      • 평양 남정리 53호분 출토 토제칠기의 보존 및 제작기법 연구 : 토제칠이배, 토제칠반, 토제칠안

        본고는 일제 강점기 조사 미등록 유물 정리사업의 일환으로 수행한 평양 남정리 53호분 출토 토제칠이배, 토제칠반, 토제칠안에 관한 연구이다. 남정리 출토 유물은 소지가 목심이 아닌 토제라는 것과 칠이배와 칠반을 얹은 칠안이 함께 출토되었다는 점에서 주목할 만하다. 토제칠이배 및 칠반 그리고 칠안의 보존처리를 통해 이들의 형태와 문양 등에 대해 알 수 있었다. 또한 칠기에 사용된 붉은색 안료의 성분을 분석하여 안료의 주성분이 진사(HgS)였음을 확인하였다. 이번 연구에서 기존에 잘 알려지지 않았던 한반도 내 위치한 낙랑의 토제칠기에 관해 살펴볼 수 있었으며 이러한 제작방법이 한나라 고유의 문화인지 아니면 재지세력이 한나라의 영향을 받아 현지화한 문화의 산물인지에 대해 생각해 볼 수 있는 기회라 생각된다. This research paper examines the lacquered pottery cup, plate and table unearthed from Tomb No. 53 at Namjeong-ri in Pyongyang, which were under conservation treatment as a part of the Project of Unregistered Artefacts Uncovered in the Japanese Colonial Era. It is very rare to have a lacquered pottery cup and plate, lying on a lacquered pottery table, as a set of grave goods. In particular, they are not wooden but pottery items. Through conservation treatment for lacquered pottery cup, plate and table, it was possible to find out their shapes and decoration patterns. The resulting analysis of the red pigment used indicates that cinnabar (HgS) is the main element. This research is significant because it examines lacquered pottery wares produced in Lelang, which has rarely been conducted. Further research will need to prove whether their production techniques were in the unique Chinese Han style or were produced by indigenous craftsmen who adopted the Chinese Han technique.

      • 신경망 모형을 이용한 무심천 수위 예측

        Casualties and loss of property are experienced year after year in Korea because of localized heavy rain in summer. Therefore, appropriate administration of water resources is required and it is needed to predict the water level using the rainfall and runoff effectively in th river. In this study, neural network model is used to overcome the uncertain model parameters in rainfall-runoff model. The first step is to calculate the accumulative rainfall, effective rainfall, discharge and so forth, on the basis of rainfall data in the basin. In order to test and the verify the water level, eleven rainfall events observed in the river basin are used, of which nine events are used for the test data and two events are used for the verification. Using the Levenverg-Maquart method, the water levels in Mushim stream are estimated. The predicted values of the water level is calibrated by the HEC-RAS model. The results show that the water levels obtained by hourly rainfall are more effective than those obtained by accumulative rainfall or effective rainfall. And the results show that water levels obtained by rainfall and the former time's water level as input layer are more accurate than those obtained by the former time's discharge as input layer.

        다발성 외상시 응급 초음파검사의 의의

        The Evaluation of blunt abdominal trauma is clinically challenging. Abdominal computed to-mography(CT) and diagnostic peritoneal lavage(DPL) has become primary diagnostic modali-ty in advanced trauma life support(ATLS). But DPL is invasive study with potential for the patients. Monitoring and resuscitation are usually suboptimal during there examinations. We examined the efficacy and role of emergency ultrasonographic(US) studies in the initial abdominal evaluation of blunt trauma patients. A prospective study was carried out on 101 blunt trauma patients from 1994, June to September. 101 patients underwent real-time US ex-amination of the chest and abdomen. Indication for emergency US included the cases of shock (systolic blood pressure<90mmHg) or tachycardia (pulse rate/min.>100) in blunt trauma pa-tients. US showed a sensitivity of 96.2%, specificity 98%, accuracy 97% in regard to indica-tion for surgery in case of blunt abdominal or thoracic trauma. US saves time, and it can be performed during the emergency care in the emergency department. US showes high sensitivity and specificty, and is the method of first choice in the evaluation of blunt trauma.

