU-1001)의 SmA* 상 범위에서 장 유도효과에 의한 경전효과 때문에 작은 경사각이 관찰되었다. KU-1001 혼합물의 전기광학 응답시간은 상온에서 약 50㎲이고, SmC*-SmA* 상전이 근처에서는 높은 전장에서 2㎲보다 빠르게 나타내었다. 또한 N* 상에서는 인가 전기장에 무관하며 10㎲의 응답시간을 나타내었다. 유전율 이방성은 주파수와 온도에 크게 의존하며, SmC* 상과 SmA* 상에서는 음의 값을 나타내었고, N* 상에서는 0에 접근하였다. 이 현상은 N* 상에서의 변전효과 때문에 일어난다. KU-1002 혼합물은 유전 이방성의 값이 음인 강유전성 액정물질이 높은 인가 전장영역에서 관찰되는 변칙적인 스위칭 거동을 하였다. A phenyl ester type of ferroelectric liquid crystal mixtures have discussed dielectric and electro-optic properties for chiral smectic C(SmC*), chiral smectic A(SmA*) and chiral nematic (N*) phases. The tilt angle exhibits a small tail in the SmA* phase due to field induced effect, which is an electroclinic effect. KU-1001 mixture shows a optical response time of about 50㎲ at room temperature and low electric field down to 2㎲ for high electric field close to the SmC*_SmA* phase transition. In the N* phase, the value of the optical response time is order of 10㎲ and almost independent of the applied field. The dielectric anisotropy Δε has been negative value in the SmC* and SmA* phases which strongly depends on frequency and temperature. It approaches zero in the N* phase. This is the favorable condition sought in order to exploit flexoelectric effect in the N* phase. Anomalous switching behaviour of FLC materials with negative dielectric anisotropy have been reported. KU-1002 mixture shows a minimum in response time at a sufficiently high field.

        비부비동 반정성 유두종의 전산화 단층촬영상과 자기공명영상의 분석

        Background: Computed tomography (CT) is commonly used to evaluate the degree of sinus involvement in case of inverted papilloma (IP). However, CT cannot differentiate tumor from adjacent inflammatory mucosa or retained secretions. By contrast, magnetic resonance imaging (MRI) has been reported to be useful in distinguishing IP from paranasal sinusitis. This study investigated whether preoperative assessment with MRI and CT accurately predict the extent of IP. Materials and methods: CT and MRI were retrospectively reviewed in 9 case of IP. Patients were categorized into staged based on CT and MRI findings according to the staging system proposed by Krouse. The involvement of IP in each sinus was also assessed. Results: Differentiation of IP from inflammatory disease may be more successful in routine case where the inflammatory mucosa has low signal intensity on T1-weighted images and very high signal intensity on T2-weighted images. CT imaging could not differentiate tumor from adjacent inflammatory mucosa or retained secretions. Conclusion: Preoperative MRI of IP can predict the location and extent of the tumor involvement in the paranasal sinuses and sometimes predicts malignant changes.

      • 전자충돌이 Mo/Pd(100)계의 저에너지 전자회절 pattern에 미치는 영향

        Mo 재질의 평판 위에 Pd(100) 결정시료를 놓고 3 keV의 Ar 이온선으로 동시에 bombardment 시킬때, Mo 원자들이 Pd(100)상에 증착 되는 현상을 발견하였다. 이렇게 증착된 Mo 원자들의 덮임율과 흡착구조, 그리고 이들 흡착구조들이 전자선 충격에 의해 받는 영향을 LEED와 AES를 이용하여 조사하였다. 흡착된 Mo 원자의 덮임율이 0.1일 때, Mo/Pd(100)로부터 어닐링(annealing) 온도 300℃와 500℃이상에서 각각 c(4×2)와 c(4×2)3+(√5×√5)R arctan 1/2 pattern이 관측되었다. 이들 pattern중 안정된 흡착구조인 (√5×√5)R arctan 1/2 는 3 keV의 전자선 충격에 의한 영향이 매우 적으나 불안정한 흡착구조인 c(4×2)는 민감하게 영향을 받고 있음을 알 수 있었다. Pd(100) sample was mounted on Mo plate in ultra high vacuum chamber, and both of Pd(100) sample and Mo plate were bombarded by 3 keV Ar ion beam. It was found that this bombardment gave rise to deposition of Mo atoms on Pd(100). The coverage and overlayer structure of Mo adatom, and an effect of the electron impact on this Mo/Pd(100) system were investigated by LEED and AES. When the coverage of Mo was 0.1, c(4×2) and c(4×2)+(√5×√5)R arctan 1/2 pattern were observed from this system at annealing temperature of 300℃ and above 500℃, respectively. When irradiating 3 keV electron beam on these surface structures, the c(4×2) corresponding to unstable overlayer structure from both LEED patterns was sensitively influenced but the (√5×√5)R arctan 1/2 not sensitively by 3 keV electron impact.

