      • 재발 또는 불응성 비호즈킨 림프종 환자에서 CDME 구제항암화학요법 후 고용량 항암화학요법 및 자가말초혈액 조혈모세포이식의 효과

        Background and objectives : The long-term survival in patients with non-Hodgkin's lymphoma (NHL) after conventional dose chemotherapy is about 35% and the rest of the patients tend to have relapse. So, in relapsed or refractory NHL, we compared the outcome of patients undergoing high-dose chemotherapy with autologous peripheral blood stem cell transplantation(APBSCT) with only salvage chemotherapy of cisplatin, dexamethasone, mitoxantrone, and etoposide(CDME). Materials and methods : From June 1993 to December 1999, 25 patients with relapsed or resistant NHL were treated with CDME regimen as salvage chemotherapy. Twelve patients were received four cycles of CDME chemotherapy, and 13 patients were received high-dose chemotherapy with APBSCT following two cycles of CDME chemotherapy. Results : The median follow-up duration was 12.8 months(range:4-68). The overall response rate was 41.7% (complete response rate 25%, partial response rate 16.7%) in 12 patients with CDME only. Thirteen patients who were treated with high-dose chemotherapy with APBSCT achieved 61.5% complete response rate and 15.4% partial response rate, with an overall response rate of 76.9%. The estimated 3-year progression-free survival rate was significantly higher among patients who received high-dose therapy than patients who received CDME only(41.5% vs 20.0%, p<0.05). And, 3-year overall survival rate was significantly higher among patients who received high-dose therapy(51.3% vs 25.0%, p <0.05). Conclusions : In relapsed or refractory NHL, CDME chemotherapy is an effective salvage chemotherapy and allow peripheral blood stem cell collection. Also, high-dose chemotherapy with APBSCT following CDME is superior to CDME salvage chemotherapy only.

        Thymoma is associated with myasthenia gravis, Pure red cell aplasia, and autoimmune diseases such as autoimmune hemolytic anemia, aplastic anemia, and hypogammaglobulinemia. It's association with aplastic anemia is rare and aplastic anemia appearing after surgical removal of thymoma is especially rare. The authors hereby report a case of aplastic anemia occuring in a patient who was diagnosed with thymoma and myasthenia gravis and had the tumor surgically removed. The patient was treated with anti-thymocyte globulin, cyclosporin, and prednisolone, and showed partial remission with hematologic improvements after 12 months.

      • Arylnitrile imine의 1,3-Dipolar Cycloaddition 反應에 關한 硏究(Ⅰ)

        These studies aim at expanding the cycloaddition of nitrile imines among the 1,3-dipolar cycloaddition, and particularly at examining the 1,3-dipo1ar cycloaddition of arylnitrile imines. 3-trimethylsilylated 5-alkyl(or aryl) tetrazoles were obtained by reacting hexame-thyldisilazane with various 5-aryltetrazole. It was then thermolyzed to form C-alkyl(or aryl)-N-trimethylsilyl nitrile imines, in which the N-H proton of arylnitrne imines had been blocked by trimethylsilyl group, and was reacted with various amyl nitrites containing C-N group. As a result, the 1,3-dipolar cycloddition products, in which the N-position had been rimethyl silylated, were obtained. Various 3,6-diaryl-4H-1,2,4-triazole derivatives were synthesized by hydrolyzing the above products which accompanied the removal of trimethylsilyl group. The 4H-1,2,4-triazole derivatives thus prepared were identified with those made by the usual method by comparing the physical properties and I. R. spectra

