        전침자극과 경피신경전기자극이 흰쥐 좌골신경 압좌손상 후 척수내 GAP-43 발현에 미치는 영향

        Purpose: The purpose of this study was to identify the effect of electroacupuncture(EA) and transcutaneous electric nerve stimulation(TENS) after sciatic nerve crush injury in rats. Methods: The EA for experimental group I (Exp I, n=15) and TENS for experimental group II (Exp II, n=15) was applied from post-injury day(PD) 1 to PD 14 after sciatic nerve injury using low frequency stimulator that gave electrical stimulation(15min/60Hz). In order observe the effect of EA and TENS, this study examined GAP-43 expression in rat lumbar spinal cord at the PD 1, PD 7 and PD 14. In addition, the stride length(SL) and toe out angle(TOA) were measured at the PD 7 and PD 4. Results; Exp I and Exp II had higher GAP-43 immunoreactivity than control group(PD 1, 7, 14). The SL of Exp I and Exp II were significantly higher than control group(PD 7, 14). The TOA of Exp I and Exp II were significantly lower than control group(PD 7, 14). Conclusion: EA and TENS application increased motor nerve recovery and expression of GAP-43 immunoreactivity after sciatic nerve crush injury. Therefore effect of TENS and EA had similar effect on nerve regeneration and functional recovery.

        A Study of EEG and Melatonin in Plasma According to Exercise Type in Elderly with Sleep Disorder

        목적 : 수면장애는 노인에게 있어 다양한 이유로 빈발하고 있다. 이러한 노인의 수면장애에서 운동 강도에 따른 지속적 인 운동습관이 수면장애의 치료적 효과를 줄 수 있는지를 밝히기 위해서 뇌파와 멜라토닌의 농도를 이용한 과학적인 방법으로 접근하는 것이 본 연구의 목적이다. 연구방법 : 일부지역의 노인시설에 거주하는 65세 이상의 노인 30명을 대상으로 하였다. 수면장애의 판단은 수면시간 6시간 이하의 경우(Kozier et al, 2004)와 PSQI(Pittsburgh Sleep Quality Index)의 점수를 동시에 만족하는 경우로 정하였다. 운동유형별 적용은 저강도의 걷기운동, 중강도의 유산소 운동, 고강도의 저항성 근력운동을 적용하였으며 측정도구는 뇌파에서 파형을 체크하기 위하여 QEEG 8-System(LAXTHA Inc. KOREA) 기기와 수면의 질을 검사 하기 위해서 수면다원검사에 사용하는 Polysomnograpy (Compumedics, Australia) 기기를 사용하였다. 그리고 TTC(triphenyltetrazolium chloride) 염색과 H & E(Hematoxylin & Eosin) 염색을 통해 조직학적 양상을 관찰하였다. 결과 : 중강도의 유산소 운동 후에 멜라토닌의 농도와 뇌파로 측정한 수면지수에는 긍정적인 유의미한 변화가 있었다. 중강도의 유산소 운동에서는 SWS가 저강도, 고강도 운동보다 유의한 효과가 있었고 SWS의 관찰지표인 델타파의 유의한 변화가 있었다. 결론 : 인체의 일주기성 리듬의 생화학적, 행동학적 기능에 변화를 가져오는 노인에게서 수면장애에 대한 지속적인 운 동은 운동의 유형에 따라 효과가 다름을 알 수 있었고 특히 유산소 운동이 다른 운동보다 노인 수면장애에 효과가 크다는 것을 멜라토닌의 농도와 뇌파를 통하여 알 수 있었다. 노인에게 맞춤형 운동을 제공하여 노인들의 삶의 질에 유용하고 건강한 말년을 제공할 수 있을 것이다. Objective : Sleep disorder caused by stress or disease to elderly currently. We tried to make clear constant exercise according to exercise intensity would have effect on sleep disorder in elderly using EEG and melatonin. Methods : Thirty subjects were over 65 years old who lived a senior facility in some parts. They were able to communicate and understand the purpose of the study. They also expressed their intension to participate actively in experiment. They should have alert consciousness and orientation about time, place, and people. Sleep disorder was assessed via below 6 hr total sleep time and Pittsburge Sleep Quality Index, which was satisfied with both of criterion at the same time. Exercises composed of low intensity walking, moderate aerobic exercise, and high intensity resistance strength. We used QEEG 8-System (LAXTHA Inc. KOREA) to check wave type and Polysomnograpy (Compumedics, Australia) to test quality of sleep. and Histologic features were observed by TTC (triphenyltetrazolium chloride) staining and H & E (Hematoxylin & Eosin) staining. Results : There was a significant sleep index and change of melatonin after aerobic exercise. There was a significant SOL, S1, S2, and SWS in aerobic exercise, but there was a significant SWS in walking and resistance strength. There was a significant change of delta wave especially in frontal and temporal region between exercises. Conclusion : They had different effects according to type of exercise, when elderly who have chemical, behavioral change of circadian rhythms did exercise consistently. Aerobic exercise had more effect on sleep disorder than other exercise. Therefore, we may supply proper exercise to elderly and high quality of life.

