      • 동종 반월상 연골 이식술의 임상적 결과 - 내측과 외측 및 동반 손상의 유무에 따른 비교 -

        목적: 동종 반월상 연골 이식술의 결과를 내측과 외측 및 동반손상 유무에 따라 비교하고자 하였다. 대상 및 방법: 1999년 12월부터 2007년 6월까지 동종 반월상 연골 이식술을 시행하고 1년이상 추시가 가능했던 52례를 대상으로 하였다. 내측 이식군은 19례, 외측 이식군은 33례였고, 단독 손상군은 18례 동반 손상군은 34례였다. 평균나이는 34.2세(18세~51세) 평균 추시기간은 42.2개월(12개월~90개월)이었다. 임상적 결과의 판정을 위하여 슬관절 운동범위, VAS 점수, IKDC 주관적 평가 점수, Lysholm 점수, Tegner 점수, 환자 만족도 및 이차 관절경 또는 추시 MRI를 시행하였다. 결과: 수술 후 평균 관절운동 범위는 130.3도 였다. VAS 점수는 수술 전 5.96점에서 3.05점으로, IKDC 주관적 평가 점수 46.5점에서 64.52점으로(p<0.001), Lysholm 점수는 61.94점에서 79.58점으로(p=0.0019) Tegner점수는 수술 전 2.97에서 3.62점으로 향상되었으며 수술의 만족도는 '매우만족', '대체로 만족'이라고 대답한경우가 61.5% 였다. 수술 후임상적 점수의 향상이 있었지만 내측과 외측 이식술 군간 그리고 단독 손상군과 동반 손상군 간의 통계적 유의한 차이는 없었다. 이차 관절경을 시행한 18례중 10례에서 이식된 동종 반월상 연골의 변연부의 치유 소견이 관찰되었고 6례에서 부분파열이 2례에서는 복합 파열이 관찰되었다. 추시 MRI 촬영이 가능하였던 16례에서 아탈구는 12례에서 관찰되었다. 결론: 반월상 연골 이식술 후 내측과 외측 이식군 간의 임상적 결과는 차이가 없었으며 동반된 연골 손상이나 불안정성에 대한 치료를 병행한 경우 동반손상의 유무는 결과에 영향을 미치지 않았다. Purpose: To compare the clinical outcomes after meniscus allograft transplantation between lateral and medial or isolated and combined procedure groups. Materials and Methods: Of the patients who had undergone arthroscopic meniscal allograft transplantation between Dec. 1997 and Jun. 2007, 52 patients were available for retrospective evaluation. Patients were grouped into lateral(33 cases) and medial(19 cases) transplant groups as well as those with isolated(18 cases) and combined(34 cases) procedure. The average age was 34.2 years and the mean follow-up period was 42.2 months. Postoperative range of motion (ROM), visual analog scale (VAS), International Knee Documentation Committee (IKDC) subjective score, Lysholm score, Tegner score, patient's subjective satisfaction, $2^{nd}$ look arthroscopy and MRI were evaluated retrospectively. Results: Mean postoperative ROM was $130.3^{\circ}$. The VAS showed an improvement from 5.96 to 3.05 at the last follow up. IKDC subjective score and Lysholm score also showed an improvement from 46.5 to 64.5 and from 61.9s to 79.58 respectively. Tegner score was improved from 2.9 to 3.6. Overall, 61.5% of patients reported they were completely or mostly satisfied with procedure. There were no significant differences noted between lateral and medial groups as well as isolated and combined groups. In 2nd look arthroscopy, 10 of 18 cases showed good pheripheal healing and there were 6 cases of partial and 2 of complex tear. We observed graft subluxation or extrusion in 12 of 16 cases who were evaluated with follow-up MRI. Conclusion: Meniscus allograft transplantation alone or in combination with other procedure showed an improvement in knee pain and clinical score. But there were no significant difference between lateral and medial groups or isolated and combined procedure groups.