      • 혈당측정기 GlucoDr™ System의 평가

        연구배경: 당뇨병 환자에서 엄격한 혈당조절은 환자의 예후와 밀접한 관련이 있다. 혈당조절을 철저히 하기 위해서는 자가 혈당 측정기의 사용이 필수적이라 할 수 있다. 또한 저혈당이 빈번하게 발생하거나, 위험요소가 있는 환자들에게도 유용하게 사용될 수 있다. 이에 저자들은 전기화학감지법의 원리를 이용하여 국내에서 처음 생산된 자가혈당측정기인 GlucoDr™ blood Glucose Testing System(ALL Medicus, Co, Korea)를 현재 국내에서 시판되고 있는 다른 제품과 비교, 평가하여 임상적 유용성을 살펴보고자 하였다 방법: 혈당측정기준장비는 YSI 2300 STAT Plus(YSI Incorporated, USA)를 사용하였으며 GlucoDr™외에 Glucocard(KDK, Japan), Precision QID(Abbott Laboratories, Co, USA), Glucotrend(Roche, USA), Surestep(Lifescan, USA)의 다른 혈당측정기를 같이 평가하였다. 정밀도, 직선성, 비교방법과의 상관관계 평가, 검체량에 따른 영향, 헤마토크리트에 따른 영향, 검사자에 따른 영향, 항응고제에 따른 영향들을 평가하였다. 결과: 본 연구에서의 주된 평가대상인 GlucoDr™의 평가결과, 정밀도를 나타내는 검사 내, 검사 간 변이계수는 8%이내였으며, 45~500mg/dL 범위에서 R²=0.9906, 0.9984 정도의 직선성을 나타내었다. 본 연구에 있어 비교방법으로 이용한 YSI 2300 STAT Plus와의 상관관계 및 정확도는 y = 1.0058x + 0.453, R = 0.9853인 것으로 나타났다. 검체량이나 검사자 및 항응고제에 따른 측정값에는 유의할만한 차이가 없는 것으로 나타났으나, 헤마토크리트가 증가 또는 감소함에 따라 최소 10%에서 최대 26% 가량의 영향을 받는 것으로 관찰되었다. 결론: GlucoDrTM는 국내 사용되는 다른 혈당측정기와 비교하여 유용할 것으로 생각된다. Background: Self-monitoing blood glucose devices are sidely used in monitoring and point-of-care testing for the management of diabetic patients. We performed the present study to evaluate the performance of the GlucoDr™ blood glucose testing system using an electrochemical technique. Method: The GlucoDr™ was evaluated for linearity, precision, comparison of method, the effect of sample volume, hematocrit concentration, reapplication, operator and application methods. Results: The GlucoDr™ showed good linearity for glucose concentrations ranging from 52mg/dl to 475mg/dl(r²=0.971). The single day, and day-to-day, CV were within 8%. Excellent correlation was found between the GlucoDr™ and YSI 2300 STAT Plus(y=1.0058x + 0.453, r²=0.9710). The sample volume, reapplication, operator and application method produced no significant effect on the test result. An overestimation in the glucose values was found with low hematocrit concentrations. There was no significant effect by the anticoagulants, with the exception of citrate. Conclusion: The GlucoDr™ showed good linearity, precision and correlation with the reference method and provided rapid and reliable result for blood glucose levels. Therefore, the GlucoDr™ seems appropriate for clinical use in the management of diabetic patients.

