Ectodermal dysplasia(외배엽 이형성증)은 외배엽성 기관에 한가지 이상의 선천적 이상소견을 특징으로 하는 질환이다. Ectodermal dysplasia는 약 120종류 이상이 보고되고 있고, 이 중 Hypohidrotic ectodermal dysplasia가 가장 잘 알려져 있으며 이는 성염색체 열성인자에 의해 유전되며 남성에서 호발한다고 한다. 임상증상으로는 무한증, 감모증, 무치증 혹은 핍지증이 가장 특징적이다. 이외에도 무피지증, 손톱이형성증, 가늘고 성긴 머리카락, 두드러진 이마, 안장코, 돌출된 안와상부, 입술의 돌출과 함께 피부가 건조하고 땀샘이 결핍되어 있어서 더위를 참지 못하고 발열이 일어나기 쉽다. 본 증례의 환자는 단국대학교 치과병원 소아치과에 내원한 6세 6개월된 남아로써 무치증을 보이는 Hypohidrotic ectodermal dysplasia로 진단되었고 환자의 심미적, 기능적 결함을 해소하기 위해 총의치로 치료를 시행하는 동안 다소의 지견을 얻어 이를 보고하는 바이다. 주요어 : 외배엽 이형성증, 무치증, 총의치

Introduction: Baclofen is a gamma-aminobutyric acid(GABA) derivative which is commonly used for hiccup, and spinal origin spasm, pain. baclofen side effects are nausea, vomit, dizziness, weakness, mental state change and respiratory depression. Baclofen is primarily excreted by glomerular fi ltration, so pharmacokinetic of baclofenis signifi cantly affected by kidney disease. In a patient receiving hemodialysis, baclofen toxicity is often reported. However, baclofen toxicity in patients with chronic kidney disease is not reported in korea. This paper reports one case of baclofen toxicity in a patients with chronic kidney disease who does not receiving hemodialysis. Case: A 78 year old patient with chronic kidney disease was admitted due to drowsy mental status after 4days of baclofen treatment in urologic department due to hiccup control. On physical examination, there were no focal neurologic defi cits. Laboratory data showed hyperkalemia and no another abnormality. Brain computed tomography and brain magnetic resonance imaging showed no evidence of acute cerebral hemorrhage or infarction. EEG showed no specifi c abnormality. However correcting serum potassium level, the patient`s mental state didn`t change. After 4days of conservative treatment with baclofen quit, neurologic abnormality was completely recovered. Because the patient maintained normal state of consciousness and no evidence of mental and neurologic abnormality, he was discharged after 12 days of hospitalized day. He is attended hospital outpatients and no abnormality is observed. Conclusions: In conclusion, Physicians should be aware of the possibility of baclofen- induced encephalopathy in old aged patients with chronic kidney disease, and start low dose baclofen in these patients.

Members of the TGA family of basic domain/leucine zipper transcription factors regulate defense genes through physical interaction with NON-EXPRESSOR OF PR1 (NPR1). Of the seven TGA family members, TGA4/octopine synthase (ocs)-element-binding factor 4 (OBF4) is the least understood. Here we present evidence for a novel function of OBF4 as a regulator of flowering. We identified CONSTANS (CO), a positive regulator of floral induction, as an OBF4-interacting protein, in a yeast two-hybrid library screen. OBF4 interacts with the B-box region of CO. The abundance of OBF4 mRNA cycles with a 24 h rhythm under both long-day (LD) and short-day (SD) conditions, with significantly higher levels during the night than during the day. Electrophoretic mobility shift assays revealed that OBF4 binds to the promoter of the FLOWERING LOCUS T (FT) gene, a direct target of CO. We also found that, like CO and FT, an OBF4:GUS construct was prominently expressed in the vascular tissues of leaf, indicating that OBF4 can regulate FT expression through the formation of a protein complex with CO. Taken together, our results suggest that OBF4 may act as a link between defense responses and flowering.