        하부 늑골골절과 masked 복부손상과의 상관관계

        Some emergency physicians neglect abdominal injuries if a patient does not complaint of abdominal pain or abdominal tenderness was absent on physical examination. But intra-abdominal injuries are correlated closely with lower rib fractures and pelvic bone fractures. In cases of lower rib fractures, intra-abdominal organs are injured as a diaphragm is elevated as high as 5th intercostal space in expiration period. When intra-abdominal solid organ is ruptured, a patient complaints of abdominal pain, but there`s no abdominal pain if small hematoma occurs in solid organs. Although the most of solid organ hematoma are resolved spontaneously without complications, sometimes delayed rupture of hematoma occurs especialy in patients with the coagulopathy. So emergency physician must evaluate the abdomen closely when the possibility of intra-abdominal injury is high. To evaluate the intra-abdominal injuries, we investigated 57 patients with lower rib fractures by ultrasonography. The patients were divided in 2 groups according to presence of abdominal symptomes; patients with abdominal symptoms(n=10). Among 57 patients, intra-abdominal injuries were found in 35 patients. In group with lower rib fractures and presence of abdominal symptoms, intra-abdominal injuries were diagnosised in 32 patients(68.9%) with 48 cases(hepatic injury; 20, spleen injury; 19, renal injury 7, hemoperitoneum without solid organ injury; 2). In group with lower rib fractures and abscence of abdominal symptoms, intra-abdominal injuries were diagnosised in 3 patients(30.0%) with 3 cases(spleen injury; 1. hemoperitoneum without solid organ injury;2). In conclusion, emergency physician must do ultrasonography to evaluate intra-abdominal injuries in patients with lower rib fractures although a patient does not complaint of abdominal pain.

        외상후 혈뇨환자에서 응급 초음파검사의 의의

        The Ultrasonography is rapid, mobile, and noninvasive diagnostic tool in emergency center. As a result, we estimate it`s value as an advocated primary diagnostic modality in the immediate evaluation of patients with multiple trauma. We investigated 60 patients with traumatic hematuria(microscopic hematuria or gross hematuria) who visited our emergency department from 1995, July to 1996, March. To evaluate the injury of urinary systems, emergency physician examined patients with hematuria by ultrasonography, and comapared the results with those of excretory urography and computed tomography. We demonstrated 21 positive findings for abnormal fluid collection and renal parenchymal injury and bladder rupture on ultrasonography. All bladder injuries were diagnosised by ultrasonography, and all major renal injury(grade Ⅲ,Ⅳ,Ⅴ) were also diagnosised by that. The accuracy of emergency ultrasound examination in traumatic hematuria was 97.0%, and sensitivity was 95.0% with specificity of 97.4%. The ultrasonography was an accurate diagnostic adjunct in detecting renal injury and bladder injury in traumatic hematuria patients. Ultrasonography provides an excellent noninvasive diagnostic tool for the initial screening study of patients with traumatic hematuria.

        다발성 외상환자에서 필수적 단순 방사선검사의 의의

        In the guidelines of Advanced Trauma Life Support(ATLS), essential plane roentgenograms (chest AP vIew, cervical spine lateral view and pelvis AP view) are recommanded when primary survey and resuscitation phase are finished in care of multiple injured patients. A prospective study was carried on 376 multiple trauma patients who admitted to Wonju Christian Hospital from July, 1995 to October. We analyzed clinical significance of essential X-ray in multiple trauma patients, and classified the patients by their status of mentality. The one is alert group when their Glasgow Coma Scale(GCS) is over 13 point, the other is disoriented mentality group when their GCS are below 12 point. The alert group was redivided by presence of clnical symptoms and/or signs, and clinical significances of essential X-ray were analyzed. Among the 376 patients, 330 patients are alert group that their GCS score is over 13 point and 46 patients are disoriented group who's GCS were below 12 point. The detection rate of abnormality in essential X-ray is higher in disoriented group than alert group(cervical-spine lateral view : 13.5% vs 3.0% cheat AP view : 0.4%, pelvis AP view : 0%). This study suggests that their are no needs of taking essential X-ray in patients who is alert in mental state and have no clinical symptoms and/or signs.