      • 중합효소연쇄반응을 이용한 HLA-C 유전자의 DNA Typing에 관한 연구

        HLA-class I 유전자군에서 HLA-C 유전자는 세포표면의 발현율이 다른 class I 유전자에 비하여 매우 낮아, 혈청학적 방법으로 측정할 경우 20∼50%에서 blank로 측정되어 정착한 형별의 구분이 어려웠다. 이러한 점을 개선하기 위하여 본 연구에서는 중합효소연쇄반응(PCR)을 이용하여 HLA-C유전자의 DNA typing방법을 개발하였으며, 한국인에서 HLA-C 대립 유전자의 분포를 구하고자 하였다. 17종의 B-림프구 표준세포주의 DNA typing 결과는 과거에 발표된 것과 일치하였다. 50명에서 혈청학적 방법과 DNA typing을 동시에 실시하여 두 방법을 비교하였을 때, 두 검체에서 차이가 있었으나 split 항원임이 밝혀졌으며, 혈청학적 방법으로 검출되지 않은 24 blank 항원에서 유전자형으로는 모두 검출이 가능하였다. 한편 한국인에서 HLA-C대립 유전자 빈도를 보면 Cw*0101이 24.2%로 가장 높게 나타났으며, Cw*1401(16.3%)과 Cw0701* (11.7%) 대립 유전자도 17% 이상의 빈도를 나타내었다. 본 연구에서 개발한 중합효소연쇄반응을 이용한 HLA-C DNA typing방법은 시간이 짧고 방법이 간단하며, 특히 blank 항원을 완전히 규명할 수 있으므로 매우 효과적인 유전자 형별구분 방법으로 임상실험에서 장기기식이나 질환관련성 연구에 활용할 수 있을 것으로 기대된다. The molecular analysis and characterization of HLA-C allelic polymorphism has been impeded by lack of serological reagents which identify individual HLA-C antigens. To provide a better definition of the HLA-C alleles, we have developed DNA typing method using polymerase chain reaction-sequence specific primer (PCR-SSP) technique Instead of serological typing. DNA samples from 17 B-lymphoblstoid cell lines and 120 Korean individuals have been typed by DNA typing. The PCR-SSP results of control cell lines were correlated well with serology. Fifty Korean Individual samples determined by serological typing were compared with HLA-C DNA typing. Twenty four blank antigens were completely determined by DNA typing, In the population genetic study, Cw*0101 (24.2%) allele was the most frequent allele in Korean. Cw*1401 (16.3%) and Cw*0701 (11.7%) alleles were also frequently found. In conclusion, the high level of discrimination for HLA-C alleles may prove useful and informative in the study of transplant survival and identify the importance of allelic differences, not readily detectable by serology, on host and donor compatibility.       • 쿨롱 장벽보다 낮은 에너지에서의 중이온 핵융합 반응

        The fusion cross sections induced by heavy-ions have been studied in the basis of the customary barrier penetration model (BPM) with the incoming wave boundary condi-tion(1WBC) and also by the direct reaction approach. The latter explained well the fusion data as well as the elastic scattering in the sub-barrier energy region, while the former underestimated the observed fusion cross sections. The shape of the fusion potential within the framework of the direct reaction theory of fusion has also been investigated. A fusion with a smooth Fermi-type surface, treating the radius and diffuseness as parameters, is assumed. It turns out that the fusion cross sections below the barrier are very sensitive to the diffuseness for a given radius parameter. It is also found that the diffuseness should be small in order to be consistent with the concept of a direct reaction, and recently accumulated data for the spin distributions of the compound nucleus.

      • 고성능 액체 크로마토그래피에 의한 Costus speciosus중 Diosgenin의 분석법

        Diosgenin in Costus specious was analysed by high-performance liquid chromatographic (HPLC) Technique. The dried, powdered sample was refluxed with the mixed solution of 3N HCl and xylene (1:1) at 95-100℃ for 4 hours. The residue obtained from the xylene layer was acetylated and then injected into HPLC. HPLC was performed using a set of four of the reversed-phase column (30㎝×3.9㎜ i.d., μ-Bondapak C_18, 10㎛) Connected in series. The solvent system was tetrahydrofuran - n - hexane-acetonitrile (1:2:7). Au UV detector was used at a wavelenght of 210㎚.

      • Silicone-pyridine에 대한 Chlorophyll-a와 -b의 혼합착물의 형광연구(Ⅳ)