        의료보험 구강요양급여비에 관한 조사연구

        The government of our country has started Medical insurance policy, one of the social security systems. However, statistical date of medical insurance benefits is not much. Especially dental data is insufficient and irrational. In order to get reliable data which offer a valuable suggestion to direction of health care insurance system, the author studied the amount of total care benefits at 1998 which revealed by the survey and analysis of medical statistic index. And it divided into medical insurance, dental insurance, korean-medicine insurance benefits. The dental insurance benefits divided into dental survey and dental disease care benefits again. The dental disease care expenses is composed of dental caries and its advanced disease care expenses, periodontal disease care expenses, other dental disease care expenses. Its date is revealed by the survey and investigation of the demands of 12 dental clinic in 1998. The major findings of the analysis are summarized as follows; 1. In Medical insurance system, the total insurance benefits(include pharmaceutical expenses) was ₩10,115,324,330,000 and the total medical insurance benefits was ₩9,730,793,383,000. The Total dental insurance benefits was ₩647,033,656,600. The ratio of total medical insurance benefits to the total insurance benefit was the 96.20%, and the ratio of dental insurance benefits to the total medical insurance benefits was the 6.65% 2. The individual dental insurance benefits was ₩13,935.7. Among the individual dental insurance benefits, the proportion of the individual dental surveying expenses was 34.62% and individual dental disease care expenses was 46.04% 3. The most part of the dental insurance benefits was dental caries and its advanced disease care expenses and periodontal disease care expenses. 4. Among the dental caries its advanced disease care benefits, the percentage of tertiary preventive expenses was 40.52%. 5. To reduce the expenditure of finance of the health care insurance system and to poromote national oral health, it is necessary to increase the primary and the secondary preventive expenses. 6. Oral diagnostic expenses, oral health care planning expenses and preventive expenses for oral disease, prosthetic expenses in medical insurance benefits didn's supplied. 7. It should be make the legal basis of supply for oral diagnostic expenses and oral health care expenses. 8. Statistic data of the dental insurance should be more rationalize.

        두경부 편팡상피세포암 세포주에서 세포주기 조절인자의 활성 및 이상:후두편평상피세포암에서 종양억제 유전자 CDKN2 유전자 발현이상

        Background: Cyclin-dependent kinase (CDK) inhibitors are family of molecules that regulate the cell cycle. The CDKN2, a CDK4 inhibitor, also called p16, has been implicated in human tumorigenesis. The CDKN2, inhibits the cyclin/CDK complexes which regulate the transition from G1 to S phase of cell cycle. There is a previous repot that homozygous deletion of CDKN2 region on chromosome 9p21 was detected frequently in astrocytoma, glioma and osteosarcoma, less frequently in lung cancer, leukemia and ovarian cancer, but not detected in colon cancer and neuroblastoma. However, little is known about the relationship between CDKN2 and laryngeal cancer. Therefore this study was initiated to investigate the role of CDKN2 in human laryngeal squamous cell carcinoma development. Materials and methods: We used 5 human laryngeal carcinoma cell lines whether they have deletion or losses of CDKN2 gene expression by DNA-PCR or RT-PCR, respectively. We examined 8 fresh frozen human laryngeal cancer tissues to detect the loss of heterozygosity(LOH) of CDKN2 PCR was performed by using micosatellite markers of short arm of human chromosome 9 (D9S126, D9S144, D9S156, D9S161, D9S162,D9S166, D9S166, D9S171,D9S200 and D9SIFNA).For informative cases, allelic loss was scored of the signal of one allele was significantly decreased in tumor DNA when compared to eh same allele in normal DNA. Results: the CDKN2 DNA deletion was observed in 3 cell lines. The CDKN2 mRNA expression was observed in only one cell line, which was very weak. LOH was detected in 7 cases(87.5%) Conclusion: These results suggest that CDKN2 plays a role in the carcinogenesis of human laryngeal squamous cell carcinoma