        농촌지역 노인들의 인지기능 장애와 사망과의 관련성

        60세이상 노인인구를 대상으로 '농촌지역 노인들의 우울 및 인지기능 장애에 관한 연구'를 한 이정애와 정향균의 연구대상 558명에 대해 농촌지역 노인들의 인지기능 장애와 사망과의 관련성을 살펴본 결과는 다음과 같다. 1) 3년동안 동안 전체 대상자의 사망률은 558명중 57명인 10.2%이었으며 353명의 정상 인지기능군 중 사망자는 30명으로 사망률 8.5%, 126명 의 경도 인지장애군중 사망자는 14명으로 사망률 11.1%, 79명의 중증 인지장애군중 사망자는 13명으로 사망률 16.5% 이었다(표 3). 2) 3년동안 전체 연구 대상자의 생존율은 0.91이었으며 정상, 경도, 중증 인지기능 장애군의 3년 생존율은 각각 0.92, 0.90, 0.86이었다. 로그 순위 검정법으로 인지기능 정상군과 경도 및 중증의 각 인지기능 장애군의 생존곡선을 비교한 결과 통계적으로 유의하지 않았다. 또한 인지기능 정상군과 경도 이상의 인지기능 장애군간 생존곡선을 비교한 결과도 유의한 차이는 없었다. 3) 혼란변인을 보정하지 않는 Cox의 비례위험 회귀 모형의 단변량분석의 결과 95% 신뢰구간(C.I. : Confidence Interval)에서 사망위험도가 유의한 변인은 연령, 월수입, 흡연습관, 신체장애 등이었으며, 인지기능 장애정도를 정상 그리고 경도 및 중증으로 분류한 분석에서는 정상군에 비해 경도 및 중증의 사망위험도가 유의하게 높지 않았으나, 인지기능 점수(MMSEK score)의 증가에 따른 분석 결과 사망위험도가 0.94로 유의하게 낮게 나타났다(표 4). 4) 잠재적 혼란변인들의 영향을 보정한 Cox의 비례위험 회귀모형의 다변량 분석의 결과 인지기능 장애정도 및 MMSEK 점수증가에 따른 사망위험도는 어느 모형에서도 인지기능 장애정도가 사망에 미치는 위험도는 통계적으로 유의하지 않았다(표 5). 5) 남녀별로 각각 인지기능 장애와 사망위험도와의 관계를 알아보기 위해 다변량 분석을 시행한 결과 인지기능 장애정도 및 MMSEK 점수 증가에 따른 사망위험도는 어느 모형에서도 인지기능 장애정도가 사망에 미치는 위험도는 통계적으로 유의하지 않았다(표 6, 표 7). 이상 본 연구는 농촌지역 노인들에서 인지기능 장애정도가 사망에 미치는 영향을 알아보고자 하였지만, 인지기능 장애정도가 사망에 미치는 영향을 통계적으로 유의하게 고찰하지 못하였다. The purpose of this study was to examine the mortality risk associated with cognitive impairment among the rural elderly. The subjective of study was 558 of 'A Study on the Depression and Cognitive Impairment in the Rural Elderly' of Jung Ae Rhee and Hyang Gyun Jung's study(1993). Cognitive impairment and other social and health factors were assessed in 558 elderly rural community residents. For this study, a Korean version of the Mini-Mental State Examination(MMSEK) was used as a global indicator of cognitive functioning. And mortality risk factors for each cognitive impairment subgroup were identified by univariate and multivariate Cox regression analysis. At baseline 22.6% of the sample were mildly impaired and 14.2% were severely impaired. As the age increased, the cognitive function was more impaired. Sexual difference was existed in the cognitive function level. Also the variables such as smoking habits, physical disorders had the significant relationship with cognitive function impairment. Across a 3-year observation period the mortality rate was 8.5% for the cognitively unimpaired, 11.1% for the mildly impaired, and 16.5% for the severly impaired respendents. And the survival probability was .92 for the cognitively unimpaired, .90 for the mildly impaired, and .86 for the severly impaired respondents. Compared to survival curve for the cognitively unimpaired group, each survival curve for the mildly and the severely impaired group was not significantly different. When adjustments models were not made for the effects of other health and social covariates, each hazard ratio of death of mildly and severely impaired persons was not significantly different as compared with the cognitively unimpaired. But, as MMSEK score increased, significantly hazard ratio of death decreased. Employing Cox univariate proportional hazards model, statistically other significant variables were age, monthly income, smoking habits, physical disorders. Also when adjustments were made for the effects of other health and social covariates, there was no difference in hazard ratio of death between those with severe or mild impairment and unimpaired persons. And as MMSEK score increased, significantly hazard ratio of death did not decrease. Employing Cox multivariate proportional hazards model, statistically other significant variables were age, monthly income, physical disorders. Employing Cox multivariate proportional hazards model by sex, at men and women statistically significant variable was only age. For both men and women, also cognitive impairment was not a significant risk factor. Other investigators have found that cognitive impairment is a significant predictor of mortality. But we didn't find that it is a significant predictor of mortality. Even though the conclusions of our study were not related to cognitive impairment and mortality, early detection of impaired cognition and attention to associated health problems could improve the quality of life of these older adults and perhaps extend their survival.