        스키손상에 대한 임상적 고찰

        We evaluated 1,743 patients with ski injuries who visited to the emergency room located in the Young Pyung Ski Resorts from Nov. 25, 1995 to Mar. 31, 1996. Among the patients, 971 patients were male and 772 were female. In age distribution, young patients from 21 to 30 years old were most common. 44.8% of ski injured patients were admitted on weekend, and 73.2% were on sun-shiny day. The most common injury mechanism was slip-down(65.5%). Among the many prevention methods of skin injury such as physical preparation prior to the skiing season, safe equipment, correcting skiing technique, consideration for other slope users and adaptation to the snow condition, etc., this survey revealed that the avoidance of a cold start is important. Simple injuries such as abrasion, contusion and simple laceration were most common as 66.0%, followed by ligament injuries(19.5%). There were 4 serious injured patients(ISS 16) who were injured by colliding with either moving subject or obstaticles along the ski slopes. Among 4 patients, 1 patients was severe head injury and died, 1 patient was spinal injury and pneumothorax, and 2 patients were hemoperitoneum. Severe injured patients are all beginning skiers and they were injured at a slope for beginning skiers. Certainly, the result of this study would indicate that skier`s experience such as total number of days skied and self-assessed ability, correlated with the risk of injury. Emergency post should be locate at the slope for beginning skiers and/or intermediate skiers, and the medical equipments for emergency care of shock, head injury, spinal injury and chest injury must be equipped at post.

      • 균열밀도에 따른 R·C 슬래브 교량의 내하력 평가

        Through the exterior features on reinforced concrete slab, crack density and crack originate deflections ratio crack density, decrease percentage of load carrying capacity, crack originate deflections ration and decrease percentage of load carrying capacity were statistically analyzed. Crack density, crack originate deflections ratio and decrease percentage of load carrying capacity are very closely related each other with 0.724, 0.821 and 0.317 of correlation coefficient within 95% of mind level. However, the correlation between crack originate deflections ratio and decrease percentage of load carrying capacity was low. Through the regression equation curve method in which crack density selected as independent variable, crack originate deflections ratio and decrease percentage of load carrying capacity as dependent variable, regression equations were presumed. Equations exist in mind level range of F(0.05, S-curve showed the highest, conformance with 79.5%, 72.2%. The regression equation between CRD or LCCD and CD was also shown. The overall judgement of load carrying capacity of bridges can be possible through the moreover investigation of crack density, the time needed precision could be valuable data for lower maintenance cost, the degree of deterioration of bridges and safety judgement.

        혼수상태의 외상환자에서 초음파를 이용한 흉부 및 복부손상의 진단

        The need for rapid diagnosis and treatment of life-threatening thoracic and intra-abdominal injury result in controversy over the appropriate triage of unconscious blunt trauma patients. To aid in early decisions for these patients, a prospective analysis of 98 patients with glasgow coma scale(GCS) scores≤8 was undertaken. Although intra-thoracic and intra-abdominal injuries were frequently identified based on systolic blood ressure, the use of clinical signs alone resulted in more missed injuries than did using the emergency ultrasonography. In normotensive patients(n=34), intra-thoracic injuries was identified in one patient(2.9%), and intra-abdominal injuries were in 7 patients(20.6%). In shock(systolic blood pressure<90mmHg) Pa-tients(n=64), thoracic injuries and intra-abdominal injuries were diagnosed in 8(12.5%) and 27 patients(42.3%), thoracic and abdominal combined injuries were identified in 11 patients(17.2%). The sensitivity, specificity, and accuracy of emergency ultrasonography were 96.3%, 91.7%, 94.9%. This study suggests that all unconscious trauma patients undergo immediate emergency ultrasonography to prevent missing life-threatening injuries.

        AVPU scale과 Glasgow Coma Scale의 상관관계 : 외상환자의 의식상태 평가법 THE EVALUATION METHODS OF MENTAL STATUS IN TRAUMA

        Background: The emergency treatment of multiple trauma patient includes rapid evaluation of mental status, triage and resuscitation. The principles of Advanced Trauma Life Support consist of primary survey & resuscitation, secondary surveys and definitive cares. In primary survey, the mental status is evaluated by AVUP scale, and GCS(Glasgow coma scale) is used in secondary survey because the calculation of GCS needs more times than AVPU scale. Purposes: Is there a correlation between AVPU scale and GCS as an evaluation method of mental status in trauma? Subjects: 434 injured patients(male 321, female 113, mean age 35.9 years) who had visited the emergency center of Wonju Christian Hospital was evaluated prospectively. The mental status was evaluated by AVPU scale and GCS by emergency physician as soon as the patients was admitted to emergency center. Results: Alert patients as AVPU scale were ranged from 14 to of GCS, patients who responded to verbal command were from 9 to 14 of GCS, patients who responded to pain stimuli were from 4 to 10 of GCS, and patients who did not respond to pain stimuli were from 3 to 5 of GCS. The linear correlation of AVPU scale and GCS was seen(r2=0.90). Conclusion: This study suggest that AVPU scale was vey closely correlated with GCS.