        클로필-a와 -b를 혼합물의 상태로 1:2, 1:4, 1:6의 polydimethylethylsilane(0-2 -pyridylethyl methylsilane)에 Mg-pyridine 착물로 디에틸에테르중에서 결합시키고 중합체 사슬을 통한 에너지 이동을 연구하였다. Chl-b에 대한 Chl-a의 첨가량이 증가할수록 Chl-a의 혼합비(5:1, 3:1, 1:1)에 따라 형광수득율이 증가하였다. 반대로 Chl-b가 Chl-a에 비하여 첨가량이 증가할수록 형광수득율이 감소하였다. 최대 형광세기는 Chl-a와 Chl-b에 대하여 결합한 pyridine의 당량농도에서 나타났다. Chl-a에 의하여 흡수된 에너지는 중합체의 사슬을 통하여 Chl-b로 이동하였고 그 이동경향은 dimer>tetramer>hexamer의 순서로 증가하였다. The chlorophyll-a and -b have been bound 1:2, 1:4 and 1:6 polydimethyl ethyl silane(0-2-pyridylehylmethylsilane) as the mixed state to form the complex compound of Mg atom on chlorophyll and pyridine on the polymer in diethyl ether, and studied about the energy migration through the polymer chain in the same solvent. The more chl-a were added to chl-b the more increased fluorescence yields of chl-b were shown it to match for the mixed ratios of chl-a: chl-b in the order of 5:1, 3:1 and 1:1, while chl-a was shown the more decreased fluorescence yields according as the more chl-b were added. The maximum fluorescence intensity were appeared at equivalent pyridine concentration bound to the chl-a and -b. The absorbed energy by chl-a were transfered to chl-b through the chain, and the migration tendencies were on the increase of dimer>tetramer>hexamer.

      • 역상 크로마토그래피를 이용한 혈청에서의 Metronidazole 분석에 관한 연구

        Reversed phase liquid chromatography를 이용하여 혈액과 복강에서의 metronidazole의 농도를 측정하기 위해서 mobile phase 조건을 phosphate buffer (KHPO₄)/acetonitrile (5 mmol, pH 3.0, 92/8, v/v)로 하여 정량하여 다음과 같은 결과를 얻었다. Retention time은 hydroxy metabolite가 3.4분, metronidazole이 5.3분, tinidazole이 12.0분이었고 capacity factor는 hydroxy metabolite가 1.0, metronidazole이 2.2, tinidazole이 6.0이었다. 또한 Separation factor는 hydroxy metabolite와 metronidazole에서 2.2, metronidazole과 tinidazole에서 2.7이었다. 그리고 이론단수는 hydroxy metabolite가 5184, metronidazole이 3364, tinidazole이 3969이었다. Within run precision에서의 coefficient variation은 125㎍/ml에서 1.6%, 250㎍/ml에서 0.8%, 500㎍/ml에서 0.3%였으며 day to day precision에서 coefficient variation은 125㎍/ml에서 1.4%, 250㎍/ml에서 1.3%, 500㎍/ml에서 1.3%였다. 한편 분석회수율은 125㎍/ml에서 87.9%, 250㎍/ml에서 81.9%, 500㎍/ml에서 90.7%로 나타났다. Using reversed phase liquuid chromatography with UV detector was applied for metronidazole and its metabolites assay in human serum. The conditions with μ Bondapak ?? Column adopted for separation of metronidazole compouds were solvent system, acetonitrile/KH₂PO₄(5.0 mmol/1, pH3.0 8:92, v/v) at flow rate of 2.0ml/min. The results were follows. In chromatographic properties of metronidazole and its metabolites by reversed phase liquid chromatography; retention times of hydroxy metabolites, metronidazole, tinidazole were 3.4 min, 5.3 min, 12.0min and capacity factor(K') were 1.0, 2.2, 6.0 and separation factor (α)were 2.2, 2.7 and number of theoretical plates were 5184, 3364,3969 Within day precision studies of metronidazole showed coefficient variation 1.6% at 125㎍/ml, 0.6% at 250㎍/ml, 0.3% at 500㎍/ml. Day to day precision studies of metronidazole gave coefficient variations 2.9% at 1.4㎍/ml, 1.3% at 250㎍/ml, 1.3% at 500㎍/ml. In recovery of hydroxy metabolites and metronidazole was 87.9% at 125㎍/ml, 81.9% at 250㎍/ml, 90.7% at 500㎍/ml respectively.