        Clinical interest has lately been roused by evidence that comprehension of synaptic plasticity may be based on the theoretical opinion. This paper describes perception of synaptic plasticity. Especially processes of long term potentiation(LTP) and long term depression(LTD) are discussed. Recently, it is assessed to genetical parts from development of molecular biology. Therefore this review also represents aspect of molecular events of synaptic plasticity.

      • GaAlAs 레이저 조사가 근타박상이 유발된 흰쥐 골격근내 혈관내피성장인자 발현에 미치는 영향

        Skeletal muscle regeneration is a vital process for various muscle myopathies and muscular adaptation to physiological overload. Angiogenesis is the key event in the process of muscle regeneration, and vascular endothelial growth factor(VEGF) plays an important role in it. The purpose of this study was to evaluate the effect of GaAlAs(830nm) laser and immunoreactivity of VEGF on angiogenesis after muscle contusion injury. Muscle contusion injury was induced in the triceps surae muscle by dropping a metal bead(31.4g). GaAlAs laser irradiation(power 20 mW, frequency 2000 Hz, treatment time 15 min) was applied directly to the skin of injured muscle daily for seven days. The experimental group I was irradiated immediately by laser after injury, whereas the experimental group II was irradiated after 1 day of injury. The control group was non-irradiated. The results of this study were as follows. 1. In morphological observation, there were no significant changes in experimental and control groups for 7 days. At 3 days, however, the splited muscle fibers were observed in experimental groups, and the muscle atrophy and granular tissue viewed at 7 days in control group. 2. The VEGF was expressed in muscle fiber that located in the interspace between gastrocnemius and soleus muscles. As the time coursed, the immunoreactivity of VEGF also seemed to be strong in the individual muscle fibers. 3. The experimental group I & II showed higher immunoreactivity of VEGF than control group(p<0.05). Then, the experimental group I showed higher than group II especially(p<0.05). These data suggest GaAlAs semiconduct diode laser irradiation(830nm) enhanced angiogenesis in the skeletal muscle induced contusion injury, and immediate laser irradiation after injury promoted the angiogenesis greatly than after 1 day of injury.

