최근 충북지역에서 분리된 Gram 양성 세균종과 항생물질의 감수성 양상

임상검체에서 분리되는 병원균을 주기적으로 분석하고 이에 대한 항균제 감수성의 변화 양상을 파악하는 것은 임상에서의 적절한 항균제의 선택이나 내성균의 감독과 조절을 위해 필수적이라 할 수 있다. 저자들은 충북대학병원에 1996년 1년간 내원 환자의 미생물 배양 검사에서 분리, 동정된 1,689 균주중 Gram양성인 542 균주의 분리 빈도 및 주요 균종에 대하여 항생제 감수성 양상을 조사하였다. Gram 양성 균종은 S. aureus, S. pneumoniae, S. epidermidis, S. hemolyticus, coagulase negative Staphylococcus (CNS), Enterococcus faecalis 순으로 분리되었다. 한편, 항생제 감수성 시험 결과 S. aureus의 항생제 감수성은 penicillin과 gentamicin에 대한 저항성이 현저히 높았으며, teicoplanin과 vancomycin(MlC50 0.5㎍/ml)에 감수성이 높은 것으로 나타났다. CNS의 경우도 S. aureus와 대동 소이한 결과를 보였으나, S. epidermidis의 경우에는 ciprofloxacin과 clindamycin에 감수성이 높은 것으로 나타났다. Enterococcus spp.는 vancomycin, penicillin, tetracycline 등에 저항성 이 매우 높았으며, 검사한 모든 항생제에 대하여 대부분의 감수성이 zig-zag pattern을 보였다. In order to control resistant strains and to properly select the antimicrobial agents, it is of quite importance to know current trends of bacterial species and changing patterns of antimicrobial resistance rates. The authors studied the results of 542 Gram-positive strains among 1,689 strains isolated at Chung-buk National University Hospital in 1996. The frequently isolated Gram-positive microorganisms were Staphylococcus aureus, Streptococcus, pneumoniae, Staphylococcus epidermidis and Enterococcus faecalis in descending order. S. aureus showed high resistance to penicillin, gentamicin, and susceptibility to teicoplanin and vancomycin. Coagulase negative Staphylococcus was highly resistant to all of the antibiotics used in this experiment except teicoplanin and vancomycin. Enterococcus were highly resistant to vancomycin, penicillin and tetracycline. MIC of Gram- positive oaganisms was appeared to be zig-zag pattern.

Estimation problems of parameters of the failure process and the repair effect in repairable systems are considered. We propose estimation procedures in repairable systems without preventive maintenances. The fallure process is modeled by a proportional age reduction model (Brown. Mahoney, Sivazlian [5]) which is able to consider both aging and repair effects. Maximum likelihood method is used to estimate the repair effect and parameters of intensity function simultaneously. Simulations are performed to evaluate the accuracy of estimators. A numerical example is also presented.
호흡기감염증에 대한 Cefuroxime Axetil(Zinnat�)의 임상효과

To evaluate the efficacy and safety of Zinnat in patients with respiratory tract infections, 20 patients were treated with Zinnat (250 mg b.i.d). Among 20 cases, 5 cases had pneumonia, 5 cases exacerbation of chronic bronchitis, 4 cases acute bronchitis, 3 cases bronchiectasis with infection, and acute tonsilitis, bronchial asthma with infection and emphysema with infection 1 case, respectively. The response was cure in 12 cases, partial response in 6 cases and no response in 2 cases. There were no significant clinical and laboratory side effects except dyspepsia and nausea (1 case) and diarrhea (1 case). Zinnat was considered to be useful agent against bacterial respiratory tract infections.

Despite research to treat delayed onse muscle soreness(DOMS), no effective treatment has been reported. The purpose of this study is to investigate the effect of a hold-relax technique and cryotherapy on DOMS. Thirty-three subjects were randomly assigned to one of three treatment groups: control, hold-relax technique, or hold-relax technique and cryotherapy. DOMS was induced in the non-dominant biceps muscle through repeated eccentric contractions. Resting elbow joint position, flexion and extension(universal goniometer), pain(Visual Analogue Scale; VAS), and WBC count(blood analysis) were measured one hour before DOMS was induced and 24, 48, and 72 hours after DOMS was induced. The data were analyzed by repeated measure of two-way ANOVA. The results of this study were summarized as follows: 1) While analysis showed no significant differences between groups in relation to resting elbow joint position, there were significant differences over time, especially at 24, 48 hours after DOMS was induced compared with resting elbow joint position before DOMS was induced. 2) While analysis showed no significant differences between groups in relation to range of flexion, there were significant differences between rage of flexion before DOMS was induced and range of flexion 72 hours after DOMS was induced. 2) There were no significant differences between groups or over time in relation to range of extension. 4) While analysis showed no significant differences between groups in relation to VAS, there were significant differences over time in different hours. 5) There were no significant differences between groups or over time in relation to WBC count. 6) There were no interactions between groups or over time in all variables. This results suggested that hold-relax technique and cryotherapy were not effective to reduce DOMS.
Panax ginseng Improves Senile Testicular Function in Rats