      • Expression of the cannabinoid type I receptor and prognosis following surgery in colorectal cancer

        <P>The cannabinoid system has been considered to be a potential target of colorectal carcinoma therapy. The aim of this study was to address the correlation between cannabinoid type 1 (CB1) receptor expression and disease severity/outcomes in patients with colorectal cancer (CRC). CB1 receptor expression was analyzed by immunohistochemistry using tissue microarrays in consecutive patients who underwent surgical resection (n=534). CB1 receptor expression was categorized as a high (≥66%) vs. low (<66%) immunopercentage as a median split, and was analyzed in relation to disease severity and overall survival. CB1 receptor expression was observed in 409 patients (76.6%). Low CB1 receptor expression was more frequently identified in stage IV than in stage I/II or III cancer (P<0.01 for both). In stage IV CRC, high vs. low CB1 expression was correlated with a statistically significant poorer overall survival (P=0.033) that was independent of age, R0 resection, tumor differentiation and chemotherapy [hazard ratio (HR), 1.805; 95% confidence interval (CI), 1.042–3.094; P=0.035]. However, CB1 expression was not observed to be correlated with patient survival following surgery in stage I/II or III cancer. The high immunoreactivity of the cannabinoid type 1 receptor is a significant prognostic factor following surgery in stage IV CRC.</P>

        Mangiferin Ameliorates Scopolamine-Induced Learning Deficits in Mice

        <P>The aim of this study was to evaluate the effects of <I>Anemarrhena asphodeloides</I> <SMALL>BUNGE</SMALL> (AA) on cholinergic memory deficits in mice. This agent has previously been used as an antipyretic, anti-inflammatory, anti-diabetic, and antidepressant in traditional Chinese medicine. Mangiferin was isolated from AA and showed a dose-dependent inhibition of acetylcholinesterase (AChE) activity (IC<SUB>50</SUB> value, 62.8 μ<SMALL>M</SMALL>). Cholinergic dysfunction was induced in mice by administering scopolamine, and the animals were then tested using the passive avoidance test as well as the Morris water maze test. Mangiferin (20 mg/kg, <I>p.o.</I>) significantly reversed scopolamine-induced deficits in the passive avoidance test, and also improved escape latencies in training trials and increased swimming times in the Morris water maze test (<I>p</I><0.05). Mangiferin also reduced acetylcholine and tumor necrosis factor (TNF)-α levels induced by scopolamine in mice brain (<I>p</I><0.05) and inhibited nuclear factor (NF)-κB activation in scopolamine or TNF-α-stimulated BV-2 microglial cells. These results suggest that mangiferin can improve long-term cholinergic memory deficits by AChE inhibition or cholinergic receptor stimulation and inhibition of NF-κB activation.</P>

        Original Articles : Morphologic Analysis of Water-Cooled Bipolar Radiofrequency lesions on Egg White in Vitro

