Is the Internet advancing democracy? As the Internet has been instrumental in gathering information and public support all over the world, many debates take place regarding the impact of information technology on civil participation and democracy. However, evaluations on the impacts of the Internet as the major causes for the demonstrations run the extremes. This paper analyzes the content of Internet bulletin boards involved in the 2008 protest against U.S. beef imports in Korea. Are debates in public opinion formation on the Internet being communicated in a rational and reasonable manner? How has democracy been realized through the debates that were actively carried out through online channels? This paper will explore the possibility of forming public opinions using the Internet through the qualitative analysis of how the Internet was used in citizens' political participation. This research seeks to examine how debates through the Internet are carried out in view of deliberative democracy using three aspects - information provision, the diversity of participation groups and the exchange of ideas. The level of deliberation is assessed through content analysis of cyber bulletin boards of ‘Agora Political Discussion Forum’ over the 2008 demonstrations against U.S. beef imports.

The author analyzed 158 cases of intracramal anenrysm surgically treated in the Department of Nenrosurgery, Yonsei University Medical College, from Jan. 1965 to Sept. 1976. Of the 158 cases 125 underwent direct incracranial approach (clipping, ligature, coating, wrapping, proximal vessel occlusion, and trapping) for the treatment of; the anenrysms, and 33 cases underwent proximal carotid ligation in the neck as the choice of treatment. The aneurysms arising in the posterior communicating artery (46) are most common, followed by anterior communicating artery (43), middle cerebral artery M₂ portion (22), and carotid bifurcation (12) in order. We also found cavernous aneurysm (2), ophthalmic artery aneurysms (3), anterior choroidal artery anurrysm (2), anterior cerebral artery A₁ portion aneurysm (6), distal anterior cerebral artery aneurysm (2), middle cerebral artery Mi portion aneurysm (8), Ms portion aneurysm. (2), posterior cerebral artery aneurysm (1) and basilar artery aneurysm (1). Multiple aneurysms were found in 8 cases. There was no significant difference according to sex. There were 18 cases over 60 years of age, of whom 2 were 75 years old. On this basis, we cannot consider age an absolute contraindication to direct intsacranial aneurysmal surgery. Overall mortality rate for direct intracranial approach was 12% and that of proximal carotid ligation was 18.2%. If patients who were operated on in emergency because of large intracranial hematoma of repeated, or chain ruptured of aneurysm are excluded, the mortality rate for purely elective cases using the intracranial approach drops to 4. 3%. After introduction of the surgical microscope and newly designed temporary clipping system, 37 patients were operated on with only one death (2.7%), a patient who had repeated ruptures of the posterior communicating artery aneurysm and was operative on only 7 days after the last bleeding. So recently we have not had to pay much attention to the mortality rate of the intracranial attack for the aneurysms. There are instances when imperfection which cannot be seen by the naked eye are noticed only after the aneurysmal neck is magnified from 6 to 40 times with use of microscope. In these cases, it is obvious that correction of clipping as well as coating, which prevents rupture of the aneuryemal sac and reinforces, or prevents slipping of the clip are inevitable. Various factors, such as grade of the patient's condition vasospasm, cerebral edema, presence of intracranial hematoma, status of intracranial pressure, preoperative management, timing of surgery, points of operative technique according to the sites of aneurysms, postoperative complications and their management, all of which affect the clinical and postoperative course, are discussed.

Purpose: Under the South Korea’s unique health insurance structure, any new surgical technology must be evaluated first by the government in order to consider whether that particular technology can be applied to patients for further clinical trials as categorized as ‘New Health Technology,’ then potentially covered by the insurance sometime later. The aim of this meta-analysis was to assess the safety and efficacy of transanal total mesorectal excision (TaTME) for rectal cancer, activated by the National Evidence-based Healthcare Collaborating Agency (NECA) TaTME committee. Methods: We systematically searched Ovid-MEDLINE, Ovid-Embase, Cochrane, and Korean databases (from their inception until August 31, 2019) for studies published that compare TaTME with laparoscopic total mesorectal excision (LaTME). End-points included perioperative and pathological outcomes. Results: Sixteen cohort studies (7 for case-matched studies) were identified, comprising 1,923 patients (938 TaTMEs and 985 LaTMEs). Regarding perioperative outcomes, the conversion rate was significantly lower in TaTME (risk ratio, 0.19; 95% confidence interval, 0.11–0.34; P < 0.001); whereas other perioperative outcomes were similar to LaTME. There were no statistically significant differences in pathological results between the 2 procedures. Conclusion: Our meta-analysis showed comparable results in preoperative and pathologic outcomes between TaTME and LaTME, and indicated the benefit of TaTME with low conversion. Extensive evaluations of well-designed, multicenter randomized controlled trials are required to come to unequivocal conclusions, but the results showed that TaTME is a potentially beneficial technique in some specific cases. This meta-analysis suggests that TaTME can be performed for rectal cancer patients as a ‘New Health Technology’ endorsed by NECA in South Korea.

Sandmeyer 반응으로 5-Amino-6-methyluracil로 부터 5-iodo-, 5-chloro- 및 5-bromo-6-methyluracil을 간편하게 합성하는 방법을 새로히 마련하였다. 이 방법에 따르면 5-halo-6-methyluracil이 70% 이상의 좋은 수득률로 얻어지며 부반응을 수반하지 않으므로 생성물의 정제가 쉽고 편리하게 5-halouracil을 얻을 수 있다는 이점이 있다. A new conventional method for the preparation of 5-iodo, chloro-, and bromo-6-methyluracil by Sandmeyer reaction was described. According to this procedure, 5-halo-6-methyluracils have been prepared in high yields (up to 70%) without any difficulties to obtain highly pure products. No appreciable competing reaction was observed.