We reported previously that the administration of Korean red ginseng water extract (KRG-WE) protected the guinea pig testis against damage induced by 2,3,7,8-tetrachlorodibenzo-p-dioxin (a potent endocrine disruptor). We also found that crude saponin from ginseng was the active ingredient responsible for this protection. Here, we examined the biological role of KRGWE in an animal model of age-induced dysfunction of spermatogenesis. Twenty-four male Sprague-Dawley (six 2-monthold and eighteen 12-month-old) rats were used. The young and old control groups received only vehicle. The ginseng saponin (GS)- and KRG-WE-treated groups received GS (40 ㎎/㎏ body weight/day) and KRG-WE (200 ㎎/㎏ body weight/day), respectively, for 4 months. The number of cells, Sertoli cell index, Johnsen’ score, and sex hormone levels decreased significantly with age. However, the administration of KRG-WE and GS markedly improved the number of germ cells, seminiferous tubular size, and Johnsen’ score in the old rats. Ginseng produced a distinct testicular histological improvement in old rats. KRG-WE and GS elevated testosterone levels, while attenuating the aberrant increase in follicle stimulating hormone and luteinizing hormone levels. Sperm kinematics evaluated by a computer-assisted sperm analyzer demonstrated improvement in the percentage of motile sperm, progressive sperm motility, and curvilinear velocity associated with sperm quality, supporting the beneficial role of red ginseng in senile spermatogenesis. Overall, the total water extract had a more potent effect than the corresponding saponin fraction. In conclusion, Korean red ginseng rejuvenated age-induced testicular dysfunction. Additionally, the total water extract was more potent than the corresponding saponin fraction.

The purpose of this study was to investigate the effects of a workplace-based work-conditioning program (WCP) on the management of work-related musculoskeletal disorders (WRMDs) in an automo-bile-parts manufacturing company. In total, 1,110 subjects with WRMDs participated in workplace-based WCP emphasizing function-centered management and ergonomics. We investigated the incidence of WRMDs variables (number of persons diagnosed with industrial accident-related WRMDs, number of cases of WRMD) and financial benefits (cost of workers' compensation insurance and lost work days related to WRMDs) per year before and after WCP. Additionally, we compared self-reported pain intensity and functional disability in subjects with musculoskeletal pain before and after the WCP. Pain intensity was measured using a visual analog scale (VAS), and functional disability was measured by the neck disability index (NDI) and the Oswestry disability Index (ODI). The number of person diagnosed with in dustrial accident-related WRMDs, the number of cases of WRMD, the cost of workers' compensation in surance and lost work days related to WRlVIDs per year decreased by 51%, 37%, 34%, and 47%, respectively, and VAS, NDI, and am scores decreased significantly after implementation of WCP (p

We have previously reported that intraperitoneal (i.p.) injection of Deer antler extract demonstrated preventive and therapeutic effects against testicular toxicity induced by 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD) in male rats. Forty rats (250 ::t 20g) were divided into 4 groups: normal controls (group 1) received vehic1e and saline; group 2 (single TCDD-treated) received TCDD (20 μ g/kg, i.p.) and saline (p.o.); groups 3 and 4 received Deer ant1er extract from Deer ant1er (DA) at daily oral dose of 10 and 20 mg/kg, respectively, and vehic1e (i.p.) for 6 wks from 1 wk after TCDD-exposure. Weights of kidney, spleen and testis of the TCDD-exposed rat were significantly decreased. Moderate swe11ing of hepatocytes, hyperchromatism, acidophi1ic cytoplasm and cytoplasmic vacuolation, single cell necrosis, small groups of degenerating hepatocytes surrounded by mononuc1ear cells and some loss of lobular architecture were observed in G2 animals. on the other hand, histopathological damage was moderately to markedly alleviated in groups 3 and 4, but pretreatment of Deer antler was more effective than the simultaneous treatment. From these results, we could understand the active compounds of Deer antler and further prove previously reported preventive and therapeutic effect of Deer antler against TCDD-induced toxicity in male rats.
