한국인 성인 남성 폐쇄성수면무호흡 환자의 측모 두부 방사선계측학적 비교

본 연구는 비만도에 따른 한국인 남성 폐쇄성수면무호흡 환자의 측모 두부 방사선계측학적 특성을 파악하기 위하여 시행되었다. 이를 위하여 계명대학교 의과대학 동산의료원 수면클리닉에 수면장애를 주소로 내원하여 수면다원검사 후 치과에서 측모 두부방사선계측사진 촬영을 한 87명의 성인 환자들을 체질량지수(BMI)와 수면무호흡지수(AHI)에 따라 비비만 단순코골이군(Non-obese, simple snorers), 비만 단순코골이군(Obese, simple snorers), 비비만 수면 무호흡군(Non-obese, OSA patients), 비만 수면무호흡군(Obese, OSA patients)의 4군으로 나누어 비교하였다. 그결과, 4군 중 비만 수면무호흡군의 수면무호흡지수가 가장 컸으며, 비만 수면무호흡군보다 비비만 수면무호흡군의 하악각이 더 크고 혀 길이는 더 작았다. 또한, 비비만 수면무호흡군보다 비만수면무호흡군의 설골이 더 전하방에 위치하였고, 수면무호흡지수에 영향을 미치는 기여 인자는 비만 수면무호흡군에서는 혀 길이, 비비만 수면무호흡군에서는 설골의 후방위치였다. 이처럼 비만 수면무호흡 환자와 비비만 수면무호흡 환자의 측모 두부방사선계측학적 특성과 기여 인자가 다르게 나타나므로, 치료방법도 따라서 다르게 선택해야 할 것이다. 비만 수면무호흡 환자들에게는 먼저 체중감량이 권고되어야 할 것이고, 비비만 수면무호흡 환자들은 폐쇄부위에 따라 구강 내 장치나 Nasal CPAP (continuous positive airway pressure), UPPP (uvulopalatopharyngoplasty) 등이 추천될 수 있을 것이다. Objective: The purpose of this study was to compare the cephalometric measurements of obese and non-obese Korean male patients with obstructive sleep apnea syndrome (OSA). Methods: Eighty-seven adults who had visited the Sleep Disorder Clinic Center in Keimyung University, Daegu, Korea were examined and evaluated with polysomnography (PSG) and lateral cephalogram. They were divided into 4 groups (non-obese simple snorers, obese simple snorers, non-obese OSA patients, obese OSA patients) according to AHI (Apnea-Hypopnea Index) and BMI (Body Mass Index). Results: The obese OSA group had the highest AHI among the 4 groups. The non-obese OSA group had a significantly steeper mandibular angle and shorter tongue length than the obese OSA group. The hyoid bone of the obese OSA group was positioned anterior and inferior as compared with the non-obese OSA group. Multiple regression analysis showed that tongue length in the obese OSA group and retroposition of hyoid bone in the non-obese OSA group were significant determinants for the severity of AHI. Conclusions: From a cephalometric point of view, the obese and non-obese pateints with OSA may be characterized by different pathogeneses. Therefore, they have to be managed by individualized treatment. For the obese OSA patients, weight control must be advised as a first choice and for the non-obese OSA patients, oral appliance, nasal CPAP, UPPP and others could be chosen according to the obstructive sites.

Glucose is essential for testicular function; the uptake of carbohydrate-derived glucose by cells is mediated by glucose transporters (GLUTs). In the present study, we investigated the activity of GLUT1 and GLUT3, the two main isoforms of GLUTs found in testes, in the left scrotal and right abdominal testes of a German Shepherd dog. Immunohistochemical analysis showed that GLUT1 immunoreactivity was absent in the scrotal and abdominal testes. In contrast, weak to moderate GLUT3 immunoreactivity was observed in mature spermatocytes as well as spermatids in the scrotal testis. In the abdominal testis, relatively strong GLUT3 immunoreactivity was detected in Leydig cells only and was absent in mature spermatocytes and spermatids. GLUT3 immunoreactivity was significantly decreased in the tubular region of abdominal testis and significantly increased in the extra-tubular (interstitial) region of abdominal testis compared to observations in the each region of scrotal testis, respectively. These results suggest that GLUT3 is the major glucose transporter in the testes and that abdominal testes may increase the uptake of glucose into interstitial areas, leading to an increased risk of developing cancer.