      • Capsaicin 적용 후 손상된 흰쥐 척수내 c-fos와 HSP70의 발현

        C-fos는 원종양유전자(proto-oncogene)인 v-fos의 세포 동족체로써, 성장인자나 신경전달 물질에 의해 수분 내에 다양한 형태의 세포에서 활성화된다. Fos단백질은 스트레스와 통증 과정의 신호전달기전에서 세포활동을 조절하는 3차전령으로 활동한다. 열충격 단백질(Heat shock protein : 이하 HSP)은 계통발생학적으로 초기 척추 동물에서부터 발현되며 생체방어체계의 중요한 인자로 세포가 고열, 외상, 허혈 등의 스트레스에 직면했을 때 발현이 증가하는 단백질로 알려져 있다. 본 연구에서는 캡사이신(capsaicin)으로 말초 신경병변을 유발시킨 후 통각신경활성의 지표로 이용되는 원종양 유전자인 c-fos의 발현과 열 또는 스트레스로 야기되는 손상에 대한 조직의 방어작용으로 발현되는 HSP 70의 발현을 동시에 관찰함으로서, 급성으로 유발된 말초 신경병변의 확인과 동시에 실험동물 체내에서 방어적인 역할을 밝히는 일환으로 이 실험을 실시하였다. 본 실험의 결과는 다음과 같다; 1. 척수 등쪽뿔 천층(Laminae I and II)에서 각각 c-fos와 HSP70을 항원으로 하는 면역조직화학적 방법으로 염색한 표본에서 0.9% NaCI 투여 2시간 후 c-fos와 HSP70의 양성을 나타내는 세포는 전혀 없음을 알 수 있었다. 2. 척수 등쪽뿔 천층에서 c-fos 단백질을 항원으로 하는 면역조직화학적 방법으로 염색한 표본에서 Capsaicin 투여 2시간 후 c-fos 단백질에 양성을 나타내는 세포가 많이 발현됨을 육안적 관찰로서 알 수 있었다. 3. 척수 등쪽뿔 천층에서 HSP70을 항원으로 하는 면역조직화확적 방법으로 염색한 표본에서 Capsaicin 투여 2시간 후 HSP7O의 양성을 나타내는 세포가 보통수준으로 발현됨을 육안적 관찰로서 알 수 있었다. 이 실험의 결과로 볼 때, 화학적인 신경병변 유발물질에 의한 손상을 방어하기 위해서 체내에는 내인성 물질이 형성될 것이라는 추측과 c-fos 가 다른 유전자의 발현을 유도한다는 점을 함께 고려 하였을때, Capsaicin에 의한 말초 신경병변에서 c-fos 발현이 많이 나타나는 것은 손상을 방어하는 물질의 생성에 관여하기 때문이며, 방어물질 중 이 실험에서 본 HSP70도 증가한 내인성 방어물질의 하나라고 할 수 있을 것이다.

        Vascular endothelial growth factors(VEGFs) constitute a group of structurally and functionally related growth factor that modulate many important physiological functions of endothelial cells, especially angiogenesis. This paper explain substance, which participate in signaling transduction of VEGF, including Bcl-2, caspase, focal adhesion kinase(FAK), integrin ${\alpha}v{\beta}3$, MAP kinase, nitric oxide(NO)and prostacyclin(PGI2). Physical therapy enhance angiogenesis for repairment of injury which as wound healing, muscle contusion, cerebrovascular disease, rheumatoid arthritis. Therefore this review assist understanding for mechanism of physical therapy as therapeutic angiogenesis.

      • 근타박상시 치료용 초음파가 혈관내피성장인자와 Substance-P 발현에 미치는 효과에 대한 면역조직화학적 연구

        Therapeutic angiogenesis is the controlled induction or stimulation of new blood vessel formation to reduce unfavourable tissue effects caused by local hypoxia and to enhance tissue repair. Therapeutic ultrasound can be considered as a physical agent to deliver therapeutic angiogenesis. The purpose of this study was to evaluate the effect of therapeutic ultrasound after muscle contusion injury by observed immunoreactivity of vascular endothelial growth factor(VEGF) that plays an important role in angiogenesis and substance-P in pain transmission. Ultrasound irradiation(1MHz, $1W/cm^2$, continuous mode, treatment time 5 min) was applied through water submersion technique to 1 limb daily by kept off 5cm from muscle belly of gastrocnemius. The result of this study were as follows. 1. In morphological observation, there were no significant changes excepts of 7 days. At 7 days, granular tissue viewed abundantly in control group. In other groups, general feature were increased interspace of muscle fiber; centronucleated muscle fiber; collapsed of muscle and nerve tissue; appeared inflammatory cell. 2. The VEGF was expressed in interspace of muscle fiber. Especially, at 7 days in experimental group, VEGF was showed in connective tissue surrounding gastrocnemius muscle. 3. The VEGF was higher expressed in experimental group at 2 and 3 days, but in control group at 7 days. These data suggest therapeutic ultrasound enhanced production of VEGF in the early day relatively, therefore stimulated angiogenesis in the skeletal muscle induced contusion injury. Also therapeutic ultrasound may stimulate pain relief by diminish of substance-P in dorsal horn of spinal cord.

