저자들은 56세 여성 환자의 왼쪽 쇄골 하 정맥에 시행한 이식형 약제주입기 카테터의 삽입 후 발생한 비정상 위치에 대한 증례를 보고하고자 한다. 카테터의 끝은 환자의 늘 어진 가슴, 체위, 신체의 움직임, 카테터의 유동성 등의 영향으로 천자된 정맥 내에서 견인되고 뒤쪽으로 이동하였다. 환자는 어떠한 증상도 호소하지 않았지만 비정상 위치의 카테터는 심각한 합병증을 유발할 수 있기 때문에 제거되었다. 따라서 시술자는 약제주입기 삽입 전에 환자의 신체 상태와 단순 흉부 촬영상을 확인하고 카테터의 비정상 위치가 발생할 수 있는 위험 징후를 알아차려야 하겠다.

감염성 대동맥류는 세균성 감염에 의한 동맥벽의 미란과 파괴로 생긴 동맥류를 말한다. 그 원인 병원체로는 그람 양성균, 그람 음성균, 진균, 항산균 등이 있다. 폐렴막대균은 비뇨생식기계 감염, 지역사회 폐렴, 간농양, 내인성 안내염 등을 흔하게 발병시키나 감염성 동맥류의 원인으로는 드물다. 최근 들어, 특히 동부 아시아 지역에서 폐렴막대균으로 인한 간농양의 발생과 이에 동반된 내인성 안내염의 보고가 증가하고 있는 추세이지만 아직까지 폐렴막대균 감염성 동맥류 A mycotic aneurysm is a localized, irreversible arterial dilatation resulting from destruction of the vessel wall by infection. A 66 year old male underwent functional neck dissection due to squamous cell carcinoma on the tongue base. The preperative finding of electrocardiography(ECG) was nonspecific. After the anesthetic induction, the blood pressure was unstable and hypotension continued all through the operation. Continuous intravenous dopamine infusion was done to correct the hypotension 4 hours after the start of operation. But the hypotension was not corrected and ST segment depression of ECG was more aggravated. Seven hours after the start of operation, cardiac arrest occurred. We performed cardiopulmonary resuscitation immediately and the heart beat was restored at 20 minutes after the resuscitation. Consciousness was recovered at the end of operation. Ventilatory support was gibe in the intensive care unit. The patient discharged without sequelae 20 days after the admission. The circulatory collapse was thought to be attributed to incompletely treated hypovolemia, myocardial depression and compromising coronary blood flow producing ST segment depression.
Second Trimester Maternal Serum Marker and Adverse Pregnancy Outcome

Objective The aim of this study is to assess the risk of adverse pregnancy outcome by maternal serum markers in Quad test and to compare the existing cutoff (2.0 MoM) with 95 percentile cutoff generated from our consecutive population in predicting adverse pregnancy outcome. Methods We generated 95 percentile cutoff as our own reference using 3,000 consecutive women who performed Quad test. Except for follow-up loss, fetal aneuploidy and anomaly, 2,598 women were analyzed for the assessment of adverse pregnancy outcome consisted of preeclampsia (PE), preterm birth (<32 weeks), low birth weight and fetal death in utero. Results We confirmed high levels of alpha-fetoprotein (AFP), human chorionic gonadotropin (hCG), and inhibin A are associated with development of PE, preterm birth, and low birth weight. In general, 95 percentile cutoff corresponded similarly with 2.0 MoM except AFP. Combination of serum markers increased the odds ratio for predicting adverse pregnancy outcome. The women with higher level of both AFP and inhibin A (2.0 ≥ MoM) had 9.8 times higher risk for PE, 24.8 times higher risk for preterm birth, and 5.6 times higher risk for low birth weight. The women with higher level of AFP, hCG, and inhibin A had 8.2 times higher risk for PE, 29.4 times higher risk for preterm birth. Notably, negative predictive value of serum markers for PE and preterm birth are over 98% either in alone and in combination. Conclusion Maternal serum makers either in alone and in combination offer valuable information on the risk assessment of adverse pregnancy outcome.

The objective of this study was to determine the effects of physically effective neutral detergent fiber (peNDF) content in total mixed ration (TMR) on dry matter intake, digestibility, and chewing activity in fattening Hanwoo (Bos taurus coreanae) heifers. The experiment was designed as a replicated $3{\times}3$ Latin square using 12 heifers. Fattening heifers were offered one of three diets [high (T1), medium (T2), and low (T3) peNDF] obtained by different mixing times (3, 10, and 25 min) for the same TMR feed. The peNDF content of TMR was determined by multiplying the proportion of dry matter retained by a 1.18 mm-screen in a Penn State Particle Separator by the dietary NDF content. The $peNDF_{1.18}$ content was 30.36%, 29.20%, and 27.50% for the T1, T2, and T3 diets, respectively (p<0.05). Dry matter intake was not affected by peNDF content in TMR. Total weight gain in T1 group was significantly higher (p<0.05) than in T2 and T3 groups. However, weight gain did not differ between T2 and T3 groups. The feed conversion ratio decreased with an increase in the peNDF content (T1: 12.18, T2: 14.17, and T3: 14.01 g/g). An increase in the peNDF content of TMR was associated with a linear increase in the digestibility of dry matter, crude protein, crude fiber, neutral detergent fiber, and acid detergent fiber (p<0.05). Also, an increase in peNDF content of the TMR resulted in a linear increase in the number of chews in eating and ruminating (p<0.05), and consequently in the number of total chews (p<0.05). These results indicate that peNDF content affects digestibility and chewing activity. Consequently, the peNDF content of TMR should be considered for improving feed efficiency, digestibility, body weight gain, and performance in fattening heifers.
미세튜브 열교환기의 장착을 고려한 구조건전성에 관한 연구

본 연구의 목적은 유한요소 해석을 이용하여 항공기 엔진에 장착된 미세 튜브형 열교환기의 작동 하중에 대한 특성을 파악하고 구조안전성을 평가하는 것이다. 작동 하중은 열-기계하중이 고려되었다. 항공기 엔진 부품의 특수성으로 인하여 구조적 강성확보뿐만 아니라 고효율, 저중량, 최소체적을 만족하는 형상 설계가 요구 된다. 브레이징한 미세튜브의 기계적물성치 확보를 위해 고온 인장 실험을 수행하였다. Ansys 12.1의 자체 모델러를 이용하여 형상을 모델링하고 유한요소해석을 수행하였다. 열-구조 연성해석을 통하여 튜브를 제외한 모듈형(modular type) 열교환기에 대한 설계방법을 제시하고 구조적 건전성을 평가하였다. The objective of this study is to predict the structural characteristics of a heat exchanger mounted on an aircraft engine using finite element analysis. The plastic fracture and life of the heat exchanger were estimated by a thermo-mechanical analysis. Tensile tests were conducted under high temperature conditions (700, 800, 900, 1000 K) using five specimens to obtain the mechanical properties of the Inconel 625 tubes. To assess the structural characteristics of the heat exchanger, the full and partial models were applied under the operating conditions given by the thermo-mechanical and inertial load. As a result, the case, tubesheet, flange, and mounting components have a reasonable safety margin to the allowable stress assuming a fatigue strength of Inconel 625 of 10000 cycles under 1000 K.