        Background: The aim of this study was to document the optimal spacing of two cannulae to form continuous strip lesions and maximal surface area by using water-cooled bipolar radiofrequency technology. Methods: Two water-cooled needle probes (15 cm length, 18-gauge probe with 6 mm electrode tip) were placed in a parallel position 10, 20, 24, 26, and 28 mm apart and submerged in egg white. Temperatures of the probes were raised from 35oC to 90oC and the progress of lesion formation was photographed every 1 minute with the increase of the tip temperature. Approximately 30 photographs were taken. The resultant surface areas of the lesions were measured with the digital image program. Results: Continuous strip lesions were formed when the cannulae were spaced 24 mm or less apart; monopolar lesions around each cannula resulted if they were spaced more than 26 mm apart. Maximal surface areas through the formation of continuous strip lesion were 221 mm2, 375 mm2, and 476 mm2 in 10, 20, and 24 mm, respectively. Summations of maximal surface area of each monopolar lesions were 394 mm2 and 103 mm2 in 26 and 28 mm, respectively. Conclusions: Water-cooled bipolar Radiofrequency technology creates continuous "strip" lesions proportional in size to the distance between the probes till the distance between cannulae is 24 mm or less. Spacing the cannulae 24 mm apart and treating about 80oC for 24 minutes maximizes the surface area of the lesion.

      • Tankyrase-mediated β-catenin activity regulates vasopressin-induced AQP2 expression in kidney collecting duct mpkCCDc14 cells

        <P>Aquaporin-2 (AQP2) mediates arginine vasopressin (AVP)-induced water reabsorption in the kidney collecting duct. AVP regulates AQP2 expression primarily via G(s)alpha/cAMP/PKA signaling. Tankyrase, a member of the poly(ADP-ribose) polymerase family, is known to mediate Wnt/beta-catenin signaling-induced gene expression. We examined whether tankyrase plays a role in AVP-induced AQP2 regulation via ADP-ribosylation of G protein-alpha (G alpha) and/or beta-catenin-mediated transcription of AQP2. RT-PCR and immunoblotting analysis revealed the mRNA and protein expression of tankyrase in mouse kidney and mouse collecting duct mpkCCDc14 cells. dDAVP-induced AQP2 upregulation was attenuated in mpkCCDc14 cells under the tankyrase inhibition by XAV939 treatment or small interfering (si) RNA knockdown. Fluorescence resonance energy transfer image analysis, however, revealed that XAV939 treatment did not affect dDAVP-or forskolin-induced PKA activation. Inhibition of tankyrase decreased dDAVP-induced phosphorylation of beta-catenin (S552) and nuclear translocation of phospho-beta-catenin. siRNA-mediated knockdown of beta-catenin decreased forskolin-induced AQP2 transcription and dDAVP-induced AQP2 expression. Moreover, inhibition of phosphoinositide 3-kinase/Akt, which was associated with decreased nuclear translocation of beta-catenin, diminished dDAVP-induced AQP2 upregulation, further indicating that beta-catenin mediates AQP2 expression. Taken together, tankyrase plays a role in AVP-induced AQP2 regulation, which is likely via beta-catenin-mediated transcription of AQP2, but not ADP-ribosylation of G alpha. The results provide novel insights into vasopressin-mediated urine concentration and homeostasis of body water metabolism.</P>

        Design and characterization of a fungal bioaerosol generator using multi-orifice air jets and a rotating substrate

        <P><B>Abstract</B></P><P>Fungal bioaerosols, which are major constituents of ambient airborne microorganisms, are well known for their adverse health effects. However, thus far, the effective control of fungal bioaerosols has rarely been studied. A major reason for this is a lack of stable and reliable methods of generating fungal bioaerosols that can simulate real environmental dispersal. As a first step towards developing an efficient means of controlling fungal bioaerosols, we designed and evaluated a new fungal bioaerosol generator. The multi-orifice air jets and the rotating substrate inside the generator were used to uniformly scan a fungal culture Petri plate and to maintain uniformity of the production rate of fungal bioaerosols.</P><P>We conducted the experimental tests on <I>Aspergillus versicolor</I> and <I>Cladosporium cladosporioides</I>. The particle size distribution and the total particle number concentration of generated fungal bioaerosols were measured using a particle size distribution analyzer and a condensation particle counter. The effects of the air flow rate and the rotating speed of the substrate on the generation of fungal bioaerosols were investigated. The results demonstrate that the fungal bioaerosol generator can produce mono-dispersed fungal bioaerosols under various experimental conditions and that it is possible to control the rates of production of fungal bioaerosols by adjusting the flow rate through the fungal generator and the rotating speed of the substrate.</P>