      • 저강도레이저 조사가 근육압좌손상 후 척수분절의 EGF 발현에 미치는 영향

        Low energy laser irradiation(LELI) therapy in physical therapy is widespread but the mechanisms are not fully understood. The purpose of the present study was to examine the epidermal growth factor(EGF)'s expression within lumbar spinal cord which corresponding with crushed extensor digitorum longus(EDL) of rats after low-power laser irradiation applied. After a crushed injury on the right EDL, low-power laser irradiation was applied by using 2000mW, 2000Hz, 830nm GaAlAs(Gallium-aluminum-arsenide) semiconductor diode laser. The laser treatment was performed with 10 minutes daily for 3days. After EDL crush injury, EGF immunoreactive positive neurons in experimental group were progressively decreased from the first to third days. Especially 1 day subgroup is highly expressed in dorsal horn(Lamina I, II, III) and around of central cannal of spinal cord(Lamina VII). Control group was only expressed slightly at 3 days. This study suggests that LELI stimulate that release and migration of EGF in spinal cord, which distict to wound site, therfore promote wound healing of EDL crush injury.

      • GaAlAs 다이오드 레이저 조사가 장지신근 압좌손상 후 요수분절의 TGF-$\beta$ 발현에 미치는 영향

        Low intensity laser irradiation is potential physical agent that triggers the muscle regeneration by previous study. In muscle regeneration, a number of growth factors also promotes that is triggered in response to muscle damage. The transforming growth factor(TGF)-$\beta$ is involved in the activation of cell proliferation and the inhibition of cell differentiation in muscle regeneration. This is secreted not only autocrine system but also paracrine and endocrine. Therefore, We investigated that effects of Gallium aluminum arsenide(GaAlAs) diode laser for the expression of TGF-$\beta$ on lumbar spinal cord after extensor digitorum muscle crush injury. After laser irradiation, the immunoreactivity of TGF-$\beta$ was increased bilaterally in gray mater of spinal cord. Especially, in 1 day, experimental group was highed than control, and in 3 day, lateral motor nucleus were storong immunoreactivy of TGF-$\beta$. Also, in 1 and 2 day, TGF-$\beta$ was showed in white mater as well as gray mater, but in 3 day, only showed in gray mater. These data may suggests to the establishment of laser irradiation on spinal cord for skeletal muscle injury.

        흰쥐 좌골신경 압좌손상 후 척수분절의 저강도 레이저 조사가 운동기능 회복에 미치는 영향

        The purpose of the present study was to examine the functional recovery of the crushed sciatic nerve of rats after low-power laser irradiation applied to the corresponding segments of the spinal cord. After a crushed injury on the left sciatic nerve in rats. low-power laser irradiation was applied transcutaneously to corresponding segments of the spinal cord immediately after suture the wound by using 2000 mW, 2000Hz, 830 nm CaAIAs(Gallium-aluminum-arsenide) semiconductor diode laser. The laser treatment was performed with 10 minutes daily for 4 successive weeks. Functional recovery was evaluated per weekly following injury by sciatic function index(SFI),using data obtained by walking track analysis. For four weeks after crush injury, experimental group had significantly greater functional improvement than control group(${\alpha}$=0.05). In a experimental group, SFI was significantly increased for three weeks, but control group not increased for two weeks. This study suggests that low-power laser irradiation applied directly to the spinal cord can improve functional recovery of the crushed sciatic nerve in rats.