Valacyclovir is a prodrug of acyclovir, transformed to acyclovir by first-pass metabolism in the liver. In recent years, valacyclovir has largely replaced acyclovir in the treatment of herpesvirus infections, because it is more effective by oral administration. Valacyclovir is used increasingly to treat herpes zoster, although neuropsychiatric symptoms [valacyclovir neurotoxicity (VAN) or acyclovir neurotoxicity], may accompany use of this drug. This paper reports two cases of neurotoxicity of valacyclovir in a patients with end-stage renal disease who was undergoing maintanence hemodialysis. A 68-year-old male, receiving hemodialysis three times a week, prescribed valacyclovir 1000 mg/day on every dialysis day, for herpes zoster of left thigh . After he took his second dose orally, he developed lethargy and disorientation. His mental status progressed to semicoma. Daily conventional hemodialysis was started. After three sessions of hemodialysis, the patient became alert A 64-year-old male who was being treated with hemodialysis was admitted for dysarthria and euphoria. He took 1,000 mg of valacyclovir twice for hepes zoster of fourth level of thoracic nerve. After two days of treatment with oral valacyclovir, he had several neurologic symptoms as mentioned above. We performed daily hemodialysis and after three session of hemodialysis, he fully recovered. In conclusion, valacyclovir can induce life-threatening neurotoxicity, especially in ESRD patients with appropriate dose reduction. But which can be effectively managed by hemodialysis.

Primary dissection of the renal artery is rare. Spontaneous renal artery dissection can be associated with diseases such as medial degeneration, neurofibromatosis, syphilitic arteritis, tuberculosis, polyarteritis nodosa, Marfan syndrome, fibromuscular dysplasia, or Ehlers-Danlos syndrome (EDS). Among these causes, EDS related renal artery dissection is very rare worldwide and has not been previously reported in Korea. EDS are a group of heritable connective tissue disorders characterized by fragility of the skin and hypermobility of the joints. We describe the case history of a young man who presented with left side flank pain, hypermobility of the hand joints and showed left renal artery dissection on computed tomography and angiography that turned out to be the first complication of vascular type EDS.

Background: Acute pyelonephritis (APN) is a relatively common bacterial infection. Bilateral APN may be cause of acute kidney injury(AKI) that is also independent risk factor for the development or progression of chronic kidney disease(CKD). Although several cases of AKI due to APN have been described, CKD from APN in healthy man has rarely been reported. Case: A 52-year-old man was admitted to our emergency department with general weakness that lasted 2 weeks. He was diagnosed with bilateral APN using abdominal CT and ultrasonography that showed edematous swelling of both kidneys. His serum creatinine rose to 9.79 mg/dL. But, his GFR was in normal range 2 months ago. He did not have medical history was unremarkable for risk factors of AKI. He was treated with intraveonus antibiotic Therapy(imipenem for 29 days, cilastatin for 9 days, vancomycin for 3 days, ciprofi oxacin for 5 days). At discharge, his MDRD eGFR was 29 mL/min/1.73 m2. After discharge, he visited OPD many times. His urinalysis constantly showed pyuria, and bacteriuria. A renal biopsy performed 1 year after the AKI showed chronic tubulointerstitial nephritis, moderately advanced. At that time, his MDRD eGFR was still 30 mL/min/1.73 m2. Conclusions: In previously healthy man, chronic kidney disease is very rare in the course of APN without urinary tract obstruction, renal hypoperfusion due to hypotension, exposure to nephrotoxic agents or other structural anomalies. Here, we describe the case with a literature review.
Thrombospondins Mediate the Adhesion of Osteoblast to Extracelluar Matrix

Thrombospondins (TSP-l, TSP-2) are secretory extracellular glycoproteins that are involved in a variety of physiological processes such as tumor cell adhesion, invasion, and metastasis. The present study was undertaken to elucidate the involvement of thrombospondins in the adhesion of osteoblast-like cells using the TSP-l or TSP-2 antisense MG63 and MC3T3-El cell lines. For downregulation of TSPs expression, we prepared antisense constructs for TSP-l and TSP-2 using the pREP4 an episomal mammalian expression vector, which be able to produce the specific antisense oligonucleotides around chromosome. MG63 and MC3T3-El osteoblast-like cells were transfected with the antisense constructs and nonliposomal Fugene 6, and then selected under hygromycin B (50 μg/ml) treatment for 2 weeks. Western blot analysis revealed that expression of the TSP proteins was downregulated in the antisense cell lines. The cell adhesion assay showed that adhesive properties of TSP-l and TSP-2 antisense MG63 cells on the polystyrene culture plate were reduced to 17% and 21% of the control cells, respectively, and those of the TSP-l and TSP-2 antisense MC3T3-El cells also decreased to 19% and 27% of control, respectively. Adhesion of TSP-l and TSP-2 antisense MC3T3-El cells on Type I collagen-coated culture plate decreased to 27% and 76%, respectively. These results indicate that TSP-l and TSP-2 proteins may have an important role in adhesion of osteoblast-like cells to extracellular matrix.