<P>PURPOSE: To prospectively evaluate the effects of z-axis spatial resolution and tube current on the sensitivity of a commercially available computed tomographic (CT) colonography computer-aided diagnosis (CAD) system for polyp detection by using pig colon phantoms. MATERIALS AND METHODS: Ninety-six polyps were created and analyzed in 14 pig colon phantoms. CT colonography was performed by using a 16-detector CT scanner at 0.75-mm collimation; 10, 50, 100, and 160 mAs; and a pitch of 1.5. At each milliampere-second setting, the CT images were reconstructed with a section thickness (ST) of 1.5 mm and a reconstruction increment (RI) of 1.3 mm. To evaluate the effect of z-axis spatial resolution, CT images were also reconstructed at 100 mAs with various SI and RI combinations (respectively: 1.0 and 0.7 mm, 3.0 and 2.0 mm, 3.0 and 3.0 mm, 5.0 and 5.0 mm). The phantom data were then analyzed by using a CAD program. CAD performance with different CT parameters was calculated and compared in terms of per-polyp sensitivity and number of false-positive (FP) findings per data set. RESULTS: At a constant tube current of 100 mAs, the polyp detection rate was significantly higher in data sets obtained with SI and RI combinations of 1.0 and 0.7 mm, respectively (81% [78/96]), and 1.5 and 1.3 mm, respectively (75% [72/96]), than in those obtained with the three thicker ST-RI settings (27% [26/96] to 64% [61/96]) (P < .01). A similar trend was observed, regardless of polyp size or morphology. However, the number of FP findings at the 1.0 mm and 0.7 mm setting (8.9 per phantom) was also significantly greater than that at the thicker ST-RI settings (4.0-6.1 per phantom) (P < .05). At a constant z-axis spatial resolution (1.5-mm ST, 1.3-mm RI), CAD polyp detection rate and number of FP findings per phantom remained nearly constant-close to 78% (75/96) and 6.1, respectively-at various tube current settings. CONCLUSION: CAD performance in polyp detection at CT colonography is highly dependent on z-axis spatial resolution. serAtopic dermatitis is a chronic, inflammatory disease of the skin with increased transepidermal water loss. Both an abnormal inflammatory response and a defective skin barrier are known to be involved in the pathogenesis of atopic dermatitis. Protease activated receptor 2 (PAR2) belongs to a family of G-protein coupled receptors and is activated by both trypsin and a specific agonist peptide, SLIGKV-NH2. PAR2 is expressed in suprabasal layers of the epidermis and regulates inflammatory responses and barrier homeostasis. In this study, we show that nordihydroguaiaretic acid (NDGA) inhibits the PAR2-mediated signal pathway and plays a role in skin barrier recovery in atopic dermatitis. Specifically, NDGA reduces the mobilization of intracellular Ca2+ in HaCaT keratinocytes by down-regulating inflammatory mediators, such as interleukin-8, thymus and activation-regulated chemokine and intercellular cell adhesion molecule-1 in HaCaT keratinocytes. Also, NDGA decreases the protein expression of involucrin, a differentiation maker of keratinocyte, in both HaCaT keratinocytes and normal human epidermal keratinocytes. We examined NDGA-recovered skin barrier in atopic dermatitis by using an oxazolone-induced atopic dermatitis model in hairless mice. Topical application of NDGA produced an increase in transepidermal water loss recovery and a decrease in serum IgE level, without weight loss. Accordingly, we suggest that NDGA acts as a PAR2 antagonist and may be a possible therapeutic agent for atopic dermatitis.