        저비중 척추마취시 국소마취제의 용적이 마취 높이에 미치는 영향

        The Effect of the Injected Volume on the Spread of Spinal Anesthesia with Hypobaric Tetracaine Dong Woo Han, M.D., Seung Jun Kim, M.D., Sun Joon Bai, M.D. Kyeong Mee Oh, M.D., and Yong Taek Nam, M.D. Department of Anesthesiology, Yonsei University College of Medicine, Seoul, Korea Background: The properties of hypobaric spinal anesthesia are excellent in sensory and motor block with low concentration solutions, increased venous return, and hemodynamic stability. Much volume is needed with hypobaric spinal anesthesia because low concentration solutions are used. The purpose of this study was to examine the effects of the injected volume on the level of anesthesia in hypobaric spinal anesthesia during a total hip replacement. Methods: Twenty-seven adult patients scheduled for a total hip replacement were assigned randomly to two groups, one with 0.1% (n = 14), and the other with 0.2% (n = 13) hypobaric tetracaine spinal anesthesia. 8 mg of tetracaine was used for male patients and 7 mg for female patients. Hence the injected volumes were 8 ml for male and 7 ml for female patients in 0.1% solution and 4 ml for male and 3.5 ml for female patients in 0.2% solution, respectively. Epinephrine was mixed to either solution in concentrations of 1 : 200,000. The speed of injection was fixed at the rate of 0.2 ml/sec. Results: The height of sensory block with the 0.1% was one dermatome higher (T5.7) than with the 0.2% tetracaine solution (T6.7). The time for sensory block (11.4 min vs 12.3 min) and the time for motor block (10.0 min vs 15.0 min) with 0.1% was shorter than that with the 0.2% tetracaine solution. The duration of sensory block and the duration of motor bllock were not significantly different in either groups. Complete motor block was achieved in all patients. Conclusions: Both 0.1% and 0.2% hypobaric tetracaine spinal anesthesia are suitable for a total hip replacement. However we got better results with 0.1% than with 0.2% retracaine such as rapid and high sensory block with rapid motor block. (Korean J Anesthesiol 2001; 41: 159~164)

      • Genomic Heterogeneity in Clinical Strains of Mycobacterium tuberculosis, M. terrae Complex, M. gordonae, M. avium-intracellulae Complex and M. fortuitum by Pulsed-Field Gel Electrophoresis

        Clinical strains of Mycobacterium tuberculosis, M. terrae complex, M. gordonae, M. avium-intracellulae complex, and M. fortuitum from Korean patients were isolated and analyzed by comparing large restriction fragment (LRF) patterns produced by digestion of genomic DNA with infrequent-cutting endonucleases like AsnI and XbaI. and pulsed-field gel electrophoresis (PFGE). Three M. tuberculosis, two M. terrae complex, two M. gordonae, two M. avium-intracellulae complex, and two M. fortuitum strains were compared by using AsnI and XbaI. and this allowed easy visual separation of all epidemiologically unrelated strains. PFGE exhibits different DNA restriction patterns which are easy to compare. Genome size of the strains roughly ranged from 3020 to 3335 kb. The LRF patterns are useful for epidemiologic studies of tuberculosis with regard to drug resistance.

        전격성 간부전증을 동반한 조직구성 세포탐식성 지방충염 1 예

        Histiocytic cytophagic panniculitis was first described in 1980 by Winkelmann, et al. It is characterized by the presence of fever, serositis, inflammatory panniculitis, pancytopenia, coagulation defects, hepatosplenomegaly, and subcutaneous nodules resulting from the infiltration of lympho-histiocytes in the dermis and subcutaneous adipose tissue. A characteristic finding is the presence of bean-bag histiocytes containing phagocytized red blood cells, lymphocytes, and platelets. We experienced a case of histiocytic cytophagic panniculitis in a 30-year-old woman with a clinical picture of jaundice, recurrent chronic panniculitis, pancytopenia, hepatosplenomegaly, and a terminal, febrile illness with bleeding diathesis. In the skin biopsy and bone marrow study, characteristic histiocytes with prominent phagocytic activity and so-called bean-bag appearance were identified in subcutaneous fat as well as in the bone marrow. The patient died with DIC due to fulminant hepatic failure.

        Nucleoside Diphosphate Kinase and Flagellin from <i>Pseudomonas aeruginosa</i> Induce Interleukin 1 Expression via the Akt/NF-κB Signaling Pathways

        <P>Inflammatory responses are a first line of host defense against a range of invading pathogens, consisting of the release of proinflammatory cytokines followed by attraction of polymorphonuclear neutrophils (PMNs) to the site of inflammation. Among the many virulence factors that contribute to the pathogenesis of infections, nucleoside diphosphate kinase (Ndk) mediates bacterially induced toxicity against eukaryotic cells. However, no study has examined how Ndk affects inflammatory responses. The present study examined the mechanisms by which <I>Pseudomonas aeruginosa</I> activates inflammatory responses upon infection of cells. The results showed that bacterial Ndk, with the aid of an additional bacterial factor, flagellin, induced expression of the proinflammatory cytokines interleukin-1α (IL-1α) and IL-1β. Cytokine induction appeared to be dependent on the kinase activity of Ndk and was mediated via the NF-κB signaling pathway. Notably, Ndk activated the Akt signaling pathway, which acts upstream of NF-κB, as well as caspase-1, which is a key component of inflammasome. Thus, this study demonstrated that <I>P. aeruginosa</I>, through the combined effects of Ndk and flagellin, upregulates the expression of proinflammatory cytokines via the Akt/NF-κB signaling pathways.</P>
