Colorectal cancer is common and the liver and lung are frequent metastatic sites. But single metastasis from colorectal cancer to the central airway is extremely rare. In a fatal central airway obstruction, generally, the rigid bronchoscopic interventions or surgical procedures under general anesthesia were the preferred management. However in patients who are not candidates for surgical resection, bronchoscope-guided multidisciplinary approaches offered a safe and effective alternative in the palliative management of respiratory symptoms related with airway obstruction. Of these, argon plasma coagulation (APC) is a valuable tool in treating superficial bleeding and debulking granulation tissue and tumors. In addition cryotherapy relies on repeated freeze/thaw cycles for tissue destruction, and spares cartilaginous structures due to their poor vascularity. Herein, we report an interesting case of the successful debukling management with combined APC and cryotherapy guided by flexible bronchoscopy under the simple sedation in a patient with impending total airway obstruction from colorectal cancer. Emphysema is a form of chronic obstructive pulmonary disease (COPD) that is de- fined by abnormal and permanent enlargement of the airspaces distal to the terminal bronchioles. Lung volume reduction surgery is a surgical treatment for patients with advanced emphysema. However, surgical morbidity is high and non-pulmonary comorbidities may preclude surgery. The incidence of tracheal post-intubation stenosis has increased because of the increasing number of patients who require mechanical ventilator support. Resection of the stenotic segment and reconstruction or Montgomery T-tube insertion are considered the ideal management. But these procedures require introduction with a rigid bronchoscope, surgical procedures, and an operating room under general anesthesia. Recently, as alternative options, airway stents have been extensively introduced with various results of success, and low intra-operative and early postoperative complications rates. However stenting for subglottic stenosis (SGS) remains a difficult therapeutic problem for the interventional pulmonologist in respect of its location relating stent migration and granulation formation as well as shear stress inducing 2ndary tracheo-laryngeal injury. Herein, we reported a case of 74-years-old man who treated with double covered-SEMS, guided by flexible bronchoscope and fluoroscope, for post-intubation subglottic stenosis that disrupted end-to-end anastomosis. S-345 Castleman``s disease in intrapulmonary lymph node presenting as solitary pulmonary nodule

Castleman's disease is an uncommon lymphoproliferative disorder which can occur wherever lymph nodes are presens. Castleman's disease in the chest develops usually in mediastinal or hilar lymph node. A 53-year-old woman was admitted to our hospital for further evaluation of a pulmonary nodule detected incidentally in health check up.Computer tomography demonstrated that this nodule was 27×25mm, well-defined in the right upper lobe. On MRI of chest MRI, which was performed to further diagnosis, the nodule was located on perihilar area of the right upper lobe and showed high signal intensity in T2 weight imaging and rapid and strong enhancement in dynamic contrast image, suspected to be a sclerosing hemangioma. We performed right upper lobectomy with lymph node dissection and the pathologic examination revealed that the pulmonary nodule was Castleman's disease arising from an intrapulmonary lymph node. We here report a rare case of Castleman's diseas in intrapulmonary lymph node presenting as solitary pulmonary nodule, suggesting that Castleman's disease should be considered in patinet with a nodule in perihiarl area.
Primary Pulmonary Melanoma with Distant Metastasis Diagnosed on PET-CT Scanw

Melanoma is a cutaneous malignant neoplasm of melanocytes. Primary malignant melanoma (MM) of the lung is very rare. Although previous reports have described the radiologic features of pulmonary MM, its rarity makes the issues still unknown. Furthermore, there is little information regarding diagnostic application and/or usefulness of [18F]-fluorine-2-fluoro-2-de-oxy-D-glucose positron emission tomography (FDG-PET)-computed tomography (CT). Herein, we introduce a primary MM of the lung with multiple metastases, focusing on the radiologic imaging including PET-CT imaging. The pulmonary mass was presented as a round, well-demarcated, and multi-lobulated mass with an increased uptake of FDG on PET-CT images.

Surgical resection remains as the treatment of choice for non-small cell lung cancer (NSCLC) in early stage. Despite some patients with inadequate respiratory reserve, cardiac dysfunction, vascular disease, or other medical co-morbidities are unable to tolerate the stress during surgery or postoperative period. The need for less invasive alternative therapy has increased. Filling up the need, radiofrequency ablation (RFA) plays a key role in improving and replacing the existing therapies including surgery or chemotherapy. Recent studies have shown that lung RFA is one of the most reliable alternative therapeutic tool for all the stage of lung cancer. Long term follow-up data of pulmonary RFA in terms of safety and efficacy have rarely reported. In fact, there was no report of complete remission (CR) achieved by RFA in advanced lung cancer. We reported a case of achievement of complete remission state for ten years by RFA combined with adjuvant chemotherapy for advanced stage of lung cancer.
HEVC에서 화면 간 예측 모드들 간의 율-왜곡 파라미터 분석

최근 표준화가 진행되고 있는 HEVC는 H.264/AVC의 기본 구조를 유지하면서 각 부호화기의 성능을 향상시키는 방향으로 진행되고 있다. 기존의 H.264/AVC는 화면 간 예측에서 예측 정보를 보내지 않고 복호화가 가능한 SKIP 모드와 예측 정보를 보내주는 화면 간 예측 모드들을 사용한다. 그런데 SKIP 모드는 율-왜곡 측면에서 극단적인 특성을 갖기 때문에 SKIP 모드와 다른 화면 간 예측 모드들 사이에 넓은 율-왜곡 파라미터 간격이 존재하며, 이는 화질 저하의 원인이 된다. 본 논문에서는 HEVC에서도 동일한 문제점이 존재하는지 분석하기 위해 SKIP 모드와 화면 간 예측 모드들의 율-왜곡 파라미터에 대한 분석을 수행하였다. 분석 결과를 바탕으로 HEVC에서도 부호화 효율을 향상시키기 위하여 새로운 화면 간 예측 모드가 필요함을 보였다.
