Ventricular septal defect remains an infrequent but devastating complication of acute MI. Approximately 1∼3% of cases of acute myocardial infarction are complicated by ventricular septal rupture in the prethrombolytic era and the incidence has decreased to 0.2% with reperfusion therapy. This mechanical complication usually occurs within the first 10 to 14 days when necrotic tissue is most abundant and the collateral coronary circulation is not well developed. This lesion is generally associated with complete coronary obstruction rather than severe stenosis. Rupture of the ventricular septum is a severe mechanical complication of acute myocardial infarction, usually resulting in death unless surgical repair is performed. The bad prognosis of this event within the first 2 weeks indicates the need for early surgical rapair. This complication is more frequent after the first acute myocardial infarction in the elderly and secondary to a transmural myocardial infarction. We report two cases of ventricular septal rupture after acute myocardial infarction.

Objective : Acute basilar artery occlusion (BAO) is associated with severe neurological dysfunction and high mortality rates. The benefits of mechanical thrombectomy in BAO have not been explored in recent clinical trials. Therefore, we analyzed outcomes of stent retriever mechanical thrombectomy for BAO, and compared with anterior circulation occlusions (ACO). Methods : In total, 161 consecutive patients (24 BAO, 137 ACO) who underwent mechanical thrombectomy with the stent retriever between January 2013 and August 2016 enrolled in our study. All patients underwent clinical assessment with the National Institutes of Health Stroke Scale (NIHSS) and modified Rankin scale (mRS). Radiological results were used to evaluate thrombolysis in cerebral infarction (TICI) scores and successful recanalization was defined by TICI 2b or 3. Results : Mean NIHSS scores at 24 hours and 30 days were significantly higher in the BAO group than the ACO group (p=0.021, p=0.001). mRS at 90 days was significantly higher in the BAO group (4.2) compared with the ACO group (3.0) (p=0.003). The BAO group (2.0) performed fewer stent retriever passages than the ACO group (2.7) (p=0.049). There were no significant differences between the two groups in terms of complications, but the BAO patients experienced a higher mortality (16.6%) rate than ACO patients (5.8%) (p=0.001). In subgroup analysis of BAO, patients with short procedure times achieved successful recanalization (p=0.001) and successfully recanalized patients exhibited more favorable mRS at 90 days (p=0.027). Conclusion : In our study, mechanical thrombectomy of BAO patients showed worse clinical outcome and higher mortality rate than ACO patients. However, mechanical thrombectomy with a stent retriever in BAO is an effective treatment, because successfully recanalized patients showed good clinical outcome in BAO patients.
A Novel Soft-Switching Full-Bridge PWM Converter with an Energy Recovery Circuit

This paper proposes a new phase-shift full-bridge DC-DC converter by applying energy recovery circuits to a conventional full-bridge DC-DC converter in plasma display panel applications. The converter can achieve soft-switching in main-switches by an extra auxiliary resonant network even with the wide operating condition of both output load and input voltage. The un-coupled design guidelines to the main bridge-leg component parameters for soft-switching operation contribute to conduction loss reduction in the transformer primary side leading to efficiency improvement. The auxiliary switches in the resonant network also operate in zero-current switching. This paper analyzes the operation modes of the proposed scheme and presents the key design guidelines through steady state analysis. Also, the paper verifies the validity of the circuits by hardware experiments with a 1kW DC/DC converter prototype.

To investigate the effect of extreme high temperature on the cardiopulmonary function, we measured the heart rate, blood pressure, lung volumes, and breath-holding time before and during exposure to 70℃ environment in male adults. 1) The heart rate demonstrated exquisite sensitivity to extreme high temperature; exposure to heat increased the heart rate from 75 bpm to 124 bpm(65% increase). 2) There was a tendency for both systolic and diastolic pressure to decrease, which was not statistically significant. 3) While the exposure increased the respiratory rate from 19/min to 28/min (47% increase), it decreased the tidal volume from 566ml to 314ml. 4) The vital capacity decreased from control of 4.31 L to 3.26 L during exposure. The exposure also decreased the maximum breathing capacity (before exposure, 99 L; during exposure, 56 L0. 5) The breath-holding time decreased from 48 sec to 38 sec during exposure.
Evaluating the results of the Momguard noninvasive prenatal test

Purpose: To evaluate the performance of the Momguard noninvasive prenatal test by tracing the 'screen positive' results based on preliminary samples from Korean cohorts. Materials and Methods: This preliminary study is based on data collected by the LabGenomics Clinical Laboratory (Seongnam, Korea) with informed consent. Only pregnant women who underwent both the Momguard test and karyotyping were included in this study. Momguard test results were compared with those of the karyotyping analysis. Results: Among the 38 cases with 'screen positive' results by Momguard, 30 cases also had karyotyping results available. In three trisomy (T) 18 and three T13 cases, the Momguard results were concordant with the karyotyping results. For the T21 cases, except for one case belonging to the mid-risk zone, Momguard results from 23 out of 24 cases matched the karyotyping results. Conclusion: Momguard is a highly reliable screening tool for detecting T13, T18, and T21 cases in independent Korean cohort samples.
